Multidisciplinary approach for post-liver transplant recurrence of hepatocellular carcinoma: A proposed management algorithm

A large number of liver transplants have been performed for hepatocellular carcinoma(HCC), and recurrence is increasingly encountered. The recurrence of HCC after liver transplantation is notoriously difficult to manage. We hereby propose multi-disciplinary management with a systematic approach. The patient is jointly managed by the transplant surgeon, physician, oncologist and radiologist. Immunosuppressants should be tapered to the lowest effective dose to protect against rejection. The combination of a mammalian target of rapamycin inhibitor with a reduced calcineurin inhibitor could be considered with close monitoring of graft function and toxicity. Comprehensive staging can be performed by dual-tracer positron emission tomography-computed tomography or the combination of contrast computed tomography and a bone scan. In patients with disseminated recurrence, sorafenib confers survival benefits but is associated with significant drug toxicity. Oligo-recurrence encompasses recurrent disease that is limited in number and location so that loco-regional treatments convey disease control and survival benefits. Intra-hepatic recurrence can be managed with graft resection, but significant operative morbidity is expected. Radiofrequency ablation and stereotactic body radiation therapy(SBRT) are effective alternative strategies. In patients with more advanced hepatic disease, regional treatment with trans-arterial chemoembolization or intra-arterial Yttrium-90 can be considered. For patients with extra-hepatic oligorecurrence, loco-regional treatment can be considered if practical. Patients with more than one site of recurrence are not always contraindicated for curative treatments. Surgical resection is effective for patients with pulmonary oligo-recurrence, but adequate lung function is a prerequisite. SBRT is a non-invasive and effective modality that conveys local control to pulmonary and skeletal oligo-recurrences.

Cu2ZnSnS4（CZTS） is a promising photovoltaic absorber material,however,efficiency is largely hindered by potential fluctuation and a band tailing problem due to the abundance of defect complexes and low formation energy of an intrinsic Cu Zn *** to CZTS by group I,II,or IV element replacement to circumvent this challenge has grown research *** this work,using a hybrid （HSE06） functional,we demonstrated the qualitative similarity of defect thermodynamics and electronic properties in Cu2MgSnS4（CMTS） to *** show Sn Mg to be abundant when in Sn-and Cu-rich condition,which can be detrimental,while defect properties are largely similar to CZTS in Sn-and *** Sn-and Cu-poor chemical potential,there is a general increase in formation energy in most defects except Sn Mg,Cu Mg remains as the main contribution to p-type carriers,and Sn Mg may be detrimental because of a deep defect level in the mid gap and the possibility of forming defect complex Sn Mg+Mg *** diffusion is studied using generalized gradient approximation,and we find similar vacancy diffusion properties for Cu vacancy and lower diffusion barrier for Mg vacancy,which may reduce possible Cu-Mg disorder in *** findings further confirm the feasibility of CMTS as an alternative absorber material to CZTS and suggest the possibility for tuning defect properties of CZTS,which is crucial for high photovoltaic performance.

Abstract Generalized B-splines have been employed as geometric modeling and numerical simu- lation tools for isogeometric analysis （IGA for short）. However, the previous models used in IGA, such as trigonometric generalized B-splines or hyperbolic generalized B-splines, are not the unified mathematical representation of conics and polynomial parametric curves/surfaces. In this paper, a unified approach to construct the generalized non-uniform B-splines over the space spanned by {α（t）,β（t）,ξ（t）, η（t）, 1, t,……. , tn-4} is proposed, and the corresponding isogeometric analysis framework for PDE solving is also studied. Compared with the NURBS-IGA method, the proposed frameworks have several advantages such as high accuracy, easy-to-compute derivatives and integrals due to the non-rational form. Furthermore, with the proposed spline models, isogeometric analysis can be performed on the computational domain bounded by transcendental curves/surfaces, such as the involute of circle, the helix/helicoid, the catenary/catenoid and the cycloid. Several numerical examples for isogeometrie heat conduction problems are presented to show the effectiveness of the proposed methods.

