Bone Mineral Density in Patients with newly Diagnosed Inflammatory Bowel Disease: Frequency and Risk Factors in Tunisian Population. Results of a Prospective Study

Background and Aims: Osteopenia and osteoporosis are frequently encountered with Inflammatory Bowel Disease (IBD). Our aims were to determine the prevalence of low bone mineral density (BMD) in patients with recently diagnosed IBD, and to assess predictive factors of reduced BMD. Patients and Methods: Prospective study conducted from January 2008 to December 2012 and involved patients with IBD treated in the Department of Gastroenterology of the Internal Security Forces Hospital. The data collected included: age, gender, body mass index (BMI), diagnostic delay, disease activity, and disease localization. Laboratory findings included serum calcium, phosphate, albumin, hemoglobin, and C-reactive protein. BMD was assessed by dual energy X-ray absorptiometry (DEXA) of the lumber spine and femoral neck. According to WHO criteria, osteopenia was defined as a T-score between -1 and -2 SD, and osteoporosis as a T-score less than -2 SD. Results: A total of 34 patients (17 men, 17 women) were enrolled. Mean age was 37.1 ± 13.8 years (range 16 - 62). Twenty-two patients (65%) had Crohn’s disease (CD) and 12 patients (35%) had ulcerative colitis (UC). Mean BMI was 20.5 ± 4 kg/m2. Low BMD occurred in 50% of patients (12 CD, 5 UC). Thirteen patients (38.2%) exhibited osteopenia and 4 patients (10.8%) showed osteoporosis. Mean vertebral T-score was -0.933 ± 1.41 (range -4.1 to 1.7) and BMD in this site was 1.079 ± 0.17 g/cm2 (range 0.674 to 1.380). Mean femoral T-score was -0.398 ± 1.2 (range -3.1 to 2.4) and BMD in this site was 0.990 ± 0.173 g/cm2 (range 0.633 to 1.600). There was a positive correlation between T-score and albuminemia. Low BMI was found to be predictive factor of reduced BMD at the moment of IBD diagnosis. However, no correlation was found between BMD and the other studied variables (age, gender, smoking, history of fracture, disease location, duration of disease, activity, small bowel resection, serum calcium level, phosphate, C-reactive protein and hemoglobin)

Endophytic flora plays a vital role in the colonization and survival of host plants, especially in harsh environments, such as arid regions. This flora may, however, contain pathogenic species responsible for various troublesome host diseases. The present study is aimed at investigating the diversity of both cultivable and non-cultivable endophytic fungal floras in the internal tissues(roots and leaves) of Tunisian date palm trees(Phoenix dactylifera). Accordingly, 13 isolates from both root and leaf samples, exhibiting distinct colony morphology, were selected from potato dextrose agar(PDA) medium and identified by a sequence match search wherein their 18S–28S internal transcribed spacer(ITS) sequences were compared to those available in public databases. These findings revealed that the cultivable root and leaf isolates fell into two groups, namely nectriaceae and Pleosporaceae. Additionally, total DNA from palm roots and leaves was further extracted and ITS fragments were amplified. Restriction fragment length polymorphism(RFLP) analysis of the ITS from 200 fungal clones(leaves: 100; roots: 100) using HaeIII restriction enzyme revealed 13 distinct patterns that were further sequenced and led to the identification of Alternaria, Cladosporium, Davidiella(Cladosporium teleomorph), Pythium, Curvularia, and uncharacterized fungal endophytes. Both approaches confirmed that while the roots were predominantly colonized by Fusaria(members of the nectriaceae family), the leaves were essentially colonized by Alternaria(members of the Pleosporaceae family). Overall, the findings of the present study constitute, to the authors' knowledge, the first extensive report on the diversity of endophytic fungal flora associated with date palm trees(P. dactylifera).

