Gastrointestinal stromal tumors(GISTs)are mesenchymal tumors that originate from the gastrointestinal tract,mostly from the stomach.GISTs are derived from the myenteric interstitial cells of Cajal and are caused by several mutations in the c-kit and platelet-derived growth factor receptor genes.Clinically,GISTs are detected by endoscopic and imaging findings and are diagnosed by immunostaining.Surgery is the first line of treatment,and if the tumor is relatively small,minimally invasive surgery such as laparoscopy is performed.In recent years,neoadjuvant therapy has been administered to patients with GISTs that are suspected of having a large size or infiltration to other organs.Postoperative adjuvant imatinib is the standard therapy for high-risk GISTs.It is important to assess the risk of recurrence after GIST resection.However,the effect of tyrosine kinase inhibitor use will vary by the mutation of c-kit genes and the site of mutation.Furthermore,information regarding gene mutation is indispensable when considering the treatment policy for recurrent GISTs.This article reviews the clinicopathological characteristics of GISTs along with the minimally invasive and multidisciplinary treatment options available for these tumors.The future perspectives for diagnostic and treatment approaches for these tumors have also been discussed.

Fabrication of independent nickel microstructures with anodizing of aluminum, laser irradiation, and electrodeposition

Independent microstructures made of Ni metal were fabricated by fivesequential processes: porous anodic oxide film formation, pore sealing, laser irradiation, Nielectroplating, and removal of the aluminum substrate and anodic oxide films. Aluminum plates androds were anodized in an oxalic acid solution to form porous type anodic oxide films, and thenimmersed in boiling distilled water for pore sealing. The anodized and pore-sealed specimens wereirradiated with a pulsed neodymium-doped yttrium aluminum garnet (Nd-YAG) laser beam in a Ni platingsolution to remove anodic oxide film locally by rotating and moving up I down with anXYZ(theta)-stage. Nickel was deposited at the area where film had been removed by cathodicpolarization in the solution before removing the aluminum substrate and anodic oxide films in NaOHsolutions. Cylindrical or plain network structures were fabricated successfully.

Ultrasonic Measurement of Two-Dimensional Liquid Velocity Profile Using Two-Element Transducer

The flow field or multidimensional velocity distribution of the coolant in fuel rod bundles of the reactor core in pressurized water reactors (PWRs) is an important parameter that is revealed through experimental investigations. This paper presents the two-dimensional (2D) velocity profile measurement using a two-element ultrasonic transducer with both elements acting as a transceiver. The size of the transducer is minimized for compactness, leading to a narrow sound field appropriate for applications in fuel rod bundle flow. Furthermore, the transducer’s sound pressure is evaluated via simulations and experimental measurements. In order to confirm the ability of the ultrasonic velocity profiler (UVP) with a two-element transducer, the experimental measurement is conducted in turbulent horizontal pipe flow. The 2D velocity vector profile is obtained, and then the measurement in swirling flow is conducted. The 2D velocity profile in an axial and radial plane is obtained utilizing the UVP measurement. Lastly, the ability of the UVP to derive the 2D velocity profile in the narrow area of the rod bundles is demonstrated.

Thrombocytopenia after liver transplantation:should we care?

Transient thrombocytopenia is a common phenomenon after liver transplantation. After liver transplantation(LT), platelet count decreases and reaches a nadir on postoperative days 3-5, with an average reduction in platelet counts of 60%; platelet count recovers to preoperative levels approximately two weeks after LT. The putative mechanisms include haemodilution, decreased platelet production, increased sequestration, medications, infections, thrombosis, or combination of these processes. However, the precise mechanisms remain unclear. The role of platelets in liver transplantation has been highlighted in recent years, and particular attention has been given to their effects beyond hemostasis and thrombosis. Previous studies have demonstrated that perioperative thrombocytopenia causes poor graft regeneration, increases the incidence of postoperative morbidity, and deteriorates the graft and decreases patient survival in both the short and long term after liver transplantation. Platelet therapies to increase perioperative platelet counts, such as thrombopoietin, thrombopoietin receptor agonist, platelet transfusion, splenectomy, and intravenous immunoglobulin treatment might have a potential for improving graft survival, however clinical trials are lacking. Further studies are warranted to detect direct evidence on whether thrombocytopenia is the cause or result of poor-graft function and postoperative complications, and to determine who needs platelet therapies in order to prevent postoperative complications and thus improve post-transplant outcomes.

