The presence of an emerging brominated flame retardant, decabromodiphenylethane （DBDPE）, has been confirmed in Spanish sewage sludge. Thirty one samples from different urban wastewater treatment plants （WWTPs） were analyzed for this brominated flame retardant. DBDPE was positively identified and quantified in all samples at lower concentrations （47.0 ± 29.7 ng/g dry weight （dw）; mean ± sd） than those obtained for decabromodiphenyl ether （BDE-209）（290 ± 236 ng/g dw; mean ± sd） in a previous study. Influence of the WWTP characteristics in the pollutant levels was evaluated. No significant correlations were obtained between DBDPE concentrations and the population or sewage sludge production rate associated with the plants, neither wastewater treatment method. Sources of DBDPE in the sludge were also evaluated. Data indicate a common origin for DBDPE and BDE-209, which may be related to leaching processes during the use and disposal of consumer products containing these chemicals. Nevertheless, DBDPE contents are not influenced by industrial activities, which suggests that the infusion of DBDPE commercial mixture is not a source of this chemical into the environment, and indicates that the use of DBDPE in the Spanish industry is still low compared to deca-BDE.

AIM:To study the effect of moxibustion on Zusanli or Liangmeng point on gastric mucosa injury in stress-induced ulcer rats and its correlation with the expression of heat shock protein 70 (HSP70). METHODS:Sixty healthy sd rats (30 males,30 females) were divided into control group,injury model group,Zushanli point group,Liangmeng point group. Stress gastric ulcer model was induced by binding cold stress method. Gastric mucosa ulcer injury (UI) index was calculated by Guth method. Gastric mucosa blood flow (GMBF) was recorded with a biological signal analyzer. Protein content and gene expression in gastric mucosal HSP70 were detected by immunohistochemistry (IHC) and reverse transcription polymerase chain reaction (RT-PCR). Thiobarbital method was used to determine malondialdehyde (MDA) content. Gastric mucosal endothelin (ET) and prostaglandin E2 (PGE2) were analyzed by radioimmunoassay. RESULTS:High gastric mucosal UI index,high HSP70 expression,low GMBF and PGF2,elevated MDA and ET were observed in gastric mucosa of rats subjected to cold stress. Moxibustion on Zusanli or Liangmeng point decreased rat gastric mucosal UI index,MDA and ET. Conversely,the expression of HSP70,GMBF,and PGE2 was elevated in gastric mucosa after pretreatment with moxibustion on Zusanli or Liangmeng point. The observed parameters were significantly different between Zusanli and Liangmeng points. CONCLUSION:Pretreatment with moxibustion on Zusanli or Liangmeng point protects gastric mucosa against stress injury. This protection is associated with the higher expression of HSP70 mRNA and protein,leading to release of PGE2 and inhibition of MDA and ET,impairment of gastric mucosal index.

