Clinical outcomes of elderly South-East Asian patients in primary percutaneous coronary intervention for ST- elevation myocardial infarction

Objective To evaluate the clinical characteristics and in-hospital outcomes of elderly South-East Asian patients undergoing primary pereutaneous coronary intervention （PPCI）. Methods From January 2009 to December 2012, 1268 patients （86.4% male, mean age of 58,4 ± 12.2 years） presented to our hospital for ST-elevation myocardial infarction （STEMI） and underwent PPCI. They were divided into two groups： elderly group defined as age _〉 70 years and non-elderly group defined as age 〈 70 years. Data were collected retrospectively on baseline clinical characteristics, door-to-balloon （D2B） time, angiographic findings, therapeutic modality and hospital course. Results The elderly group constituted 19% of the study population with mean age 76.6 ± 5.0 years. There was a higher proportion of female gender and ethnic Chinese patients in the elderly group when compared with the non-elderly group. The former was less likely to be smokers and have a significantly higher prevalence of hypertension. The mean D2B time was significantly longer in the elderly group. They also had a significantly higher incidence of triple vessel disease and obstructive left main disease. The use of radial artery access, glyeoprotein 2b/3a inhibitors and drug-eluting stents during PPCI were also significantly lower. In-hospital mortality was significantly higher in the elderly group. The rate of cardiogenic shock and inhospital complications were also significantly higher. Conclusions Our registry showed that in-hospital mortality rate in elderly South-East Asian patients undergoing PPCI for STEMI was high. Further studies into the optimal STEMI management strat- egy for these elderly patients are warranted.

Liver transplant(LT)is the curative treatment for patients with hepatocellular carcinoma(HCC).Bridge therapies are local treatments given to patients on the LT waitlist,to prevent tumor progression and to reduce the dropout *** presentation:We reported a 40-year-old man diagnosed with Barcenola-Clinic Liver Cancer BCLC intermediate stage HCC and Child-Pugh A5 hepatitis B virus cirrhosis who underwent combined bridge therapies to ***,the patient received transarterial chemoembolization(TACE)for two times and showed a partial *** he underwent stereotactic body radiation therapy(SBRT)with a total dose of 45 Gy in 3 *** months later,the tumor size and serum protein induced by Vitamin K absence or antagonists-II,alpha fetoprotein levels decreased *** June 2019 a suitable donor was found and his LT was successfully ***:We propose that a combination of TACE and SBRT was feasible as bridge therapy for HCC patients on the LT waitlist.

AIM:To determine the dry eye(DE)rate and its relationship with disease stage in patients with primary ***:A cross-sectional study included 432 patients with primary hypertension(with an equal number of patients in each group:144 in stage Ⅰ,Ⅱ,and Ⅲ hypertension)and 144 healthy subjects as a control *** Ocular Surface Disease Index(OSDI)and Schirmer Ⅰ test without anesthetics were conducted on all 576 *** with OSDI scores<13 and Schirmer Ⅰ values equal to or under 10 mm were diagnosed with ***:The ratio of DE in hypertension patients was higher than in the control group(41.7%versus 18.8%;P<0.001).The proportion of patients with DE increased gradually according to the hypertension stage:27.1% in stage Ⅰ,40.3% in stage Ⅱ,and 57.6% in stage Ⅲ,P<***,duration of hypertension,plasma urea,creatinine,and high-sensitivity C-reactive protein(CRP-hs)levels in hypertension patients with DE were higher than those without DE,P<*** age,a long duration of hypertension,diabetes mellitus,elevated plasma creatinine,and CRP-hs levels were independent factors associated with DE in primary hypertension patients,P<***:DE is a common disorder associated with advanced age,a long duration of hypertension,diabetes mellitus,elevated plasma CRP-hs,and creatinine levels in patients with primary hypertension.

Karst habitats are hotspots of diversity and endemism. Their naturally fragmented distributions across broad geographic landscapes have led to the complex array of smaller evolutionary ecosystems that present unique challenges from a conservation *** assessments of karst habitats have revealed that these ecosystems contain an almost unparalleled level of endemism, and many site-restricted species remain undescribed, thus posing considerable challenges for effective conservation *** rock-dwelling species, such as geckos, may be particularly prone to such isolation. In this paper, we discuss one such genus, i.e.,Hemiphyllodactylus, and explore its diversity across karst landforms in Yunnan Province, southwestern China. Based on morphological and genetic data, we describe two new species of Hemiphyllodactylus from karst habitats in Simao District and Yanshan County. A phylogenetic tree for Hemiphyllodactylus was constructed using 1?039 base pairs(bp) of the mitochondrial NADH dehydrogenase subunit 2 gene(ND2). The Simao and Yanshan specimens can be distinguished from all other congeners within their respective subclades based on uncorrected genetic pairwise distances greater than 6.3% and 4.3%respectively, as well as significant morphological differences. The discovery and description of these two new species brings the total number of described Hemiphyllodactylus species in China to 14and indicates many more undescribed species from unsurveyed karst regions await *** findings suggest that karst ecosystems in Yunnan support a higher diversity of Hemiphyllodactylus than previously *** study also highlights the importance of karst ecosystems as refugia for sitespecific endemic species and the need for heightened conservation efforts.

