i-Gel Supraglottic Airway for Inexperienced Anesthesia Residents

i-gel (Intersurgical Ltd., Wokingham, UK) is a new supraglottic airway device with a cuff made of thermoplastic elastomer gel. We retrospectively studied easiness of insertion and troubles in ventilation when this device was used by less experienced anesthesiologists. Consecutive eleven cases undergoing surgery under general anesthesia were studied. i-gel was successfully inserted in 7 cases (63%) at the first attempt, 2 cases at the second attempt without changing the size. In one case, the size was changed at the second attempt, which resulted in the successful insertion. There was one case of failed insertion even at the second attempt. Fiberoptic observation of the glottis showed direct contact of the cuff to the arytenoids cartilage in cases with successful insertion, however, spastic glottis, defined as contact of vocal cords was observed in 4 cases, and down-folding of the epiglottis was observed in 2 cases. During maintenance of anesthesia, ventilation trouble was observed in 3 cases. We conclude that i-gel should only be used with sufficient knowledge, preparations, skills or support from experienced physician to solve problems related to the safe use of supraglottic airway devices.

Background Although adhesive strapping(AS)for pediatric umbilical hernia(UH),which was once obsolete,has been reconsidered as a common practice in Japan,its efficacy is still *** study aimed to evaluate its efficacy by reviewing related *** A comprehensive literature search of PubMed,Cochrane,Google Scholar,and Igaku Chuo Zasshi via Ichushi-Web was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses *** studies reporting on the UH closure rate after AS compared with observation-only management were *** A total of 10 cohort studies were included,and the overall UH closure rate was not statistically significant(p=0.31,risk ratio(RR)=0.76,95%confidence interval(CI)0.45 to 1.28).However,there were significant differences in the UH closure rate at the age of 6 months(p<0.01,RR=0.55,95%CI 0.41 to 0.75)and the efficacy of preventing protruding umbilici with redundant skin(p=0.049,RR=0.16,95%CI 0.03 to 0.99).Conclusions Although the efficacy of AS on UH compared with observation-only management did not differ in terms of the UH closure rate,the application of AS may be effective for faster UH closure and the prevention of protruding ***,due to the high heterogeneity of the study,further large-scale studies,particularly randomized controlled trials,are warranted to reach a conclusion.

To analyze the molecular events that occur in the developing mandible, we examined the expression of 8803 genes from samples taken at different time points during rat postnatal mandible development. Total RNA was extracted from the mandibles of 1-day-old, 1-week-old, and 2-week-old rats. Complementary RNA (cRNA) was synthesized from cDNA and biotinylated. Fragmented cRNA was hybridized to RGU34A GeneChip arrays. Among the 8803 genes tested, 4344 were detectable. We identified 148 genes with significantly increased expression, and 19 genes with significantly decreased expression. A comprehensive analysis appears to be an effective method of studying the complex process of development.

When individuals are not good at grasping coincidence events in the gambling mechanism,they take particular note of lay beliefs in *** the lay beliefs in luck,that concerning“strength of luck”is often specific for gambling *** conducted a survey that compared gamblers and non-gamblers,and examined whether this way of thinking is related to gambling *** Internet survey was completed by 550 people.A cluster analysis based on gambling frequency extracted three clusters(non-,moderate-,and heavy-gambler groups).The South Oaks Gambling Screen(SOGS)score was highest in the heavy-gambler *** were seen in three items:a belief in differences in luck among individuals;a belief in the“flow of Tsuki(in luck)”;and conscious behaviors regarding *** all three,the scores of the moderate-and heavy-gambler groups tended to be higher than those of the non-gambler ***“strength of luck”and“strength of luck in gambling”,the heavy-gambler group perceived that they had the strongest luck,followed,in order,by the moderate-and non-gambler *** three factors could lead to sustained gambling *** who felt that their“strength of luck in gambling”was strong tended to be pathological *** result supports a previous study.

Constraining the timing of fault zone formation is fundamentally important in terms of geotectonics to understand structural evolution and brittle fault *** paper presents the first authigenic illite K-Ar age data from fault gouge samples collected from the Red River Shear Zone at Lao Cai province,*** fault gouge samples were separated into three grain-size fractions（〈0.1 μm,0.1-0.4 μm and 0.4-1.0 μm）.The results show that the K-Ar age values decrease from coarser to finer grain fractions（24.1 to 19.2 Ma）,suggesting enrichment in finer fraction of morerecently grown authigenic *** timing of the fault movement are the lower intercept ages at 0%detrital illite（19.2 ± 0.92 Ma and 19.4 ± 0.49 Ma）.In combination with previous geochronological data,this result indicates that the metamorphism of the Day Nui Con Voi（DNCV） metamorphic complex took place before ca.26.8 *** about 26.8 Ma-25 Ma,the fault strongly acted to cause the rapid exhumation of the rocks along the Red River-Ailoa Shan Fault Zone（RR-ASFZ）.During brittle deformation,the DNCV slowly uplifted,implying weak movement of the *** brittle deformation might have lasted for ca.5 Ma.
