Physical activity decreases in patients on the liver transplant waiting list and influences postoperative outcome-a prospective cohort study

background:Physical deconditioning affects patients suffering from end-stage liver disease(ESLD).Liver transplantation(LT)is the only curative option for *** evidence suggests that pre-habilitation is beneficial in reducing post-surgical morbidity and *** investigated physical activity(PA)in patients awaiting LT in a country with long waiting ***:Prospective,single center,longitudinal study in bern,Switzerland between June 2019 and February 2020(halted due to SARS-CoV-2 pandemic),with follow-up data up to six months *** were instructed to use a wrist tracker(Fitbit)to monitor PA,which was assessed using mixed-effects generalized linear *** study was approved by the local ethics committee(bASEC ID 2019-00606).Results:Thirty-five patients were included[71%male,median 59 years,body mass index(bMI)28 kg/m^(2),lab Model End-Stage Liver Disease(MELD)11],17(49%)pre-frail and 5(14%)frail according to the Liver Frailty Index(LFI).Twenty-eight patients underwent transplantation with 0 ninety-day mortality and 15(53.6%)composite adverse clinical *** daily steps were 4,661[interquartile range(IQR),1,685-8,609]and weekly moderate PA(MPA)was 41 min(IQR,0-127 min).Longitudinal analysis showed that female patients and patients on nutritional support had an increase in MPA between weeks 20 and 40.A significant decrease was seen in MPA after week 40,whilst no significant association was seen with age,Child-Pugh Score,LFI or quality of life at time of *** was significantly associated with the occurrence of the composite clinical endpoint after week 30 of waiting time(odds ratio 0.882,P=0.026).World Health Organization(WHO)-recommended MPA was significantly associated with less adverse composite clinical outcomes(P<0.001).Conclusions:In patients listed for LT,MPA decreased over time,showing a significant association with adverse outcome,specifically after week 30 on the waiting *** data support the implem

RANDOMIZED POLICY OF A POISSON INPUT QUEUE WITH J VACATIONS

This paper studies the operating characteristics of an M/G/1 queuing system with a randomized control policy and at most J vacations. After all the customers are served in the queue exhaustively, the server immediately takes at most J vacations repeatedly until at least N customers are waiting for service in the queue upon returning from a vacation. If the number of arrivals does not reach N by the end of the jth vacation, the server remains idle in the system until the number of arrivals in the queue reaches N. If the number of customers in the queue is exactly accumulated N since the server remains idle or returns from vacation, the server is activated for services with probability p and deactivated with probability （l-p）. For such variant vacation model, other important system characteristics are derived, such as the expected number of customers, the expected length of the busy and idle period, and etc. Following the construction of the expected cost function per unit time, an efficient and fast procedure is developed for searching the joint optimum thresholds （N＊,J＊） that minimize the cost function. Some numerical examples are also presented.

Monomer-activated Copolymerization of Ethylene Oxide and Epichlorohydrin: In Situ Kinetics Evidences Tapered block Copolymer Formation

The monomer-activated anionic ring-opening copolymerization (AROP) of ethylene oxide (EO) and epichlorohydrin (ECH) using tetraoctylammonium bromide as an initiator and triisobutylaluminum (i-bu3Al) as an activator was studied. The properties of the copolymers as well as the microstructure have been analyzed in detail via an in situ NMR kinetics study. The statistical copolymers exhibited molecular weights ranging from 2350 g·mol^-1 to 38000 g·mol^-1 (measured by SEC, PEG-standards) and moderate dispersities of 1.27-1.44. The thermal property tests revealed both a glass transition and melting for all copolymers, supporting a block-like nature. Applying in situ NMR kinetic measurements, the reactivity ratios of EO and ECH were determined to be strongly disparate, i.e., rEo = 9.2 and rECH = 0.10. This shows that the simple one-pot statistical anionic copolymerization of EO and ECH via the monomer-activated AROP resulted in the formation of strongly tapered, block-like structures. Furthermore, post-polymerization functionalization of the reactive chloromethyl groups by nucleophilic displacement was investigated for the copolymers. Copolymerization of EO and ECH offers a broad platform for further functionalization and therefore the possibility to prepare a variety of multifunctional PEGs.

Unilateral mimicking bilateral bPPV- a forgotten entity? Characteristics of a largecohort of patients, comparison with posterior canal bPPV and clinical implications

Objective: Unilateral mimicking bilateral benign paroxysmal positional vertigo (umb-bPPV) was attributed to inappropriate head positioning during testing of the posterior canal. Despite its inclusion in theDiagnostic criteria for the classification of vestibular disorders of the barany Society, the clinical characteristics and treatment responsiveness of this bPPV subtype have not been intensively ***: Records of patients with bPPV seen at a single outpatient dizziness clinic during the years 2000e2020 were reviewed. Eighty seven patients with umb-bPPV and 86 random patients with posteriorcanal bPPV (p-bPPV) were retrieved. Their demographics and bPPV characteristics were ***: Patients' and bPPV characteristics were similar in umb- and p-bPPV except for the prevalence ofmales in the umb-bPPV group. No differences were found between treatment responsiveness and recurrences in both groups. The recurrence rate of umb-bPPV was not influenced by age, gender, bPPV side,duration of symptoms or treatment responsiveness during the first ***: In accordance with our hypothesis about mixed canalo- and cupulolithiasis as the underlying mechanism of umb-bPPV, patients did not differ in characteristics and treatment responsivenessfrom p-bPPV patients. Recognition of umb-bPPV is important since inappropriate treatment can cause anunnecessary delay in therapy success.
