Clinical outcomes for men with positive surgical margins after radical prostatectomy—results from the South Australian Prostate Cancer Clinical Outcomes Collaborative community-based registry

Objective Positive surgical margins(PSMs)after radical prostatectomy(RP)indicate failure of surgery to completely clear *** confer an increased risk of biochemical recurrence(BCR),but how more robust outcomes are affected is *** study investigated factors associated with PSMs following RP and determined their impact on clinical outcomes(BCR,second treatment[radiotherapy and/or androgen deprivation therapy],and prostate cancer-specific mortality[PCSM]).Methods The study cohort included men diagnosed with prostate cancer(pT2-3b/N0/M0)between January 1998 and June 2016 who underwent RP from the South Australian Prostate Cancer Clinical Outcomes Collaborative *** associated with risk of PSMs were identified using Poisson *** impact of PSMs on clinical outcomes(BCR,second treatment,and PCSM)was assessed using competing risk *** Of the 2827 eligible participants,28%had PSMs—10%apical,6%bladder neck,17%posterolateral,and 5%at multiple *** follow-up was 9.6 years with 81 deaths from prostate cancer *** of PSM increased with higher pathological grade and pathological tumor stage,and greater tumour volume,but decreased with increasing surgeon volume(odds ratio[OR]:0.93;95%confidence interval[CI]:0.88-0.98,per 100 previous prostatectomies).PSMs were associated with increased risk of BCR(adjusted sub-distribution hazard ratio[sHR]2.5;95%CI 2.1-3.1)and second treatment(sHR 2.9;95%CI 2.4-3.5).Risk of BCR was increased similarly for each PSM location,but was higher for multiple margin *** found no association between PSMs and *** Our findings support previous research suggesting that PSMs are not independently associated with PCSM despite strong association with *** PSM rates remains an important objective,given the higher likelihood of secondary treatment with associated comorbidities.

Background:Immune checkpoint inhibitors(ICIs)are effective in a subset of patients with metastatic solid ***,the patients who would benefit most from ICIs in biliary tract cancer(BTC)are still *** and methods:We molecularly characterized tissues and blood from 32 patients with metastatic BTC treated with the ICI pembrolizumab as second-line ***:All patients had microsatellite stable(MSS)type *** of the 32 patients achieved partial response(PR),with an objective response rate(ORR)of 9.4%(95%confidence interval[CI],2.0–25.2)and nine showed stable disease(SD),exhibiting a disease control rate(DCR)of 37.5%(95%CI,21.1–56.3).For the 31 patients who had access to PD-1 ligand 1(PD-l1)combined positive score(CPS)testing(cut-off value≥1%),the ORR was not different between those who had PD-l1-positive(PD-l1+;1/11,9.1%)and PDl1-(2/20,10.0%)tumors(p=1.000).The tumor mutational burden(TMB)of PD-l1+BTC was comparable to that of PD-l1-BTC(p=0.630).TMB and any exonic somatic mutations were also not predictive of pembrolizumab *** analysis of blood and tumor samples demonstrated a relatively high natural killer(NK)cell proportion in the peripheral blood before pembrolizumab treatment in patients who achieved tumor ***,the tumors of these patients presented high enrichment scores for NK cells,antitumor cytokines,and Th1 signatures,and a low enrichment score for cancer-associated ***:This study shows the molecular characteristics associated with the efficacy of pembrolizumab in BTC of the MSS type.

Probing anomalous top-Higgs couplings at the Hl-lHC via H→WW~* decay channels

We study the prospects of probing the anomalous tHq(q=u,c)couplings via SS2l or 3l signatures at the High luminosity(Hl-lHC)run of the 14 Te V CERN *** focus on signals of the tH associated production followed by the decay modes t→b?^+ν_?and H→W W~*,and ■ production followed by the decay modes t→b?^+ν_?and■where?=e,μ.Based on two types of H→W W~*decay topologies,one assuming the semileptonic decay mode H→W W~*→?^+νjj and the other the fully leptonic decay mode■,we perform a full simulation for signals and *** is shown that,at the future Hl-lHC,the branching ratio Br(t→uh)(Br(t→ch))can be probed to 1.17(1.56)×10^(-3)for the same-sign di-lepton channel,and to7.1×10^(-4)(1.39×10^(-3))for the 3l channel at 3σsensitivity.

