The outbreak of corona virus disease 2019(COVID-19)imposes a major challenge in managing patients undergoing surgical *** this study,we analyzed clinical and transmission features of 25 cases of COVID-19 from a single thoracic department,including 13 patients and 12 health care *** were 13 males and 12 *** median age of the patients was 61(range:51 to 69)*** median age of the health care staff was 35(range:22 to 51)*** the end of follow-up date(Mar.3,2020),there were 16 non-severe cases(64%)and 9 severe cases(36%),5 cases were dead(20%).Nineteen(76%)of the infected cases were confirmed by SARS-CoV-2 nucleic acid test,the rest were clinically diagnosed as suspected COVID-19 cases,and 19(76%)of the infected cases had positive exposure *** found that COPD was significantly associated with severity and death(P-0.040,and P=0.038,respectively),and chest operation was significantly associated with death for COVID-19 patients(P-0.039).A potential“super spreader"may be the source of the transmission before the implementation of quarantine and comprehensive *** was concluded that COVID-19 is associated with poor prognosis for patients undergoing thoracic operation,especially for those with *** of comprehensive protective measures is important to control nosocomial infection.

AIM: To compare the efficacy of hepatic resection(HR) and transarterial chemoembolization(TACE) for patients with solitary huge(≥ 10 cm) hepatocellular carcinoma(HCC).METHODS: Records were retrospectively analyzed of 247 patients with solitary huge HCC, comprising 180 treated by HR and 67 by TACE. Long-term overall survival(OS) was compared between the two groups using the Kaplan-Meier method, and independent predictors of survival were identified by multivariate analysis. These analyses were performed using all patients in both groups and/or 61 pairs of propensity score-matched patients from the two groups. RESULTS: OS at 5 years was significantly higher in the HR group than the TACE group, across all patients(P = 0.002) and across propensity score-matchedpairs(36.4% vs 18.2%, P = 0.039). The two groups showed similar postoperative mortality and morbidity. Multivariate analysis identified alpha-fetoprotein ≥ 400 ng/m L, presence of vascular invasion and TACE treatment as independent predictors of poor ***: Our findings suggest that HR can be safe and more effective than TACE for patients with solitary huge HCC.

AIM To investigate the efficacy and safety of postoperative adjuvant transcatheter arterial chemoembolization(PA-TACE) in preventing tumor recurrence and improving survival in Barcelona Clinic liver Cancer(BCLC) early(A) and intermediate(B) stage hepatocellular carcinoma(HCC) patients with microvascular invasion(MVI).METHODS A total of 519 BCLC A or B HCC patients treated by liver resection alone or followed by PA-TACE between January 2012 and December 2015 were studied retrospectively. Univariate and multivariate analyses were performed to investigate the risk factors for recurrence-free survival(RFS) and overall survival(OS). Multiple logistic regression was used to identify the clinicopathological characteristics associated with MVI. The rates of RFS and OS were compared among patients with or without MVI treated with liver resection alone or followed by PA-TACE. RESULTS Univariate and multivariate analyses demonstrated that serum AFP level > 400 ng/m L, tumor size > 5 cm, tumor capsule invasion, MVI, and major hepatectomy were risk factors for poor OS. Tumor capsule invasion, MVI, tumor size > 5 cm, HBV-DNA copies > 1 x 104 IU/m L, and multinodularity were risk factors for poor RFS. Multiple logistic regression identified serum AFP level > 400 ng/m L, tumor size > 5 cm, and tumor capsule invasion as independent predictors of MVI. Both OS and DFS were significantly improved in patients with MVI who received PA-TACE as compared to those who underwent liver resection alone. Patients without MVI did not show a significant difference in OS and RFS between those treated by liver resection alone or followed by *** PA-TACE is a safe adjuvant intervention and can efficiently prevent tumor recurrence and improve the survival of BCLC early-and intermediate-stage HCC patients with MVI.

The low perioperative morbidity and shorter hospital stay associated with laparoscopic hepatectomy have made it an often-used option at many liver centers, despite the fact that many patients with hepatocellular carcinoma have cirrhosis, which makes the procedure more difficult and dangerous. Type of surgical procedure proves not to be a primary risk factor for poor outcomes after hepatic resection for hepatocellular carcinoma, the available evidence clearly shows that laparoscopic hepatectomy is an effective alternative to the open procedure for patients with early-stage hepatocellular carcinoma, even in the presence of cirrhosis. Whether the same is true for patients with intermediate or advanced disease is less clear, since laparoscopic major hepatectomy remains a technically demanding procedure.

Background and Aims:This study was designed to analyze the effects of age and clinicopathological characteristics on prognosis of Chinese patients with hepatocellular carcinoma(HCC).Methods:The clinical data of 2032 HCC patients who were first diagnosed with HCC and underwent curative hepatectomy in our hospital between January 2006 and January 2011 were retrospectively ***:Younger HCC patients(agelevels,higher preoperative liver function,and more frequent vascular invasions than older *** younger patients were suitable for anatomical hepatectomy,and their tumors were found to be at a highly advanced *** recurrence-free survival and overall survival rates of younger HCC patients were significantly worse than those of older patients but this difference disappeared after propensity score *** analysis of pre-matched samples showed that age≤40 years was one of the independent risk factors associated with poor overall ***:Younger patients showed different clinicopathological characteristics than older patients,such as higher rates of hepatitis B infection and advanced *** recurrence-free survival and overall survival rates of younger HCC patients after hepatectomy may be similar to those of older patients.
