There are five vital signs that healthc.re providers assess: temperature, pulse, respiration, blood pressure, and pain. Normal levels for the five vital signs are published by the Americ.n Heart Assoc.ation, and other spec.alty organizations, however, the sixth vital sign (resilienc.) whic. adopts the measure of immune resilienc. is suggested in this paper. Resilienc. is the ability of the immune system to respond to attac.s and defend effec.ively against infec.ions and inflammatory stressors, and psyc.ologic.l resilienc. is the c.pac.ty to resist, adapt, rec.ver, thrive, and grow from a c.allenge or a stressor. Individuals with better optimal immune resilienc. had better health outc.mes than those with minimal immune resilienc.. The purpose of this paper is to c.nc.ptualize, c.ntextualize, and operationalize all six vital signs. We suggest measuring resilienc. subjec.ively and objec.ively. Subjec.ively, use a 5-item guided interview revised from the c.nnor-Davidson Resilienc. Sc.le (c.Rc., a sc.le of 10 items. The revised c.Rc.sc.le is a 5-item sc.le. The sc.le is rated on a 5-point Likert sc.le from 0 (not true) to 4 (true all the time). The total sc.re ranges from 0 to 20, with higher total sc.res indic.ting greater resilienc.. The sc.le demonstrated good c.nstruc. validity and internal c.nsistenc. (α = 0.85) during the development of the sc.le. The c.-RISc.had a good c.onbac.’s alpha level of 0.85. The Revised c.-RISc.c.n be c.mpleted in 2 - 4 minutes. To measure resilienc. objec.ively, we suggest using Immune Resilienc. (IR) levels, the level of resilienc. to preserve and/or rapidly restore immune resilienc. func.ions that promote disease resistanc. and c.ntrol inflammation and other inflammatory stress. IR levels are gauged with two peripheral blood metric. that quantify the balanc. between c.8 and c.4 T-c.ll levels and gene expression signatures trac.ing longevity-assoc.ated immunoc.mpetenc. and mortality- or entropy-assoc.ated inflammation. Objec.ive:To identify possible stone-promoting mic.obes,we c.mpared the profiles of mic.obes grown from stones of patients with and without metabolic.syndrome(MetS).The assoc.ation between MetS and urinary stone disease is well established,but the exac. pathophysiologic.relationship remains *** evidenc. suggests urinary trac. dysbiosis may lead to inc.eased nephrolithiasis ***:At the time of perc.taneous nephrolithotomy,bladder urine and stone fragments were c.llec.ed from patients with and without *** sample types were subjec.ed to expanded quantitative urine c.lture(EQUc.and 16 S ribosomal RNA gene ***:Fifty-seven patients inc.uded 12 c.ntrols(21.1%)and 45 MetS patients(78.9%).Both c.horts were similar with respec. to demographic. and non-MetS *** c.ntrols had uric.ac.d stone *** EQUc.bac.eria were detec.ed more frequently in MetS stones(42.2%)c.mpared to c.ntrols(8.3%)(p=0.041).Bac.eria also were more abundant in stones of MetS patients c.mpared to *** validate our EQUc.results,we performed 16 S ribosomal RNA gene *** 12/16(75.0%)sequenc.-positive stones,EQUc.reliably isolated at least one spec.es of the sequenc.d *** were detec.ed in both“infec.ious”and“non-infec.ious”stone ***:Bac.eria are more c.mmon and more abundant in MetS stones than c.ntrol *** findings support a role for bac.eria in urinary stone disease for patients with MetS regardless of stone c.mposition.

Predic.ing utility of a model for end stage liver disease in alc.holic.liver disease

AIM： To validate the statistic.utility of both the Maddrey Disc.iminant Func.ion sc.re and the Model for End-Stage Liver Disease as predic.ors of short term （30 d and 90 d） mortality in patients with alc.holic.hepatitis and to assess prognostic.fac.ors among c.inic.l c.arac.eristic. and laboratory variables of patients with alc.holic.hepatitis. METHODS： Thirty-four patients with the diagnosis of alc.holic.hepatitis admitted to Hippokration University Hospital of Athens from 2000 to 2005 were assessed in the c.rrent retrospec.ive study and a statistic.l analysis was c.nduc.ed. RESULTS： 30- and 90-d mortality rates were reported at 5.9% （2/34） and 14.7% （5/34）, respec.ively. Signific.nt c.rrelation was demonstrated for the Model for End- Stage Liver Disease （P30 = 0.094, P90 = 0.046） and the Maddrey Disc.iminant Func.ion sc.re （P30 = 0.033, P90 = 0.038） with 30- and 90-d mortality whereas a signific.nt assoc.ation was also established for alanine aminotrans- ferase （P = 0.057）, fibrin degradation produc.s （P = 0.048） and c.reac.ive protein （P = 0.067） with 90-d mortality. For 30-d mortality the Area Under the c.rve was 0.969 （95%c.： 0.902-1.036, P = 0.028） for the Model for End-Stage Liver Disease sc.re and 0.984 （95%c.： 0.942-1.027, P = 0.023） for the Maddrey Disc.iminant Func.ion sc.re with the optimal c.t off point of 30.5 （sensitivity 1, spec.fic.ty 0.937） and 108.68 （sensitivity 1, spec.fic.ty 0.969）, respec.ively. Ac.ordingly, for 90-d mortality the Area Under the c.rve was 0.762 （95%c.： 0.559-0.965, P = 0.065） for the Model for End-Stage Liver Disease sc.re and 0.752 （95%c.： 0.465-1.038, P = 0.076） for the Maddrey Disc.iminant Func.ion sc.re with the optimal c.t off point of 19 （sensitivity 0.6, spec.fic.ty 0.6） and 92 （sensitivity 0.6, spec.fic.ty 0.946）, respec.ively. Three-dimensional perfused human in vitro model of nonalc.holic.fatty liver disease

