The adsorption/plasma decomposition with the combination of adsorption honeycomb-sheets and a plasma element is a new technology for small-sized apparatuses to decompose volatile organic compounds （VOCs） at concentrations lower than about 100 ppm. The feasibility of the prototype adsorption/plasma decomposition apparatus was evaluated with the simulated exhausts containing one VOC component and with real exhausts from a painting booth and an adhesion factory. The apparatus decomposed VOCs effectively at the painting booth exhaust but not always satisfactorily at the adhesion factory exhaust. The performance test results with real exhausts were discussed with respect to the concentration and discharge pattern of the exhausts and the basic properties of the system such as cooperation of adsorption and plasma reaction and the concentration dependence of the performance.

BACKGROUND Ventricular arrhythmias,such as ventricular tachycardia and fibrillation,are the main causes of death in patients with aconite poisoning.CASE SUMMARY A 51-year-old man presented to our emergency department because he was vomiting after ingesting aconite root to attempt suicide.On arrival,the patient was hemodynamically unstable,and his electrocardiogram revealed polymorphic ventricular extrasystoles and non-sustained ventricular tachycardia.Amiodarone was immediately administered for ventricular arrhythmia.However,the patient remained unresponsive.We administered continuous intravenous landiolol as the ventricular arrhythmia worsened,gradually suppressing it.The patient returned to sinus rhythm 16 h after arriving at the hospital.Some aconitum alkaloids act on voltage-gated Na+-channels and induce ventricular or supraventricular tachyarrhythmias.Landiolol suppresses sympathetic nerve activity through its blocking effect,preventing arrhythmia.CONCLUSION Landiolol can be a therapeutic option for amiodarone-refractory ventricular arrhythmias caused by aconite intoxication.

Objective The Japan Coma Scale(JCS)is widely used in clinical practice to evaluate levels of consciousness in Japan.There have been several studies on the usefulness of JCS in adults.However,its usefulness in evaluating children has not been reported.Therefore,this study aimed to assess the usefulness of the JCS for the prediction of mortality in children.Methods This is a multicenter cohort study which used data from a national trauma registry(Japan Trauma Data Bank).This study included patients under 16 years of age who were treated between 2004 and 2015.The primary outcome measure was in-hospital mortality.Two models were used to examine each item of the Glasgow Coma Scale(GCS)and the JCS.Model A included the discrete levels of each index.In model B,data regarding age,sex,vital signs on arrival to hospital,the Injury Severity Score,and blunt trauma were added to each index.The effectivity of the JCS score was then evaluated using the area under the curve(AUC)for discrimination,a calibration plot,and the Hosmer-Lemeshow test for calibration.Results A total of 9045 patients were identified.The AUCs of the GCS and JCS were 0.929(95%confidence interval(CI)0.904 to 0.954)and 0.930(95%CI 0.906 to 0.954)in model A and 0.975(95%CI 0.963 to 0.987)and 0.974(95%CI 0.963 to 0.985)in model B,respectively.The results of the Hosmer-Lemeshow test were 0.00(p=1.00)and 0.00(p=1.00)in model A and 4.14(p=0.84)and 8.55(p=0.38)in model B for the GCS and JCS,respectively.Conclusions We demonstrated that the JCS is as valid as the GCS for predicting mortality.The findings of this study indicate that the JCS is a useful and relevant tool for pediatric trauma care and future research.

Objective:To assess the incidence of asymptomatic unruptured renal artery pseudoaneurysm(RAP)on contrast-enhanced computed tomography(CE-CT)after robot-assisted partial nephrectomy(RAPN)without parenchymal renorrhaphy.Methods:From May 2016 to December 2017,78 patients underwent RAPN for renal tumors.Inner suture was performed in the opened collecting system or renal sinus,whereas parenchymal renorrhaphy was not.For hemostasis,the soft coagulation system was used,and absorbable hemostats were placed on the resection bed.CE-CT was carried out within 7 days after surgery.Data on these patients were prospectively collected.A single radiologist determined the diagnosis of RAP.Results:Median(range)data were as follows:Patient age,65(19-82)years;radiographic tumor size,30(12-95)mm;operating time,166(102-294)min;warm ischemic time,16(7-67)min;and blood loss,15(0-4450)mL.One patient(1.6%)required a perioperative blood transfusion.No patient required conversion to open surgery or nephrectomy.CE-CT was carried out at median 6(3-7)days after surgery.CE-CT showed no RAP development in all 61 patients.Urinary leakage was not observed.One patient had acute cholecystitis,a postoperative complication classified as Clavien-Dindo grade higher than 3,which was treated with cholecystectomy.Positive surgical margin was identified in four patients(6.6%).Conclusion:RAPN using soft coagulation and absorbable hemostats without renorrhaphy appears to be feasible and safe.Our technique could eliminate the risk of RAP.

High circulating tumor cell concentrations in a specific subtype of gastric cancer with diffuse bone metastasis at diagnosis

AIM: To clarify the biological feature contributing to gastric cancer with diffuse bone metastases at diagnosis.METHODS: The participants visited the Department of Clinical Oncology, Akita University Hospital, from January 2014 to August 2015. The selection criterion for gastric cancer with diffuse bone metastases at diagnosis includes over 29 hot spots of bone scintigraphy. Circulating tumor cell were collected from 20 m L of peripheral venous blood drawn using a Cell Search kit and a Cell Tracks Auto Prep system by SRL, a clinical laboratory. The endpoints of this study were correlations between circulating tumor cells(CTC) count and therapeutic outcomes. RESULTS: Among 39 patients with gastric cancer, 5 patients met the criterion. The incidence of this subtype was 12.8%. CTC counts ranged from 235 to 6440 cells/7.5 m L of peripheral blood(median of 1724). These values were much higher than common gastric cancers(2 cells). In chemo-sensitive cases, CTC counts decreased within 14 d(median) from 275, 235 and 1724 to 2, 7 and 66, respectively. On the other hand, CTC counts increased after treatment failure or insensitive case from 2, 7 and 6440 to 787, 513 and 7885, respectively. The correlation between CTC count and survival time showed a trend, but did not reach significance(Y = 234.6- 0.03 X, P = 0.085).CONCLUSION: High CTC count is a biological hallmark of this subtype, and can be used as a direct and definitive indicator of therapeutic outcome.

