After HAART (highly active antiretroviral therapy), which is a powerful antiviral drug regimen, was introduced into HIV therapy for AIDS patients, AIDS mortality decreased dramatically. The incidence of encephalopathy among AIDS patients was reported to be 10% to 40% in the past, but the introduction of HAART resulted in a decrease in HIV encephalopathy and an increase in mild CNS-related deficits, which affect the quality of life of AIDS patients. We report the case of an AIDS patient who developed schizophrenia-like delusions associated with encephalopathy. Administration of an antipsychotic drug and hypnotics was effective in relieving the symptoms of patients with insomnia, depressed mood, and delusions. Further information on the neuronal mechanism underlying the mental symptoms observed in our patient will be necessary to understand the process of pathogenesis of the encephalopathy and to devise an adequate clinical strategy for treatment.

Malnutrition and Its Association with the Mortality of Patients with Femoral Intertrochanteric Fractures: A Retrospective Analysis of Japanese Patients

Background: The incidence of femoral intertrochanteric fractures in older adults is higher than that of femoral neck fractures;however, both conditions are often analyzed together as proximal femoral fractures. Considering the difference in treatment, postoperative complication, and mortality risk, these two fractures should be analyzed separately. This study aimed to analyze 1-year mortality and its risk factors in patients with surgically treated femoral intertrochanteric fractures. Methods: Consecutive patients with intertrochanteric fractures who underwent surgical interventions at our institution between January 2017 and December 2021 were retrospectively reviewed. A total of 238 patients were eligible for inclusion in this study. Patients’ demographic and clinical information were retrospectively collected. Patients were divided into the 1-year mortality (n = 16) and survival (n = 222) groups. The incidence of 1-year mortality and its independent risk factors were investigated using univariate and multivariate logistic regression analyses. Results: The mean age of patients was 85.6 ± 8.5 years. The 1-year mortality rate was 6.7% (16/238). Preoperative albumin level, the Geriatric Nutritional Risk Index (GNRI), and malnutrition status (GNRI p = 0.02, p = 0.02, and p = 0.0011, respectively). Multivariate analysis showed that malnutrition status (GNRI p = 0.035) was an independent risk factor for 1-year mortality. Conclusion: Malnutrition status assessed using GNRI (GNRI < 92) was an independent risk factor for 1-year mortality. Our findings suggest that GNRI may be an effective screening tool for predicting postoperative 1-year mortality of patients with surgically treated femoral intertrochanteric fractures.

AIM:To explore the usage of choroidal thickness measured by swept-source optical coherence tomography(SS-OCT)to detect myopic macular degeneration(MMD)in high myopic ***:Participants with bilateral high myopia(≤−6 diopters)were recruited from a subset of the Guangzhou Zhongshan Ophthalmic Center-Brien Holden Vision Institute High Myopia Cohort ***-OCT was performed to determine the choroidal thickness,and myopic maculopathy was graded by the International Meta-Analysis for Pathologic Myopia(META-PM)Classiﬁ*** of MMD was defined as META-PM category 2 or ***:A total of 568 right eyes were included for *** with MMD(n=106,18.7%)were found to have older age,longer axial lengths(AL),higher myopic spherical equivalents(SE),and reduced choroidal thickness in each Early Treatment Diabetic Retinopathy Study(ETDRS)grid sector(P<0.001).The area under the receiver operating characteristic(ROC)curves(AUC)for subfoveal choroidal thickness(0.907)was greater than that of the model,including age,AL,and SE at 0.6249,0.8208,and 0.8205,*** choroidal thickness of the inner and outer nasal sectors was the most accurate indicator of MMD(AUC of 0.928 and 0.923,respectively).An outer nasal sector choroidal thickness of less than 74μm demonstrated the highest odds of predicting MMD(OR=33.8).CONCLUSION:Choroidal thickness detects the presence of MMD with high agreement,particularly of the inner and outer nasal sectors of the posterior pole,which appears to be a biometric parameter more precise than age,AL,or SE.

When the disastrous tsunamis hit East Japan on 11 March 2011, the hi-tech information transmission system that is the pride of Japan was unable to send out the Tsunami Warnings effectively;many lives were lost as a result. Subsequently, the Japan Meteorological Agency(JMA) set up a committee of experts to improve the overall system. Last year as a result of their meetings, a report on The Improvement of the Information Text for the Tsunami Warnings was announced. The changes are, however, problematic from the linguistics perspective as the committee did not include any experts in the field. This study examines a range of recordings of the Tsunami Warnings and the Evacuation Instructionsi given for the East Japan tsunami disaster from the perspective of linguistics, especially pragmatics, whose main objective is to discover how recipients interpret verbal messages. The aim of this article is to propose further improvement to the transmission system of Tsunami Warnings and Evacuation Instructions. This study also emphasizes the necessity and importance of reexamining the overall disaster information transmission issue in Japan, from the linguistics perspective.

A Hybrid Vision Method for Autonomous Guided Vehicle Navigation

Background:Emerging data suggested that liquid biopsy such as detection of circulating tumor cells(CTCs)and cell-free tumor DNA analysis augments the management of patients with urothelial cancer(UC).We presented our pilot experience of liquid biopsy using the Ion Torrent platform to detect CTCs and genomic alterations in UC.Materials and methods:Blood or urine samples from 16 patients were subjected to CTC and plasma/urine cell-free tumor DNA isolation for next generation sequencing(NGS)using the Ion S5 system to detect mutations among 50 oncogenes on the Ion AmpliSeq Cancer Hotspot Panel.Results:The Ion Torrent platform detected a higher number of CTCs than those in previous studies using the CellSearchTM ***,mutations were detected in 13/16(81.3％)patients with a median number of 18(range 12-25).NGS isolated 17 hotspot mutations from 11 genes and 41 novel genomic alterations from 24 genes,some of which are supposed to be clinically actionable.Conclusions:The Ion Torrent platform efficiently detected CTCs compared with previous *** with the present system also allowed for detection of gene alterations which are likely to be therapeutic targets and provided an attractive tool to guide personalized therapy for patients with advanced UC.

For the autonomous guided vehicle (AGV) used mainly in unfixed work fields, a machine vision method was proposed for the navigation system, in which a series of navigation-signs are placed along the travel route. The navigation system detects and recognizes these signs, and accordingly informs the travel control system. In order for the navigation to have balanced ability of 1) covering a large area and 2) recognizing details of the sign, the proposed vision method was designed to be a hybrid one, using both the stereo vision and the traditional 2D template matching. The former implemented a coarse recognition function for above 1), and the later implemented a fine recognition function for above 2). The results from the coarse recognition were used in the fine recognition for the gaze control to input suitable 2D image of the signs. Experiments on a prototype system show the feasibility of the proposed hybrid method in achieving the objective specifications for a typical AGV.

This research is dedicated to develop a safety measurement for human-machine cooperative system, in which the machine region and the human region cannot be separated due to the overlap and the movement both from human and from machines. Our proposal here is to automatically monitor the moving objects by image sensing/recognition method, such that the machine system can get enough information about the environment situation and about the production progress at any time, and therefore the machines can accordingly take some corresponding actions automatically to avoid hazard. For this purpose, two types of monitor systems are proposed. The first type is based on the omni directional vision sensor, and the second is based on the stereo vision sensor. Each type may be used alone or together with another type, depending on the safety system's requirements and the specific situation of the manufacture field to be monitored. In this paper, the description about these two types are given, and as for the special application of these image sensors into safety control, the construction of a hierarchy safety system is proposed.
