Background: Type 2 diabetes is a metabolic disease characterized by chronic hyperglycemia. The latter plays an essential role in inflammation by stimulating the production of pro-inflammatory cytokines or by increasing their secretion by an oxidative mechanism. These cytokines control the hepatic synthesis of an inflammatory protein called C-Reactive Protein (CRP). High or average values of CRP would have a predictive value of cardiovascular diseases and type 2 diabetes. The relationship between low CRP values and the risk of developing type 2 diabetes and its cardiovascular complications is not sufficiently studied. The hsCRP could serve as a predictive biomarker of risk of onset, follow-up and prognosis for type 2 diabetes and its cardiovascular complications. To answer this problem, we conducted this study, the aim of which was to study the predictive role of hsCRP in the risk of occurrence of type 2 diabetes and its cardiovascular complications. Materials and Methods: This is a prospective and cross-sectional case-control study involving 200 participants including 100 control women and 100 women with type 2 diabetes (mean age was respectively 49.89 years ± 8.26 & 51.92 years ± 7.18;p = 0.066). The interviews were conducted on the basis of a questionnaire. Physical examination collected biometric data and cardiovascular constants. The biochemical parameters such as hsCRP were analyzed by an automated Abbott device. Results: We noted that hsCRP was significantly higher in type 2 diabetic subjects compared to control subjects (p (rho = 0.40, p systolic blood pressure (rho = 0.30, p = 0.003), diastolic blood pressure (rho = 0.28, p = 0.006), total body fat (rho = 0.48, p Conclusion: hsCRP has a prognostic value in the evaluation of cardiovascular risk. It seems to play an important role in the pathogenesis of type 2 diabetes mellitus and its cardiovascular complications. Contribution of Garcinia kola to the Productivity and Economic Profitability of the Community Agricultural Area of Kedougou: Financial and Comparative Analysis of Production Factors

Inland fishing predominates in the Kedougou region, which has no coastline. This type of fishing does not yet meet the local population’s demand for fish products. In response to this concern, a Community Agricultural Estate housing an integrated fish farm was set up in the commune of Bandafassi, in the village of Itato. since its creation, this production unit has been faced with the problem of sourcing high-quality, low-cost, monosex male Oreochromis niloticus fry. In order to overcome this constraint, the present research focuses on the contribution of Garcinia kola to the productivity and economic profitability of the Itato farm. The aim of the research is to assess fish production in the experimental set-up and the production costs of tilapia in a controlled environment. The comparative study of the various production factors shows disparities only in the input factor, where scenarios 2 and 3 use additional products. These are 17-α-methytestosterone for scenario 2 and Garcinia kola for scenario 3. These products significantly interfere with fish production, with a fairly high mortality rate for scenario 2 (25% for two production cycles/ cohort2 (B5, B6, B7 and B8) treated with 17-α-methytestosterone). As for scenarios 1 and 3, mortalities are 5% with or without recourse to additional products (G. kola). In addition, average fish production for the three (03) scenarios is estimated at 28687.5 kg/2 cycles. It varies from one scenario to another, i.e. 30937.5 kg/2 cycles for scenarios 1 and 3 and 24187.5 kg/2 cycles for scenario 2. It is therefore higher in scenarios 1 and 3 than in scenario 2. This difference is due to the fairly large losses of individuals in scenario 2. Furthermore, the analysis of the profit and loss accounts for tilapia production varies from one scenario to another depending on the type of farm: 476 Franc CFA for scenario 1, 610 Franc CFA for scenario 2 and 472 Franc CFA for scenario 3 (F CFA = franc of the French Colonies of Africa). Prostate Cancer in the Thies Region, s.negal: Epidemiological, Diagnostic and Therapeutic Aspects

Introduction: Prostate cancer is the leading urological cancer. It is the most common cancer in men over 50. Objective: To determine the epidemiological, diagnostic and therapeutic characteristics of prostate cancer in hospitals in the Thiès region. Patients and Methods: We conducted a descriptive study from January 1sup>stsup>, 2015 to December 31sup>stsup>, 2020. We included all cases of primary prostate cancer confirmed on histology. Results: We collected data on 318 cases of primary prostate cancer during the study period. Mean patient age was 72.7 years (Range: 49;94 years). Family history of prostate cancer was found in 22 patients (6.91%). The average consultation time was 18.6 months. The circumstances of discovery were dominated by obstructive voiding disorders (97.16%). Digital Rectal examination was suggestive in 55.40% of patients. PsA level was above 20 ng/ml in 76.7% of patients. Prostatic adenocarcinoma was the only histological type (100%). localized cancer represented 7.2% and locally advanced cancer occurred in 36.5% of cases, while metastatic cancer accounted for 56.3%. Radical prostatectomy was performed in 3.18% of cases. Mortality rate was estimated at 8.50% after 1 year. Conclusion: Prostate cancer is the leading urological cancer in the Thies region. It is characterized by the predominance of locally advanced and metastatic forms.