In Front of Bilateral Pseudo Tumoral Ovarian Mass: Think about Tuberculosis

Ovarian and peritoneal localizations of tuberculosis are rare. They raise the problem of differential diagnosis with malignancy. In this regard, we reported the case of a 25-year-old woman who presented with bilateral pseudo tumoral ovarian mass mimicking an advanced ovarian carcinoma with ascites. Extemporaneous examination of the peritoneum confirmed the diagnosis of tuberculosis and avoided bilateral annexectomy that could compromise the fertility of the patient.

Diagnosis of Fucosidosis through Cutaneous Manifestations

Angiokeratoma corporis diffusum is a dermatological hallmark of several inherited lysosomal storage disorders including Anderson-Fabry disease and other enzyme deficiencies such as fucosidosis. We report a 4-year-old boy with neurodevelopment delay who was diagnosed as having fucosidosis following recognition of dermatological signs, angiokeratoma and telangiectasies. The diagnosis was confirmed by leukocyte oligosaccharide enzyme analysis.

Approaches for Evaluating the Groundwater Suitability for Domestic and Agricultural Uses in Guenniche Plain, Northern Tunisia

In add and semi-add regions groundwater is often the only natural resource for water supply. Suitability of groundwater for domestic and irrigation purposes as well as its hydrochemical characteristics was estimated in the aquifer of Guenniche plain in Northern Tunisia. Samples, collected in March and July 2007, were analysed for a range of chemical parameters （pH, conductivity, TDS, calcium, magnesium, sodium, potassium, sulphate, chloride, carbonate and bicarbonate）. The mean concentration of cations （mg.L-~） indicated that Na＋〉 Ca2＋〉 Mg2＋〉 K＋. Similarly, the mean concentration of anions （mg.L-1） displayed the following decreasing trend： Cl-〉 SO42〉 HCO3〉 CO32-. A comparison of groundwater quality parameters in relation to specified limits for drinking water showed that the concentrations of EC, TDS, Ca2＋, Na＋, Cl-, SO42 and total hardness exceeded the desirable limits in many water samples. Majority of the samples were not suitable for drinking purposes. Furthermore, suitability of groundwater for irrigation was evaluated based on sodium absorption ratio, sodium percent, permeability index, residual sodium carbonate, magnesium hazard and Kelly＇s ratio. All these quality parameters revealed that the groundwater is of good to suitable category for irrigation.

Objective:To assess the antioxidant activity of aerial parts of Allium roseum *** *** Regel.(*** *** *** Regel.)for the first time,as well as to isolate the main bioactive ***:The chloroformic extract of Allium roseum(***)and their fractions obtained by subjection to a chromatographic study were tested for their antioxidant activities by using 2,2-diphenyl-2-picrylhydrazyl and 2,2′-azinobis-(3-ethylbenzothiazoline-6-sulfonic acid)*** activity-guided purification was conducted to isolate five compounds in pure form where their structures were identified by means of nuclear magnetic resonance analyses,including 1D and 2D nuclear magnetic resonance ***:The evaluation of the antioxidant activity of chloroformic extract and their fractions from *** *** *** *** interesting *** active chloroformic extract afforded five isolated compounds where their structures were identified as β-sitosterol(1),chrysoeriol(2),luteolin(3),apigenin(4),andβ-sitosterol 3-O-β-D-glucoside(5).All the compounds were isolated for the first time from the *** *** *** *** three flavonoids(2–4)exhibited the highest antioxidant activity with IC_(50) values of 62.28,21.26 and 513.42μg/mL,respectively(2,2-diphenyl-2-picrylhydrazyl assay)and 218.00,73.50 and 877.66μg/mL,respectively[2,2′-azinobis-(3-ethylbenzothiazoline-6-sulfonic acid)assay].An important value of trolox equivalent antioxidant capacity(2.10 mmol/L)was reported for luteolin(3).Conclusions:These results may suggest that the *** *** *** *** great potential as a source of a natural preservative ingredient in beneficial for natural health products.

