To investigated the usefulness of a novel slim type ball-tipped FlushKnife (FlushKnife-BTS) over ball-tipped FlushKnife (FlushKnife-BT) in functional experiments and clinical *** order to evaluate the functionality of FlushKnife-BTS, water aspiration speed, resistance to knife insertion through the scope, and waterjet flushing speed were compared between FlushKnife-BTS and BT. In clinical practice, esophageal endoscopic submucosal dissection (ESD) performed using FlushKnife-BTS or BT by an experienced endoscopist between October 2015 and January 2016 were retrospectively reviewed. The treatment speed and frequency of removing and reinserting the knife to aspirate fluid and air during ESD sessions were *** experiments revealed that water aspiration speed by the endoscope equipped with a 2.8-mm working channel with FlushKnife-BTS was 7.7-fold faster than that with conventional FlushKnife-BT. Resistance to knife insertion inside the scope with a 2.8-mm working channel was reduced by 40% with FlushKnife-BTS. The waterjet flushing speed was faster with the use of FlushKnife-BT. In clinical practice, a comparison of 6 and 7 ESD using FlushKnife-BT and BTS, respectively, revealed that the median treatment speed was 25.5 mm2/min (range 19.6-30.3) in the BT group and 44.2 mm2/min (range 15.5-55.4) in the BTS group (P = 0.0633). However, the median treatment speed was significantly faster with FlushKnife-BTS when the resection size was larger than 1000 m2 (n = 4, median 24.2 mm2/min, range 19.6-27.7 vs n = 4, median 47.4 mm2/min, range 44.2-55.4, P = 0.0209). The frequency of knife replacement was less in the BTS group (median 1.76 times in one hour, range 0-5.45) than in the BT group (7.02 times in one hour, range 4.23-15) (P = 0.0065).CONCLUSIONOur results indicate that FlushKnife-BTS enhances the performance of ESD, particularly for large lesions, by improving air and fluid aspirat

In a retrospective study of 1224 transthoracic echocardiograms performed between January 2011 and December 2013, we evaluated the spectrum of congenital heart disease (CHD) diagnosed at a tertiary referral centre in maiduguri, north-eastern Nigeria. Diagnosis of CHD was made in 88 (8.3%) subjects, comprising 23 (26.1%) adults and 65 (73.9%) aged less than 18 years. Forty six (52.3%) of those with CHD were females, while 42 (47.7%) were males. The frequencies of the CHD in decreasing order were: ventricular septal defect 23 (26.1%), tetralogy of Fallot (TOF) 14 (15.9%) and atrial septal defect (ASD) and atrioventricular septal defect (AVSD) were 11 (12.5%) each. One of the patients with AVSD had Ellis Van Creveldt syndrome. Six (6.8%) cases of patent ductus arteriosus (PDA) were diagnosed in those younger than 18 years, while all the 5 (5.7%) cases of Ebstein’s anomaly were diagnosed in adults. There were 6 (6.8%) cases of Eisenmenger syndrome involving three cases of AVSD, one case of ASD and two cases of Ebstein’s anomaly. Timely definitive cares for these patients are still lacking in Nigeria and many areas of sub-Saharan Africa. We recommend sensitization of all relevant clinicians to actively look for congenital heart defects. Pulse oximetry and postnatal echocardiographic new-born screening which were previously validated should be implemented at secondary and tertiary levels, and efforts should be made towards providing the needed care for patients with CHD.

The dissolution kinetics of a Nigerian chalcopyrite ore in hydrochloric acid was studied in this article. Acid concentration, reaction temperature, and ore particle size were chosen as experimental parameters. The chemical and morphological studies of the ore before and after leaching at optimal conditions were carried out by X-ray diffraction （XRD） and scanning electron microscopy （SEm）. It is revealed that increasing the acid concentration and system temperature and decreasing the ore particle size greatly enhances the dissolution rate. The dissolution kinetics was found to follow the shrinking core model for the diffusion control mechanism where the activation energy （Ea） of 32.92 ***1-1 was obtained for the process and supported by morphological changes at a higher dissolution of 91.33%.

