BACKGROUND: In the setting of severe sepsis and septic shock, mortality increases when lactate levels are ≥ 4 mmol/L. However, the consequences of lower lactate levels in this population are not well understood. The study aimed to determine the in-hospital mortality associated with severe sepsis and septic shock when initial lactate levels are < 4 mmol/***: This is a retrospective cohort study of septic patients admitted over a 40-month period. Totally 338 patients were divided into three groups based on initial lactate values. Group 1 had lactate levels < 2 mmol/L; group 2: 2–4 mmol/L; and group 3: ≥ 4 mmol/L. The primary outcome was in-hospital ***: There were 111 patients in group 1, 96 patients in group 2, and 131 in group 3. The mortality rates were 21.6%, 35.4%, and 51.9% respectively. Univariate analysis revealed the mortality differences to be statistically significant. Multivariate logistic regression demonstrated higher odds of death with higher lactate tier group, however the findings did not reach statistical ***: This study found that only assignment to group 3, initial lactic acid level of ≥ 4 mmol/L, was independently associated with increased mortality after correcting for underlying severity of illness and organ dysfunction. However, rising lactate levels in the other two groups were associated with increased severity of illness and were inversely proportional to prognosis.

该文对美国南卡罗莱纳州两条(开级配沥青混合料)OGFC试验路进行了针对性研究与总结,以确定不掺加纤维是否会对OGFC混合料的性能产生影响。其中一条试验路包含3种OGFC混合料:掺加木质素纤维的热拌沥青混合料(HMA)、未掺纤维而采用废轮胎橡胶粉改性的沥青混合料(GTR),未掺纤维的温拌沥青混合料(WMA);另一条试验路包含两种:HMA、WMA。试验结果表明:(1)不掺加纤维对混合料的析漏损失、渗水率、抗磨耗性、间接拉伸强度和抗疲劳强度并未产生不利影响;(2)虽然没有掺加木质素纤维,采用温拌技术生产的温拌沥青混合料的抗疲劳强度仍然高于HMA混合料;(3)温拌沥青混合料可能会减少摊铺初始阶段(可流动的沥青混合料首先通过冷的材料传送装置,然后到达冷的摊铺装置)的热量损失,而较低的热量损失能够改善摊铺初始位置横向冷接缝附近OGFC面层的密度稳定性,从而可能改善路面性能。

The Treatment of Partial-Thickness Burns with a Hydroconductive Wound Dressing: Clinical and Mechanistic Effects

OBjECTIVE The goal of the study was to determine the potential for the KeratinoSens assay to identify electrophile allergens within a botanical extract *** In the KeratinoSens assay,the induction of a luciferase gene under the control of the antioxidant response element(ARE) derived from the human gene AKR1C2 gene is *** parallel,cytotoxicity is assessed by both Neutral Red Uptake(NRU)and MTT *** samples included gluteraldehyde(GA),dimethyl maleate(DM)and cinnamic aldehyde(CA)spiked into four different botanical extracts(including different excipient solvent systems).Test concentrations ranged up to 1 g·L(of complete extract)and a test concentration was considered positive if the relative viability was≥70%and the fold induction of luciferase was 1.5x relative to the solvent *** was measured relative to the EC1.5 of the neat sensitizer as a function of sensitizer concentration and extract *** independent trials were performed on each test *** No appreciable cytotoxicity was observed with any of the *** spiked at 1%to 2%were sufficient to induce a positive up regulation of the ARE below the maximum test material concentration of up to 1 g·*** responses occurred at cell survivals well in excess of 70%.The botanical matrix impacted recovery for both gluteraldehyde and dimethyl maleate *** recovery of the GA spike required at least a -3 fold increase in concentration relative to the chemical alone and botanical extract #3 reduced the activity below *** DM and CA showed activity at about the same effective concentrations as the neat chemical although the DM showed reduced activity in botanical extract #3 as *** These data suggest that the KeratinoSens assay has the potential to identify electrophile allergens within a botanical extract matrix.

Objectives: Edema in partial-thickness burn wounds can decrease tissue perfusion, increase tissue ischemia, and deepen the burn injury. We report the results of a clinical trial comparing the effectiveness of a hydro-conductive dressing to our standard burn dressing at removing edema fluid from partial-thickness burns and present the proposed mechanisms of action of the hydroconductive dressing. Methods: An internally controlled comparison of two wound dressings was performed on 10 patients with non-contiguous partial-thickness burns. Each patient served as his/her own control. One burn was treated with our standard burn dressing and the other with hydroconductive dressing. Dressings were weighed prior to application, removed at 24 and 48 hours, weighed, and new pre-weighed dressings applied. Weight gain of each dressing at 24 and 48 hours was determined. Statistics were applied using the Student’s paired T-test. The VAS pain scale was measured prior to, during, and after each dressing change. Results: At 24 hours, the hydroconductive dressing had increased 85.6% ± 29.3% in weight compared with 61.3% ± 32.7% for the control (P = 0.053). For the second 24 hour period, the respective numbers were statistically significantly different at 59.7% ± 23.4% vs. 34.2% ± 19.1% (P = 0.038). Averaging the weight gain over the two dressing periods demonstrated that the differences were highly statistically significant as the hydroconductive dressing increased in weight by 71.0% ± 20.3% compared with 44.5% ± 17.4% for the gauze dressing (P = 0.005). VAS scores revealed no statistical differences. Mechanisms of action included capillary, hydroconductive, and electrostatic actions. Conclusions: A hydroconductive dressing designed to draw off excessive wound fluid removes more wound edema fluid than standard burn gauze dressings.