目的探讨蒙药壮西-4调控miR-204对骨质疏松大鼠成骨细胞增殖的影响及机制。方法将60只sd大鼠随机分为空白对照组、模型对照组、阳性对照组、低、中和高剂量组各10只。建模各组给予维A酸溶液灌胃建立骨质疏松模型,连续14 d。阳性对照组给予戊酸雌二醇灌胃,低、中、高剂量组分别给予不同剂量(50、100、200 mg/kg)蒙药壮西-4灌胃,空白对照组和模型对照组给予同体积的生理盐水灌胃,连续42 d。采用骨密度(BMD)仪测定大鼠第4腰椎及右股骨的BMD值,逆转录-聚合酶链反应(RT-PCR)血清miR-204表达量,酶联免疫吸附试验(ELISA)检测血清骨桥蛋白(OPG)、核因子κB受体活化因子配体(RANKL)含量。取MC3T3-E1细胞,分为对照组、促进组、抑制组和药物组,分别加入生理盐水、miR-204模拟物、miR-204抑制剂和蒙药壮西-4。采用RT-PCR法检测miR-204表达量,Western印迹检测RANKL、OPG蛋白表达量,噻唑蓝(MTT)检测细胞增殖情况,计算细胞存活率。结果各组第4腰椎和右股骨BMD值、血清miR-204表达量、血清OPG含量差异有统计学意义(均P<0.05);空白对照组明显高于建模各组,模型对照组明显低于低、中、高剂量组、阳性对照组,阳性对照组和中剂量组明显高于低、高剂量组(均P<0.05)。各组血清RANKL含量差异有统计学意义(均P<0.05),空白对照组明显低于建模各组(均P<0.05),模型对照组明显高于低、中、高剂量组和阳性对照组(均P<0.05),阳性对照组和中剂量组明显低于低、高剂量组(均P<0.05)。各组miR-204表达量、OPG蛋白表达量、细胞存活率差异有统计学意义(均P<0.05),促进组和药物组明显高于对照组和抑制组,抑制组明显低于对照组(均P<0.05)。各组RANKL蛋白表达量差异具有统计学意义(均P<0.05),促进组和药物组明显低于对照组和抑制组,抑制组明显高于对照组(均P<0.05)。结论蒙药壮西-4能增加骨质疏松大鼠BMD,其可能机制为通过上调miR-204表达促进大鼠成骨细胞增殖。

Testicular cancer （TC） is the most common solid cancer in men between the third and fourth decade of life. Due to successful treatment approaches, TC survivors （TCSs） have long life expectancy, but with numerous potential long-term sequelae, including sexual dysfunction. We investigated predictors of long-term normal sexual function （SF） recovery in TCSs. Sociodemographic, medical, and psychometric data were analyzed in 143 Caucasian-European TCSs, who underwent orchiectomy at a single institution. Health-significant comorbidities were scored with the Charlson Comorbidity Index （CCI）. Patients completed the International Index of Erectile Function （IIEF）. Statistical models tested the association between predictors （including age at surgery, body mass index, CCI, and adjuvant therapy： radiotherapy [RT], chemotherapy [CT], CT followed by retroperitoneal lymph node dissection [RPLND] and RPLND alone） and the long-term recovery of normal SF （defined as IIEF-erectile function [EF] ≥26, and sexual desire [sd], intercourse satisfaction [IS] orgasmic function [OF], and overall satisfaction [OS] domain scores in the upper tertiles）. At a mean follow-up of 86 months, 35 （25.5%） TCSs had erectile dysfunction （ED）, with 16 （11.2%） experiencing severe ED. Median time of EF recovery was 60, 60, and 70 months after CT, RT, and RPLND, respectively. Only adjuvant RT emerged as an independent predictor of nonrecovery of normal EF （HR： 0.55, P = 0.01）. Neither adjuvant CT nor CT plus RPLND or RPLND alone significantly impaired the recovery of normal erections. Adjuvant therapy was not associated with impaired recovery of normal sexuality as a whole, considering the IIEF-sd, -OF, -IS, and OS domains.

BACKGROUND In paediatric patients with complicated nephrotic syndrome(NS), rituximab(RTX) administration can induce persistent IgG hypogammaglobulinemia among subjects showing low basal immunoglobulin G(IgG) *** To evaluate the effect of RTX on IgG levels and infections in patients with complicated NS and normal basal IgG *** We consecutively enrolled all patients with complicated NS and normal basal IgG levels undergoing the first RTX infusion from January 2008 to January 2016. Basal IgG levels were dosed after 6 wk of absent proteinuria and with a maximal interval of 3 mo before RTX infusion. The primary outcome was the onset of IgG hypogammaglobulinemia during the follow-up according to the IgG normal values for age [mean ± standard deviation(sd)].RESULTS We enrolled 20 patients with mean age at NS diagnosis of 4.2 ± 3.3 years. The mean age at the first RTX infusion was 10.9 ± 3.5 years. Eleven out of twenty patients(55%) developed IgG hypogammaglobulinemia. None of these patients showed severe or recurrent infections. Only one patient suffered from recurrent acute otitis media and underwent substitutive IgG infusion. Three patients undergoing only the two "starting doses" experienced normalization of IgG levels. Using Kaplan-Meier analysis, the cumulative proportion of patients free of IgG hypogammaglobulinemia was 57.8% after the first RTX dose, 51.5% after the third dose, 44.1% after the fourth dose, and 35.5% after the fifth *** RTX can induce IgG hypogammaglobulinemia in patients with pre-RTX IgG normal values. None of the treated patients showed severe infections.