Prognosis Factors of Urinary Quality of life in Parkinson Disease

Introduction: Quality of life in Parkinson disease is not necessary linked to motor symptoms. It’s correlated of overactive bladders and prostatic symptoms. Prognosis factors of urinary quality of life are unknown. Objectives: Our study aims to find prognosis factors of quality of life associated specifically of urinary disorders in PD. Patients and methods: We conducted a transversal, analytic and descriptive study in Physical Medicine, and neurology departments, Fann Teaching Hospital, Dakar and included patients followed for treatment of PD. Urinary disorders, quality of life and functional autonomy were assessed respectively by Urinary symptoms Profile (UsP), Qualiven short Form, schwab and England scale. Results: 38 patients presented PD, with a mean age of 60.89 ± 13.6 years and sex-ratio of 2.45. Mean duration of PD was 3.1 ± 2.9 years. Urinary disorders were found in 47.36% and dominated by incontinence (88.88%) and overactive bladders (88.88%) which were minor in more than 55%. Quality of life was impaired in 88.88% of cases. Minor forms (43.75%) were predominant and constraint dimension (50%) was the most severely altered. Prognosis factors for quality of life of urinary disorders were PD stages (0.046) and functional autonomy (0.042). Discussion and conclusion: Urinary disorders in PD are common, impaired quality of life, especially the constraint dimension, depends on stage of evolution of PD and functional autonomy of patients.

Background: Until recently it has not been possible to isolate the mechanical behavior of individual muscles during passive stretching. Muscle shear modulus(an index of muscle stiffness) measured using ultrasound shear wave elastography can be used to estimate changes in stiffness of an individual muscle. The aims of the present study were(1) to determine the shear modulus—knee angle relationship and the slack angle of the vastus medialis oblique(VMO), rectus femoris(RF), and vastus lateralis(Vl) muscles;(2) to determine whether this differs between the ***: Nine male rowers took part in the study. The shear modulus of VMO, RF, and Vl muscles was measured while the quadriceps was passively stretched at 3°/s. The relationship between the muscle shear modulus and knee angle was plotted as shear modulus—knee angle curve through which the slack angle of each muscle was ***: The shear modulus of RF was higher than that of VMO and Vl when the muscles were stretched over 54°(all p significant difference was found between the VMO and Vl(all p > 0.05). The slack angle was similar among the muscles: 41.3°± 10.6°, 44.3°± 9.1°, and 44.3°± 5.6° of knee flexion for VMO, RF, and Vl, respectively(p = 0.626).Conclusion: This is the first study to experimentally determine the muscle mechanical behavior of individual heads of the quadriceps during passive stretching. Different pattern of passive tension was observed between mono-and bi-articular muscles. Further research is.needed to determine whether changes in muscle stiffness are muscle-specific in pathological conditions or after interventions such as stretching protocols.

To monitor the quality characteristics of a process, appropriate graphical and statistical tools must be used. These tools are capable of showing the evolution over time of the behavior of the quality characteristics (measurable or countable) and detecting situations that seem to present certain anomalies. The control chart is one of these tools widely used in quality management. In the process of managing the COVID-19 pandemic, this tool will make it possible to know at all times whether the parameters monitored such as the positivity rate, the recovery rate, and the mortality rate, are under control and to act accordingly. Monitoring cure and mortality rates will also show us the effectiveness of the treatments used.

Association between smoking,pancreatic insulin secretion and insulin resistance in Chinese subjects with or without glucose intolerance

Background There are studies suggesting smoking may increase the risk of type 2 diabetes. Effects of smoking on insulin secretion and insulin resistance （IR） are, however, controversial. Methods This is a cross-sectional study. since there were very few smokers among Hong Kong Chinese women, only men （n=-1068） were analyzed in this report. Fasting and 2-hour plasma glucose and insulin were measured. Insulinogenic index as well as beta-cell function and IR based on homeostatic model assessment （HOMA） by computer model （HOMA Calculator v2.2） were calculated. Results Of the 1068 men, 147 had newly diagnosed diabetes, 131 newly diagnosed impaired glucose tolerance （IGT） and 790 were non-diabetic normal controls. smokers had similar fasting and 2-hour insulin levels, insulinogenic index and HOMA derived beta-cell function as compared to non-smokers in the groups with diabetes, IGT or normal oral glucose tolerance test （OGTT）. IR was also similar between smokers, ex-smokers and non-smokers in those with normal OGTT. In men with IGT or diabetes, after adjustment for age and body mass index, smokers were more insulin resistant as compared to non-smokers （IR, IGT： 1.59±1.07 vs 1.03±0.54, P〈0.05; diabetes： 1.96±1.36 vs 1.06±0.45, P〈0.01）. With logistic regression analysis, comparing smokers and non-smokers, IR was independently associated with smoking （odds ratio （95% C/）, IGT： 2.23 （1.05, 4.71）; diabetes： 3.92 （1.22, 12.58））. None of the other insulin parameters enter into the model among those with normal OGTT or comparing ex-smokers and non-smoker or smokers and ex-smokers. Conclusions In Chinese men, smoking did not show any direct association with insulin levels and pancreatic insulin secretion. smoking men with IGT or diabetes appeared more insulin resistant than their non-smoking counterparts.