背景麻醉是一个发展很快的医学专业。癌症患者切除肿瘤时进行麻醉,但是我们无法追溯这些药对于对癌症患者发病率以及引起癌症转移和诱变是否有影响。神经肌肉接头的阻断对许多外科手术而言非常重要。在手术和ICU,如果有神经肌肉阻断剂,甚至不再需要镇静和镇痛,患者便能停止自主呼吸,加强控制通气。研究其对癌症的影响一直很少。神经肌肉阻断剂有两种主要类型,去极化和非去极化神经肌肉阻断剂。目前,临床上常用的是非去极化肌松药,而顺式阿曲库铵和维库溴铵作为中效肌松药常为首选。维库溴铵和阿曲库铵与乙酰胆碱竞争突触后神经肌肉接点。他们将竞争性结合到胆碱能受体的运动终板电位上来对抗乙酰胆碱的作用,从而阻滞神经肌肉传递。一项全面的文献研究显示,维库溴铵在致癌遗传毒性方面的特征性研究不如其在临床毒性,不良反应以及在乳酸盐脱氢酶和肌酸磷酸激酶细胞表达等方面的研究那样广泛。顺式阿曲库铵是一种神经肌肉阻滞剂,在生理范围的p H和温度下,可产生自发性退化反应,即霍夫曼消除。这种代谢过程导致两种主要化学物的合成N-甲基四氢罂粟碱和丙烯酸盐。因为这种独特的代谢过程,它经常被推荐用于伴有肾脏及肝脏疾病患者,以及需要肌松的呼吸窘迫患者。早期认为高浓度阿曲库铵对大鼠肝细胞有毒作用。这项发现因顺阿曲库铵和阿曲库铵抑制人肝癌细胞(Hep G2)和脐静脉上皮细胞(HUVEC)的增殖而被支持。相似的,顺阿曲库铵抑制新生及成年大鼠外周神经细胞的存活及轴突生长也被证明。以上结果表明,顺阿曲库铵以及维库溴铵在人类多种组织和器官中发挥肿瘤抑制作用。这项研究的目的是调查曲库和阿曲库铵在上述提到的两种肿瘤细胞的体外实验中的影响。方法通过有效的细胞培养技术,肺癌细胞(A549)和肝癌细胞(Hep G2)作为体外实验模型,在健康状态下生长。癌细胞在经顺式阿曲库安或维库溴胺的处理后进行一系列的体外实验包括细胞增殖实验CCK-8,细胞毒性试验,细胞周期,迁移实验,划痕实验,侵袭实验,应用DAPI染色法和TUneL法做凋亡实验。对照组没有加入顺式阿曲库胺或维库溴胺,实验组均加入不同浓度的顺式阿曲库安或维库溴胺(1μm,2μm,3μm,5μm,10μm,and 15μm)。只有3μm浓度用于做划痕实验,细胞周期和两种凋亡实验。结果本实验应用了两种人癌细胞,即A549和Hep G2细胞。两种肌松药均显示出细胞抑制作用,但顺式阿曲库胺的效果最明显。两种肌松药在癌细胞中的抑制作用比较,在Hep G2细胞中的作用强于A549细胞。两种细胞经顺式阿曲库胺或维库溴胺作用后,细胞增值率均显著下降。两种细胞均在大于等于3μm时观察到明线效果。在此研究中,细胞周期的结果显示:两种细胞经处理后,G0/G1期细胞增多,S期细胞数量减少。然而,在经处理后的两种细胞系中,仅发现少量的G2/m期的降低。我们还发现,与非整数倍的未处理细胞相比较,顺式阿曲库胺处理后的细胞为0,而经维库溴铵处理后的细胞为7。关于两种肌松药对两种人癌细胞的影响,这进一步说明本实验中应用的非去极化肌松药可以抑制A549和Hep G2细胞的生长。另外,本实验发现顺式阿曲库胺或维库溴胺与顺铂可产生协同作用来抑制肺癌细胞生长。并且细胞减少和抑制作用有显著意义。另外,经处理后两种细胞系的迁移与侵袭作用均有显著性下降。并且经顺式阿曲库胺处理后的效果较维库溴胺处理后更加明显。经顺式阿曲库胺处理后,细胞多处在凋亡2期或3期,然而对照组细胞只有少数细胞在凋亡或正处于凋亡状态。48小时后,实验组中平均18%(2.3)的A549细胞和26.5%(0.71)的Hep G2细胞在凋亡的不同阶段,且大多数在凋亡的核裂解期。对照组中,只有平均0.95%(0.07)的A549细胞和1.25%(0.07)的Hep G2细胞处于凋亡状态,大多数细胞在生长正常期,仅有少数处于凋亡1期。两种细胞系均有显著的统计学差异。类似于DAPI的观察分析,与对照组相比,经过末端标记的细胞在细胞凋亡实验中能观察到显著的增长(细胞凋亡实验阳性)。48小时的顺阿处理,对照组中,未标记的细胞中有平均1%(0.07)的A549和1.2%(0.28)Hep G2在显微镜下可观察到阳性细胞(P<<0.05)。另一方面,试验组19%(1.41)的A549和24.7%(0.99)Hep G2均可在显微镜下观察到阳性细胞(P<<0.05)。结论本实验发现顺式阿曲库胺或维库溴胺可抑制肺癌细胞(A549)和肝脏癌症(Hep G2)生长。然而,顺式阿曲库胺在大于等于临床浓度时会引起的抑制作用,维库溴胺仅在高于插管浓度时产生抑制作用。此外,本研究也证明了顺式阿曲库胺和维库溴胺潜在的协同顺铂活性的作用。这两种非去极化神经组织药物显著的抑制肺癌与肝癌细胞的转移与侵袭作用。本实验中的所有结论证明顺式阿曲库胺或维库溴胺可被选择作为肺癌患者癌切除术

