Aortic valve replacement reduces mortality in moderate aortic stenosis:a systematic review and meta-analysis

BACKGROUND With the introduction of transcatheter aortic valve replacement and an evolving understanding of the natural progression and history of aortic stenosis,the potential for earlier intervention in appropriate patients is promising;however,the benefit of aortic valve replacement in moderate aortic stenosis remains *** Pubmed,Embase,and the Cochrane Library databases were searched up until 30th of December 2021 using keywords including moderate aortic stenosis and aortic valve *** reporting all-cause mortality and outcomes in early aortic valve replacement(AVR)compared to conservative management in patients with moderate aortic stenosis were *** ratios were generated using random-effects meta-analysis to determine effect *** 3470 publications were screened with title and abstract review,which left 169 articles for full-text *** these studies,7 met inclusion criteria and were included,totalling 4,827 *** studies treated AVR as a time-dependent co-variable in cox-regression multivariate analysis of all-cause *** with surgical or transcatheter AVR was associated with a 45% decreased risk of all-cause mortality(HR=0.55[0.42-0.68],I2=51.5%,Pthe overall cohort with appropriate sample sizes,with no evidence of publication,detection,or information biases in any of the *** In this systematic review and meta-analysis,we report a 45% reduction in all-cause mortality in patients with moderate aortic stenosis who were treated with early aortic valve replacement compared to a strategy of conservative *** control trials are awaited to determine the utility of AVR in moderate aortic stenosis.

the putative Jambil meta-carbonatites of Swat,northern Pakistan,occur as discrete intrusions into the Proterozoic Manglaur Formation,which are difficult to be distinguished from nearby calc-silicate marble because both rock types experienced regional metamorphism during Himalayan orogenesis that resulted in similar mosaic textures and mineral *** are often significant repositories of economic mineral resources and,therefore,are important to be distinguished from calc-silicate *** present new geochemical and geochronology data to distinguish between the two rock types and interpret the petrogenesis and tectonic evolution of the Jambil *** rock chemical data from the Jambil meta-carbonatites show characteristically high rare earth element(REE),Sr contents and lack of negative Eu anomaly,consistent with average calcio-carbonatite values worldwide and an igneous *** than 0.5 wt.% SrO in the metacarbonatites and SrO> 0.15 wt.% in constituent rock forming calcite are discriminating signatures of the Jambil ***,the Jambil meta-carbonatites are relatively depleted in Rb,Nb,Ta,Ti,Zr and Hf,relatively enriched in Ba,th,Sr,and have a high LREE/HREE ratio when normalized to primitive *** carbon and oxygen isotope compositions vary from-3.5‰ to-4.3‰and from 9.7‰ to 12.3‰,*** geochemical characteristics indicate generation of the carbonatites through small degree of partial melting from a carbonated eclogitic ***-situ,U/Pb analysis of titanite indicates that the Jambil meta-carbonatites were emplacement at 438 ±3 *** combined with regional geological observations,we interpret the emplacement of the Jambil metacarbonatites to have taken place during the Silurian back arc extension within greater Gondwana and mark a transition from a compressional tectonic regime,brought about by collision of microcontinental blocks along the northern margin of Gondwana,to post-orogenic

Inhibitory activity of polysaccharide extracts from three kinds of edible fungi on proliferation of human hepatoma smMC-7721 cell and mouse implanted S180 tumor

AIM To determine the activities ofpolysaccharide extracts from Flammulina velutipes (Curt. ex Fr. ) Sing (FV), Lentinusedodes (LE) and Agaricus bisporus Sing (AB)on the proliferation of human hepatoma smMC-7721 cells in vitro and on mouse implanted S-180tumors in *** the polysaccharide extracts were isolated from the fruit bodies of FV, LE and AB by the methods of hot-water extraction, Sevag’sremoval of proteins, ethanol precipitation,trypsin digestion and ethanol fractionalprecipitation. Human hepatoma smMC-7721 cells were treated with 50 mg/L Polysaccharide extracts, and the mitosis index, mitochondria activity and cell proliferation were detected at different times in both control and experimental groups. the mice with S-180 implanted tumors were injected with the polysaccharide extracts at 24 mg/ kg body weight for 9 d and the tumorweight was measured on the 15th *** the mitosis index of hepatoma cells in vitro could be significantly decreased by treatment with the polysaccharide extracts fromthe three kinds of edible fungi (P thecell numbers and mitochondria activity of smMC7721 cells treated with polysaccharide extracts were lower than those in control groups (P the inhibition rates of polysaccharide extracts against implanted S-180 tumors in mice were 52.8%, 56.6% and 51 .9% respectivelycompared with that in c0ntrol ***0N the POIysaccharide extractsfrom the three kinds of edible fungi could inhibitnot only the Cultured malignant cells in vitfO butalso impIanted Sl80 tum0r i0 vivo.