Assessment of Physicians’ Knowledge of Clean Intermittent Urinary self-Catheterization in an African Country: The Case of s.negal

Introduction: Clean intermittent self-catheterization is the gold standard for the management of urinary retention. But its use remains limited in our practice. Objective: We are conducting this study to assess physicians’ knowledge of its practice in order to promote its promotion. Material and Methods: We conducted a cross-sectional, prospective, descriptive and analytical study in two hospitals in Dakar for two months. The resident doctors were interviewed on the basis of a questionnaire developed based on the recommendations of the French society of Physical Medicine and Rehabilitation (sOFMER). Results: 54 medical residents returned the questionnaire. 63% of physicians surveyed defined the clean intermittent self-catheterization as a sterile intermittent self-catheterization. 70.9% of the doctors surveyed had cited urinary retention as an indication for the clean intermittent self-catheterization;53.7% advocated sterile gloves for the catheterization;29.6% proposed a frequency of one to two times and 2 to 4 catheterizations per day. 70.4% of doctors recommended indicating Cytobacteriological examination of urine in case of symptoms of urinary tract infection. Antibiotic therapy from the outset was offered by 83.3% of doctors in case of symptoms of urinary tract infection under intermittent self-sounding. 59.3% were unaware of clean intermittent self-catheterization. Discussion and Conclusion: Apart from the indica-tions, the clean intermittent self-catheterization remains unknown by res-ident doctors and its practice remains to be mastered, hence the need to establish a course on its practice at the faculty of medicine.

Unsteady Natural Convection between Two Eccentric Hemispheres

Natural convection heat transfer in open or closed cavities takes place in different engineering areas. The hemispherical cavity is a part of basic geometries although it is not widely studied. The present paper reports the numerical study of natural convection in a closed hemispherical annulus delimited by two vertically eccentric hemispheres filled with newtonian fluid (air in this case with Pr = 0.7) is conducted. The inner hemisphere is heated by a heat flux of constant density and the outer one is maintained isothermal. Based on the Bouss.nesq assumptions, the governing equations are numerically studied using unsteady natural convection formulated with vorticity and stream-function variables. These equations are written by using bispherical coordinates system and solved by using a finite difference method. The effect of the control parameters such as the Rayleigh number (103 ≤ Ra ≤ 106) or the eccentricity (e = ±0.2, ±0.5, 0) in the dynamic and thermal behaviours of the fluid is investigated.

Diffuse Cervico-Facial Cellulitis: Epidemiological and Clinical Aspects of 297 Cases

Introduction: Cervicofacial cellulitis is a medical and surgical emergency, it is an extensive infection of the aponeurotic spaces of the face and neck. Although frequent in black Africa rather than in developed countries, they remain associated with a poor prognosis and few studies have been done on this subject. Their management must be early, probabilistic and then adapted to the bacteriological results of the pus taken. As s.negal is one of the many sub-saharan African countries without recent data on cervicofacial cellulitis, we felt it was important to review the situation. The aim of our study was to describe the epidemiological and clinical aspects of cervicofacial cellulitis in the ENT department of lamine s.ne DIOP of the Fann National University Hospital of Dakar. Materials and Method: This was a retrospective study conducted over 5 years from January 2017 to December 2021 in the ENT-CCF lAMIne s.ne DIOP Department of the CHNU FANN in Dakar. Our data sources were hospitalization registers and medical records of patients hospitalized for cervicofacial cellulitis. Our data were analyzed using the spss 12 for windows software. Results: During this 5-year period, we collected 297 cases of cervicofacial cellulitis, i.e. 19% of the hospitalizations. The average age was 35 years with a sex ratio of 2.23. The notion of taking NsAIDs was found in 90.57% of the cases, the immunocompromised background was present in 18 patients, i.e. 6.06% of the cases. The site of the swelling was sub-mento-maxillary in 49% of the cases and anterior cervical in 5% of cases. The portal of entry was dental in 189 patients, i.e. 64% of cases, and unknown in 3%. 4 deaths were recorded, i.e. 1.35% of patients hospitalized for cervicofacial cellulitis, and 1 case of mediastinitis with polyseritis was transferred to the Department of Thoracic and Cardiovascular surgery, which had a favourable outcome. Conclusion: Cervicofacial cellulitis is a fairly common infection with a poor prognosis