Experimental investigation was conducted to characterize the responses of pseudo-ductile cementitious composites (PDCCs) when subjected to uniaxial and biaxial *** PDCCs is a class of fiber reinforced cementitious composites with ultra-high ductility by using a low volume fraction (2%) of polyvinyl alcohol (PVA) *** different strength grades of PDCC were examined with cubic specimen size of 100 mm in the *** specimens were loaded with a servo-hydraulic jack at different stress *** principle stresses and strains of the specimens were recorded,and the failure modes with various stress states were *** test results indicated that the ultimate strength of PDCCs increased due to the lateral confinement in the other principal stress direction,and the maximum ultimate strength occurred at the biaxial stress ratio of 0.25,which was very different from common concrete *** the PDCC specimens,the biaxial strength may be lower than the uniaxial strength when subjected to biaxial compression with the stress ratio of 1.0,and the failure mode showed a shear-type failure because of the bridging effect of ***,a failure criterion was proposed for PDCCs under biaxial compression.

This study was conducted to screen the commercial herbicides for algae control in the aquarium. Three herbicides of ametryn, atrazine and metribuzine were tested at concentrations ranging from 0.625 to 10.00 ppm. It is found that ametryn was the most effective herbicide that inhibited algal growth. Ametryn concentration that caused 50% inhibition on algae growth after exposure for 21 days was 0.335 ppm. Survival rate of oruamental fish, Harlequin rasbora （Trigonostigma heteromorpha） and growth rate of aquatic plant （Elodea canadensis） exposed to 10 ppm ametryn were compared with those exposed to algaecide. The result showed that there was no significant difference in survival rate of Harlequin rasbora when exposed to ametryn, algaecide and dechlorinized tap water （control）. However, growth rate of E. canadensis was lower after exposure of ametryn compared to those exposed to algaecide and tap water. These results suggest that ametryn has potential to be used as an algal inhibitor in aquarium.

Objectives:*** examine the characteristics of patients with erectile dysfunction in a family physician led erectile dysfunction clinic;*** review association of chronic disease spectrum and erectile dysfunction;*** review initial treatment pattern and ***:Retrospective case series ***:All consecutive patients seen in a regional hospital family physician led erectile dysfunction clinic from April 2014 to March *** outcome measures:*** severity of erectile dysfunction,based on International Index of Erectile Function(IIEF-5).*** associated chronic comorbidities of *** patterns and patient ***:One hundred and eighty three patients presented with erectile dysfunction(ED)with mean age 58.7(range 23 to 82)years old were seen during the study *** hundred and twenty seven patients(69.4%)had comorbidity of chronic diseases,including 50.8%had hypertension,38.8%had diabetes mellitus and 33.9%had *** mean body mass index was 25.2 kg/m2,the mean blood pressure was 137.3/79.5 mm Hg(1 mm Hg=0.133 kPa).According to IIEF-5 score,50.3%,30.6%and 18.6%had severe,moderate and mild erectile dysfunction *** average duration of ED before seeking medical help was 3.9 *** 5(PDE5)inhibitors were prescribed to 119 patients(65%),and 57.1%of them achieved good *** nine patients(15.8%)were referred to other specialty for further management,including 27.6%had contraindication for PDE5 ***:High proportion of erectile dysfunction patients had comorbitiy of chronic dis-eases.57.1%of those patients receiving PDE5 inhibitors showed good response.

Gastrointestinal(GI)cancers are the major cause of cancer-related mortality *** imaging is an important auxiliary means for the diagnosis,assessment and prognostic prediction of GI *** is an emerging and effective technology to decipher the encoded information within medical images,and traditional machine learning is the most commonly used *** advances in deep learning technology have further promoted the development of *** the field of GI cancer,although there are several surveys on radiomics,there is no specific review on the application of deep-learning-based radiomics(DLR).In this review,a search was conducted on Web of Science,PubMed,and Google Scholar with an emphasis on the application of DLR for GI cancers,including esophageal,gastric,liver,pancreatic,and colorectal ***,the challenges and recommendations based on the findings of the review are comprehensively analyzed to advance DLR.
