Purpose:To compare cartilage tympanoplasty(CT)combined with eustachian tube balloon dilatation(ETBD)and cartilage tympanoplasty alone as a surgical treatment modality for adhesive otitis media(AdOM)in terms of graft healing,audiological outcomes,and impact on life style,using Chronic Otitis Media Outcome Test 15(COMOT-15).Methods:50 patients with AdOM were randomly classified into 2 groups:25 patients for cartilage tympanoplasty only(CT group)and 25 patients for cartilage tympanoplasty combined with eustachian tube balloon dilatation(CT t ETBD group).Clinical outcomes in both groups were compared at 3 and 6 months of follow up.Results:There was no significant difference in graft healing between the two groups.Postoperative COMOT-15 scores significantly decreased in both groups with a significant difference between the groups with regard to the decrease in COMOT-15 scores at 3 and 6 months of follow-up(P<0.05).Hearing improvement was achieved,as the mean preoperative ABG was 26.5±5.4 and 27.1±4.6 dB,and the mean postoperative ABG at 6 months was 19.4±4.4 and 14.6±3.9 dB in the CT and the CT t ETBD groups,respectively.The difference in the magnitude of ABG reduction in the two groups was significant at 3 and 6 months of postoperative follow-up(P<0.05)in favour of the CT t ETBD group.Conclusion:ETBD can increase the success rate of cartilage tympanoplasty in patients with AdOM by enhancing the audiological outcome and quality of life.

The National Institute for Occupational Safety and Health(NIOSH)conducted a comprehensive monitoring program in a room-and-pillar mine located in Southern Virginia.The deformation and the stress change in an instrumented pillar were monitored during the progress of pillar retreat mining at two sites of different geological conditions and depths of cover.The main objectives of the monitoring program were to better understand the stress transfer and load shedding on coal pillars and to quantify the rib deformation due to pillar retreat mining;and to examine the effect of rib geology and overburden depth on coal rib performance.The instrumentation at both sites included pull-out tests to measure the anchorage capacity of rib bolts,load cells mounted on rib bolts to monitor the induced loads in the bolts,borehole pressure cells(BPCs)installed at various depths in the study pillar to measure the change in vertical pressure within the pillar,and roof and rib extensometers installed to quantify the vertical displacement of the roof and the horizontal displacement of the rib that would occur during the retreat mining process.The outcome from the monitoring program provides insight into coal pillar rib support optimization at various depths and geological conditions.Also,this study contributes to the NIOSH rib support database in U.S coal mines and provides essential data for rib support design.

The continuous development of cyberattacks is threatening digital transformation endeavors worldwide and leadsto wide losses for various organizations. These dangers have proven that signature-based approaches are insufficientto prevent emerging and polymorphic attacks. Therefore, this paper is proposing a Robust Malicious ExecutableDetection (RMED) using Host-based Machine Learning Classifier to discover malicious Portable Executable (PE)files in hosts using Windows operating systems through collecting PE headers and applying machine learningmechanisms to detect unknown infected files. The authors have collected a novel reliable dataset containing 116,031benign files and 179,071 malware samples from diverse sources to ensure the efficiency of RMED approach.The most effective PE headers that can highly differentiate between benign and malware files were selected totrain the model on 15 PE features to speed up the classification process and achieve real-time detection formalicious executables. The evaluation results showed that RMED succeeded in shrinking the classification timeto 91 milliseconds for each file while reaching an accuracy of 98.42% with a false positive rate equal to 1.58. Inconclusion, this paper contributes to the field of cybersecurity by presenting a comprehensive framework thatleverages Artificial Intelligence (AI) methods to proactively detect and prevent cyber-attacks.

Do statins reduce the mortality rate in stroke patients treated with systemic thrombolysis in a 5-year single-center study?