Under the aim of finding effective rehabilitation solutions,the difference between the extents of fatigue of each muscle used in different motions are compared.Previous research suggested methods for estimating muscle torque and muscle tension on the basis of a musculoskeletal model.As a result,it has become possible to quantitatively identify the extent of fatigue in each muscle during motion.Therefore,to evaluate muscle fatigue more quantitatively,driving power and angular momentum are focused on.Based on the driving torque of joints and the muscle torque calculated by using a three-dimensional musculoskeletal model,a method for calculating the driving power of joints and the angular momentum of a muscle is proposed.This method is applied to analysis of different motions.Moreover,fatigue of muscles is quantitatively analyzed on the basis of driving power and angular momentum.

This study describes an ultrasonic velocity profiler that uses a new ultrasonic array transducer with unique 5-element configuration, with all five elements acting as transmitters and four elements as receivers. The receivers are designed to reduce the amount of uncertainty. As the fluid moves through this setup, four Doppler frequencies are obtained. The multi-dimensional velocity information along the measurement line can be reconstructed. The transducer has a compact geometry suitable for a wide range of applications, including narrow flow areas. The transducer’s basic frequency and sound pressure are selected and evaluated to be compatible with the application. First, to confirm the measurement ability, the measurement of the developed system in two-dimensional flow is validated by comparing it to the theoretical data. The uncertainty of measurement was within 15%. Second, the three-dimensional measurement in turbulent and swirling flow is proved experimentally to check the applicability of the proposed technique.

Electrocatalytic evolution of oxygen on NiCu particles modifying conductive alumina/nano-carbon network composite electrode

In this paper, a novel electrically conductive alumina/nano-carbon network (NCN) composite material was used as an electro- catalyst carrier. A NiCu/Al2O3 /NCN composite electrode was prepared by electrodepositing NiCu particles onto the surface of the conductive alumina/NCN composite. Morphology, composition, crystalline structure and electrochemical properties of the NiCu/Al2O3 /NCN composite electrode were investigated. The results showed that NiCu particles can be deposited onto the surface of the alumina/NCN composite by a coelectrodeposition method. NiCu particles in the form of solid solution with face-centered cubic (fcc) structure were relatively uniformly distributed over the carbon layer of the conductive ceramic between alumina grains. As-resulted NiCu/Al2O3 /NCN composite electrode had a remarkably enhanced electrochemical activity and high stabilization for oxygen evolution reaction, which indicated its potential application with enhanced performance to oxygen evolution reaction (OER). Moreover, based on the electrochemical measurement, the mechanism of the OER on the NiCu/Al2O3 /NCN composite electrode was discussed.

A Breast Density Classification System for Mammography Considering Reliability Issues in Deep Learning

In a convolutional neural network (CNN) classification model for diagnosing medical images, transparency and interpretability of the model’s behavior are crucial in addition to high classification accuracy, and it is highly important to explicitly demonstrate them. In this study, we constructed an interpretable CNN-based model for breast density classification using spectral information from mammograms. We evaluated whether the model’s prediction scores provided reliable probability values using a reliability diagram and visualized the basis for the final prediction. In constructing the classification model, we modified ResNet50 and introduced algorithms for extracting and inputting image spectra, visualizing network behavior, and quantifying prediction ambiguity. From the experimental results, our proposed model demonstrated not only high classification accuracy but also higher reliability and interpretability compared to the conventional CNN models that use pixel information from images. Furthermore, our proposed model can detect misclassified data and indicate explicit basis for prediction. The results demonstrated the effectiveness and usefulness of our proposed model from the perspective of credibility and transparency.
