Heart Failure in Children: Epidemiological, Etiological and Diagnostic Aspects in Diamniadio Children’s Hospital (Senegal)

Introduction: Heart failure is a major cause of morbidity and mortality in children. The authors aimed to report the epidemiological and etiological characteristics of heart failure (HF) in children in Senegal. Patients and Methods: This was a retrospective study conducted at the Children’s Hospital of Diamniadio (CHD) from 01-01-2016 to 31-12-2017. Children aged 0 to 15 years with heart failure confirmed by echocardiography were included. Results: Sixty-six children were admitted for heart failure. The prevalence of HF was 5.3‰. The mean age of the children was 41.59 months ± 53.35 months (range: 0 to 168 months). The 0 - 5 age group was the most represented (n = 48;72.72%). The HF concerned 38 boys and 28 girls, a sex ratio (M/F) of 1.36. The majority of our patients came from the semi-urban area (42.4%). The etiology of HF was represented in order of frequency by congenital heart disease, rheumatic heart disease and cardiomyopathy with 59.09% of cases, 22.73% of cases and 7.58% of cases respectively. It was unknown in 4.54% of the cases. At echocardiography, the left ventricular ejection fraction (LVEF) was impaired (less than 50%) in 19 patients, in 29.2% of cases with an average of 64% +/- 15.16% (extremes: 24% and 86%). Anemia was the comorbidity most commonly associated with their condition (n = 33%;50%). Conclusion: Heart failure in children is uncommon. The main etiology is represented by congenital heart disease.

Renewable Energy Powered Membrane Process for Removal of Excess Fluoride Ions and Safe Drinking Water Supply in Rural Areas in Senegal

The analysis results made on drinking water from the rural areas of Kaolack,Diourbel and Fatick central region of Senegal showed the excess of fluoride and chloride *** on the area,the fluoride concentration in the water can reach 3.5 mg/*** values obtained are completely above of WHO(World Health Organization)*** objective of this study is to assess the performances of membrane filtration units powered by renewable energies for fluoride and salinity excess removal in remote village in *** membrane filtration units have been installed in rural areas of regions such as Kaolack,Fatick and *** membrane filtration units are equipped by the LPRO(Low Pressure Reverse Osmosis)membranes with 9 m^(2)of surface.A rejection rate of fluoride and chloride ions obtained is 99.33%and 95.25%*** conversion rates are ranged from 45%to 65%.These results clearly show that membrane processes can be used in Africa,especially in isolated rural areas,with the combination of renewable ***,more than twenty membrane filtration units are installed in Senegal and provide drinking water of very good quality for populations living in rural *** prospect is to expand it on a larger scale,which is already underway,with the construction of a desalination plant in Dakar.

The aim of this study was to evaluate the dose delivered to patients during scannographic examinations at the Kaolack Regional Hospital (one of the 14 regions of Senegal located in the center-west of the country, 192 km from Dakar) and to compare these irradiation doses at diagnostic reference levels found in the literature in order to optimize our scanning protocols. To achieve this goal, we carried out a prospective study involving 218 CT scans. These examinations focused on cerebral, thoracic, abdominopelvic and lumbar spine acquisitions made in adults and cerebral only in children. We compared the median values of dosimetric indicators (CTDIvol and DLP) per acquisition with NRDs in the literature. During the course of this study, we found a dosimetric ratio lower than that of the NRDs for thoracic, abdomino-pelvic and lumbar spine CT scans in adults and a significant dosimetric exceedance for brain scans in adults and in children. These results encourage us to extend these dosimetric evaluations to other hospital structures in order to establish rigorous monitoring of dosimetric indicators and to respect the principles of radiation protection and good practice.

