Sequential transcatheter arterial chemoembolization and portal vein embolization before right hemihepatectomy in patients with hepatocellular carcinoma

Background:Recent studies showed that sequential selective transcatheter arterial chemoembolization(TACE)and portal vein embolization(PVE)provided better future liver remnant(FLR)regeneration rate and disease-free survival following surgery compared with PVE alone.The present study aimed to clarify whether preoperative sequential TACE and PVE before right hemihepatectomy can reduce postoperative hepatocellular carcinoma(HCC)recurrence and improve long-term disease-free and overall survival.Methods:Recurrence and survival outcomes were retrospectively evaluated in 205 patients with HCC who underwent right hemihepatectomy by a single surgeon from November 1993 to November 2017.Patients were divided into four groups according to the procedure performed before the surgery:sequential TACE and PVE(TACE-PVE),PVE-only,TACE-only,or na?ve control groups.The baseline patient and tumor characteristics,postoperative outcomes,recurrence-free survival and overall survival were analyzed.Results:Baseline patient and tumor characteristics upon diagnosis were similar in all four groups,while sequential TACE and PVE were well tolerated.The TACE-PVE group had a higher mean increase in percentage FLR volume compared with that of the PVE-only group(17.46%±6.63%vs.12.14%±5.93%;P=0.001).The TACE-PVE group had significantly better overall and disease-free survival rates compared with the other groups(both P<0.001).Conclusions:Sequential TACE and PVE prior to surgery can be an effective therapeutic strategy for patients with HCC scheduled for major hepatic resection.The active application of preoperative sequential TACE and PVE for HCC would allow more patients with marginal FLR volume to become candidates for major hepatic resection by promoting compensatory FLR hypertrophy without the deterioration of basal hepatic functional reserve or tumor progression.

Treatment of hepatocellular carcinoma with portal venous tumor thrombosis: A comprehensive review

The natural history of hepatocellular carcinoma(HCC)with portal vein tumor thrombosis(PVTT)is dismal(approximately 2-4 mo),and PVTT is reportedly found in 10%-40%of HCC patients at diagnosis.According to the Barcelona Clinic Liver Cancer(BCLC)Staging System(which is the most widely adopted HCC management guideline),sorafenib is the standard of care for advanced HCC(i.e.,BCLC stage C)and the presence of PVTT is included in this category.However,sorafenib treatment only marginally prolongs patient survival and,notably,its therapeutic efficacy is reduced in patients with PVTT.In this context,there have been diverse efforts to develop alternatives to current standard systemic chemotherapies or combination treatment options.To date,many studies on transarterial chemoembolization,3-dimensional conformal radiotherapy,hepatic arterial chemotherapy,and transarterial radioembolization report better overall survival than sorafenib therapy alone,but their outcomes need to be verified in future prospective,randomized controlled studies in order to be incorporated into current treatment guidelines.Additionally,combination strategies have been applied to treat HCC patients with PVTT,with the hope that the possible synergistic actions among different treatment modalities would provide promising results.This narrative review describes the current status of the management options for HCC with PVTT,with a focus on overall survival.

＜正＞Purpose:To evaluate the efficacy of transjugular liver biopsy（TJLB） in patients who have undergone dual living donor liver transplantation（LDLT） and analyze the necessity of biopsy from dual grafts. Materials:We assessed results from 173 biopsy sessions in 73 patients including 56 TJLB sessions（28 patients）,95 percutaneous biopsy sessions,and 22 open biopsy sessions.The TJLB patients had received dual left lobes（n=23）,right and left lobes（n=4） and dual left lateral segments（n=1）.TJLB samples were taken using a Quick-Core biopsy needle or a biopsy forceps in cases with unfavorable vena cava-hepatic venous angle.We analyzed the technical success rate for TJLB and the consistency of histologic diagnosis based on samples from both grafts according to their CT enhancing patterns.Percutaneous and open biopsy samples were also included in the analysis of the consistency of histologic diagnosis. Results:Except for 23 nontarget grafts,TJLB was successful in 83 of 89（93%） target grafts using a Quick-Core biopsy needle（n=68） or a biopsy forceps（n=15）.Liver tissues from 78 of 83（94%） grafts were adequate for histologic diagnosis.Liver tissues of dual grafts were available in 99 sessions,including 27 TJLB sessions. Histologic diagnoses were inconsistent in 21 sessions with consistent（n=14） or inconsistent（n=7） enhancing patterns of dual grafts.The consistency of histologic diagnosis correlated significantly with the enhancing patterns of the dual grafts（p＜0.001）. Conclusion:TJLB can be used to obtain adequate liver tissue for histologic diagnosis in dual LDLT recipients.Biopsies should be obtained from each graft owing to the possibility of inconsistent histologic diagnoses of dual grafts regardless of enhancing patterns.