The coronavirus disease 2019(COVID-19)pandemic has been linked to an increased prevalence of mental health disorders,particularly anxiety and ***,the COVID-19 pandemic has caused stress in people worldwide due to several factors,including fear of infection;social isolation;difficulty in adapting to new routines;lack of coping methods;high exposure to social media,misinformation,and fake reports;economic impact of the measures implemented to slow the contagion and concerns regarding the disease ***-19 patients have elevated levels of pro-inflammatory cytokines,such as interleukin(Il)-1β,Il-6,and tumor necrosis factor-α,and other inflammation-related ***,invasion of the central nervous system by the novel severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)may potentially contribute to neuroinflammatory alterations in infected ***,a consequence of psychological stress due to the COVID-19 pandemic,may also play a role in the development of anxiety and depressive symptoms in the general *** that neuroinflammation plays a significant role in the pathophysiology of depression and anxiety,this study investigated the effects of SARS-CoV-2 on mental health and focused on the impact of the COVID-19 pandemic on the neuroinflammatory pathways.

Effect of Aging Treatment on Microstructural and Mechanical Characteristics of PEO Coatings on Mg-Al Alloy

Cast Mg-6 wt pct Al alloy solution-treated at 683 K for 16 h and aged at 498 K was coated by plasma electrolytic oxidation （PEO） *** Mg-6 wt pct Al alloy aged for 16 h exhibited the highest microhardness and wear *** PEO coating,however,the microhardness and wear resistance of coatings on Mg- 6 wt pct Al alloy showed a tendency to decrease with increasing aging time,which was in aggrement with the change of thickness with aging *** addition,the coatings on solution-treated Mg-6 wt pct Al alloy had better microhardness and wear resistance than those on aged Mg-6 wt pct Al ***,it can be understood that the aging treatment has a deleterious influence on the mechanical properties of coatings on Mg-6 wt pct Al alloy.

The Southern Annular Mode(SAM) in PMIP2 Simulations of the last Glacial Maximum

The increasing trend of the Southern Annular Mode （SAM） in recent decades has influenced climate change in the Southem Hemisphere （SH）.How the SAM will respond increased greenhouse gas concentrations in the future remains *** the variability of the SAM in the past under a colder climate such as during the last Glacial Maximum （lGM） might provide some understanding of the response of the SAM under a future warmer *** analyzed the changes in the SAM during the lGM in comparison to pre-industrial （PI） simulations using five coupled ocean-atmosphere models （CCSM,FGOAlS,IPSl,MIROC,HadCM） from the second phase of the Paleoclimate Modelling Intercomparison Project （PMIP2）.In CCSM,MIROC,IPSl,and FGOAlS,the variability of the simulated SAM appears to be reduced in the lGM compared to the PI simulations,with a decrease in the standard deviation of the SAM ***,four out of the five models suggest a weaker SAM amplitude in the lGM consistent with a weaker SH polar vortex and westerly winds found in some proxy records and model *** weakening of the SAM in the lGM was associated with an increase in the vertical propagation of Rossby waves in southern high latitudes.

