Background: Pulmonary arterial hypertension (PAH) and chronic thromboembolic pulmonary hypertension (CTEPH) are serious diseases with similar pathophysiologic aspects. The prognosis of patients with these conditions is highly uncertain, particularly incident cases. Methods: A Cox proportional hazards model was applied to a group of 85 patients (81% women, mean age 52 (18 - 82) years) with PAH (80%) and non-surgical CTEPH (20%) to evaluate risk factors for mortality. The following variables were included in the model: age, etiology, baseline 6-minute walk test (6 mWT), cardiac index, and improvement in the 6 mWT following initiation of first medical treatment. Results: In the multivariate analysis, the response to treatment, assessed by an improvement on the 6 mWT, was the most relevant prognostic factor in these patients (RR, 4.832 (95% CI, 1.888 - 12.364);p = 0.001). The remaining variables studied in this model had less influence on the prognosis: age > 50 years (RR, 0.744 (95% CI, 0.26 - 2.133);p = 0.582);etiology of connective tissue disease-associated PAH (RR, 3.145 (95% CI, 0.995-9.946);p = 0.051) or CTEPH (RR, 0.654 (95% CI, 0.179 - 2.387);p = 0.521) with respect to idiopathic PAH;baseline 6 mWT (RR, 1.173 (95% CI, 0.599 - 4.895);p = 0.315);or cardiac index (RR, 2.295 (95% CI, 0.793 - 6.642);p = 0.125). Conclusions: There is a high degree of uncer-tainty regarding the prognosis of PAH and CTEPH at the start of appropriate treatment. Our results support the idea that the initial treatment response is of paramount importance as prognostic factor in these patients.

Automated image systems to characterize aquatic organisms improve research and enable fast response to environmental risk situations. In November 2015, a dam in Mariana City-MG (Brazil) collapsed and led to the disposal of mud tailings from the mining process to the Doce River. The accident resulted in several casualties and incalculable damage to surrounding communities and the environment. The mud increased water turbidity, an essential condition to the functioning of the image analysis systems, and directly affected the characterization of the organisms, making it impossible to distinguish copepods in the mud, due to the blurred outline. To get a quick response evaluating environmental situations, this work aimed to develop and test different algorithms characterizing and classifying copepods by their size (length and area) using in situ images acquired by the Lightframe On-Sight Keyspecies Investigation device. Field tests were carried out under different turbidity levels throughout the gradient observed in the coastal zone adjacent to the Doce River. The best algorithm reduced nearly 50% of the noise in some images when compared with manual treatment and led to 96% accuracy in measurement and counting. Semi-automated devices that perform post-processing corrections are suitable for fast environmental evaluation under high turbidity scenarios.

Objective: To isolate Candida albicans(C. albicans) from the urine of hospitalized patients and assess the virulence factors and antifungal susceptibility profiles of the ***: Yeasts were identified using the chromogenic medium CHROMagar?, the VITEK~?2 system, hypertonic Sabouraud broth, tobacco agar, polymerase chain reaction,and DNA sequencing. The evaluated virulence factors were proteinase production,phospholipase production, and biofilm production on polystyrene. The broth microdilution technique was used to determine the minimum inhibitory ***: All yeasts isolated from urine were identified as C. albicans using both classical and molecular methods. Although 91.3% of the isolates showed no phospholipase activity, 56.5% showed strong proteinase activity and 91.7% produced biofilm. All microorganisms were sensitive to fluconazole, voriconazole and amphotericin B, but 56.5% of the yeasts showed resistance to ***: C. albicans isolates from urine have a high capacity for virulence and can be associated with infectious processes. Furthermore, the high percentage of isolates resistant to itraconazole is important because this antifungal agent is commonly used to treat fungal infections in the hospital environment.

Proportion of Cesarean Deliveries According to Hospital Funding in São Paulo Metropolitan Area, Brazil

Introduction: There has been an increase in the proportion of cesarean deliveries, especially in developing countries. Methods: Cross-sectional study of all hospital live births from mothers living in Sao Paulo Metropolitan Area, Brazil. Data were obtained from the Live Birth Information System, which is linked to the National Database of Health Establishments. Hospitals were classified as public, private or mixed. Descriptive analysis and exponential regression were conducted to evaluate time trends. Poisson regression was applied to analyze each hospital type to identify risk factors and the attributable risk fraction for cesarean section. Results: There was an annual increase (1.4%) of cesarean deliveries between 2000 and2013. In2009, the percentage of cesarean deliveries was 53% overall and 83% in private hospitals. The primary risk factor for cesarean delivery was delivery in a private hospital. Other risk factors that were stronger in public hospitals included the following: advanced maternal age, high maternal education, nulliparity, high number of prenatal visits, multiple pregnancy, hospital with low annual volume of deliveries, birth outside the city of residence and white skin color. Discussion: These results may be explained by the obstetric care model of private hospitals, which is not multiprofessional. Prenatal care and delivery are conducted by the same doctor and rarely include the input of another professional, such as a midwife. In addition, the mode of delivery influences the professional’s payment.

