irritable bowel syndrome subtypes differ in body awareness, psychological symptoms and biochemical stress markers

AiM: To elucidate the differences in somatic, psycho-logical and biochemical pattern between the subtypes of irritable bowel syndrome (iBS). METHODS: Eighty iBS patients, 30 diarrhoea pre-dominant (D-iBS), 16 constipation predominant (C-iBS) and 34 alternating iBS (A-iBS) underwent physi-otherapeutic examinations for dysfunctions in body movements and awareness and were compared to an apparently healthy control group (AHC). All groups an-swered questionnaires for gastrointestinal and psycho-logical symptoms. Biochemical variables were analysed in blood. RESULTS: The D-iBS group showed less body aware-ness, less psychological symptoms, a more normal sense of coherence and psychosocial rating as well as higher C-peptide values. C-iBS had a higher degree of body dysfunction and psychological symptoms, as well as the lowest sense of coherence compared to controls and D-iBS. They also demonstrated the most elevated prolactin levels. A-iBS had the lowest degree of body disturbance, deteriorated quality of life and affected bi-ochemical pattern. All subtypes had higher pain scores compared to controls. in addition they all had signifi -cantly increased triglycerides and elevated morning cortisol levels, however, without statistical signifi cance compared with the ***: iBS subtypes showed different pro-files in body awareness, somatic and psychological symptoms and in biochemical variables. D-iBS differed compared to the other groups by lowered body aware-ness, less psychological symptoms and a higher sense of coherence and elevated C-peptide values. C-iBS and A-iBS subtypes suffered more from depression and anxiety, associated with a lower quality of life. These differences may be important and will be taken into account in our treatment of these patients.

Aspects of the non-pharmacological treatment of irritable bowel syndrome

irritable bowel syndrome(iBS) is one of the most commonly diagnosed gastrointestinal conditions. it represents a significant healthcare burden and remains a clinical challenge. Over the years iBS has been described from a variety of different perspectives; from a strict illness of the gastrointestinal tract(medical model) to a more complex multi-symptomatic disorder of the brain-gut axis(biopsychosocial/psychosomatic model). in this article we present aspects of the pathophysiology and the non-pharmacological treatment of iBS based on current knowledge. Effects of conditioned stress and/or traumatic influences on the emotional system(top-down) as well as effects on the intestine through stressors,infection,inflammation,food and dysbiosis(bottom-up) can affect braingut communication and result in dysregulation of the autonomic nervous system(AnS),playing an important role in the pathophysiology of iBS. Conditioned stress together with dysregulation of the autonomic nervous system and the emotional system may involve reactions in which the distress inside the body is not recognized due to low body awareness. This may explain why patients have difficulty identifying their symptoms despite dysfunction in muscle tension,movement patterns,and posture and biochemical functions in addition to gastrointestinal symptoms. iBS shares many features with other idiopathic conditions,such as fibromyalgia,chronic fatigue syndrome and somatoform disorders. The key to effective treatment is a thorough examination,including a gastroenterological examination to exclude other diseases along with an assessment of body awareness by a body-mind therapist. The literature suggests that early interdisciplinary diagnostic cooperation between gastroenterologists and body-mind therapists is necessary. Re-establishing balance in the AnS is an important component of iBS treatment. This article discusses the current knowledge of body-mind treatment,addressing the topic from a practical point of view

Arctic sea-ice extent: no record minimum in 2023 or recent years

Arctic sea-ice extent reaches its minimum each year in September. On 11 September 2023 the minimum was 4.969 million square kilometers(***^(2)). This was not a record low, which occurred in 2012, when the minimum was 4.175 ***^(2), 0.794 ***^(2) less than the minimum in 2023. However, the ice extent had decreased by 0.432 ***^(2) compared with 2022. nevertheless, the summer melting in 2023 was remarkably less than expected when considering the strong heat waves in the atmosphere and ocean, with record temperatures set around the world. in general, there is a high correlation between the long-term decrease in sea-ice extent and the increasing CO_(2) in the atmosphere, where the increase of CO_(2) in recent decades explains about 80% of the decrease in sea ice in September, while the remainder is caused by natural variability.

This study investigates the effect of aluminium oxide nanoparticlesaddition to biodiesel blends made from dairy milk scum on performance and combustion characteristics of the diesel *** dispersion of Al2O3nanoparticles in B20 blends at different concentrations was done with the help of *** number of blends were prepared for the *** Machine learning algorithms(Random Forest（RF）and CatBoost) was used for the *** results show that in comparison to biodiesel blends without nanoparticles,the kinematic viscosity and density is higher for fuel blends with *** these Fuel blends have higher calorific *** blends exhibited reduced Brake Specific Fuel Consumption（BSFC） of2.85% than the blends without nano particles(Dairy Scum Methyl Ester Biodiesel 20%+neat Diesel 80%（in volume）,DSMEB20),57.14% less CO,40.8% less hydrocarbon,and increased nOxemissions compared to conventional diesel,contributing to the development of environmentally friendly and renewable biofuel blends with *** B20nP30 is the optimal blend for performance and emission *** study concludes with findings on enhanced Brake Thermal Efficiency（BTE） of 26.29% in 3×10-5（in volume） Al2O3nanoparticle-blended DSME B20 and other DSME B20 fuel blends,emphasizing the importance of optimal nanoparticle *** correlation matrix shows how engine load,efficiency measures（BTE,BSFC）,and emissions（CO,CO2,nOx,Smoke） are connected in complex *** results help us understand the complicated dynamics of engine performance and emission characteristics ***'s diagram for BTE and BSFC shows that CatBoost-based BTE models perform superior to RF-based models during the training as well as testing *** results were obtained for CO and CO2emission results.

Dedifferentiated fat cells:A cell source for regenerative medicine

The identification of an ideal cell source for tissue regeneration remains a challenge in the stem cell field. The ability of progeny cells to differentiate into other cell types is important for the processes of tissue reconstruction and tissue engineering and has clinical, biochemical or molecular implications. The adaptation of stem cells from adipose tissue for use in regenerative medicine has created a new role for adipocytes. Mature adipocytes can easily be isolated from adipose cell suspensions and allowed to dedifferentiate into lipidfree multipotent cells, referred to as dedifferentiated fat(DFAT) cells. Compared to other adult stem cells, the DFAT cells have unique advantages in their abundance, ease of isolation and homogeneity. Under proper condition in vitro and in vivo, the DFAT cells have exhibited adipogenic, osteogenic, chondrogenic, cardiomyogenc, angiogenic, myogenic, and neurogenic potentials. in this review, we first discuss the phenomena of dedifferentiation and transdifferentiation of cells, and then dedifferentiation of adipocytes in particular. Understanding the dedifferentiation process itself may contribute to our knowledge of normal growth processes, as well as mechanisms of disease. Second, we highlight new developments in DFAT cell culture and summarize the current understanding of DFAT cell properties. The unique features of DFAT cells are promising for clinical applications such as tissue regeneration.

Bleeding risk assessment in elderly patients with acute coronary syndrome
