目的用人胚肾(HEK)细胞研究纯心脏钠通道电流(Nav1.5)频率、电压依赖性及快、慢钠通道失活以及利多卡因对其影响。方法用膜片钳技术,观察两次刺激脉冲间隔钠电流对频率,电压依赖性阻滞程度。通过两种不同刺激程序产生快、慢钠电流并记录其失活过程。并观察0.1 mmol/l利多卡因对钠电流的频率依赖性阻滞。结果电压钳制在-140 mV时钠电流抑制随频率刺激脉冲间隔时间缩短而阻滞加重。完全恢复正常平均值为33.2±5.77ms(n=9),当电压钳制在-90,-100 mV时,钠电流完全恢复正常延迟至1 015 ms。快钠电流失活随钳制电压降低而加重,并呈现负相关(r=-0.97,Pl/l利多卡因在-100 mV电压钳制下,导致20%Na+电流抑制,静息状态下产生轻微阻滞;且抑制随刺激频率增加而增加。结论HEK细胞Nav1.5通道电流频率依赖性阻滞恢复与刺激频率、钳制电压高低有关;快钠电流失活随钳制电压降低而加重并呈负相关;利多卡因对HEK细胞Nav1.5通道有加重频率依赖性阻滞作用且随刺激频率增加而增加。

Purpose: This study tests the hypothesis that steroid administration improves the outcome of biliary atresia (BA) by evaluating the efficacy of postoperative steroid use on surgical outcomes in infants with BA. Methods: Steroid use and outcomes in patients with BA were retrospectively analyzed at a tertiary pediatric hospital. Institutional review board approval was obtained. Results: Kasai portoenterostomy (PE) was performed in 43 patients with BA treated from 1992 to 2004 (16 boys and 27 girls). Twenty-one PE patients received steroids and 22 did not. Portoenterostomy was successful in 24 patients (55.8%) with consistent serum bilirubin less than 2 mg/dl. Sixteen (66%) received postoperative steroids. A normal postoperative bilirubin was achieved at 6 months in 16 (76%) of 21 patients with steroids compared with 8 (37%) of 22 in untreated controls (Fisher’s Exact test, P = .01). Of the 43 patients, 19 (44%) required liver transplantation, including 7 (37%) of 19 with steroids vs 12 (63%) of 19 without (P = .2). Twenty-eight infants developed cholangitis (fever with and without changes in hepatic function): 25 after PE and 3 after transplant. Of the 25, 12 (48%) received steroids. Seven died (16%) (range, 7 months to 4 years): 2 while awaiting transplantation (received steroids) and 5 after transplantation (1 received steroids and 4 were untreated). Survival was 86%(18/21) in patients with steroids and 82%(18/22) in those without. Transplant survival (74%)was comparable to previously reported historical controls (82%). Conclusions: The Kasai PE continues to be the procedure of choice in infants with BA younger than 3 months. A significantly improved clearance of postoperative jaundice and lower serum bilirubin levels were observed in patients receiving steroids. However, steroids had no effect on the incidence of cholangitis, need for liver transplantation, and overall survival. 目的:在兔模型中利用局部麻醉来研究两种麻醉剂的眼心反射发生率及其预防,同时我们还研究了局部麻醉对角膜愈合的影响。方法:健康的成年新西兰白兔(雌雄皆有)48只,分成两组(A组和B组),分别用氯胺酮(A组,24例)、异丙酚(B组,24例)麻醉。在此双盲研究中,通过用或不用局部麻醉(40g/l利多卡因,5g/l盐酸丙美卡因,5g/l布比卡因)对眼睛的不同干预,记录各组眼心反射发生率。应用每种局部麻醉后,通过对眼睛的临床检查,组织病理学,胶原染色以及投射电子显微等方法对术后1d长达7d角膜毒性和愈合进行评估。结果:在氯胺酮的麻醉下未出现眼心反射,而在异丙酚的麻醉下,心率显著下降(P<0.01)。因此,局部麻醉可以成功地防止眼心反射的发生并且不影响角膜愈合。结论:局部麻醉没有任何局部副作用,可以被推荐用来预防眼心反射。

Tree species in coastal forests may exhibit specialization or plasticity in coping with drought through changes in their stomatal morphology or activity, allowing for a balance between gas exchange and water loss in a periodically stressful environment. To examine these responses, we sought to answer two primary research questions: a) how is variation in B. simaruba’s stomatal traits partitioned across hierarchical levels, i.e., site, tree, and leaf;and b) is variation in stomatal traits an integrated response to physiological stress expressed across the habitat gradient of Florida Keys forests? At eight sites distributed throughout the Keys, five leaves were collected from three mature trees for stomatal analysis. leaf carbon stable isotope ratio (δ13C) was determined to infer the changes in water use efficiency caused by physiological stress experienced by each tree. The results showed that substantial proportions of the total variance in three traits (stomatal density, stomatal size, and δ13C) were observed at all levels, suggesting that processes operating at each scale are important in determining trait values. A significant negative correlation between stomatal density and size across scales was observed. Path model analysis showed that environmental variables, distance to ground water and ground water salinity, affect leaf δ13C indirectly, via its effects on stomatal traits, not directly to leaf δ13C. Therefore, the combination of small and densely distributed stomata seems to represent a strategy that allows B. simaruba to conserve water under conditions of physiological drought induced by either higher ground water salinity or flooding stress at very low elevation.