　以多孔块状硅体为模板,通过一步浸渍、炭化、酸处理工艺,制备出结构可控、孔隙联通且具有多重孔隙的铸型炭体。分别采用氮吸附技术、高分辨透射电镜和扫描电镜对模板硅、硅碳复合物及相应炭体进行了结构表征。从微米尺度分析,该炭体是由高度联接的枝状结构单元为骨架构筑而成。与模板硅相比较,此炭体是硅体的正复本。从纳米尺度分析,这种炭体枝状骨架是由分布均匀且相互连接的中孔构成,此中孔及其孔壁对应于模板硅的孔壁及中孔,是多孔硅的负复本。此外构成中孔的炭骨架本体又含有大量炭化过程产生的微孔。因此该法合成的铸型炭具有孔隙高度发达和结构层层嵌套的特点。

Biobehavioral assessment of the anxiety disorders:Current progress and future directions

It is difficult to accurately assess and differentially diagnose the anxiety *** current system of assessment relies heavily on the subjective measures of client self-report,clinical observation,and clinical ***,recent technological advances may enable practitioners to utilize objective,biobehavioral measures of assessment in a clinical *** current body of literature on two of these biobehavioral tools(eyetracking and electrocardiogram devices) is promising,but more validation and standardization research is needed to maximize the utility of these ***-tracking devices are uniquely capable of providing data that can be used to differentially diagnose anxiety disorders from both other commonly comorbid and misdiagnosed *** eye-tracking and electrocardiogram devices are able to provide change-sensitive assessment *** objective,real-time feedback can assist clinicians and researchers in assessing treatment efficacy and symptom *** developed wearable and highly portable electrocardiogram devices,like the wearable fitness and behavior tracking devices used by many consumers,may be particularly suited for providing this feedback to *** these biobehavioral devices would supply an objective,dimensional component to the current categorical diagnostic assessment *** posit that if adequate funding and attention are directed at this area of research,it could revolutionize diagnostic and on-going assessment practices and,in doing so,bring the field of diagnosis out of the 20 th century.

Orthopaedic care provided by the 14th combat support hospital in support of humanitarian and disaster relief after hurricane Maria in Puerto Rico

On September 20,2017 Hurricane Maria,a category 4 hurricane,made landfall on the eastern coast of Puerto *** was preceded by Hurricane Irma,a category 5 hurricane,which passed just off the coast 13 d *** destruction from both Hurricane Irma and Maria precipitated a coordinated federal response which included the Federal Emergency Management Agency(FEMA)and the United States *** United States Army dispatched the 14^th Combat Support Hospital(CSH)to Humacao,a city on the eastern side of the island where Maria made *** mission of the 14^th CSH was to provide medical humanitarian aid and conduct disaster relief operations in support of the government of Puerto Rico and *** the 14^th CSH deployment to Puerto Rico,1157 patients were evaluated and ***-seven operative cases were performed to include 23 orthopaedic *** mean age of the orthopaedic patients treated was 45.7 years(range 13-76 years).the most common operation was irrigation and debridement of open contaminated and/or infected *** presented a mean 10.8 d from their initial injury(range 1-40 d).Fractures and infections were the most common diagnoses with the greatest delay in treatment from the initial date of *** deployment of the 14^th CSH to Puerto Rico was unique in its use of air transport,language and local customs encountered,as well as deployment to a location outside the continental United *** factors coupled with the need for rapid deployment of the 14^th CSH provided valuable experience which will undoubtedly enable future success in similar endeavors.