Perineal rectosigmoidectomy for strangled rectal prolapse:A case report

Strangled rectal prolapse, the rare disease, is a proctological emergency. Its management is controversial. When it is not reducible and signs of ischemia are present, the Altemeierperineal rectosigmoidectomy remains the best treatment. This study aimed to report our experience on the management of strangled rectal prolapse about 1 case in a 45-year-old man, a holder of a rectal prolapse for 2 years. On admission, he had strangled prolapse for 24 hours with edema. After a vain attempt of manual reduction and installation of necrosis after 48 hours, he had an Altemeier rectosigmo?dectomy. The postoperative course was uneventful and the patient was discharged on the 6th postoperative day. The results were very good, after one year follow-up.

Preliminary Evaluation of Soils from Kwalkwalawa （Sokoto） and Bakolori （Talata mafara） Regions Used for Rice and Fishing Activities

Kwalkwalawa is an area noted for extensive rice production and fishing activities, while Talata mafara is a site used mainly for rice production. Soils of the two areas were taken and evaluated for potentialities of rice-cum-fish culture. Three replicate soil samples were collected in October 2004 and 2005 from three sampling sites within the study area at 0-15 cm and 15-30 cm depth. Analysis of the samples indicated that at Kwalkwalawa the soil pH was 7.35 and 7.28 for 0-15 cm and 15-30 cm depth, respectively, and 6.75 and 6.67 for Talata mafara, respectively, during 2004. During 2005, the pH at Kwalkwalawa was 6.75 and 7.18, while at Talata mafara ranged between 5.63 and 5.14. Organic carbon content at Kwalkwalawa estimated during 2004 was 1.12 g/kg and 0.81 g/kg for 0-15 cm and 15-30 cm, whereas at Talata mafara amounted 0.93 g/kg and 0.11 g/kg. During 2005, noted values of organic carbon at Kwalkwalawa lowered to 0.45-0.28 g/kg and at Talata mafara lowered to 0.38-0.06 g/kg. Total nitrogen （N） at Kwalkwalawa during 2004 was 0.04 g/kg and 0.03 g/kg for 0-15 cm and 15-30 cm, at Talata mafara it was both 0.04 g/kg for both depths. During 2005, at Kwalkwalawa, total N was 0.04 g/kg and 0.03 g/kg, and at Talata mafara it was 0.05 g/kg for both depths. Available phosphorus （P） at Kwalkwalawa during 2004 was 1.93 mg/kg and 2.03 mg/kg for 0-15 cm and 15-30 cm, at Talata mafara it was 1.59 mg/kg and 1.63 mg/kg. During 2005, available P was 5.82 mg/kg and 6.13 mg/kg for Kwalkwalawa and 1.67 mg/kg and 1.91 mg/kg for Talata mafara, at 0-15 cm and 15-30 cm, respectively. Analysis of clay content indicated that at Kwalkwalawa the clay content was 11 g/kg for both depths during 2004 and 16.5 g/kg during 2005, and the soil texture was classified as sandy loam during both seasons. At Talata mafara during 2004, the clay content was 73 g/kg and 93 g/kg for 0-15 cm and 15-30 cm. During 2005, it was 84 g/kg and 103 g/Kg. The effect of the ethanolic extract Khaya senegalensis on some biochemical parameters was tested in rabbits with normal glucose and glucose overload. Phytochemical screening after three batches of 6 rabbits race (TBD) were treated with the extract in oral doses of respectively 10, 25 and 50 mg/kg body weight for 30 days;three other groups receiving respectively these doses of the extract were placed under a priori temporary overload of glucose;a control group received instead distilled water treatment. Blood glucose, triglycerides and total cholesterol were measured by spectrophotometry and weight gain of the animals was performed at the beginning of the experiment and every 2 weeks during the study period. The phytochemical screening revealed the presence of polyphenolic compounds (majority) of saponins of anthracene derivatives and steroids in the extract. The test results indicate that under the dose of 50 mg/kg body weight (BW) there was a significant decrease (p < 0.05) in blood glucose after 24 hours. The extract had no effect on the lipid profile or body weight of rabbits.