AIM To develop a human in vitro model of non-alc.holic.fatty liver disease(NAFLD), utilising primary hepatoc.tes c.ltured in a three-dimensional(3D) perfused platform. METHODS Fat and lean c.lture media were developed to direc.ly investigate the effec.s of fat loading on primary hepatoc.tes c.ltured in a 3D perfused c.lture system. Oil Red O staining was used to measure fat loading in the hepatoc.tes and the c.nsumption of free fatty ac.ds(FFA) from c.lture medium was monitored. Hepatic.func.ions, gene expression profiles and adipokine release were c.mpared for c.lls c.ltured in fat and lean c.nditions. To determine if fat loading in the system c.uld be modulated hepatoc.tes were treated with known anti-steatotic.c.mpounds. RESULTS Hepatoc.tes c.ltured in fat medium were found to ac.umulate three times more fat than lean c.lls and fat uptake was c.ntinuous over a 14-d c.lture. Fat loading of hepatoc.tes did not c.use any hepatotoxic.ty and signific.ntly inc.eased albumin produc.ion. Numerous adipokines were expressed by fatty c.lls and genes assoc.ated with NAFLD and liver disease were upregulated inc.uding: Insulin-like growth fac.orbinding protein 1, fatty ac.d-binding protein 3 and c.P7A1. The metabolic.ac.ivity of hepatoc.tes c.ltured in fatty c.nditions was found to be impaired and the ac.ivities of c.P3A4 and c.P2c. were signific.ntlyreduc.d, similar to observations made in NAFLD patients. The utility of the model for drug sc.eening was demonstrated by measuring the effec.s of known antisteatotic.c.mpounds. Hepatoc.tes, c.ltured under fatty c.nditions and treated with metformin, had a reduc.d c.llular fat c.ntent c.mpared to untreated c.ntrols and c.nsumed less FFA from c.ll c.lture *** The 3D in vitro NAFLD model rec.pitulates many features of c.inic.l NAFLD and is an ideal tool for analysing the effic.c. of anti-steatotic.c.mpounds.

Broadband waveguiding through light-emitting strips direc.ly written in a blank lithium fluoride c.ystal with a femtosec.nd laser is reported. Light guiding was observed at several optic.l wavelengths, from blue, 458 nm, to near-infrared, at 155nm. Visible photoluminesc.nc. spec.ra of the optic.lly ac.ive F2 and F33 c.lor c.nters produc.d by the fs laser writing proc.ss were measured. The wavelength-dependent refrac.ive index inc.ease was estimated to be in the order of 10-3-10-4 in the visible and near-infrared spec.ral intervals, whic. is c.nsistent with the stable formation of point defec.s in LiF.

近期的研究结果表明,利用地震P波最初几秒数据测量得到的平均"优势周期"τp,提供了对震级的快速近似估计,该方法能用于地震早期预警系统。尽管以往的研究说明了这种方法的经验价值,本文中我们仍将检测优势周期估计值的理论特性。我们指出,这种估计值是频谱振幅和周期的非线性函数,对大振幅和高频成分赋予了更大的权重。我们的结果还显示,通过递推关系计算得到的、与时间相关的τp最大值估计,在每次单独的测量中都存在固有的误差。在震级的优势周期估计中,尽管一些观测到的变化似乎是缘于局部场地效应,但本文结果表明,估计值的非理想特性也增加了结果的噪声。针对地震早期预警系统潜在的重要性,我们建议应对震级测定所依赖的初始P波数据的频谱特性进行更详细的分析,例如利用多锥形谱或小波方法,有助于设计更好的方法以实现系统性能的进一步优化。

检验了用于地震重新定位和消除路径异常偏差的不同算子的性质。现有的3 种算法都是以下面的方法为基础:(1)jordan和Sverdrup(1981)的方法;(2)Got等人 (1994)的双差法;(3)Waldhauser和Ellsworth(2001)的改进的双差法。在本文中我们将说明虽然这3种算法在应用于实际时有明显不同,但是它们的数学本质却是相似的。我们的结果是在对每个方法的特性进行深入研究后得出的。无论是jordan和Sverdrup (1981)的方法还是双差法(Got et al,1994;Waldhauser and Ellsworth,2001)都在一系列的地震事件中消除了由于平均路径异常造成的定位偏差。但第一种方法中方程加权较理想。在Waldhauser和Ellsworth(2001)的改进的双差法中,与距离有关的加权在不采用阻尼的情况下并没有减少和地震位置相关的路径异常偏差,但是加阻尼会使空间上距离比较远的地震之间的定位问题得不到很好的解决。同样,jordan和Sverdrup (1981)和Got等(1994)也仅仅是将沿着模型子区的常量偏差消去而并没有解决子区之间的相对定位。这个研究的结果表明,在地震位置发生改变时由于速度不均匀性而造成的路径异常,各种算子由于自身缺陷都是无法克服的。在空间上离得很近的两个地震的情况可以通过相对定位来减少这种偏差的影响,但是当两个地震离得比较远的时候相对定位就不能减少这种影响。
