AIM: The role of intravenous pantoprazole in treatment of patients with high-risk bleeding peptic ulcers following endoscopic hemostasis remains uncertain. We therefore conducted the pilot prospective randomized study to assess whether intravenous pantoprazole could improve the efficacy of H2-antagonist as an adjunct treatment following endoscopic injection therapy for bleeding ***: Patients with active bleeding ulcers or ulcers with major signs of recent bleeding were treated with distilled water injection. After hemostasis was achieved, they were randomly assigned to receive intravenous pantoprazole or ***: One hundred and two patients were enrolled in this prospective trial. Bleeding recurred in 2 patients (4%) in the pantoprazole group (n = 52), as compared with 8 (16%) in the ranitidine group (n = 50). The rebleeding rate was significantly lower in the pantoprazole group (P = 0.04).There were no statistically significant differences between the groups with regard to the need for emergency surgery (0% vs2%), transfusion requirements (4.9+5.9 vs5.7:1:6.8 units), hospital days (5.9+3.2 vs7.5:1:5.0 d) or mortality (2% vs 2%).CONCLUSION: Pantoprozole is superior to ranitidine as an adjunct treatment to endoscopic injection therapy in high-risk bleeding ulcers.

AIM: To evaluate the factors affecting the early tumor recurrence within one year in cirrhotic patients having a single small hepatocellular carcinoma (HCC) after complete tumor necrosis by radiofrequency ablation (RFA)***: Thirty patients with a single small HCC received RFA therapy by a RFA 2000 generator with LeVeen needle. Tri-phase computerized tomogram was followed every 2 to3 mo after RFA. The clinical effects and tumor recurrence were ***: The initial complete tumor necrosis rate was 86.7%. Twenty-two patients were followed for more than one year. The local and overall recurrence rates were 13.6% and 36.4%, 33.3% and 56.2%, 46.6% and 56.2%at 12, 24 and 30 mo, respectively. No major complication or procedure-related mortality was found. The risk factors for early local tumor recurrence within one year were larger tumor size, poor pathologic differentiation of tumor cells and advanced tumor staging. The age of patients with new tumor formation within one year was relatively younger (55.1±8.3 vs 66.7±10.8, P = 0.029).CONCLUSION: Large tumor size, poor pathologic differentiation of tumor cells and advanced tumor staging are the risk factors for early local tumor recurrence within one year, and young age is the positive predictor for new tumor formation within one year.

AIM: Bleeding and perforation are the major and serious complications associated with endoscopic polypectomy. To develop a safe and effective method to resect hyperplastic polyps of the stomach, we employed rubber bands to strangulate hyperplastic polyps and to determine the possibility of inducing avascular necrosis in these ***: Forty-seven patients with 72 hyperplastic polyps were treated with endoscopic banding ligation (EBL). On 14 days after endoscopic ligation, follow-up endoscopies were performed to assess the outcomes of the strangulated ***: After being strangulated by the rubber bands,all of the polyps immediately became congested (100 %),and then developed cyanotic changes (100 %) approximately 4 minutes later. On follow-up endoscopy 2 weeks later, all the polyps except one had dropped off. The only one residual polyp shrank with a rubber band in its base, and it also dropped off spontaneously during subsequent *** complications occurred during or following the ligation ***: Gastric polyps develop avascular necrosis following ligation by rubber bands. Employing suction equipment, EBL can easily capture sessile polyps. It is an easy, safe and effective method to eradicate hyperplastic polyps of the stomach.

Spatial Interpolation of Daily Precipitation in China:1951-2005

Climate research relies heavily on good quality instrumental data; for modeling efforts gridded data are needed. So far, relatively little effort has been made to create gridded climate data for China. This is especially true for high-resolution daily data. This work, focuses on identifying an accurate method to produce gridded daily precipitation in China based on the observed data at 753 stations for the period 1951-2005. Five interpolation methods, including ordinary nearest neighbor, local polynomial, radial basis function, inverse distance weighting, and ordinary kriging, have been used and compared. Cross-validation shows that the ordinary kriging based on seasonal semi-variograms gives the best performance, closely followed by the inverse distance weighting with a power of 2. Finally the ordinary kriging is chosen to interpolate the station data to a 18 km× 18 km grid system covering the whole country. Precipitation for each 0.5°×0.5° latitude-longitude block is then obtained by averaging the values at the grid nodes within the block. Owing to the higher station density in the eastern part of the country, the interpolation errors are much smaller than those in the west （west of 100°E）. Excluding 145 stations in the western region, the daily, monthly, and annual relative mean absolute errors of the interpolation for the remaining 608 stations are 74%, 29%, and 16%, respectively. The interpolated daily precipitation has been made available on the internet for the scientific community.