The present study investigated the association between pre-treatment with a cholesterol-lowering drug(statin) or new setting hereon and the effect on the mortality rate in patients with acute ischemic stroke who received intravenous systemic thrombolysis. During a 5-year period(starting in October 2008), 542 consecutive stroke patients who received intravenous systemic thrombolysis with recombinant tissue plasminogen activator(rt-PA) at the Department of Neurology, University Hospital Schleswig-Holstein, Campus Lübeck, Germany, were included. Patients were characterized according to statins. The primary endpoint was mortality;it was assessed twice: in hospital and 3 months after discharge. The secondary outcome was the rate of symptomatic intracerebral hemorrhage. Of the 542 stroke patients examined(mean age 72 ± 13 years;51% women, mean National Institutes of Health Stroke Scale(NIHSS) score 11), 138 patients(25.5%) had been pretreated with statin, while in 190 patients(35.1%) statin therapy was initiated during their stay in hospital, whereas 193(35.6%) never received statins. Patients pre-treated with statin were older and more frequently had previous illnesses(arterial hypertension, diabetes mellitus and previous cerebral infarctions), but were comparably similarly affected by the stroke(NIHSS 11 vs. 11;P = 0.76) compared to patients who were not on statin treatment at the time of cerebral infarction. Patients pretreated with statin did not differ in 3-month mortality from those newly treated to a statin(7.6% vs. 8%;P = 0.9). Interestingly, the group of patients pretreated with statin showed a lower rate of in hospital mortality(6.6% vs. 17.0;P = 0.005) and 3-month mortality(10.7% vs. 23.7%;P = 0.005) than the group of patients who had no statin treatment at all. The same effect was seen for patients newly adjusted to a statin during the hospital stay compared to patients who did not receive statins(3-month mortality: 7.1% vs. 23.7%;P < 0.001). Underground coal mines use mechanical bolts in addition to other types of bolts to control the rib deformation and to stabilize the yielded coal ribs.Limited research has been conducted to understand the performance of the mechanical bolts in coal ribs.Researchers from the National Institute for Occupational Safety and Health(NIOSH)conducted this work to understand the loading characteristics of mechanical bolts(stiffness and capacity)installed in coal ribs at five underground coal mines.Standard pull-out tests were performed in this study to define the loading characteristics of mechanical rib bolts.Different installation torques were applied to the tested bolts based on the strength of the coal seam.A typical tri-linear load-deformation response for mechanical bolts was obtained from these tests.It was found that the anchorage capacity depended mainly on the coal strength.Guidelines for modeling mechanical bolts have been developed using the tri-linear load-deformation response.The outcome of this research provides essential data for rib support design.

Infectious diseases resemble a great threat to the human health according to World Health Organization where about 17% of all deaths(z9.2 million deaths) in 2013 recorded are related to infectious diseases. The pathogenic microorganisms such as bacteria, viruses,fungi and parasites are the principle causes of infectious diseases. Ebola, AIDS, dengue,hepatitis, malaria, tuberculosis and schistosomiasis are among 216 infectious diseases found where the immunity represents the first line defense in infection. Lipidomic includes examination of different biological lipids in the biological cell. The lipidomic research covers all aspects of individual lipid molecule including its structure, function,connection with other cell constituents such as protein, lipid, and metabolite in both health and disease conditions. Details of cell biology obtained from different pathogens(viruses, bacteria, and parasites) provide a great data on molecular structure of hostpathogen relation and consequently on infection process. The lipids here play a very important role in many processes involved in host-pathogen relations. The role of lipid in host-pathogen link includes many processes in(1) structural host constituents,(2) host recognition,(3) intracellular transferring, and(4) energy and resource homeostasis during pathogen duplication. There are many lipid phosphatases, kinases, and lipases molecules that greatly involved in these processes and controlling pathogen expression and infection progress. The cell lipid metabolism depends on an adequate energy stores that push the infection to be accelerated and disease symptoms to be appeared. Consequently, future lipidomics studies are the basic for detecting the lipid role in host-pathogen relations which help in therapy advances and biomarkers development.

Globally, the third cause of males cancer and the fourth cause of females cancer is colon cancer(CC). In Egypt, high CC percentage occurs in children and in individuals below 40 years of age. The complete loss of biological enzyme function is the main cause of CC and consequently CC increased in smoking and pollution exposure. The aim of this review is to focus on the application of metabolome as a physiological tool that can play an important role in preventing CC incidence by natural products and hormones. The dietary factors, intestinal micro-flora and endogenously produced metabolites are the main three causes that produce free radicals in the colon. A correlation occurs between the enzyme activity and CC polymorphisms or property. Nowadays metabolome is applied with the progress of different analytical methods, data bases and tools for cancer predication and stimulation especially in CC cases. Metabolism is defined as intracellular chemical reactions that produce chemical substances and energies sustaining life. Metabolic pathway networks are also composed of links that are defined as transformation of chemical structures between two metabolites and an enzyme reaction. The most important advantage of metabolome is its ability to analyze metabolites from any source, regardless of origin, where the application of liquid chromatography combined with mass spectra in metabolome analysis to a series of cancer cell lines that were progressively more tumorigenic due to the induction of 1,2,3 or 4 oncogenes to cell lines could be a metabolome example application. In conclusion, natural products and hormones are very important in preventing CC in humans and animal models where both natural products and hormones play a significant and important effect in regulating physiological process especially in CC cases. In this situation, metabolome must increase in its application in the future for the diagnosis of CC cases.