在美国,大多数学校讲授的数学是学校"数学",它与数学家所从事的数学有着本质的不同.为此要区分以下4种差异:(1)约定俗成的数学与学校数学之间的差异;(2)理解方式与思维方式之间的差异;(3)成熟的学习者与被动的学习者之间的差异;(4)知识传授与知识参与这两种教学模式之间的差异.根据Harel的对偶原则来设计一个数学任务时,应该考虑学生已有的思维方式和理解方式,如激发学生学习某一特定概念的需要,促进理想的思维方式,阻止不合适的思维方式以及评估学生的概念性理解.

【目的】对顶果木灰霉病病原菌进行鉴定,以明确其分类地位,为顶果木灰霉病的防控提供参考依据。【方法】从广西南宁市良凤江国家森林公园苗圃基地采集顶果木灰霉病病害标本,对病害进行症状描述、组织分离和致病性测定,观察致病菌的形态学特征并进行r DNA-ITS序列分析。【结果】分离获得顶果木灰霉病致病菌代表菌株DGM5-6。DGM5-6菌株在PDA上长势良好,毛绒状菌落初期呈淡黄白色,后期逐渐变为灰白色;气生菌丝呈放射状生长,开始为白色,生长后期为灰色;病菌分生孢子梗单生,灰色至褐色,分枝,顶端膨大成球形,其上有多个小梗,梗上聚生卵形、圆形或椭圆形的分生孢子;分生孢子呈浅灰色至无色,单胞,大小为10.0-12.5μm×7.5-10.0μm。DGM5-6菌株ITS基因扩增片段长度为535 bp,其r DNA-ITS序列与Gen Bank中灰葡萄孢（Botrytis cinerea *** Fr）序列的同源性达99%。【结论】引起广西顶果木灰霉病的病原为灰葡萄孢（Botrytis cinerea *** Fr）,该病原菌寄主范围广,应结合苗圃清园、圃地治理、合理密植、生物防治和化学防治等进行防控。

Objectives A nurse duty roster is usually prepared monthly in a hospital *** is common for nurses to make duty shift requests prior to scheduling.A ward manager normally spends more than a working day to manually prepare and subsequently to optimally adjust the schedule upon staff requests and hospital *** study aimed to develop an automatic nurse roster scheduling system with the use of open-source operational research tools by taking into account the hospital standards and the constraints from *** Artificial intelligence and end user tools operational research tools were used to develop the code for the nurse duty roster scheduling *** compare with previous research on various heuristics in employee scheduling,the current system was developed on open architecture and adopted with real shift duty requirements in a hospital *** The schedule can be generated within 1 min under both hard and soft constraint *** hard constraints are fulfilled and most nurse soft constraints could be *** with those schedules prepared manually,the computer-generated schedules were more optimally adjusted as real time interaction among nurses and management *** generated schedules were flexible to cope with daily and hourly duty changes by redeploying ward staff in order to maintain safe staffing *** An economical but yet highly efficient and user friendly solution to nurse roster scheduling system has been developed and adopted using open-source operational research *** open-source platform is found to perform satisfactorily in scheduling *** system can be implemented to different wards in hospitals and be regularly updated with new hospital polices and nurse manpower by hospital information personnel with training in artificial intelligence.

＜正＞In this paper,we propose a fuzzy adaptive controller design for a class of networked control systems（NCS） with the presence of network induced delay,data packet dropout and unknown time-delay controlled *** mathematical model of Smith predictor,which is combined with fuzzy adaptive controller and fuzzy compensator time-delay,is designed to adjust online its parameters according to the changing of the system’s *** results based on TrueTime Beta2.0 simulation platform demonstrate that our design significantly improves the response of system over unknown time-delay.

In this paper, we dealt with the problems of network induced delay and randomly varying time-delay （RVTD） controlled plant in networked control systems （NCS）. These inherent challenges make the conventional control methods, e.g, a mathematical model of Smith predictor combined with fuzzy adaptive controller, more difficult to meet the quality requirements for NCS stability. Specifically, based on analyzing the existing techniques, we propose a novel method to efficiently compensate the RVTD for NCS. This so-called RVTD compensator for the Smith predictor using Neural network estimation scheme has not only the features of simple Smith predict structure, but also the characteristics of adaptivity, stability, and fast response. The simulation results via TrueTime Beta2.0 platform demonstrate that our design significantly improves the performance of NCS.