Multispectral observations from unmanned aerial vehicles(UAVs)are currently used for precision agriculture and crop phenotyping applications to monitor a series of traits allowing the characterization of the vegetation ***,the limited autonomy of UAVs makes the completion of flights difficult when sampling large *** the throughput of data acquisition while not degrading the ground sample distance(GSD)is,therefore,a critical issue to be *** propose here a new image acquisition configuration based on the combination of two focal length(f)optics:an optics with f=4:2 mm is added to the standard f=8 mm(SS:single swath)of the multispectral camera(DS:double swath,double of the standard one).Two flights were completed consecutively in 2018 over a maize field using the AIRPHEN multispectral camera at 52 m *** DS flight plan was designed to get 80%overlap with the 4.2 mm optics,while the SS one was designed to get 80%overlap with the 8 mm *** a result,the time required to cover the same area is halved for the DS as compared to the *** georeferencing accuracy was improved for the DS configuration,particularly for the Z dimension due to the larger view angles available with the small focal length *** to plant height estimates demonstrates that the DS configuration provides similar results as the SS ***,for both the DS and SS configurations,degrading the quality level used to generate the 3D point cloud significantly decreases the plant height estimates.

Background: Depressive disorder is often accompanied by physiological changes that may adversely affect the course of medical illness, including an increase in pro-inflammatory cytokines. Methods: We examine the effects of religious cognitive behavioral therapy (RCBT) vs. conventional CBT (CCBT) on pro-/anti-inflammatory indicators and stress hormones in 132 individuals with major depressive disorder (MDD) and chronic medical illness who were recruited into a multi-site randomized clinical trial. Biomarkers (C-reactive protein and pro-inflammatory cytokines TNF-α, Il-1β, IFN-γ, Il-6, Il-12-p70), anti-inflammatory cytokines (Il1ra, Il-4, Il-10), and stress hormones (urinary cortisol, epinephrine, norepinephrine) were assessed at baseline, 12 weeks, and 24 weeks. Differential effects of baseline religiosity on treatment response were also examined, along with effects of religiosity on changes in biomarkers over time independent of treatment group. Biomarker levels were log transformed where possible to normalize distributions. Mixed models were used to examine trajectories of change. Results: CRP increased and Il-4, Il-10, and epinephrine decreased over time, mostly in the opposite direction expected (except epinephrine). No significant difference between RCBT and CCBT was found on average trajectory of change in any biomarkers. Religiosity interacted with treatment group in effects on Il-6, such that CCBT was more effective than RCBT in lowering ll-6 in those with low religiosity whereas RCBT appeared to be more effective than CCBT in those with high religiosity. Higher baseline religiosity also tended to predict an increase in pro-inflammatory cytokines INF-γ and Il-12 (p70) and urinary cortisol over time. Conclusions: RCBT and CCBT had similar effects on stress biomarkers. CCBT was more effective in reducing Il-6 levels in those with low religiosity, whereas RCBT tended to be more effective in those with high religiosity. Unexpectedly, higher baseline religiosity w

Background: Religious practices/experiences (RPE) may produce positive physiological changes in patients with major depressive disorder (MDD) and chronic medical illness. Here, we report cross-sectional relationships between depressive symptoms, RPE and stress biomarkers (pro-/anti-inflammatory measures and stress hormones), hypothesizing positive associations between depressive symptoms and stress biomarkers and inverse associations between RPE and stress biomarkers. Methods: We recruited 132 individuals with both MDD and chronic illness into a randomized clinical trial. First, stress biomarkers in the baseline sample were compared to biomarker levels from a community sample. Second, relationships between depressive symptoms and biomarkers were examined, and, finally, relationships between RPE and biomarkers were analyzed, controlling for demographics, depressive symptoms, and physical functioning. Results: As expected, inflammatory markers and stress hormones were higher in our sample with MDD compared to community participants. In the current sample, however, depressive symptoms were largely unrelated to stress biomarkers, and were unexpectedly inversely related to proinflammatory cytokine levels (TNF-α, Il-1β). likewise, while RPE were largely unrelated to stress biomarkers, they were related to the anti-inflammatory cytokine Il-1RA and the stress hormone norepinephrine in expected directions. Unexpectedly, RPE were also positively related to the proinflammatory cytokine IFN-γ and to IFN-γ/Il-4 and IFN-γ/Il-10 ratios. Conclusions: little evidence was found for a consistent pattern of relationships between depressive symptoms or religiosity and stress biomarkers. Of the few significant relationships, unexpected findings predominated. Future research is needed to determine whether religious interventions can alter stress biomarkers over time in MDD.

目的 :探讨在急诊条件下早期应用无创血流动力学监护系统的必要性和可行性 ,评估其早期揭示循环功能不良和在早期高危急诊外科患者复苏治疗中的指导作用。方法 :用无创血流动力学监护系统监测 1 5 6例高危急诊外科患者 ,对其中 4 8例同时进行了有创血流动力学监护系统 (Swan Ganz法 )的观察 ,并将两种方法所得结果进行比较。结果 :无创血流动力学监护系统所获得的心排血量 (CO)和心脏指数 (CI)值与有创血流动力学监护系统所获得的值非常接近 ,在急诊条件下 ,r=0 .6 9,r2 =0 .87,Pl· min- 1 · m- 2 。在 ICU条件下进一步比较两种方法所得结果 :r=0 .84 ,r2 =0 .93,Pl· m in- 1· m- 2。结论 :在急诊条件下应用无创血流动力学监护系统可显示创伤初期和复苏早期阶段的低血压、低 CI、低经皮氧张力 (Ptc O2 )、低血氧饱和度 (Sa O2 )和高经皮二氧化碳张力 (Ptc CO2 )的变化过程 ,并且为早期复苏治疗提供直观、可靠、连续性的依据 ,对预防由于长时间组织低血流量和低灌注所致的休克。