Place of Exercise Echocardiography in Stable Coronary Artery Disease in Sub-Saharan Africa:A Series in Senegal

Introduction: Exercise echocardiography is a non-invasive technique that occupies a special place for the detection of stable coronary disease. The main objective of this study was to report our experience and assess our practice in the diagnosis of stable coronaryartery disease. Methodology: We conducted a retrospective study, descriptive over a period of 34 months, from December 1, 2016 to September 30, 2019. All the patients received during the study period for stress echocardiography as part of a suspicion of stable coronary artery disease were included. A total of 100 examinations were completed during the study period. Results: Ninety (90) exams were studied. There were 56 men or a sex-ratio of 1.64. The mean age was of 57.3 ± 10 years. Hypertension (31 patients or 62%) was the most frequent risk factor. Typical chest pain was noticed in 45 patients (52.3%). The pre-test probability was intermediate in 77 patients (89.5%). Echocardiography at rest was normal in 86 patients (95.6%). A total of 62 tests were negative (69%) and 02 (2.2%) were non-contributory. Ten tests (10) of 26 electrical positive tests were associated with segmental kinetics disorders. In the 10 patients who had exercise kinetic disorders, 08 had coronary angiography. It was normal in 04 of them. The positive predictive value of exercise echography was 50% in our study. In a patient with a negative exercise echocardiography with a high clinical probability of coronary disease, the coronary angiography showed a two-vessel impairment with an intermediate lesion of Cx2 and an intermediate lesion of RCA2. It thus constitutes a false negative. Sensitivity was 80% in our study. We noticed an incident like non-sustained ventricular tachycardia. Conclusion: Exercise echocardiography is a safe and reliable examination for the diagnosis of stable coronary artery disease. A good selection of patients based on the clinical probability of coronary ischemia should improve our sensitivity. This is even more

In sub-Saharan Africa, Sorghum (Sorghum bicolor) is an important cereal for both human being and animals. Unfortunately, its production is confronted to soils with deficiency of phosphorus. Traditional use of mineral phosphate on this culture fertilization is expensive and may cause contamination. It is thus necessary to seek more efficient and economic reasonable techniques to improve sorghum growth. Arbuscular mycorrhizal fungi (AMF) constitute a reference for phosphorus improvement and plant nutrition. This study aimed to investigate the effects of AMF strains (Rhizophagus irregulare, Glomus aggregatum, G. mosseae) on growth of sorghum cultivated in greenhouse on Sangalkam soil (Senegal) sterilized with or without Tilemsi natural phosphate (PNT). The phosphorus can represent until 0.2% of the dry weight of the plant. Two fertilizers were used separately and together to doses of 20 g by strain, 100 mg and 200 mg of PNT. The experiment lasted for 120 d. Results showed that mycorrhizal colonization intensity varied between 40% and 80% for all treatments. AMF inoculation increased sorghum plant height and biomass, regardless of PNT amendment. The inoculation permits to bring strain of AMF that intervene efficiently in the transportation and the availability of phosphorus for the plant.

This research studies the capability of the Weather Research and Forecasting model coupled with the Chemistry/Aerosol module(WRF-Chem)with and without parametrization to reproduce a dust storm,which was held on 27th June 2018 over Sahara *** authors use satellite observations and ground-based measurements to evaluate the WRF-Chem *** sensitivities of WRF-Chem Model are tested on the replication of haboob features with a tuned GOCART aerosol *** of simulations with satellite and ground-based observations show that WRF-Chem is able to reproduce the Aerosol Optical Depth(AOD)distribution and associated changes of haboob in the meteorological fields with temperature drops of about 9℃and wind gust 20 m·s–*** WRF-Chem Convec-tion-permitting model(CPM)shows strong 10-meter winds induced a large dust emission along the leading edge of a convective cold pool(LECCP).The CPM indicates heavy dust transported over the West African coast(16°W-10°W;6°N-21°N)which has a potential for long-distance travel on 27th June between 1100 UTC and 1500 *** daily precipitation is improved in the CPM with a spatial distribution similar to the GPM-imERG precipitation and maxi-mum rainfall located at the right *** well as raising a large amount of dust,the haboob caused considerable dam-age along its route.

