Pancreatic cancer remains a disease with an overall grim prognosis,even in the limited number of patients who are amenable for resection attempted at ***,pancreatic cancer demonstrate a predisposition for invasive growth and distant metastasis-even in the very early stages of the ***,while surgical resection is the strongest factor for long-term survival,early recurrence after surgery is associated with a poor prognosis(1).In light of this knowledge and,despite the lack of good level I data,it seems that patients and practitioners have been voting with their feet regarding systemic therapy;an increasing number of institutions are giving systemic treatment before surgery with the hopes of better disease control and avoiding futile ***,the contemporary practice is based on an“radiant autonomy”-as it would seem,that pretty much any regimen and combination of drugs and modalities will be offered in current practice,with the lack of randomized studies for most options but often with an institutional signature to preference and delivery.

An increasing number of patients are being referred to pancreatic centres around the world due to often incidentally discovered cystic neoplasms of the *** evaluation and management of pancreatic cystic neoplasms is a controversial topic and with existing guidelines based on a lack of strong evidence there is discordance between centres and guidelines with regard to when to offer surgery and when to favour *** frequency,duration and modality of surveillance is also controversial as this is resource-consuming and must be balanced against the perceived benefits and risks *** there is consensus that the risk of malignancy should be balanced against the lifeexpectancy and comorbidities,the indications for surgery and surveillance strategies vary among the ***,the tug of war between surveillance or resection *** we discuss the recommendations from guidelines with further accumulating data and emerging reports on intraductal papillary mucinous neoplasm in the literature.

BACKGROUND: suspension trauma syndrome is a life-threatening event that occurs when a person is "trapped" in a prolonged passive suspension. It is most commonly seen in people who engage in occupational or sport activities that require harness suspension. The aim of this study is to identify the predisposing factors, pathophysiology, and management of suspension ***: A review and analysis of the literature published in English and spanish from 1972 to 2020 on suspension trauma were performed. search sources were Pub Med, Medline, Cochrane Library, MesH, Up To Date, and Google scholar. Articles referring to suspension trauma associated with other injury mechanisms(traumatic impact injuries, drowning, asphyxiation, or bleeding), case reports, and pediatric population were ***: Forty-one articles were identified. Of these, 29 articles related to mechanism, pathophysiology, and management of individuals who suffered prolonged suspension trauma without associated traumatic injuries were included in the study. We encountered several controversies describing the putative pathophysiology, ranging from blood sequestration in the lower extremities versus accumulation of metabolic waste and hyperkalemia to dorsal hook-type harness as a trigger cause of positional asphyxia;to vascular compression of femoral vessels exerted by the harness causing decreased venous return. Pstients suspended in a full-body harness with dorsal hook showed more hemodynamic alterations in response to the compressive effect on the rib cage, causing a reduction in perfusion by presenting a decrease in pulse pressure. Management strategies varied across ***: Progress has been made in individualizing the population at risk and in the management of suspension trauma. We recommend the formation of consensus definitions, larger cohort or registry studies to be conducted, and experimental animal models to better understand the mechanisms in order to develop management

AIM:To investigate the epidemiological trends in inci-dence and mortality of perforated peptic ulcer(PPU)in a well-defined Norwegian population. METHODs:A retrospective,population-based,single-center,consecutive cohort study of all patients diag-nosed with benign perforated peptic *** were both gastric and duodenal ulcer patients admitted to stavanger University Hospital between January 2001 and December *** with a malignant neoplasia diagnosis,verified by histology after biopsy or resection,were *** were identified from the hospitals administrative electronic database using pertinent ICD-9 and ICD-10 codes(K25.1,K25.2,K25.5, K25.6,K26.1,K26.2,K26.5,K26.6).Additional searches using appropriate codes for relevant laparoscopic and open surgical procedures(e.g.,JDA 60,JDA 61,JDH 70 and JDH 71)were performed to enable a complete identification of all *** demographics,presentation patterns and clinical data were retrieved from hospital records and surgical *** and adjusted incidence and mortality rates were estimated by using national population demographics data. REsULTs:In the study period,a total of 172 patients with PPU were *** adjusted incidence rate for the overall 10-year period was 6.5 per 100 000 per year(95%CI:5.6-7.6)and the adjusted mortality rate for the overall 10-year period was 1.1 per 100 000 per year(95%CI:0.7-1.6).A non-significant decline in ad-justed incidence rate from 9.7 to 5.6 occurred during the *** standardized mortality ratio for the whole study period was 5.7(95%CI:3.9-8.2),while the total 30-d mortality was 16.3%.No difference in in-cidence or mortality was found between ***,for patients≥60 years,the incidence increased over 10-fold,and mortality more than 50-fold,compared to younger *** admission rates outside office hours were high with almost two out of three(63%) admissions seen at evening/night time shifts and/or during *** observed season

The Ryman-Laikre (R-L) effect is an increase in inbreeding and a reduction in total effective population size ( ) in a combined captive-wild system, which arises when a few captive parents produce large numbers of offspring. To facilitate evaluation of the R-L effect for scenarios that are relevant to marine stock enhancement and aquaculture, we extended the original R-L formula to explicitly account for several key factors that determine , including the numbers of captive and wild adults, the ratio of captive to wild (), productivity of captive and wild breeders, and removal of individuals from the wild for captive breeding. We show how to provide quantitative answers to questions such as: What scenarios lead to no loss of effective size? What is the maximum effective size that can be achieved? and What scenarios insure that will be no smaller than a specified value? Important results include the following: (1) For large marine populations, the value of becomes increasingly important as the captive contribution increases. Captive propagation will sharply reduce unless the captive contribution is very small or is very large (∼10 or higher). (2) Very large values of are only possible if wild is tiny. Therefore, large wild populations undergoing captive enhancement at even modest levels will suffer major reductions in effective size unless wild is a tiny fraction of the census size (about 10 or lower).

Can molecular biomarkers replace a clinical risk score for resectable colorectal liver metastasis?

In resectable colorectal liver metastasis(CRLM) the role and use of molecular biomarkers is still controversial. several biomarkers have been linked to clinical outcomes in CRLM, but none have so far become routine for clinical decision making. For several reasons, the clinical risk score appears to no longer hold the same predictive value. some of the reasons include the ever expanding indications for liver resection, which now increasingly tend to involve extrahepatic disease, such as lung metastases(both resectable and non-resectable) and the shift in indication from "what is taken out"(e.g., how much liver has to be resected) to "what is left behind"(that is, how much functional liver tissue the patient has after resection). The latter is amenable to modifications by using adjunct techniques of portal vein embolization and the associating liver partition and portal vein ligation for staged hepatectomy techniques to expand indications for liver resection. Added to this complexity is the increasing number of molecular markers, which appear to hold important prognostic and predictive information, for which some will be discussed here. Beyond characteristics of tissue-based genomic profiles will be liquid biopsies derived from circulating tumor cells and cell-free circulating tumor DNA in the blood. These markers are present in the peripheral circulation in the majority of patients with metastatic cancer disease. Circulating biomarkers may represent more readily available methods to monitor, characterize and predict cancer biology with future implications for cancer care.