通过预先用锌诱导后大鼠做力竭性运动，来观察锌诱导金属硫蛋白（MT）合成对心肌的保护作用及其可能机制。方法：选用健康雄性sd大鼠30只，体重220～250g，随机A、B、C分为3组，即：A组为对照组；B组为一次性力竭运动组；C组为锌＋一次性力竭运动组，每组10只。满8周后，测定其心肌组织中的MT含量、组织中钙的含量、巯基（-SH）和丙二醛（MDA）含量、以及心肌中的谷胱甘肽过氧化物酶（GSH-px）和Na^＋-K^＋-ATP酶的活性。结果：1）与对照组相比，力竭运动组大鼠心肌组织中MT的含量较对照组显著降低（P〈0．05）；心肌组织中MDA含量和总钙量显著增高（P〈0．05）；-SH含量以及GSH-px活性和Na^＋-K^＋-ATP酶的活性显著降低（P〈0．05）。2）与力竭运动组相比；锌＋力竭运动组大鼠心肌组织中MT含量较力竭运动组显著增高（P〈0．05）；心肌组织中MDA含量和总Ca^2＋量显著降低（P〈0．05）；-SH含量及GSH-px活性和Na^＋-K^＋-ATP酶的活性显著增高（P〈0．05）。结论：预先用锌诱导可减少力竭运动后心肌组织中MT含量的显著下降，降低心肌组织脂质过氧化水平，提高抗氧化酶的活性，防止钙超载，对力竭运动后心肌细胞的损伤具有保护作用。MT和金属锌密切相关，二者的相互作用可能共同参与保护力竭运动后机体组织的损伤，并促进损伤组织的快速修复。

Risk Factors for Fall in Elderly Patients:Follow-Up Study after Hospital Discharge

Introduction: Increased occurrences of falls after discharge are reported, especially in elderly people. Falls are a major cause of disability and identification of risk factors associated with falls is required to plan preventive actions. This study aims to determine the occurrence of falls in the three months after discharge and risk factors in elderly patients. Materials and Methods: 100 patients over 65 admitted to an Internal Medicine Ward participated. Questionnaires were given during hospitalization and three months after discharge. Follow-up information was unavailable for 31 patients (25 deceased, 6 unreachable). Results: Of those analyzed 52% were males with 80 ± 8.1 years (mean ± sd). Polymedication (p = 0.002), use of psychoactive drugs (p = 0.007), analgesics (p = 0.034) and walking devices (p = 0.006) were associated with a higher incidence of falls 6 months before hospitalization. Post-discharge follow-up was obtained for 69 patients: 18 reported falling during the follow-up. There was a higher risk of fall in patients with a history of falls in the 6 months before admission (p = 0.015 RR = 2.76). Patients who had one or more falls after discharge had a significantly shorter length of hospital stay compared to those who didn’t fall (p = 0.012). In multivariate logistic regression, we found that patients who were hospitalized more than 7 days had a lower risk of falling in the post-discharge period (OR = 0.195, p = 0.017) independently of the history of falls 6 months prior to admission. Conclusions: Further studies are required to validate the risk factors identified after discharge and to evaluate preventive measures. Elderly patients discharged from an Internal Medicine Ward should be screened to determine the risk of falls, specifically previous history of falls and medication. These patients should integrate a fall prevention program.
