Epidemiological Aspects of Diseases Seen in Pediatric Surgery Consultation at the Kara Teaching Hospital

Background: Pediatric surgery in developing countries faces serious problems of inadequate infrastructures and equipment. Since July 2019, a new department was created in the north of Togo. It is in this context that this study is initiated, with the aim of taking stock of the epidemiological distribution of pediatric surgical diseases. Material and methods: This was a retrospective and descriptive study over a period of eighteen months, from October 30, 2019 to April 30, 2021. It took place in the pediatric surgery department of the Kara teaching hospital. It concerns pathologies seen at the consultation in the department collected in consultation register. Epidemiological parameters were studied. Results: In eighteen (18) months, two hundred and fifty-nine children, were seen in pediatric surgery consultation at the Kara Teaching Hospital. The annual frequency of consultations was 172.66. The monthly frequency was 14.39. The average age of the children was 4.56 years with extremes of 2 days and 15 years. There were 194 boys (74.90%) and 65 girls (25.10%). At the consultation, whatever the reason, the examination was normal in 17 children and 242 children had abnormalities. Two hundred and twelve children (81.85%) had each one abnormality and 30 children (11.58%) had each at least two. A total of 298 diseases were reported in the 242 children. These were 218 congenital diseases (73.15%) dominated by urological abnormalities with 37.16% of cases and 80 acquired diseases (26.85%). Pelagic fish,including sardines and anchovies(Order Clupeiformes),are the most common species taken by artisanal marine fisheries along the Togolese *** investigated fisher involvement as well as fish captures over a period of 10 years,particularly of the European anchovy(Engraulis encrasicolus).Our results indicate that 60%of fishers operated from the Loméfishing harbour,most fishers being Ghanaians working seasonally in Togo.63.7%of all the fishers used canoes with outboards,a higher percentage compared to the previous *** fishing gear type were identified,with bottom gillnet and surface gillnet being the most commonly ***,in the most important fishing camp in the country in terms of fish production(Loméfishing harbour),all fishers used shark ***,fisheries catches did not change significantly across years,but anchovy fishing effort and catch per unit of effort(CPUE)declined over the study period,suggesting some depletion of the species stocks on the Togolese coast and a demotivation of fishers.

Place of Selective Tubal Catheterization in the Management of Female Infertility in Togo

Objective: To determine the effectiveness of selective tubal catheterization in the management of female infertility due to proximal tubal obstruction. Method: This was a longitudinal descriptive study, conducted over a period of 24 months, which included 73 patients presenting with objectified bilateral proximal tubal obstruction after standard HSG. The intervention was performed on an outpatient basis, during the follicular phase with negative β-hCG assay the day before, in the interventional radiology room and under antibiotic coverage. Confirmatory hysterosalpingography was performed as the first step followed by selective tubal catheterization after the failure of spontaneous tubal opacification. The parameters studied related to socio-epidemiological, clinical and radiological data. Results: The age of our patients was between 24 and 42 years with an average of 33.97 years. The average duration of infertility was 3.95 years, with a predominance of primary infertility in 83.56% of cases. Voluntary termination of pregnancy (38.89%) and fibromyomas (33.33%) were the most represented gynecological-obstetrical antecedents. Selective tubal catheterization was successful in 92.14% of cases (129/140 tubes). It was possible bilaterally in 93.02% of cases and unilaterally in 6.98% of cases. The confirmatory HSG allowed a spontaneous opacification of 4.10% of the fallopian tubes. At the end of the procedure, all the recanalized tubes were opacified;62.01% of them were normal, against 37.99% pathological with a preponderance of inflammatory tubes 26.61% followed by hydrosalpinx in 5.03% of cases. No major complications were encountered. The fertility rate was 23.29%. Conclusion: Selective tubal catheterization is a simple technique, without major complications with an efficiency close to natural fertility. It should be proposed as the first intention before any other procedure in the treatment of infertility by proximal tubal obstruction.

Study of Descriptive Determinants of Moral Harassment at Work in a Factory in Senegal

Introduction: Globalization and the attendant contemporary system of work organization, industrial competitiveness and workplace hostility could be a source of stress. Behaviors considered to be hostile tend to constitute the foundation on which moral harassment at work rests and contributes significantly to work place mental health. Methodology: We conducted a quantitative, cross sectional descriptive study between April 1 and June 30, 2018 at the Grands Moulins de Dakar “GMD”. We used a questionnaire containing the socio-demographic data to which we added the Masson Meret and Steiner questionnaire called, “Ambiance in professional environment”. We used the Epi info software for data entry and analysis. Results: Sample population was 100 subjects with average age of 42.18 years ± 7.6, a male predominance of 91% and average length of service in the company was 15.27 years ± 10.1. Hostile behavior was more frequently reported than stress, with 49% and 28% respectively. Among the hostile behavior we found criticism in the professional field in 35% of the cases and rumors mongering was observed in 25% of the cases. The most reported stress symptom was general fatigue (45%), hostile behavior was more common in males (40 - 49 years of age and those with more than 10 dependents). Of the officers who felt generally unwell, 78.9% were victims of hostile behavior. Conclusion: Hostile behavior forms the basis of moral harassment at work and their devious nature makes them difficult to detect, this contrasts with its harmful consequences on the health of the staff. Potentially harmful workplace stress has become more frequently encountered in the contemporary workplace and has constituted an occupational hazard. The most common symptoms reported are feeling unwell and general fatigue.