Co_(3)O_(4) was synthesized on carbon paper(CP)using a facile method to improve electrochemical nitrate-toammonia conversion efficiency.The resulting Co_(3)O_(4)-CP electrode demonstrated an exceptional Faradaic efficiency of almost 100% across a broad range of application conditions,with a peak NH3 yield of 3.43 mmol h^(-1) cm^(-2)(2.25 mol gCo^(-1) h^(-1)).

Continuous renal replacement therapy (CRRT) is the preferred dialysis modality in critical care settings for patients with hemodynamic instability. Nafamostat mesylate (NM) is an anticoagulant commonly used (mainly in Japan) during CRRT in patients with high bleeding risk. In this study, we evaluated the pharmacokinetics of NM during CRRT. Patients undergoing CRRT therapy and using NM as the anticoagulant in the intensive care unit were enrolled in the study. Blood was collected from the CRRT circuit just after blood removal, just before and after the membrane for CRRT, and from the filtrates after the membrane. NM concentrations were measured using high-performance liquid chromatography. NM was detected in the intracorporeal circulation during CRRT in some cases, and liver enzymes were severely elevated in almost all of the cases. Coagulation time was prolonged even before the initiation of NM administration in these cases and may be associated with liver damage. This study suggests that NM dosage should take into account liver damage assessed by elevated liver enzymes.

Investigation of Inspiratory Circuit Condensation due to Room Temperature in a Heated Humidifier during Mechanical Ventilation

Heating and humidification control during mechanical ventilation is important. Water condensation can occur due to insufficient humidification. Condensation in the breathing circuit can cause various adverse events. In this study, we investigated whether the occurrence of tubing condensation in the breathing circuit could be affected by room temperature using a simulated respiratory circuit with a humidifier. The room temperature was set at 23°C, 25°C, and 27°C, and mechanical ventilation was performed for 8 h. The inspired gas was appropriately heated and humidified according to the manufacturer’s instructions. The weight of the circuit was measured every 2 h at room temperature to estimate the amount of condensation. During the mechanical ventilation, condensation continued to increase at a room temperature of 23°C, but only for the first 2 h at 25°C, and did not occur at 27°C. The room temperature of the indoor environment was one of the factors generating water condensation in the breathing circuit with a humidifier even when a heater wire in the breathing circuit was appropriately operated.

E26 transformation-specific-1 （ETS-1）, an ETS family transcription factor, has been reported to play an important role in a variety of physiological and pathological processes, but clinical implications of ETS-1 expression in prostate cancer （PCa）, particularly high-risk cases, including response to androgen-deprivation therapy （ADT） have yet to be elucidated. We examined the expression of ETS- 1 using immunohistochemical staining of paraffin-embedded prostate carcinoma tissue obtained by needle biopsy from 69 mostly advanced PCa patients. ETS-1 expression was compared with the clinicopathological characteristics of the 69 patients, including 25 who underwent ADT as a primary treatment. As a result, PCa patients with higher expression of ETS-1 were significantly more likely to be of high stage and high Gleason score （P〈O.05）. There was no significant association between ETS-1 expression and the initial prostate-specific antigen （PSA） level. In the 25 patients treated by ADT, the staining score for ETS-1 was significantly associated with rapid development of castration-resistant disease within 24 months （P〈O.05）, whereas the Gleason score and PSA level were not. In conclusion, increased ETS-1 expression was associated with a higher stage, higher Gleason score and shorter time to castration-resistant progression. These data suggest that immunostaining for ETS-1 could be a molecular marker for predicting a poor clinical outcome for PCa patients, oarticularlv those with hi^h-risk disease.

Beh et's disease complicated by multiple aseptic abscesses of the liver and spleen

Aseptic abscesses are an emergent entity and have been described in inflammatory bowel disease,especially in Crohn's disease,and in other diseases.However,aseptic abscesses associated with Beh et's disease are extremely rare.We report a Japanese male diagnosed with an incomplete type of Beh et's disease who developed multiple aseptic abscesses of the spleen and liver.In 2002,the spleen abscesses were accompanied by paroxysmal oral aphthous ulcers and erythema nodosum.As the patient's response to antibiotic treatment was inadequate,a splenectomy was performed.Severe inflammatory cell infiltration,largely of polymorphonuclear neutrophils,was observed without evidence of bacterial or fungal growth.Although the patient had no history of ocular symptoms or genital ulcers,a diagnosis of incomplete Beh et's disease was made according to the Japanese diagnostic criteria because of the presence of paroxysmal arthritis and epididymitis since 2002.In 2005,multiple liver abscesses developed with right hypochondrial pain and seemed to be attributed to Beh et's disease because the abscesses yielded negative results during a microbiologic investigation and failed to go into remission under antibiotic therapy.Oral prednisone(15 mg/d) was started in May 2006,and the abscesses dramatically disappeared 4 wk after treatment.Although the patient had a relapse of the liver abscesses in association with the tapering of prednisone,the augmentation of prednisone dosage yielded a response.The abscesses of the liver and spleen were strongly suggested to be attributed to Beh et's disease.Clinician should be aware of the existence of aseptic abscesses as uncommon manifestations of Beh et's disease.