Understanding the driving mechanism of the divergence of hoarding strategies of animals(i.e.,scatter-hoarding vs.larder-hoarding)is crucial to understand the ecological and evolutionary implications of plant-animal interactions.Zhang et al.(2022)compiled a global-scale dataset from the published literature including 183 species of seed-hoarding rodents,and concluded that phylogenetic conservatism,functional traits,and environmental factors counted for the hoarding behavior divergence of rodents.A more thought-provoking question may be asked:Do these variables contribute equally or not to explain the divergence of hoarding strategies?Here,we re-analyzed Zhang et al's(2022)dataset using partial Ralk calculation(Ives 2019)to parse out the relative contri-lik butions of these variables.

Perenniality:From model plants to applications in agriculture

To compensate for their sessile nature,plants have evolved sophisticated mechanisms enabling them to adapt to ever-changing environments.One such prominent feature is the evolution of diverse life history strategies,particularly such that annuals reproduce once followed by seasonal death,while perennials live longer by cycling growth seasonally.This intrinsic phenology is primarily genetic and can be altered by environmental factors.Although evolutionary transitions between annual and perennial life history strategies are common,perennials account for most species in nature because they survive well under year-round stresses.This proportion,however,is reversed in agriculture.Hence,perennial crops promise to likewise protect and enhance the resilience of agricultural ecosystems in response to climate change.Despite significant endeavors that have been made to generate perennial crops,progress is slow because of barriers in studying perennials,and many developed species await further improvement.Recent findings in model species have illustrated that simply rewiring existing genetic networks can lead to lifestyle variation.This implies that engineering plant life history strategy can be achieved by manipulating only a few key genes.In this review,we summarize our current understanding of genetic basis of perenniality and discuss major questions and challenges that remain to be addressed.

Risk factors for lymph node metastasis in superficial esophageal squamous cell carcinoma

In this editorial,we comment on the article by Wang et al published in the recent issue of the World Journal of Gastroenterology in 2023.We focused on identifying risk factors for lymph node metastasis(LNM)in superficial esophageal squamous cell carcinoma(SESCC)patients and how to construct a simple and reliable clinical prediction model to assess the risk of LNM in SESCC patients,thereby helping to guide the selection of an appropriate treatment plan.The current standard treatment for SESCC is radical esophagectomy with lymph node dissection.However,esophagectomy is associated with considerable morbidity and mortality.Endoscopic resection(ER)offers a safer and less invasive alternative to surgical resection and can enable the patient's quality of life to be maintained while providing a satisfactory outcome.However,since ER is a localized treatment that does not allow for lymph node dissection,the risk of LNM in SESCC limits the effectiveness of ER.Understanding LNM status can aid in determining whether patients with SESCC can be cured by ER without the need for additional esophagectomy.Previous studies have shown that tumor size,macroscopic type of tumor,degree of differentiation,depth of tumor invasion,and lymphovascular invasion are factors associated with LNM in patients with SESCC.In addition,tumor budding is commonly associated with LNM,recurrence,and distant metastasis,but this topic has been less covered in previous studies.By comprehensively evaluating the above risk factors for LNM,useful evidence can be obtained for doctors to select appropriate treatments for SESCC patients.
