Clinicopathological features of alpha-fetoprotein producing early gastric cancer with enteroblastic differentiation

AIM To investigate clinicopathological features of early stage gastric cancer with enteroblastic differentiation(GCED).METHODS We retrospectively investigated data on 6 cases of early stage GCED and 186 cases of early stage conventional gastric cancer(CGC: well or moderately differentiated adenocarcinoma) who underwent endoscopic submucosal dissection or endoscopic mucosal resection from September 2011 to February 2015 in our *** was defined as a tumor having a primitive intestine-like structure composed of cuboidal or columnar cells with clear cytoplasm and immunohistochemical positivity for either alpha-fetoprotein, Glypican 3 or SALL4. The following were compared between GCED and CGC: age, gender, location and size of tumor, macroscopic type, ulceration, depth of invasion, lymphatic and venous invasion, positive horizontal and vertical margin, curative resection *** Six cases(5 males, 1 female; mean age 75.7 years; 6 lesions) of early gastric cancer with a GCED component and 186 cases(139 males, 47 females; mean age 72.7 years; 209 lesions) of early stage CGC were investigated. Mean tumor diameters were similar but rates of submucosal invasion, lymphatic invasion, venous invasion, and non-curative resection were higher in GCED than CGC(66.6% vs 11.4%, 33.3% vs 2.3%, 66.6% vs 0.4%, 83.3% vs 11% respectively, P < 0.01). Deep submucosal invasion was not revealed endoscopically or by preoperative biopsy. Histologically, in GCED the superficial mucosal layer was covered with a CGC component. The GCED component tended to exist in the deeper part of the mucosa to the submucosa by lymphatic and/or venous invasion, without severe stromal reaction. Minute signet ring cell carcinoma occurring in gastric hyperplastic polyp

We describe a 45-year-old woman with minute signet ring cell carcinoma occurring in a gastric hyperplastic polyp. A biopsy specimen obtained from the gastric hyperplastic polyp revealed signet ring cell carcinoma. Endoscopic mucosal resection (EMR) was performed to confirm the diagnosis. Histological examination of the EMR specimen revealed focal signet ring cell carcinoma in the hyperplastic polyp. There are few cases of gastric hyperplastic polyp associated with signet ring cell carcinoma.

Acquired amegakaryocytic thrombocytopenia previously diagnosed as idiopathic thrombocytopenic purpura in a patient with hepatitis C virus infection

We report the first case of a patient with hepatitis C virus(HCV) infection and idiopathic thrombocytopenic purpura(ITP), who later developed acquired amegakaryocytic thrombocytopenia(AAMT), with autoantibodies to the thrombopoietin(TPO) receptor(c-Mpl). A 64-year-old woman, with chronic hepatitis C, developed severe thrombocytopenia and was diagnosed with ITP. She died of liver failure. Autopsy revealed cirrhosis and liver carcinoma. In the bone marrow, a marked reduction in the number of megakaryocytes was observed, while other cell lineages were preserved. Therefore, she was diagnosed with AAMT. Additionally, autoantibodies to c-Mpl were detected in her serum. Autoantibodies to c-Mpl are one of the causes of AAMT, acting through inhibition of TPO function, megakaryocytic maturation, and platelet formation. HCV infection induces several autoantibodies. HCV infection might also induce autoantibodies to c-Mpl, resulting in the development of AAMT. This mechanism may be one of the causes of thrombocytopenia in patients with HCV infection.

Property of critical excitation for moment-resisting frames subjected to horizontal and vertical simultaneous ground motions

It has often been reported that, when building structures are subjected to near-fault earthquake ground motions, horizontal and vertical impulsive inputs may cause critical damage during the first few seconds. In practical design of building structures, however, the safety check, taking into account the effect of multi-component ground motions, is hardly conducted except the design of important structures such as high-rise buildings and nuclear power plants. Ftirthel＇more, it is not clear how the correlation of multi-component ground motions influences the actual safety of structures. In this paper, the detailed property of critical excitation is discussed in association with the relationship between the characteristics of ground motions and those of structures. The properties of various auto power spectral density （PSD） functions of the horizontal and vertical ground motions are investigated, and those of the critical cross PSD function of these two-directional ground motions are found by a devised algorithm in a feasible complex plane. A closed-form expression is derived from the critical relation of the auto PSD functions of the simultaneous inputs. This critical excitation method provides us with a new approach for earthquake-resistant design against the possible future earthquake which causes the critical damages to buildings.

