Severe pediatric COVID‑19:a review from the clinical and immunopathophysiological perspectives

Background Coronavirus disease 2019(COVID-19)tends to have mild presentations in ***,severe and critical cases do arise in the pediatric population with debilitating systemic impacts and can be fatal at times,meriting further attention from ***,the intricate interactions between the pathogen virulence factors and host defense mechanisms are believed to play indispensable roles in severe COVID-19 pathophysiology but remain incompletely *** sources A comprehensive literature review was conducted for pertinent publications by reviewers independently using the PubMed,Embase,and Wanfang *** keywords included“COVID-19 in children”,“severe pediatric COVID-19”,and“critical illness in children with COVID-19”.Results Risks of developing severe COVID-19 in children escalate with increasing numbers of co-morbidities and an unvaccinated *** respiratory distress stress and necrotizing pneumonia are prominent pulmonary manifestations,while various forms of cardiovascular and neurological involvement may also be *** immunological processes are implicated in the host response to COVID-19 including the type I interferon and inflammasome pathways,whose dysregulation in severe and critical diseases translates into adverse clinical *** inflammatory syndrome in children(MIS-C),a potentially life-threatening immune-mediated condition chronologically associated with COVID-19 exposure,denotes another scientific and clinical conundrum that exemplifies the complexity of pediatric *** the considerable dissimilarities between the pediatric and adult immune systems,clinical trials dedicated to children are lacking and current management recommendations are largely adapted from adult *** Severe pediatric COVID-19 can affect multiple organ *** dysregulated immune pathways in severe COVID-19 shape the disease course,epitomize the vast functional diversity of the

Background Respiratory syncytial virus(RSV)is the leading global cause of respiratory infections and is responsible for about 3 million hospitalizations and more than 100,000 deaths annually in children younger than 5 years,representing a major global healthcare *** is a great unmet need for new agents and universal strategies to prevent RSV infections in early life.A multidisciplinary consensus development group comprising experts in epidemiology,infectious diseases,respiratory medicine,and methodology aims to develop the current consensus to address clinical issues of RSV infections in *** sources The evidence searches and reviews were conducted using electronic databases,including PubMed,Embase,Web of Science,and the Cochrane library,using variations in terms for"respiratory syncytial virus","RSV","lower respiratory tract infection","bronchiolitis","acute","viral pneumonia","neonatal","infant""children",and"pediatric".Results Evidence-based recommendations regarding diagnosis,treatment,and prevention were proposed with a high degree of *** supportive care remains the cornerstone for the management of RSV infections,new monoclonal antibodies,vaccines,drug therapies,and viral surveillance techniques are being rolled *** This consensus,based on international and national scientific evidence,reinforces the current recommendations and integrates the recent advances for optimal care and prevention of RSV *** improvements in the management of RSV infections will require generating the highest quality of evidence through rigorously designed studies that possess little bias and sufficient capacity to identify clinically meaningful end points.

Fast Blood Oxygenation through Hemocompatible Asymmetric Polymer of Intrinsic Microporosity Membranes

Membrane technology has attracted considerable attention for chemical and medical applications,among *** organs play important roles in medical science.A membrane oxygenator,also known as artificial lung,can replenish O_(2) and remove CO_(2) of blood to maintain the metabolism of patients with cardiopulmonary ***,the membrane,a key component,is subjected to inferior gas transport property,leakage propensity,and insufficient *** this study,we report efficient blood oxygenation by using an asymmetric nanoporous membrane that is fabricated using the classic nonsolvent-induced phase separation method for polymer of intrinsic *** intrinsic superhydrophobic nanopores and asymmetric configuration endow the membrane with water impermeability and gas ultrapermeability,up to 3,500 and 1,100 gas permeation units for CO_(2) and O_(2),***,the rational hydrophobic–hydrophilic nature,electronegativity,and smoothness of the surface enable the substantially restricted protein adsorption,platelet adhesion and activation,hemolysis,and thrombosis for the ***,during blood oxygenation,the asymmetric nanoporous membrane shows no thrombus formation and plasma leakage and exhibits fast O_(2) and CO_(2) transport processes with exchange rates of 20 to 60 and 100 to 350 ml m^(−2) min^(−1),respectively,which are 2 to 6 times higher than those of conventional *** concepts reported here offer an alternative route to fabricate high-performance membranes and expand the possibilities of nanoporous materials for membrane-based artificial organs.

Recent reports of acute hepatitis of unknown origin in previously healthy children have been increasing *** main characteristics of the affected children were jaundice and gastrointestinal *** serum aminotransaminase levels were above 500 IU/L,with negative tests for hepatitis viruses A–*** 31 May 2022,the outbreak had affected over 800 children under the age of 16 years in more than 40 countries,resulting in acute liver failure in approximately 10%,including at least 21 deaths and 38 patients requiring liver *** was still no confirmed cause or causes,although there were several different working hypotheses,such as severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),adenovirus serotype 41,or SARS-CoV-2 superantigen-mediated immune cell ***,we review early observations of the 2022 outbreak which may inform diagnosis,treatment,and prevention in the context of an overlapping COVID-19 pandemic.