Vigilance Disorders in Permanent Night Workers: The Case of the Medical Staff

Purpose of the work: To assess the impact of fixed night shift on the vigilance of paramedical staff. Methods: The present study is an exhaustive cross-sectional survey which has been conducted at the University Hospital of Monastir, Tunisia, and it is about 92 care agents working permanently the night. The study of vigilance is based on Epworth scale and Super Lab program. Results: The average age was equal to 42.53 ± 9.45 years with a sex ratio of 1.72. Nurses accounted for 72%. The average score of alertness assessed with Epworth scale was equal to 14.5 ± 6. The use of the Super Lab software has objectified a tendency towards the increase in the average time of reaction of accomplishment of the simple task and the positive cueing task, which was evaluated while starting the shift, during the half time of the work and at the end of the shift. In addition, the average rate of errors evaluated at the beginning, half-time and at the end of the work has increased during 3 tests (simple task, positive and negative cueing tasks) without this difference being statistically significant. Conclusion: The alteration of vigilance with an ascent of the error rate among fixed night shift workers is a reality, which puts in question, not only the health of paramedical staff but also the care safety provided by these teams.

Solid-Pseudopapillary tumor of the pancreas: Clinical and radiological features. Case report and review of the literature

Solid-Pseudopapillary Tumor (SPT) of the pancreas is considered to be a rare pancreatic tumor that occurs in young females. Most SPTs are considered to be benign. However, the natural course of history has not yet been clarified. We reported a case of 25-year-old women with a patent history of splenectomy for hemolytic anemia, who presented intermittent abdominal pain and vomiting of 6 months ago. Physical examination revealed a hard mass of 8 × 5 cm in the left upper quadrant of the abdomen. Routine laboratory tests were within the normal range. CT-Scan showed an 8 × 6 cm lesion with irregular low density in the body and the tail of the pancreas. MRI showed a mass of 8 cm, surrounded by a capsule, with the irregular intensity both in T1 and T2 enhanced-images. Distal pancreatictomy was done as a definitive treatment via a Bi sub costal incision. Histopathological examination confirmed the diagnosis of SPT. There was no vascular invasion or other malignant features. The resection margins are free of lesions. The post operative course was uneventful. During post operative follow-up of 24 months, there is no sign of recurrence on CT-scan. SPT is a rare condition with a low potential for malignancy and favorable prognosis. Surgical resection is generally curative. Characteristic CT and MRI scans combined with age and sex profile should be sufficient for the diagnosis and the decision to operate.

Immunophenotyping of hematopoietic progenitor cells: Comparison between cord blood and adult mobilized blood grafts

AIM:To study the immunophenotype of hematopoietic progenitor cells from cord blood (CB) grafts (n = 39) in comparison with adult apheresis grafts (AG, n = 229) and pre-apheresis peripheral blood (PAPB) samples (n = 908) using flow cytometry ***: First, we performed a qualitative analysis of CD34+ cell sub-populations in both CB and PAPB grafts using the standardized ISHAGE protocol and a wide panel of 20 monoclonal antibodies. next, we stud-ied some parameters, such as the age of mothers and the weight of newborns, which can influence the qual-ity and the quantity of CD34+ cells from CB. RESULTS: We found that the percentage of apoptotic cells was high in CB in comparison to PAPB (PAPB: 4.6% ± 2.6% vs CB: 53.4% ± 5.2%, P newborn and the age of the mother have the influence on CD34+ cells. The follow-up of Ag CD133in the ISHAGE double platform protocol in association with CD45, CD34 and the 7’AAD shows an equal rate between the two cell populations CD133+CD45+CD34+ high and CD34+CD45+ high with a higher percentage. So, is the inclusion of Ac CD133 necessary in the pres-ent panel included in the ISHAGE methodflLast part, we showed a signif icant presence of interferon γ in CB in comparison to PAPB, the annexin showing the high number of apoptotic cells in CB. CONCLUSION: This study demonstrates that many different obstetric factors must be taken into account when processing and cryo-banking umbilical CB units for transplantation.
