randomized controlled trial of minimally invasive surgery using acellular dermal matrix for complex anorectal fistula

AIM: To compare the efficacy and safety of acellular dermal matrix (ADM) bioprosthetic material and endorectal advancement flap (erAF) in treatment of complex anorectal fistula. METHODS: Ninety consecutive patients with complex anorectal fistulae admitted to Anorectal Surgical Department of First Affi liated Hospital, Xinjiang Medical University from march 2008 to July 2009, were enrolled in this study. Complex anorectal fistula was diagnosed following its clinical, radiographic, or endoscopic diagnostic criteria. Under spinal anesthesia, patients underwent identification and irrigation of the fistula tracts using hydrogen peroxide. ADM was securely sutured at the secondary opening to the primary opening using absorbable suture. Outcomes of ADM and erAF closure werecompared in terms of success rate, fecal incontinence rate, anorectal deformity rate, postoperative pain time, closure time and life quality score. Success was defined as closure of all external openings, absence of drainage without further intervention, and absence of abscess formation. Follow-up examination was performed 2 d, 2, 4, 6, 12 wk, and 5 mo after surgery, respectively. RESULTS: No patient was lost to follow-up. The overall success rate was 82.22% (37/45) 5.7 mo after surgery. ADM dislodgement occured in 5 patients (11.11%), abscess formation was found in 1 patient, and fistula recurred in 2 patients. Of the 13 patients with recurrent fistula using erAF, 5 (11.11%) received surgical drainage because of abscess formation. The success rate, postoperative pain time and closure time of ADM were significantly higher than those of erAF (P er, no difference was observed in fecal incontinence rate and anorectal deformity rate after treatment with ADM and erAF. CONCLUSION: Closure of fistula tract opening with ADM is an effective procedure for complex anorectal fistula. ADM should be considered a first line treatment for patients with complex anorectal fistula.

Photobiomodulation:a novel approach to promote trans-differentiation of adipose-derived stem cells into neuronal-like cells

An Efficient Plant Regeneration System of Hydrangea bretschneideri Dipp via Stem Segments as Explants

Development of a coupled supersonic inlet-fan Navier–Stokes simulation method

A coupled supersonic inlet-fan Navier–Stokes simulation method was developed by using COMSOL-CFD code. The flow turning, pressure rise and loss effects across blade rows of the fan and the inlet-fan interactions were taken into account as source terms of the governing equations without a blade geometry by a body force model. In this model, viscous effects in blade passages can also be calculated directly, which include the exchange of momentum between fluids and detailed viscous flow close to walls. NASA Rotor 37 compressor test rig was used to validate the ability of the body force model to estimate the real performance of blade rows. Calculated pressure ratio characteristics and the distribution of the total pressure, total temperature, and swirl angle in the span direction agreed well with experimental and numerical data. It is shown that the body force model is a promising approach for predicting the flow field of the turbomachinery. Then, coupled axisymmetric mixed compression supersonic inlet-fan simulations were conducted at mach number 2.8 operating conditions. The analysis includes coupled steady-state performance, and effects of the fan on the inlet. The results indicate that the coupled simulation method is capable of simulating behavior of the supersonic inlet-fan system.

Is nasopharyngeal cancer really a “Cantonese cancer”?

Nasopharyngeal cancer(NPC) is endemic in Southern China,with Guandong province and Hong Kong reporting some of the highest incidences in the *** journal Science has called it a "Cantonese cancer".We propose that in fact NPC is a cancer that originated in the Bai-Yue("proto-Tai-Kadai" or "proto-Austronesian" or "proto-Zhuang") peoples and was transmitted to the han Chinese in southern China through ***,the work by John Ho raised the profile of NPC,and because of the high incidence of NPC in Hong Kong and Guangzhou,NPC became known as a Cantonese *** searched historical articles,articles cited in PubMed,Google,monographs,books and Internet articles relating to genetics of the peoples with high populations of *** migration history of these various peoples was extensively researched,and where possible,their genetic fingerprint identified to corroborate with historical *** and anthropological evidence suggest there are a lot of similarities between the Bai-Yue and the aboriginal peoples of Borneo and Northeast India;between Inuit of Greenland,Austronesian mayalo-Polynesians of Southeast Asia and Polynesians of Oceania,suggesting some common *** studies also suggest the present Cantonese,Minnans and hakkas are probably an admixture of northern han and southern *** these populations have a high incidence of *** early contact between southern Chinese and peoples of East Africa and Arabia can also account for the intermediate incidence of NPC in these regions.