Design and pricing of maintenance service contract based on Nash non-cooperative game approach

Nowadays manufacturers are facing fierce *** from the products,providing customers with multiple maintenance options in the service contract becomes more popular,since it can help to improve customer satisfaction,and ultimately promote sales and maximize profit for the *** considering the combinations of corrective maintenance and preventive maintenance,totally three types of maintenance service contracts are ***,attractive incentive and penalty mechanisms are adopted in the *** this basis,Nash non-cooperative game is applied to analyze the revenue for both the manufacturer and customers,and so as to optimize the pricing mechanism of maintenance service contract and achieve a win-win *** experiments are *** results show that by taking into account the incentive and penalty mechanisms,the revenue can be improved for both the customers and ***,with the increase of repair rate and improvement factor in the preventive maintenance,the revenue will increase gradually for both the parties.

Safety and clinical efficacy of linezolid in children:a systematic review and meta-analysis

Background We aimed to evaluate the tolerability and efficacy of linezolid in children for treating suspected and diagnosed Gram-positive bacterial *** A systematic literature search was conducted up to April 23,2021,using linezolid and its synonyms as search *** reviewers independently identified and extracted relevant randomized controlled trials and prospective cohort *** extracted studies were included in a single-rate meta-analysis of adverse events and clinical outcomes using random-effects *** A total of 1082 articles were identified,and nine studies involving 758 children were included in the *** overall proportion of adverse events was 8.91%[95%confidence interval(CI)=1.64%–36.52%],with diarrhea(2.24%),vomiting(2.05%),and rash(1.72%)being the most *** incidences of thrombocytopenia and anemia were 0.68%and 0.16%,*** specific adverse events,including rash and gastrointestinal events,were more frequent in the oral administration *** terms of efficacy,the overall proportion of clinical improvement was 88.80%(95%CI=81.31%–93.52%).Children with a history of specific bacteriological diagnosis or concomitant antibiotic therapy had a 1.13-fold higher clinical improvement than children without such *** proportion of microbial eradication was 92.68%(95%CI=84.66%–96.68%).The proportion of all-cause mortality was 0.16%(95%CI=0.00%–7.75%).Conclusions linezolid was well-tolerated in pediatric patients and was associated with a low frequency of adverse events,such as anemia,thrombocytopenia,and ***,linezolid was effective in children with diagnosed and suspected Gram-positive infections.

Interannual evolution of the chemical composition,sources and processes of PM_(2.5)in Chengdu,China:Insights from observations in four winters

The air quality in China has improved significantly in the last decade and,correspondingly,the characteristics of PM_(2.5)have also *** studied the interannual variation of PM_(2.5)in Chengdu,one of the most heavily polluted megacities in southwest China,during the most polluted season(winter).Our results show that the mass concentrations of PM_(2.5)decreased significantly year-by-year,from 195.8±91.0μg/m~3in winter 2016 to 96.1±39.3μg/m^(3)in winter *** mass concentrations of organic matter(OM),SO_()4^(2-),NH_(4)^(+)and NO_(3)^(-)decreased by 49.6%,57.1%,49.7% and 28.7%,*** differential reduction in the concentrations of chemical components increased the contributions from secondary organic carbon and NO_(3)^(-)and there was a larger contribution from mobile *** contribution of OM and NO_(3)^(-)not only increased with increasing levels of pollution,but also increased year-by-year at the same level of *** sources of PM_(2.5)were identified:combustion sources,vehicular emissions,dust and secondary *** aerosols made the highest contribution and increased year-by-year,from 40.6%in winter 2016 to 46.3% in winter *** contrast,the contribution from combustion sources decreased from 14.4% to 8.7%.Our results show the effectiveness of earlier pollution reduction policies and emphasizes that priority should be given to key pollutants(e.g.,OM and NO_(3)^(-))and sources(secondary aerosols and vehicular emissions)in future policies for the reduction of pollution in Chengdu during the winter months.

Malignant hyperthermia(MH)is a genetic disorder of skeletal muscle cells that affects muscle cytoplasmic calcium homeostasis,with high mortality and low ***,it presents with non-specific signs of a hypermetabolic response,including high fever,tachycardia,and elevated end-tidal carbon dioxide(ETCO_(2)).The successful treatment lies in the timely recognition and early use of dantrolene.[1]As an inhibitor of Ca2+release through ryanodine receptor(RYR)channels,the skeletal muscle relaxant dantrolene has proven to be both a valuable experimental probe of intracellular Ca2+signaling and a lifesaving treatment for MH.[2]Dominant mutations in the skeletal muscle RYR1 gene are well-recognized causes of both malignant hyperthermia susceptibility(MHS)and central core disease(CCD).