Maize response to nitrogen and phosphorus starter fertilisation in mineral-fertilised or manured systems

Phosphorus(P)is an essential nutrient for maize production,but in temperate areas the P uptake during early growing stages can be limited due to low soil temperature,even though the soil was tested high in *** objective of this study was to assess the effects of nitrogen and phosphorous(NP)starter fertilisation during early growth stages and its carryover until maize harvest,in mineral-fertilised or manured systems.A field experiment was carried out in north-west Italy during the 2019 and 2020 growing *** trial compared sub-surface placement of NP(diammonium phosphate)or N alone(ammonium nitrate)in bands close to the maize seed furrows,in differing long-term(lT)fertilisation managements:two doses of urea(Min-l and Min-H),two doses of bovine slurry(Slu-l and Slu-H)or two doses of farmyard manure(Fym-l and Fym-H).The two rates,low(l)and high(H),corresponded to 170 and250 kg N ha^(-1)year^(-1) *** to N fertilisation,NP starter fertilisation improved early maize growth assessed by leaf area index(lAI)and shoot dry weight(SDW)in all *** effects differed between the two years(2019:lAI+63%,SDW+67%;2020:lAI+36%,SDW+38%),as 2019was cool during the first *** lAI and SDW values were confirmed at crop flowering in the mineral-fertilised systems *** shoot growth was enhanced by NP starter fertilisation,anthesis occurred 1 day earlier in all ***,a response to NP starter fertilisation at harvest was recorded in mineral-fertilised systems only(+1.3 and+3.2 t ha^(-1) in Min-l and Min-H,respectively).The uptake of P,used as a true indicator of soil nutrient availability,increased with increasing soil Olsen P until 39 mg kg^(-1).These results suggest that soil test thresholds should be revised for points above which P fertilisation should be suspended.

INVERTING GENERATING FUNCTIONS WITH INCREASED NUMERICAl PRECISION-A COMPUTATIONAl EXPERIENCE

In this paper, we consider the numerical inversion of a variety of generating functions （GFs） that arise in the area of engineering and non-engineering fields. Three classes of GFs are taken into account in a comprehensive manner： classes of probability generating functions （PGFs） that are given in rational and non-rational forms, and a class of GFs that are not PGFs. Among others, those PGFs that are not explicitly given but contain a number of unknowns are largely considered as they are often encountered in many interesting applied problems. For the numerical inversion of GFs, we use the methods of the discrete （fast） Fourier transform and the Taylor series expansion. Through these methods, we show that it is remarkably easy to obtain the desired sequence to any given accuracy, so long as enough numerical precision is used in computations. Since high precision is readily available in current software packages and programming languages, one can now lift, with little effort, the so-called laplacian curtain that veils the sequence of interest. To demonstrate, we take a series of representative examples： the PGF of the number of customers in the discrete-time Geo^X/Geo/c queue, the same in the continuous-time M^X/D/c queue, and the GFs arising in the discrete-time renewal process.

Extent and predictors of grade upgrading and downgrading in an Australian cohort according to the new prostate cancer grade groupings

Object:To determine the extent and impact of upgrading and downgrading among men who underwent radical prostatectomy(RP)according to new grade groupings and to identify predictors of upgrading from biopsy grade Group Ⅰ and Ⅱ,and downgrading to grade Group I,in a community ***:Study participants included 2279 men with non-metastatic prostate cancer diagnosed 2006-2015 who underwent prostatectomy,from the multi-institutional South Australia Prostate Cancer Clinical Outcomes Collaborative *** of up-or down-grading was assessed by comparing biopsy and prostatectomy grade *** of biochemical recurrence(BCR)with upgrading was assessed using multivariable competing risk *** logistic regression was used to identify pre-treatment predictors of upgrading from grade Groups Ⅰ and Ⅱ,and risk group reclassification among men with low risk ***:Upgrading occurred in 35%of cases,while downgrading occurred in 13%of *** percent with grade Group I disease were upgraded following *** from grade Group I was associated with greater risk of BCR compared with concordant grading(Hazard ratio:3.1,95%confidence interval:1.7-6.0).Older age,higher prostate-specific antigen levels(PSA),fewer biopsy cores,higher number of positive cores and more recent diagnosis predicted upgrading from grade Group Ⅰ,while higher PSA and clinical stage predicted upgrading from grade Group Ⅱ.No clinical risk factors for reclassification were ***:Biopsy sampling errors may play an important role in upgrading from grade Group *** clinical assessment of grade is needed to encourage greater uptake of active surveillance.
