We would like t. emphasize t.at.t.e in-flight.(K-,N) react.ons are part.cularly useful for t.e st.dy of t.e K-nucleus int.ract.on. Since t.e react.on mechanism is well known,t.ere is lit.le ambiguit. t. derive t.e K-nucleus int.ract.on from t.e measured missing mass spect.a. Here we discuss t.e missing mass spect.a of t.e (K-,N) react.ons on t.e 12C and 16O t.rget.. t.e spect.a show an appreciable amount.of st.engt. below t.e K-nucleus t.reshold which indicat.s t.at.t.e K-nuclear pot.nt.al is st.ongly at.ract.ve. Comparison of t.e missing mass spect.a wit. t.eoret.cal calculat.ons leads t. a pot.nt.al dept. of around -190 MeV for t.e 12C(K-,n) react.on. A less deep pot.nt.al of around -160 MeV reproduces well t.at.of t.e 12C(K-,p) react.on. t.e difference can be due t. isospin dependence of t.e int.ract.on. Our dat. show t.at.t.e K-nucleus pot.nt.al is very deep t. realize kaon condensat.on in t.e core of neut.on st.rs.

It.is found t.at.st.uct.ral bifurcat.on t. zonal flow dominat.d or st.eamer-like st.t.s depends on t.e spect.al anisot.opy of t.rbulent.Et. fluct.at.on, which is governed by t.e magnet.c shear. t.e t.rbulent.elect.on t.ansport.is suppressed by enhanced zonal flows. However, it.is st.ll low even if t.e st.eamer is formed in Et. t.rbulence wit. st.ong shears. It.is shown t.at.t.e low t.ansport.may be relat.d t. t.e secondary excit.t.on of poloidal long-wavelengt. mode due t. t.e beat.wave of t.e most.unst.ble component. or a modulat.on inst.bilit.. t.is large-scale st.uct.re wit. a low frequency and a long wavelengt. may sat.rat., or at.least.cont.ibut. t. t.e sat.rat.on of Et. fluct.at.ons t.rough a poloidal mode coupling. t.e result.suggest. a low fluct.at.on level in Et. t.rbulence.

AIM:t. invest.gat. endoscopic and hist.pat.ological findings in t.e duodenum of pat.ent. wit. St.ongyloides st.rcoralis(***)hyperinfect.on. MEt.ODS:Over a period of 23 years(1984-2006),we invest.gat.d 25 pat.ent. *** hyperinfect.on who had had an esophagogast.oduodenoscopy before undergoing t.eat.ent.for *** clinical and endoscopic findings were analyzed ret.ospect.vely. RESULt.:t.ent.-four(96%)of t.e pat.ent. invest.gat.d were under immunocompromised condit.on which was mainly due t. a human t.lymphot.opic virus t.pe 1(Ht.V-1)*** abnormal endoscopic findings,mainly edemat.us mucosa,whit. villi and eryt.emat.us mucosa,were observed in 23(92%) *** degree of duodenit.s including villous at.ophy/dest.uct.on and inflammat.ry cell infilt.at.on corresponded t. t.e severit. of t.e endoscopic findings. t.e hist.pat.ologic yield for ident.fying larvae was 71.4% by duodenal *** endoscopic findings of duodenit.s were more severe in pat.ent. whose biopsies were posit.ve for larvae t.an t.ose whose biopsies were negat.ve(Endoscopic severit. score:4.86±2.47vs 2.71 ±1.38,Pt.dy clearly demonst.at.s t.at.in addit.on t. st.ol analysis,endoscopic observat.on and biopsies are very *** also emphasize t.at.*** and Ht.V-1 infect.ons should be ruled out.before immunosuppressive t.erapy is administ.red in endemic regions.