Multifocal Tuberculosis Revealed by Facial Paralysis

Introduction: Tuberculosis is a chronic infectious disease that remains a global public health problem. Children, accounting for 10% of cases, are particularly vulnerable to this disease. Based on a case of multifocal tuberculosis in a 14-month-old infant, the authors conducted a literature review on this condition. Observation: The case involved a 14-month-old infant referred from a health center for persistent symptoms including anemia, fever, and lateral deviation of the mouth during crying. Clinical examination revealed an infectious and meningeal syndrome, as well as facial paralysis. The results of brain imaging, cerebrospinal fluid examination, fundus examination, and gastric lavage fluid examination led to the conclusion of multifocal tuberculosis in the infant without association with HIV. Improvement was observed one week after the start of antituberculosis treatment, with complete recovery without sequelae after 12 months. Conclusion: Although rare, multifocal tuberculosis does exists and its diagnosis remains challenging in infants. Early treatment generally leads to favorable outcomes, especially in the absence of HIV coinfection.

Introduction: According to the WHO, suicide is a real public health problem worldwide, and several risk factors have been identified, including pain and chronic illness. Objective: The aim of this study was to assess the suicidal risk in sickle-cell patients. Method: This was a descriptive and analytical cross-sectional study of sickle-cell patients followed up at the University Clinic of Blood Diseases of Hubert Koutoukou Maga National University Hospital Center from August 2022 to November 2022. The instrument used was the Ducher suicide risk self-assessment scale. Results: A total of 65 patients were included. Mean age was 27.04 ± 1.95 years, with extremes of 16 and 50 years, and a peak of 47.69% in the 20 - 30 age group. Women predominated (58.46%), with a sex ratio of 0.71. Somatic complaints accounted for 41.54% of patients’ visits, including 21.54% for pain. Suicidal risk was positive in 44.62%, including 13.85% for severe suicidal risk, and 20% attributed their desire to die to pain. The factor associated with positive suicidal risk was single parenthood (p = 0.02). Of those with a positive suicidal risk, 46% had shared the intention with close relatives (family and friends), but none had used medical services. Conclusion: These data demonstrate the high prevalence of suicidal risk among people with sickle cell disease, and therefore the need for psychosocial intervention in this specific population in Benin.

Objective:?To describe blood transfusion practices in obstetric at the Sylvanus Olympio University Hospital in Lomé. Patients and method: Prospective, cross-sectional and descriptive study, conducted from 1 June 2017 to 31 May 2018 at the gynecology-obstetrics department. The study focused on transfused pregnant, parturient and delivered women. Study parameters were sociodemographic, clinical, therapeutic and prognostic. Results: 252 patients, aged 16 to 49 with an average age of 30.4 years, referred in 74% of cases. Hemorrhage was observed in 238 cases (94.4%) and anemia in 14 cases (5.6%). Blood transfusion was urgently used in 89.7% of cases and hemorrhagic abortion was the main indication. Blood group O+ (34.5%) was the most used and blood products were obtained after two hours in 81% of cases. Conclusion: Blood transfusion is often performed urgently and for bleeding. But the Gynecology-Obstetrics department does not have any blood in reserve, whereas the Sylvanus Olympio University Hospital collection and distribution station covers less than 10% of the needs.

Background: Neonatal sepsis is a global health problem that mainly affects low- and middle-income countries. We have previously shown that early neonatal mortality is high at the Ho Teaching Hospital (HTH) of Ghana. We sought to determine the prevalence of neonatal sepsis, sepsis-related mortality, and bacterial species patterns in neonatal and young infant sepsis in this hospital. Methods: A hospital-based study was conducted in the hospital’s neonatal intensive care unit (NICU) from March to June 2018. Blood samples from 96 babies clinically diagnosed with or at risk of sepsis were cultured using the BACTEC 9050® machine. Clinical data including gravida, parity and antibiotic medication before delivery of mother and delivery type, gestation, birth weight and antibiotic medication status were collected for analysis. MALDI-TOF MS identified bacterial isolates, and their identities were confirmed via tuf gene sequence typing. The data were analyzed using GraphPad Prism 8.0.2. Results: Blood cultures were positive in 28 of the babies, with 14 and 12 representing early-onset and late-onset neonatal sepsis, respectively, and two cases of unknown sepsis type. Lumbar Spondylolisthesis in Rheumatological Practice in Lome (Togo): Frequency and Semiological Profile

Objectives: To determine the frequency, the clinical and radiological features of lumbar spondylolisthesis in a rheumatological practice in Lomé. Patients and methods: This was a cross-sectional study conducted in the rheumatology department of the Sylvanus Olympio Teaching Hospital in Lomé between January 1997 and December 2019. Medical records of patients with symptomatic spondylolisthesis with radiographic confirmation were reviewed. The Meyerding classification system was used for grading. Results: Of the 350 patients contacted, 118 consented to participate in the study, making a response rate of 33.71%. Of the 118, 104 (88.1%) were females. 109 (92.4%) presented with radicular pain and 9 (7.6%) presented low back pain. The mean age of the patients was 54.24 ± 11.19 years. The average duration of the pain was 4.56 ± 1.32 years. The spondylolisthesis level most involved was the L4 - L5 (92/118), followed by L5 - S1 (23/118). 106 (89.8%) patients had anterolisthesis only, seven (5.9%) combination anterolisthesis and retrolisthesis. The spondylolisthesis was associated with spondylosis in 110 (93.2%) patients. Spondylosis was found at level L4 - L5 in 38.98% patients, followed by L5 - S1 (24.58%), L3 - L4 (18.64%). Grade I slip was found in 108 (91.53%) patients and Grade II slips 10 (8.47%). Isthmic lysis was found in 40 (33.9%) patients and it was located at L4 in 30 (75.0%). Conclusion: Lumbar spondylolisthesis appears to be expected in Lomé patients with nonspecific back pain and mainly affects females.