A Comparison of Breast Surgeon and Nomogram-Generated Risk Predictions of Sentinel and Non-Sentinel Node Metastases

Memorial Sloan-Kettering Cancer Center (MSKCC) has developed 2 nomograms: the Sentinel Lymph Node Nomogram (SLNN), which is used to predict the likelihood of sentinel lymph node (SLN) metastases in patients with invasive breast cancer, and the Non-Sentinel Lymph Node Nomogram (NSLNN), which is used to predict the likelihood of residual axillary disease after a positive SLN biopsy. Our purpose was to compare the accuracy of MSKCC nomogram predictions with those made by breast surgeons. Two questionnaires were built with characteristics of two sets of 33 randomly selected patients from the MSKCC Sentinel Node Database. The first included only patients with invasive breast cancer, and the second included only patients with invasive breast cancer and positive SLN biopsy. 26 randomly selected Brazilian breast surgeons were asked about the probability of each patient in the first set having SLN metastases and each patient in the second set having additional non-SLN metastases. The predictions of the nomograms and breast surgeons were compared. There was no correlation between nomogram risk predictions and breast surgeon risk prediction estimates for either the SLNN or the NSLNN. The area under the receiver operating characteristics curves (AUCs) were 0.871 and 0.657 for SLNN and breast surgeons, respectively (p 0.0001), and 0.889 and 0.575 for the NSLNN and breast surgeons, respectively (p 0.0001). The nomograms were significantly more accurate as prediction tools than the risk predictions of breast surgeons in Brazil. This study demonstrates the potential utility of both nomograms in the decision-making process for patients with invasive breast cancer.

Public health decision makers rely on hospitalization forecasts to inform COVID-19 pandemic planning and resource *** forecasts are most relevant when they are accurate,made available quickly,and updated *** rapidly adapted an agent-based model(ABM)to provide weekly 30-day hospitalization forecasts(i.e.,demand for intensive care unit[ICU]beds and non-ICU beds)by state and region in North Carolina for public health decision *** ABM was based on a synthetic population of North Carolina residents and included movement of agents(i.e.,patients)among North Carolina hospitals,nursing homes,and the *** assigned SARSCoV-2 infection to agents using county-level compartmental models and determined agents’COVID-19 severity and probability of hospitalization using synthetic population characteristics(e.g.,age,comorbidities).We generated weekly 30-day hospitalization forecasts during MayeDecember 2020 and evaluated the impact of major model updates on statewide forecast accuracy under a SARS-CoV-2 effective reproduction number range of *** the 21 forecasts included in the assessment,the average mean absolute percentage error(MAPE)was 7.8%for non-ICU beds and 23.6%for ICU *** the major model updates,integration of near-real-time hospital occupancy data into the model had the largest impact on improving forecast accuracy,reducing the average MAPE for non-ICU beds from 6.6%to 3.9%and for ICU beds from 33.4%to 6.5%.Our results suggest that future pandemic hospitalization forecasting efforts should prioritize early inclusion of hospital occupancy data to maximize accuracy.

AIM To examine the effect of high doses of vitamin C(VitC) on ex vivo human platelets(PLTs).METHODS Platelet concentrates collected for therapeutic or prophylactic transfusions were exposed to:(1) normal saline(control);(2) 0.3 mmol/L VitC(Lo VitC); or(3) 3 mmol/L VitC(Hi VitC, final concentrations) and stored appropriately. The Vit C additive was preservative-free buffered ascorbic acid in water, pH 5.5 to 7.0, adjusted with sodium bicarbonate and sodium hydroxide. The doses of Vit C used here correspond to plasma Vit C levels reported in recently completed clinical trials. Prior to supplementation, a baseline sample was collected for analysis. PLTs were sampled again on days 2, 5 and 8 and assayed for changes in PLT function by: Thromboelastography(TEG), for changes in viscoelastic properties; aggregometry, for PLT aggregation and adenosine triphosphate(ATP) secretion in response to collagen or adenosine diphosphate(ADP); and flow cytometry, for changes in expression of CD-31, CD41 a, CD62 p and CD63. In addition, PLT intracellular Vit C content was measured using a fluorimetric assay for ascorbic acid and PLT poor plasma was used for plasma coagulation tests [prothrombin time(PT), partial thrombplastin time(PTT), functional fibrinogen] and Lipidomics analysis(UPLC ESI-MS/MS).RESULTS VitC supplementation significantly increased PLTs intracellular ascorbic acid levels from 1.2 mmol/L at baseline to 3.2 mmol/L(Lo VitC) and 15.7 mmol/L(Hi VitC, P 0.05). PLT function assayed by TEG, aggregometry and flow cytometry was not significantly altered by Lo or Hi VitC for up to 5 d. However, PLTs exposed to 3 mmol/L VitC for 8 d demonstrated significantly increased R and K times by TEG and a decrease in the α-angle(P < 0.05).