Anthrax is a zoonotic disease caused by Bacillus *** is potentially fatal and highly contagious *** arc the natural *** acquire the disease incidentally by contact with infected animal or animal *** the 18th century an epidemic destroyed approximately half of the sheep in *** 1900 human inhalational anthrax occured sporadically in the United *** 1979 an outbreak of human anthrax occured in Sverdlovsk of Soviet *** continued to represent a world wide *** incidence of the disease has decreased in developed countries as a result of vaccination and improved industrial *** anthrax clinically presents in three forms,***,gastrointestinal and *** 95%of human anthrax is cutaneous and 5%is *** anthrax is very rare(less than 1%).Inhalational form is used as a biological warefare agent. Penicillin,ciprofloxacin(and other quinolones),doxicyclin,ampicillin,***, clarithromycin,vancomycin,chloramphenicol,rifampicin are effective antimicrobials. Antimicrobial therapy for 60 days is *** anthrax vaccine is available. Administration of anti-protective antigen(PA) antibody in combination with ciprofloxacin produced 90%-100%*** combination of CPG-adjuvanted anthrax vaccine adsorbed(AVA) plus dalbavancin significantly improved survival.

＜正＞Brainstem A2/C2 catecholamine（CA） neurons in the solitary tract nucleus（NTS） are thought to play an important role in the control of food intake and other homeostatic *** have previously demonstrated that these neurons,which send extensive projections to brain regions involved in the regulation of appetite,are strongly and directly activated by solitary tract（ST） visceral ***,a potent orexigenic peptide released from the stomach,is proposed to act in part through modulating NTS CA neurons but the underlying cellular mechanisms are ***,we identified CA neurons using transgenic mice that express enhanced green fluorescent protein driven by the tyrosine hydroxylase promoter（th-EGFP）.We then determined how ghrelin modulates th-EGFP neurons using patch-clamp techniques in a horizontal brain slice *** inhibited the frequency of spontaneous glutamate inputs（spontaneous EPSCs） onto th-EGFP neurons,including cholecystokinin-sensitive neurons,an effect blocked by the GHSR1 antagonist,*** resulted in a decrease in the basal firing rate of NTS th-EGFP neurons,an effect blocked by the glutamate antagonist *** also dose-dependently inhibited the amplitude of ST afferent-evoked EPSCs（ST-EPSCs） in th-EGFP NTS neurons,decreasing the success rate for ST-evoked action *** addition,ghrelin decreased the frequency of mini-EPSCs suggesting its actions are presynaptic to reduce glutamate ***,inhibition by ghrelin of the ST-EPSCs was significantly increased by an 18 h *** results demonstrate a potential mechanism by which ghrelin inhibits NTS th neurons through a pathway whose responsiveness is increased during fasting.

Background： In vivo quantification of choroidal neovascularization （CNV） based on noninvasive optical coherence tomography （OCT） examination and in vitro choroidal flatmount immunohistochemistry stained of CNV currently were used to evaluate the process and severity of age-related macular degeneration （AMD） both in human and animal studies. this study aimed to investigate the correlation between these two methods in murine CNV models induced by subretinal injection. Methods： CNV was developed in 20 C57BL6/j mice by subretinal injection of adeno-associated viral delivery of a short hairpin RNA targeting sFLT-1 （***- 1）, as reported previously. After 4 weeks, CNV was imaged by OCT and fluorescence angiography. the scaling factors for each dimension, x, y, and z （ktm/pixel） were recorded, and the corneal curvature standard was adjusted from human （7.7） to mice （1 .4）. the volume of each OCT image stack was calculated and then normalized by multiplying the number of voxels by the scaling factors for each dimension in Seg3D software （University of Utah Scientific Computing and Imaging Institute, available at http：//***/cibc-software/***）. Eighteen mice were prepared for choroidal flatmounts and stained by CD31. the CNV volumes were calculated using scanning laser confocal microscopy after immunohistochemistry staining. Two mice were stained by Hematoxylin and Eosin for observing the CNV morphology. Results： the CNV volume calculated using OCT was, on average, 2.6 times larger than the volume calculated using the laser confocal microscopy. the correlation statistical analysis showed OCT measuring of CNV correlated significantly with the in vitro method （R： = 0.448, P = 0.001, n = 18）. the correlation coefficient for CNV quantification using OCT and confocal microscopy was 0.693 （n = 18, P = 0.001 ）.