Introduction: There has been limited experience with Open Heart Surgeries (OHS) in Sub-Saharan Africa. In west Africa especially in Mali, most fledgling centers are unable to overcome the myriad of challenges encountered in establishing OHS though there is a high prevalence of surgically correctable heart diseases. The aim of this paper is to review our initial experience of our first cases in developing OHS program and discuss the challenges and prospects that need to be overcome to further develop it. Methods: A total of 6 patients who underwent OHS during the first “cardiac mission” in July 2016 were included in this retrospective study. The medical records of the patients were examined and data on age, sex, diagnosis, EuroSCORE, type of surgery, cardiopulmonary bypass details, complications and length of hospital stay were extracted. Results: Six patients with a male to female ratio of 1, ages ranging between 12 and 35 years (mean of 22.5 ± 12 years) were studied. The mean of EuroSCORE was 6 ± 41. Pericardial patch closure of isolated atrial septal defect was performed in one patient. One patient had mitral valve repair for rheumatic mitral regurgitation consisting of chordal shortening with a tricuspid valvuloplasty. Three patients had mitral valve replacement with tricuspid valvuloplasty. Four patients had mitral valve replacement. Sixty-day mortality?was 0%. Conclusion: Safe conduct of open heart surgery in Mali Hospital setting is feasible. Grant financial aid is required for rapid growth of Open-Heart Surgery in this part of Sub-Saharan Africa.

背景:贝宁、马拉维、尼加拉瓜和塞内加尔的实验室开展的结核菌痰涂片检查。方法:通过实验室登记进行计算机分析确定工作量、连续痰涂片的检出率及受检人群的人口分布特征结果:42个实验室的60000多个受检查者的资料显示,每个国家每天涂片检测的数量一般为4—19(平均6)。发现一例结核病人平均检查可疑者痰涂片21个(8—50)。所有至少一次阳性结果的病例中,87％的病例在第一次检测中就呈阳性结果。病例人群分布的不同在国家和性别之间变化很大。在42个实验室中的35个实验室里,男性病例的检出率要高于女性。随着年龄的增长,女性要比男性更多数量的可疑者检查方能确诊为一个病例。结论:该研究证实抗酸杆菌涂片结果标准化记录系统具有很好的用途,它能为规程实施和了解受检人群的分布特征方面提供必要的信息。

Despite the efforts made by the Malian government with technical and financial partners in the fight against HIV and AIDS, there are no data on the evolution of people with HIV in medical consultation in antiretroviral treatment sites. The objective of this work is to fill this gap by generating factual data describing the evolution of the number of people with HIV in medical consultation in Mali. To achieve this goal, we conducted a cross-sectional study using data from the Coordination Unit of the Sectoral Committee for AIDS Control of the Ministry of Health and Public Hygiene. Sampling was comprehensive including all people living with HIV. The data collected concerned those of the years 1999 to 2014 by the ESOPE software and analyzed with Stata version 12. A Total of 39,619 people were enrolled in the study. And 26,212 people living with HIV in the 17 ESOPE sites of Mali were female. Patients aged 25-34 and 35-44 years were strongly represented in consultation in the years 2007 and 2011, and those aged 15-24 were the least frequent. HIV1 was the most dominant with 97%. The majority of patients presented for initial consultation were clinical stages 2 and 3 in all years. Cases of lost sites seen in the more common in hospitals are 62.79% at the Hospital of Mopti and 57% at the University Hospital of Point G. The number of cases of people living with HIV in medical consultation has progressively augmented from 1999-2007. Then it dropped from 2008-2010. After this number has increased by 2011, it dropped again in 2012-2014. This study made it possible to note the evolution of the patients in medical consultation in ESOPE antiretroviral treatment sites according to the years and events (safe, subsidized by activities by the global fund). The activities of sensitization and communications, case research should be strengthened by involving the community.
