The discovery of a strong association between hepatitis C virus(HCV) infection and mixed cryoglobulinemia(MC) has led to an increasingly rare diagnosis of idiopathic essential MC(EMC).The incidence of EMC is high in regions where there is a comparatively low HCV infection burden and low in areas of high infection prevalence,including *** diagnosis of EMC requires an extensive laboratory investigation to exclude all possible causes of cryoglobulin *** addition,although cryoglobulin testing is simple,improper testing conditions will result in false negative ***,we present a 46-year-old female patient with a case of EMC with dermatological and renal manifestations,highlighting the importance of extensive investigation to reacha proper *** review the need for appropriate laboratory testing,which is often neglected in clinical practice and which can result in false negative *** review also emphasizes the significance of an extended testing repertoire necessary for better patient *** a strong association of MC with HCV infection and other causes that lead to cryoglobulin formation,EMC remains a separate *** diagnosis requires proper temperature regulation during sample handling,as well as characterization and quantification of the *** of rheumatoid factor activity and complement levels in the cryoglobulin test-panel promotes better patient management and *** guidelines should be developed and implemented for cryoglobulin detection and the diagnosis of cryoglobulinemic syndrome,which will reduce variability in inter-laboratory reporting.

BACKGROUND Lipid treatment practices and levels in post-acute myocardial infarction(AMI)patients,which are crucial for secondary *** To evaluate the lipid treatment practices and lipid levels in post-myocardial infarction(MI)patients at a tertiary care hospital in *** In this cross-sectional study,we analyzed patients who had experienced their first AMI event in the past 3 *** assessed fasting and non-fasting lipid profiles,reviewed statin therapy prescriptions,and examined patient *** recommended dose was defined as rosuvastatin≥20 mg or atorvastatin≥40 mg,with target total cholesterol levels set at<160 mg/dL and target low-density lipoprotein cholesterol(LDL-C)at<55 mg/*** Among 195 patients,71.3%were male,and the mean age was 57.1±10.2 *** median duration since AMI was 36(interquartile range:10-48)months and 60% were diagnosed with ST-segment elevation *** 13.8% of patients were advised to undergo lipid profile testing after AMI,88.7% of patients were on the recommended statin therapy,and 91.8% of patients were compliant with statin *** 11.5% had LDL-C within the target range and 71.7% had total cholesterol within the target *** admission in the past 12 months was reported by 14.4%,and the readmission rate was significantly higher among non-compliant patients(37.5%vs 5.6%).Subsequent AMI event rate was also significantly higher among non-compliant patients(43.8%vs 11.7%).CONCLUSION Our study highlights that while most post-AMI patients received the recommended minimum statin therapy dose,the inadequate practice of lipid assessment may compromise therapy optimization and raise the risk of subsequent events.

A method is presented for nano-patterning a diffraction grating on human hair with a focused ion beam. Strands of brown hair are patterned with hyperbolas and Archimedean spirals whose pitches range from 540 nm to 1040 nm. Exposure of the hair strands to white light at various incident angles demonstrates that light of varying wavelengths is diffracted by the diffraction gratings. The diffraction causes the brown strands of hair to reflect light from the entire range of visible light.

This paper reports on the major cations（Ca, Mg, Na and K） and arsenic（As） compositions of surface waters collected from major creeks, rivers and lakes in Central Victoria （Australia）. The surface waters were found to be neutral to alkaline （pH 6.7-9.4）, oxidised （average redox potential （Eh） about 130 mV） and showed variable concentrations of dissolved ions （EC, about 51-4386 /μS/cm）. The concen- trations of dissolved major cations in surface waters were found to be in the order of Na〉〉Mg〉Ca〉K and in soils the contents of metals followed an order of abundance as： Ca〉Mg〉〉K〉Na. While Na was the least abundant in soils, it registered the highest dissolved cation in surface waters. Of the four major cations, the average concentration ofNa（98.7 mg/L） was attributed to the weathering of feldspars and atmospheric input. Relatively highly dissolved concentrations of Na and Mg compared with the world average values of rivers reflected the weathering of rock and soil minerals within the catchments. The As soil level is naturally high（linked to lithology） as reflected by high background soil values and mining operations are also considered to be a contributory factor. Under relatively alkaline-oxidative conditions low mobility of dissolved As （average about 7.9 μg/L） was observed in most of the surface waters with a few higher values（〉15 μg/L） around a sewage disposal site and mine railings. Arsenic in soils is slowly released into water under alkaline and/or lower Eh conditions. The efficient sink of Fe, AI and Mn oxides acts as a barrier against the As release under near neutral-oxidising conditions. High As content （average about 28.3 mg/kg） in soils was found to be associated with Fe-hydroxides as revealed by XRD and SEM analysis. The dissolved As concentration was found to be below the recommended maximum levels for recreational water in all surface waters（lakes and rivers） in the study area. The geochemical baselines and distribution of 31 elements (Al, Fe, K, Na, Mg, Ca, Mn, Ba, Cr, Zr, Ni, Sr, Zn, Y, Li, Cu, Mo, Nb, Th, Co, Ga, W, Ta, Be, Ti, Ge, Se, Bi, Te, Sc and Re) and physico-chemical parameters of the tropical surface sediments of the Terengganu River basin, Malaysia, are reported. Sediments are sandy loam to sand in texture consisting of mostly quartz, low organic matter content (average-2.68%), low CEC (average-2.02 cmol(+)/kg) and mildly acidic pH1:5 (average-5.91). Concentrations of Mn, Fe, Ba, Cr, Ni, Cu, Mo and Se were measured to be above the environmental sediment quality criteria at various locations. Lake sediments registered significantly higher Al, Fe, Ti, Mg, Ca, Mn, Te and Sc concentrations as compared to the river sediments. Most of the elements investigated showed an association with silt size fraction (2-63 μm). Among the investigated metals, Mo and Fe concentrations showed an increasing (>5-fold) and decreasing (>3-fold) trend, respectively, along the river path from the upstream to the downstream depending on the stream pH-redox conditions. The enrichment factor values (EF >5) of Cr, Ni, Mo and Se indicated enrichment from anthropogenic activities. Alkali and alkali earth metals registered a significant depletion (EF values 1,000 mg/kg) indicating contamination of sediments. This work presents the geochemical baselines of the tropical sediments as industrial development and urbanization along the north east coast of Peninsular Malaysia are advancing rapidly.