Ductility improvement methods for commercial AZ31B magnesium alloy in cold forging

In order to realize cold forging of magnesium alloys in practical application,some methods for ductility improvement of a commercial wrought AZ31B magnesium alloy(Mg-3%Al-1%Zn,mass fraction) at room temperature were *** effects of heat treatment before forging and hydrostatic pressure during forging on the ductility were investigated in cold upsetting and cup ***-temperature annealing was effective to reduce the degree of the texture anisotropy of the specimen,and it was found that the forging limit of the annealed specimen was improved in cold *** the other hand,cold cup forging of the annealed specimen was carried out with applying counter *** applying counter pressures of 100-200 MPa during forging,the critical punch stroke for forging limit of the specimen without crack was improved by 25%in punch stroke.

Neurorehabilitation using a voluntary driven exoskeletal robot improves trunk function in patients with chronic spinal cord injury: a single-arm study

Body weight-supported treadmill training with the voluntary driven exoskeleton(VDE-BWSTT) has been shown to improve the gait function of patients with chronic spinal cord injury. However, little is known whether VDE-BWSTT can effectively improve the trunk function of patients with chronic spinal cord injury. In this open-label, single-arm study, nine patients with chronic spinal cord injury at the cervical or thoracic level(six males and three females, aged 37.8 ± 15.6 years, and time since injury 51.1 ± 31.8 months) who underwent outpatient VDE-BWSTT training program at Keio University Hospital, Japan from September 2017 to March 2019 were included. All patients underwent twenty 60-minute gait training sessions using VDE. Trunk muscular strength, i.e., the maximum force against which patient could maintain a sitting posture without any support, was evaluated in four directions: anterior, posterior, and lateral(right and left) after 10 and 20 training sessions. After intervention, lateral muscular strength significantly improved. In addition, a significant positive correlation was detected between the change in lateral trunk muscular strength after 20 training sessions relative to baseline and gait speed. The change in trunk muscular strength after 20 training sessions relative to baseline was greatly correlated with patient age. This suggests that older adult patients with chronic spinal cord injury achieved a greater improvement in trunk muscle strength following VDE-BWSTT. All these findings suggest that VDE-BWSTT can improve the trunk function of patients with chronic spinal cord injury and the effect might be greater in older adult patients. The study was approved by the Keio University of Medicine Ethics Committee(IRB No. 20150355-3) on September 26, 2017.

A high-resolution climatic change since the Late Glacial Age inferred from multi-proxy of sediments in Qinghai Lake

Based on multi-proxy analysis of pollen, carbonate, TOC, TN and δ13C of organic matters, a high-resolution climatic evolution of Qinghai Lake since the Late Glacial Age is recon-structed. The results indicate that the boundary between the Last Glacial Maximum and the Late Glacial Age is at about 18.2 *** BP. The warm and wet period, which began at about 15.4 *** BP, culminated at 7.4 *** BP and came to its end at about 4.5 *** BP. After that, the climate gradually became cold and dry. The multi-proxy analysis indicates that the climate fluc-tuated greatly during the transitional period from the Late glacial Age to the Holocene, and this is in good accordance with that reflected by deep sea cores of North Atlantic, ice cores of Greenland, lake sediments in Europe, loess sequences and Guliya ice core in China. The cli-matic evolutional characteristic of the Qinghai Lake since the Late Glacial Age shows that the driving force of the East-Asia Monsoon correlates with solar radiation on the ten-thousand-year scale.

Pseudomembranous colitis (PMC) usually manifests asfever and diarrhea in hospitalized patients treated withsystemic antibiotics. We described a case of PMC withintestinal obstruction but without diarrhea. A 60-yearold man was hospitalized for chemotherapy for the treatment of Burkitt lymphoma of the stomach. The patient became febrile and complained of crampy abdominal pain during the post-chemotherapy nadir. Plain abdominal radiography showed some intestinal gas and niveau. Because stool cytotoxin assay for clostridium difficile was positive and colon fiberscopic examination showed a pseudomembrane at the left side of the colon,and a diagnosis of PMC was made. Treatment with intracolonic vancomycin administration by colonoscopy and nasoileus tube was successful. Physicians should take into account the possibility of bowel obstruction due to PMC occurring in patients undergoing chemotherapy and perform emergency colonoscopy examination of suspected cases.

Implications of excess ^(210)Pb and ^(137)Cs in sediment cores from Mikawa Bay, Japan

Four sediment cores were collected from Mikawa Bay, Japan, and excess ^210pb and ^137Cs were measured by gamma spectrometry. Sedimentation rates for the four cores were determined by ^210Pb method. The sedimentation rate range is 0.10-0.70 g/（cm^***）. The bio-mixing depth for each core is less than 7.0 cm, and was determined by the excess 210^pb profiles as well. Therefore, the bioturbation is slight. The 210^pb-derived dates coincided with the results from ^137Cs geochronology. Acceleration in sedimentation rate due to environmental alteration has been found in cores A2.5 and 05AS8, representing two depocenters due to their topography. Evidence of the Tokal Flood in 2000 was found in core 05AS8 according to the profiles of both radioisotopes and trace metals.