Multiple sclerosis(MS) is the most famous autoimmune disease attacking the central nervous system. It attacks people from age 20–50 years old and the females' attacks double than males' attacks. MS is an autoimmune disease affecting principally the central nervous system that cause nerve sheath demyelination followed by axon damage and paralysis. MS symptoms include muscle weakness, weak reflexes, muscle spasm, difficult in move, miss-coordination and unbalance with others. There are many factors may be responsible for MS: microbial, viral, smoking, stress, environmental toxins, contaminated diet, and gout. MS is wide spread in the populations in North Europe and this related to lack of vitamin D due to decrease of sunlight exposure. MS biomarkers include nitric oxide, interleukin-6, nitric oxide synthase, fetuin-A and osteopontin. MS is not a genetic disease where MS occurs when human leukocyte antigen system related genes are changed in chromosome 6. The physiology of MS is monitored by activation of immuneinflammatory, oxidative, and nitrosative stress pathways. MS is including two main steps:(1) myelin sheath destruction and formation of lesions and,(2) inflammation. Four types of MS can be distinguished: relapsing-remitting, primary progressive, secondary progressive and progressive relapsing. Nine treatments have been accepted for relapsingremitting MS type: interferon b-1a, interferon b-1b, mitoxantrone, natalizumab, glatiramer acetate, fingolimod, dimethyl fumarate, teriflunomide, and alemtuzumab, however,the only treatment used is mitoxantrone for progressive MS but many of MS treatments side effects are recorded. Complementary treatments also used in MS treatments such as:vitamin D, Yoga, medicinal plants, oxygen therapy, acupuncture and reflexology.

A practical application of photogrammetry to performing rib characterization measurements in an underground coal mine using a DSLR camera

Understanding coal mine rib behavior is important for inferring pillar loading conditions as well as ensuring the safety of miners who are regularly exposed to ribs. Due to the variability in the geometry of underground openings and ground behavior, point measurements often fail to capture the true movement of mine workings. Photogrammetry is a potentially fast, cheap, and precise supplemental measurement tool in comparison to extensometers, tape measures, or laser range meters, but its application in underground coal has been limited. The practical use of photogrammetry was tested at the Safety Research Coal Mine, National Institute for Occupational Safety and Health(NIOSH). A commercially available, digital single-lens reflex(DSLR) camera was used to perform the photogrammetric surveys for the experiment. Several experiments were performed using different lighting conditions, distances to subject,camera settings, and photograph overlaps, with results summarized as follows: the lighting method was found to be insignificant if the scene was appropriately illuminated. It was found that the distance to the subject has a minimal impact on result accuracy, and that camera settings have a significant impact on the photogrammetric quality of images. An increasing photograph resolution was preferable when measuring plane orientations; otherwise a high point cloud density would likely be excessive. Focal ratio(F-stop) changes affect the depth of field and image quality in situations where multiple angles are necessary to survey cleat orientations. Photograph overlap is very important to proper three-dimensional reconstruction, and at least 60% overlap between photograph pairs is ideal to avoid unnecessary post-processing. The suggestions and guidelines proposed are designed to increase the quality of photogrammetry inputs and outputs as well as minimize processing time, and serve as a starting point for an underground coal photogrammetry study.

BACKGROUND Bisphenol A(BPA)is present in many plastic products and food packaging.On the other hand,fertaric acid(FA)is a hydroxycinnamic acid.AIM To investigate the effect of FA on BPA-related liver,kidney,and testis toxicity,DNA breakdown,and histopathology in male rats.METHODS Thirty male albino rats were divided into five equal groups(6 rats/group):Control,paraffin oil,FA-,BPA-,and FA+BPA-treated groups.The control and paraffin oil groups were administered orally with 1 mL distilled water and 1 mL paraffin oil,respectively.The FA-,BPA-,and FA+BPA-treated groups were administered orally with FA(45 mg/kg,bw)dissolved in 1 mL distilled water,BPA(4 mg/kg,bw)dissolved in 1 mL paraffin oil,and FA(45 mg/kg,bw)followed by BPA(4 mg/kg,bw),respectively.All these treatments were given once a day for 6 wk.RESULTS BPA induced a significant decrease in serum alkaline phosphatase,acid phosphatase,sodium,potassium and chloride,testosterone,dehydroepiandrosterone sulfate,glucose-6-phosphate dehydrogenase,3β-hydroxysteroid dehydrogenase,and testis protein levels but a highly significant increase in serum aspartate aminotransferase,alanine aminotransferase,γ-glutamyl transpeptidase,lactate dehydrogenase,bilirubin,urea,creatinine,uric acid,luteinizing hormone,follicle stimulating hormone,sex hormone binding globulin,blood urea nitrogen,and testis cholesterol levels.Also,FA inhibited the degradation of liver,kidney,and testis DNA content.Oral administration of FA to BPA-treated rats restored all the above parameters to normal levels.CONCLUSION FA ameliorates BPA-induced liver,kidney,and testis toxicity,DNA breakdown,and histopathological changes.
