Red Blood Cell Transfusion in a Neonatal Tertiary Care Center: A Moroccan Study

Background: Red Blood Cell (RBC) transfusion is a common therapy in neonatal practice and data from different international neonatal centers are available, however, there is paucity of Moroccan data regarding blood components usage. We aim to study trends and outcomes of RBC transfusions in our department and emphasize the need for transfusion guidelines. Material and method: This retrospective study was performed in a Moroccan tertiary neonatal intensive care center. The records of all neonates admitted from January 2015 to March 2016 were screened for RBC usage and indications. The data were statistically analyzed by using Microsoft ExcelTM software. Results: A total of 60 neonates received 77 RBC transfusions. Postnatal age at transfusion ranged from 1 to 30 days with an average of 13 days, and 30% of our patients were aged ≥21 days. RBC Transfusion was given once in 48 patients (80%), twice in 8 patients (13.3%) and ≥ thrice in 4 patients (6.7%). More than one type of blood components was needed in 23 (38%) neonates. 70% of patients were critically ill, 60% were born before 37 weeks’ gestation and 57% were small for gestational age. Mean hemoglobin at the time of transfusion of RBC was 8.59 g/dl (2 - 11.6 g/dl). Sepsis and anemia with mechanical ventilation for severe respiratory illness were the commonest indications. All gestational ages were considered, and mortality was noted in 66% of our cases and worsening in 13%;21% of the infants showed improvement. Conclusion: Judicious implementation of guidelines for RBC transfusions would decrease their inappropriate use.

Background:Community-acquired pneumonia(CAP)is one of the five leading causes of death among children in developing countries,accounting for approximately three million deaths per *** of the modifiable risk factors of CAP may help to reduce the burden of this *** this study,the impact of the socioeconomic status(SES)on the severity and outcome of CAP among Egyptian children was ***:This was a prospective longitudinal cohort study which included 1,470 children diagnosed with CAP,aged two to 15 years(median age 5.4 years).The diagnosis of CAP was based on clinical and radiological findings.A structured questionnaire and the patients’medical records were used for the data *** subjects were divided into two groups:mild and severe *** and demographic variables were compared,and a multivariate logistic regression analysis was ***:The multivariate analysis showed that a low maternal education level(OR:3.8;95%CI:2.12-6.70;P=.0001),unavailability of adequate medical care(OR:3.1;95%CI:1.99-4.88;P=.0001),a low family income(OR:2.2;95%CI:0.99-4.78;P=.047),and parents’smoking habits(OR:2.0;95%CI:1.15-3.55;P=.014)were significant independent predictive risk factors for severe CAP among Egyptian ***:Public health measures against these socio-demographic risk factors should be identified as priorities in order to help reduce the disease burden of deaths from severe CAP among Egyptian children.

Background:Diabetic foot ulcers(DFUs)are common in patients with *** mortality rate of DFUs is ranked the highest after *** advancements in modern medicine,leech therapy,a traditional treatment method for chronic wounds in Iranian medicine,has proven effective in relieving venous ***,we aimed to observe the curative effects of leech therapy in combination with honey and curcumin dressing in a 77-year-old patient with a diabetic foot ulcer(UFC).Methods:Two medium-sized medicinal leeches were placed surrounding a grade 2 wound,based on Wagner’s classification system,located on the right first *** patient reported a visual analogue scale(VAS)score of *** leech therapy,the wound was covered with honey and curcumin,followed by oral administration of ciprofloxacin for 10 ***:Pain increased immediately after leech therapy(VAS:9-10)but decreased significantly(VAS:6)2 days after the *** the end of the 2nd day,pain completely *** 3 weeks,there was no wound on the *** 12 weeks,there were no traces of the ***:Leech therapy in combination with honey and curcumin dressing is effective against disease progression in patients with DFUs.

Stress is associated with many professions including dental surgery. It has been the subject of several studies. However, its evaluation in orthodontics has not been sufficiently studied. Some studies that have described the stressful aspects of dentistry have reported the involvement of orthodontists, but no studies have been conducted in Morocco. The purpose of this study is to evaluate professional stress in orthodontics in Morocco. We conducted a descriptive and sectional study through a survey distributed to one hundred private orthodontist practitioners in Casablanca, Morocco. Stress signs were observed in 44% of the orthodontists. The most prominent factors of stress in orthodontics are related to patients and time. Experience of practitioners has been identified as the only factor significantly influencing their perception of stress. Thus, clinical competence and proper management of practice are the best ways to deal with daily stress in orthodontic practice.

Early treatment of leishmaniasis is critical to achieve cure, prevent psychological and social distress, and prevent transmission of disease. Untreated Leishmaniasis-cutaneous leishmaniasis, mucocutaneous leishmaniasis and visceral leishmaniasis - results in disfguring scars and high rates of morbidity and mortality in highly endemic regions of the world. However, cure rates with available therapeutics are limited due to cost, therapeutic toxicity and the growing rate of resistance. New therapeutic targets for medications and vaccine development are under investigation to provide improved healing and effcacy for the treatment of Leishmania spp.

The resources available to an individual in any given environment are finite, and variation in life history traits reflect differential allocation of these resources to competing life functions. Nutritional quality of food is of particular importance in these life history decisions. In this study, we tested trade-offs among growth, immunity and survival in 3 groups of greater wax moth （Galleria mellonella） larvae fed on diets of high and average nutritional quality. We found rapid growth and weak immunity （as measured by encapsulation response） in the larvae of the high-energy food group. It took longer to develop on food of average nutritional quality. However, encapsulation response was stronger in this group. The larvae grew longer in the low-energy food group, and had the strongest encapsulation response. We observed the highest survival rates in larvae of the low-energy food group, while the highest mortality rates were observed in the high-energy food group. A significant negative correlation between body mass and the strength of encapsulation response was found only in the high-energy food group revealing significant competition between growth and immunity only at the highest rates of growth. The results of this study help to establish relationships between types of food, its nutritional value and life history traits of G. mellonella larvae.
