Clinical utility of the platelet-lymphocyte ratio as a predictor of postoperative complications after radical gastrectomy for clinical t2-4 gastric cancer

AIM to identify simple and sensitive markers for postoperative complications after gastrectomy, the predictive values were compared among candidate preoperative factors. MEtHODS three-hundred and twelve patients with previously untreated clinical t2-4 gastric cancer who underwent a D2 standard gastrectomy(distal gastrectomy or total gastrectomy) were included in the *** between 21 parameters that can be determined by preoperative routine blood tests and clinically relevant postoperative complications(grade Ⅱ or higher according to the Clavien-Dindo classification) were evaluated. the optimal cutoff values and clinical significance of the selected markers were further evaluated by subgroup analyses according to age, body mass index, operative procedure and clinical disease *** Sixty-six patients(21.1%) experienced grade Ⅱ or higher postoperative complications. the plateletlymphocyte ratio(PLR, total lymphocyte count/platelet count × 100) exhibited the highest area under the curve value(0.639) for predicting postoperative complications among the 21 parameters, and the optimal cutoff value was determined to be 0.71(sensitivity = 70%, specificity = 56%). In the univariate analysis, the odds ratio of a low PLR for the occurrence of postoperative complications was 2.94(95%CI: 1.66-5.35, P tivariate binomial logistic analysis involving other potential risk factors identified a low PLR as an independent risk factor for postoperative complications(OR = 3.32, 95%CI: 1.82-6.25, P to age, body mass index, operative procedure and clinical disease stage, the low PLR group exhibited an increased incidence of postoperative complications. CONCLUSION the preoperative PLR is a simple and useful predictor of complications after curative gastrectomy in patients with clinical t2-4 gastric cancer.

Background. the aims of this study were to define the clinical characteristics of hepatitis B virus (HBV)-related hepatocellular carcinoma (HCC) in young adult patients without cirrhosis and to evaluate the efficacy of interferon (IFN) therapy on HCC recurrence. Methods. Of 187 patients with HBV-related HCC treated at our hospital, 4 had no liver cirrhosis and were less than 30 years of age (10, 22, 23, and 26 years). Results. At the time of diagnosis of HCC, all cases had antibody to hepatitis Be antigen (anti-HBe) and histological staging of nontumorous liver was F0 or F1, i.e., low-grade hepatitis. the mothers of all 4 young adult patients with HCC had HBV-related liver disease. three cases developed recurrence of HCC. In these patients, long-term intermittent IFN therapy after reresection of HCC resulted in long-term survival without recurrence for more than 3 years of follow-up. Conclusions. (1) Young adult patients with HCC are positive for anti-HBe, lack cirrhosis,and the route of infection seems to be mother-to-infant *** transmission of HBV and HBV DNA integration into the cellular genomic DNA during fetal life is a possible explanation of HBV-related hepatocarcinogenesis in young adults; and (2) long-term IFN therapy seems to be useful for prevention of tumor recurrence after radical operation for HBV-related HCC.

目的　克隆成人腹泻标本WH 1中B组轮状病毒组 (groupBrotavirus ,GBRV)非结构蛋白NSP2基因 ,分析其核苷酸序列 ,比较与其它GBRV基因同源性 ,预测其mRNA二级结构、蛋白质二级结构。方法　利用逆转录聚合酶链反应 (Rt PCR)技术 ,从成人腹泻标本WH 1中扩增GBRVNSP2基因 ,克隆载体pUCmt ,对NSP2基因进行核苷酸序列分析。利用GeneBee软件比较与其它GBRV毒株NSP2基因同源性 ,Rnaviz2 .0软件绘制NSP2基因的二级结构 ,PredictProtein软件分析NSP2蛋白结构。结果　成人腹泻标本WH 1中GBRV非结构蛋白NSP2基因全长 10 0 7bp ,与ADRV核苷酸序列的同源性达 98% ,与印度加尔各达分离株CAL 1达 82 % ,与IDIR(鼠 )同源性仅为 79%。氨基酸序列与ADRV的同源性达 98.4 % ,与CAL 1达 97.7% ,而与IDIR仅为 89.0 %。其mRNA折叠形成多达 2 6个发卡环状结构。NSP2蛋白是由 30 1个氨基酸残基组成的多肽 ,含有 2个潜在的N 糖基化位点和多个磷酰化位点。结论　成人腹泻标本WH 1中GBRV非结构蛋白NSP2基因和氨基酸序列与人GBRV有较高的同源性 ,而与动物中GBRV同源性相对较低 ,其中与ADRV的同源性最高 ,推测GBRVWH 1株与ADRV具有相同起源。

We have developed a logic-embedded 96-Kbit FeRAMmacro that has low-voltage operation and high-endurancefeatures for smart card *** smart card LSIwas fabricated using a 0.35 μm-standard CMOS processwith 3-level metallization and CMVP *** operation of the chip was confirmed atvoltages from 2.7 to 5.5 V with 2.5 MHz clock *** Ir-based top and bottom electrodes,the fatigueendurance of the FeRAM was improved,which wasconfimed in burn-in *** failed bits were observed ataccelerated conditions with 5.5 V and 150℃ after 10fatigue cycles.

Hemophagocytic lymphohistiocytosis (HLH) is a rare disorder characterized by high grade fever, heaptos-plenomegaly, lymphadenopathy and cytopenia in association with hypercytokinemia and macrophage activation. Epstein-Barr virus (EBV) is a common cause of HLH particularly in Asian countries. Although EBV usually infects B cells and causes infectious mononucleosis, it is detected predominantly in CD8+ t cells in EBV-associated HLH. In the present study we found the EBV genome in various lineages of the lymphoid cells including t, B, and natural killer cells in a 14-month-old boy with mild EBV-associated HLH. Furthermore, chronological changes in the copy number of EBV-DNA in each lineage are reported. Profound decrease in the number of peripheral natural killer cells at the early stage could be involved in the development of HLH.
