Despite numerous advances and emerging data,liver transplantation in the setting of gastrointestinal malignancies remains controversial outside of certain accepted *** an era of persistent organ shortage and increasing organ demand,allocation of liver grafts must be considered *** hepatocellular carcinoma and hilar cholangiocarcinoma have become accepted indications for transplantation,tumor size and standardized multi-disciplinary treatment protocols are necessary to ensure optimal patient *** more studies seeking to expand the oncologic indications for liver transplantation are emerging,it is becoming increasingly clear that tumor biology and response to therapy are key factors for optimal oncologic *** addition,time from diagnosis to transplantation appears to correlate with survival,as stable disease over time portends better outcomes *** aggressive disease pre-transplant remains difficult with current imaging and tissue sampling *** tumor size and stage are important prognostic predictors for most malignancies,patient and tumor selection protocols are *** the fields of medical and surgical oncology continue to evolve,it is clear that a protocolized interdisciplinary treatment approach is necessary for combatting any cancer *** stability over time and response to neoadjuvant therapy may be the best predictors for successful patient outcomes and can be easily incorporated in our treatment *** data evaluating liver transplantation for expanded oncologic indications such as:expanded criteria hepatocellular carcinoma,intrahepatic cholangiocarcinoma,mixed tumors,and liver limited metastatic colorectal carcinomas,incorporate multi-modal therapies and evaluation of tumor treatment *** further investigation is necessary,initial results suggest there is an expanded role for transplant surgery in malignancy in a new era of liver transplant oncolog

In the last 30 years, operative, technical and medical advances have made liver transplantation(LT) a lifesaving therapy that is used worldwide today. Global industrialization has been a contributor to morbid obesity and this has brought about the metabolic syndrome along with many downstream complications of such. Non-alcoholic steatohepatitis(NASH) has become a recognized hepatic manifestation of the metabolic syndrome and NASH cirrhosis is predicted to be the primary indication for LT in the United States by 2025. Several case series and database reviews have begun analyzing the efficacy of weight reduction surgery in the LT recipient. These data have reasonably demonstrated that weight reduction surgery in the LT recipient is a feasible endeavor. However, several questions have been raised regarding the type of weight reduction surgery, timing of surgery in relation to LT, patient and allograft survival and post-LT maintenance of weight loss to name a few. We look forward to a time when weight reduction surgery will work to improve the technical conduct of LT, improve perioperative benchmarks such as blood transfusions, intensive care unit length of stay and help to prevent recurrence of NASH cirrhosis in the medically complicated obese patient. In the meantime, well-designed prospective clinical trials that focus on the issues highlighted will help guide us in the care of these complicated patients who will soon account for the majority of the patients in our clinics.

Quaternaryammonium compounds (QACs) are cationic surfactants with broad range applications. They enter into aquatic environment through the discharge of sewage effluents and adversely affect the ecosystem due to toxicity. Modern electrochemical methods have been used for characterization and detoxification of an extensively used QAC, didodecyldimethylammonium bromide (DDAB) in a wide pH range. The diffusion coefficient of DAAB was determined by Randles-Sevcik equation. In acidic conditions the electrochemical reduction of DAAB occurred by two electrons and two protons process. However, in neutral medium the overall reduction of DAAB followed electronation-protonation mechanism involving 3e– and 3H+. Although DDAB is toxic yet it is bestowed with cancer preventing ability. Hence, for getting insights into the under-standing of DAAB anticancer effect, its DNA binding parameters has been evaluated.