Silicosis pat.ent. （SILs） and pat.ent. who have been exposed t. asbest.s develop not.only respirat.ry diseases but.also cert.in immunological disorders. In part.cular, SIL somet.mes complicat.s aut.immune diseases such as syst.mic scleroderma, rheumat.id art.rit.s （known as Caplan syndrome）, and syst.mic lupus eryt.emat.ses. In addit.on, malignant.complicat.ons such as lung cancer and malignant.mesot.elioma oft.n occurr in pat.ent. exposed t. asbest.s, and may be involved in t.e reduct.on of t.mor immunit.. Alt.ough silica-induced disorders of aut.immunit. have been explained as adjuvant.t.pe effect. of silica, more precise analyses are needed and should reflect.t.e recent.progress in immunomolecular findings. A brief summary of our invest.gat.ons relat.d t. t.e immunological effect. of silica/asbest.s is present.d. Recent.advances in immunomolecular st.dies led t. det.iled analyses of t.e immunological effect. of asbest.s and silica. Bot. affect.immuno-compet.nt.cells and t.ese effect. may be associat.d wit. t.e pat.ophysiological development.of complicat.ons in silicosis and asbest.s-exposed pat.ent. such as t.e occurrence of aut.immune disorders and malignant.t.mors, respect.vely. In addit.on, immunological analyses may lead t. t.e development.of new clinical t.ols for t.e modificat.on of t.e pat.ophysiological aspect. of diseases such as t.e regulat.on of aut.immunit. or t.mor immunit. using cellmediat.d t.erapies, various cyt.kines, and molecule-t.rget.ng t.erapies. t. evaluat. t.e clinical ut.lit. for simple pat.ent.administ.red dose adjust.ent.met.ods of insulin glargine during out.at.ent.visit. compared wit. a physician managed t.t.at.on, changes in HbA1c and t.t.l daily dose of insulin were evaluat.d in 23 pat.ent. by dividing pat.ent. int. physician-led (PL) group and self-t.t.at.on (St. group who were newly administ.red glargine basal-support.d oral t.erapy (BOt. while cont.nuing oral ant.diabet.c drugs at.t.e discret.on of t.eir at.ending physician during regular out.at.ent.visit.. In t.e PL group, one mont. aft.r init.at.on of glargine, HbA1c followed a declining t.end, alt.ough t.is was not.significant.(P = 0.07), and decreased significant.y aft.r t.o and t.ree mont.s (P t.vely). However, aft.r 12 mont.s, t.e significant.difference had disappeared. By cont.ast. in t.e St.group, HbA1c did not.significant.y decrease one mont. aft.r init.at.on of glargine, but.did drop markedly aft.r t.o and t.ree mont.s, wit. t.is t.end cont.nuing up t. 12 mont.s (P t.e differences bet.een bot. groups, we found t.at.t.e init.al dose was significant.y larger in t.e PL group (P t.e dose increased significant.y more in t.e St.group aft.r t.ree mont.s. While t.e insulin dose aft.r 12 mont.s was large in t.e St.group, no st.t.st.cally significant.difference was not.d bet.een t.e t.o groups (P = 0.14) whereas HbA1c was significant.y low in t.e St.group. In conclusion, we believe t.at.pat.ent.led basal insulin dosage adjust.ent.is an effect.ve and useful t.erapeut.c opt.on when t.ey can mast.r self-monit.ring of blood glucose.

A plain radiograph showed a mineralizat.on in t.e proximal femur wit. part.ally ost.olyt.c foci and an abnormal shadow in t.e soft.t.ssue. Magnet.c resonance imaging scans showed an inhomogeneous lesion wit. int.rmediat. t. part.ally low signal int.nsit. on t.-weight.d image and int.rmediat. t. high signal int.nsit. on t.-weight.d image. Microscopically, t.e t.mor in t.e femur is a low-grade chondrosarcoma and t.e component.of soft.t.ssue was a high-grade sarcomat.us lesion wit. an epit.elial arrangement.of t.mor cells. A diffuse immunoreact.vit. t. bot. viment.n and c-kit.(CD117) ant.bodies was det.ct.d in t.e high-grade component. A dedifferent.at.d component.is similar t. t.ose of gast.oint.st.nal st.omal t.mor (GISt.. t.is is t.e first.case of dedifferent.at.d chondrosarcoma wit. a high-grade component.mimicking a GISt.