Selenium Content in Blood of Donors from the North of Rio de Janeiro State, Brazil

Selenium is an element which participates in antioxidant enzymes. A medium and long term lack of such element is associated, mainly, with heart disease, joint and bone structure problems and thyroid activity. Selenium contents in blood reflect its ingestion and food content variation. It depends on soil characteristics, such as pH and selenium presence. There are few studies concerning selenium levels in food and blood in Brazil. Therefore, the objective of this research was to determine selenium content in a blood donor group in Rio de Janeiro state, conducted from December 2008 to March 2009. From the donated blood to Regional Blood Center of Campos dos Goytacazes, 4.0 mL were submitted to selenium analysis through atomic absorption spectrometry of hydride generation and organic matter oxidation wet basis with nitric and perchloric acid. Selenium content varied from 75 ± 16 ng·g-1 for females and 70 ± 21 ng·g-1 for males. There was no significant difference among selenium contents in blood from donors of different gender, age and residence. Seventy four percent of donors had selenium content in blood below 70 ng·mL-1, the lowest level for maximum selenoproteins production, according to the Nutritional Prevention Cancer.

Background: Skin reactions and musculoskeletal injuries caused by intramuscular injection are an increased risk for skin and soft tissue infections and may culminate in sepsis. Objective: To describe the history of an elderly patient who developed a lesion in her right arm after home administration of intramuscular iron injection in this arm and its outcome. Methods: This is a case report of a patient who participated in a study in an intensive care unit. Results: This study describes a 65-year-old patient with multiple comorbidities who was admitted to the hospital complaining of pain for two months and difficulty in moving her right shoulder associated with skin and soft tissue infections in her right arm after intramuscular iron administration by a relative for the treatment of multifactorial anemia and refractory melena. The patient worsened her general condition and was transferred to the intensive care unit. Despite the therapy instituted, the patient developed sepsis of cutaneous origin, multiple organ dysfunction, and death. Conclusions: The administration of intramuscular medication requires the participation of qualified professionals, such as nurses, mainly within the scope of the Brazilian public health system which commonly comprises patients in a context of vulnerability.

Tuberculosis is one of the 10 leading causes of death in the world. Its symptoms include fever, malaise, weakness, weight loss, chest pain, cough, expectoration, shortness of breath and sepsis, which is its most frequent complication. Due to these symptoms, many patients with tuberculosis require admission to the Intensive Care Unit, where they usually are placed on mechanical ventilation. Tuberculosis is more prevalent in the population in situations of social and economic vulnerability. The main factors that interfere with adherence to treatment and the prognosis of these patients are the patient’s support network and their socioeconomic status. We present the following report of a long-time smoker patient, with chronic kidney disease and previous treatment for tuberculosis, who was brought to the emergency room due to lowering of the sensorium, and was immediately placed on mechanical ventilation. The tests suggested sepsis with a respiratory focus, so a search for Alcohol-Acid-Resistant Bacillus was carried out, with a positive result. The patient was transferred to the Intensive Care Unit due to tachycardia and acute respiratory failure. The patient was a smoker with a high tobacco load, chronic kidney disease, in addition to precarious economic, educational and self-care conditions. This report shows that patients with tuberculosis and its complications can be stabilized through known pharmacological treatment. However, the most effective measure to interrupt the transmission of the disease remains the early diagnosis and the provision of adequate treatment, with guidelines aimed at the cessation of harmful habits, such as smoking.

The assessment of detrital zircon age records is a key method in basin analysis,but it is prone to several biases that may compromise accurate sedimentary provenance *** to ultrahigh temperature(HT-UHT)metamorphism(especially if T>850℃)is herein presented as a natural cause of bias in provenance studies based on U-Pb detrital zircon ages,since zircon from rocks submitted to these extreme and often prolonged conditions frequently yield protracted,apparently concordant,geochronological *** age spreading can result from disturbance of the primary U-Pb zircon system,likewise from(re)crystallization processes during multiple and/or prolonged metamorphic *** this contribution,available geochronological data on Archean,Neoproterozoic and Palaeozoic HT-UHT metamorphic rocks,acquired by different techniques(SIMS and LA-ICP-MS)and showing distinct compositions,are reassessed to demonstrate HT-UHT metamorphism may result in modes and age distributions of unclear geological *** a consequence,it may induce misinterpretations on UPb detrital zircon provenance analyses,particularly in sedimentary rocks metamorphosed under such extreme temperature *** evaluate the presence of HT-UHT metamorphism-related bias in the detrital zircon record,we suggest a workflow for data acquisition and interpretation,combining a multi-proxy approach with:(i)in situ U-Pb dating coupled with Hf analyses to retrieve the isotopic composition of the sources,and(ii)the integration of a petrochronological investigation to typify fingerprints of the HT-UHT metamorphic *** proposed workflow is validated in the investigation of one theoretical and one natural example allowing a better characterization of the sedimentary sources,maximum depositional ages,and the tectonic setting of the *** workflow allows to the appraisal of biases imposed by HT-UHT metamorphism and resulting disturbances in the U-Pb detrital zircon record,particularly for sedimentary