Spermatogenesis is a tightly regulated process of development for the generation of mature spermatozoa. Mammalian spermatogenesis occurs in the convoluted seminiferous tubules of the testes, where spermatogenic stem cells （SSCs） reside on sustentacular Sertoli cells of somatic origin, forming a germinal epithelial structure. Spermato genesis is accomplished in a complicated, timely controlled differentiation of SSCs into premeiotic primary spermatocytes, followed by formation of postmeiotic round spermatids and their subsequent maturation into spermatozoa.

Aim:To describe the current practise of living donor liver transplantation(LDLT)for hepatocellular carcinoma(HCC),including the patient selection criteria,surgical techniques,management of small-for-size syndrome,postoperative complications,and the results of our units,in the Liver Transplant Centre of Queen Mary Hospital,Hong Kong,one of the high-volume centres for LDLT in ***:Our centre practises careful selection for HCC patients using the University of California,San Francisco(UCSF)criteria,supplemented by alpha-fetoprotein level and the model for end-stage liver disease *** flexibility is offered to enthusiastic donors and recipients in LDLT while balancing the risks and *** pioneered in using the extended right lobe graft and the novel hepatic venoplasty technique,which lessen the risk of hyperperfusion and small-for-size syndrome with improved overall recipient *** were collected prospectively and presented as the mean values and ranges,or the number of patients in proportion of total patient ***:Of our patients,74.9% met the UCSF criteria,and 64.5% met the Milan criteria.A 5-year overall and disease-free survival rate of 78.9% and 76.3% were ***:LDLT is an ideal treatment for HCC in Hong Kong with regard to the critical organ shortage and high demand for *** current surgical techniques and post-transplant surveillance contribute to the positive outcome.

AIM To analyze the outcomes of living-donor liver transplantation(LDLT) using left-lobe(LL) or right-lobe(RL) small-for-size(SFS) *** Prospectively collected data of adult patients who underwent LDLT at our hospital in the period from January 2003 to December 2013 were reviewed. The patients were divided into the RL-LDLT group and the LL-LDLT group. The two groups were compared in terms of short-and long-term outcomes, including incidence of postoperative complication, graft function, graft survival, and patient survival. A SFS graft was defined as a graft with a ratio of graft weight(GW) to recipient standard liver volume(RSLV)(GW/RSLV) of < 50%. The Urata formula was used to estimate *** Totally 218 patients were included for analysis, with 199 patients in the RL-LDLT group and 19 patients in the LL-LDLT group. The two groups were similar in terms of age(median, 53 years in the RL-LDLT group and 52 years in the LL-LDLT group, P = 0.997) but had significantly different ratios of men to women(165:34 in the RL-LDLT group and 8:11 in the LL-LDLT group, P < 0.0001). The two groups were also significantly different in GW(P < 0.0001), GW/RSLV(P < 0.0001), and graft cold ischemic time(P = 0.007). When it comes to postoperative complication, the groups were comparable(P = 0.105). Five patients died in hospital,4(2%) in the RL-LDLT group and 1(5.3%) in the LLLDLT group(P = 0.918). There were 38 graft losses, 33(16.6%) in the RL-LDLT group and 5(26.3%) in the LL-LDLT group(P = 0.452). The 5-year graft survival rate was significantly better in the RL-LDLT group(95.2% vs 89.5%, P = 0.049). The two groups had similar 5-year patient survival rates(RL-LDLT: 86.8%, LL-LDLT: 89.5%, P = 0.476).CONCLUSION The use of SFS graft in LDLT requires careful tailormade surgical planning and meticulous operation.
