Liver abnormalities following SARS-CoV 2 infection in children 1 to 10 years of age

Objective Beginning in October 2021 in the USA and elsewhere,cases of severe paediatric hepatitis of unknown aetiology were identified in young *** the adenovirus and adenovirus-associated virus have emerged as leading aetiological suspects,we attempted to investigate a potential role for SARS-CoV 2 in the development of subsequent liver *** We conducted a study using retrospective cohorts of deidentified,aggregated data from the electronic health records of over 100 million patients contributed by US healthcare *** Compared with propensity score matched children with other respiratory infections,children aged 1-10 years with COVID-19 had a higher risk of elevated transaminases(HR(95%CI)2.16(1.74 to 2.69))or total bilirubin(HR(95%CI)3.02(1.91 to 4.78)),or new diagnoses of liver diseases(HR(95%CI)1.67(1.21 to 2.30))from 1 to 6 months after *** with pre-existing liver abnormalities,liver abnormalities surrounding acute infection,younger age(1-4 years)or illness requiring hospitalisation all had similarly elevated *** who developed liver abnormalities following COVID-19 had more pre-existing conditions than those who developed abnormalities following other *** These results indicate that SARS-CoV 2 may prime the patient for subsequent development of liver infections or non-infectious liver *** rare(~1 in 1000),SARS-CoV 2 is a risk for subsequent abnormalities in liver function or the diagnosis of diseases of the liver.

BACKGROUND Metabolic dysfunction-associated steatotic liver disease(MASLD),characterised by hepatic lipid accumulation,causes inflammation and oxidative stress accompanied by cell damage and *** injury(LI)is also frequently reported in patients hospitalised with coronavirus disease 2019(COVID-19),while preexisting MASLD increases the risk of LI and the development of COVID-19-associated *** of injury at the cellular level remain unclear,but it may be significant that severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)which causes COVID-19,uses angiotensin-converting expression enzyme 2(ACE2),a key regulator of the‘anti-inflammatory’arm of the renin-angiotensin system,for viral attachment and host cell *** To determine if hepatic ACE2 levels are altered during progression of MASLD and in patients who died with severe *** ACE2 protein levels and localisation,and histological fibrosis and lipid droplet accumulation as markers of MASLD were determined in formalin-fixed liver tissue sections across the MASLD pathological spectrum(isolated hepatocellular steatosis,metabolic dysfunction-associated steatohepatitis(MASH)+/-fibrosis,end-stage cirrhosis)and in post-mortem tissues from patients who had died with severe COVID-19,using ACE2 immunohistochemistry and haematoxylin and eosin and picrosirius red staining of total collagen and lipid droplet areas,followed by quantification using machine learning-based image pixel *** ACE2 staining is primarily intracellular and concentrated in the cytoplasm of centrilobular hepatocytes and apical membranes of bile duct ***,ACE2 protein levels are elevated in non-fibrotic MASH compared to healthy controls but not in the progression to MASH with fibrosis and in ***2 protein levels and histological fibrosis are not associated,but ACE2 and liver lipid droplet content are significantly correlated across the MASLD *** A

背景中国全科医生越来越期望能够识别和预防抑郁障碍,但其接受的心理健康培训有限,社区卫生服务中心(CHCs)也缺乏针对抑郁障碍的诊断和服务规范。WHO《心理健康差距干预指南》第2版(mhGAP-IG.v2)是为非心理健康专业人员提供的决策支持工具,可用于评估、管理、随访精神障碍、神经疾病及物质使用障碍患者。鉴于mhGAP-IG.v2的通用性,其在中国的应用应根据抑郁障碍在不同文化下的差异性和中国心理健康系统特点进行调整。方法开展两轮基于网络的德尔菲(Delphi)调查。邀请来自深圳市的全科医生小组对mhGAP-IG.v2中涉及抑郁障碍的199条表述进行五分制的赞同程度打分,即“如要把该表述用在深圳市,是否赞同调整其内容和结构”。如果>80%的全科医生“有些/肯定赞同”调整某表述,则视为全科医生对调整该表述达成共识;如果>80%的全科医生“有些/肯定不赞同”调整某表述,则视为全科医生对不调整该表述达成共识。结果全科医生对79%的表述达成了需要调整的共识,平均得分为4.26分,即全科医生“有些赞同”对mh GAP-IG.v2中涉及抑郁障碍的相关表述进行调整。具体调整建议包括:抑郁评估方法应考虑更广泛的抑郁症状,并考虑病情的发展阶段;增加抑郁筛查工具的使用指南;明确全科医生的作用,以及家庭参与和跨部门服务的转诊途径;药物治疗建议应与中国用药规范保持一致;指南所有章节要强调预防自杀;情境化健康教育;强调“以人为本”的服务方法。全科医生认为,应维持mhGAP-IG.v2中对经历抑郁发作的双相障碍患者的诊断和治疗建议。结论对mhGAP-IG.v2抑郁部分的调整研究反映出对指南的评估应考虑文化和国情背景,包括中国特色的初级卫生保健系统、健康优先领域、治疗可得性,以及多样化的社会心理教育需要。调整后的mhGAP-IG.v2可以给中国全科医学教育和全科医疗服务提供参考,也可以为其他国家的非精神病学专业人员开展心理健康服务提供有益借鉴。

Peanuts pods grow underground and mature unevenly, resulting that choosing the correct time to harvest is more complicated than other crops. Pod maturity can be determined by blasting with a pressure washer to remove outer skin of the pod (exocarp) to expose the color of the middle layer (mesocarp). The mesocarp color changes with maturity from white to yellow, orange, brown and finally black. The sum of percentage from orange, brown, and black mesocarp (OBB) color and black color (BL) represents the kernels that are mature enough to harvest. The goal of this research is to identify methodologies to estimate OBB and BL of the pods using RGB images taken in the field and validate the proposed model using other pod images. The Mahalanobis distance classification method was used to process sets of images and calculate pod area (number of pixels) corresponding to two classes (mesocarp and background) with nine different color groups. The results showed a performance of 94% effectiveness for mesocarp using Mahalanobis distance classification. Statistical regression for OBB and BL was developed based on 315 images of peanut pods taken from the field. The R2 and root mean square error of predicted and actual OBB were 0.93 and 4.1%, respectively. The R2 and root mean square error of predicted and actual BL were 0.88 and 1.8%, respectively. The validation of OBB using other images provided reasonable estimation (R2 = 0.98 and RMSE = 2.73%). This study introduces a novel, cost-effective, and non-destructive method for estimating peanut maturity using RGB imagery and Mahalanobis distance classification in the field. This innovative approach addresses the limitations of traditional methods and offers a robust alternative for real-time maturity assessment.

It is intuitive that allowing a deeper search into a game tree will result in a superior player to one that is restricted in the depth of the search that it is allowed to make. Of course, searching deeper into the tree comes at increased computational cost and this is one of the trade-offs that has to be considered in developing a tree-based search algorithm. There has been some discussion as to whether the evaluation function, or the depth of the search, is the main contributory factor in the performance of an evolved checkers player. Some previous research has investigated this question （on Chess and Othello）, with differing conclusions. This suggests that different games have different emphases, with respect to these two factors. This paper provides the evidence for evolutionary checkers, and shows that the look-ahead depth （like Chess, perhaps unsurprisingly） is important. This is the first time that such an intensive study has been carried out for evolutionary checkers and given the evidence provided for Chess and Othello this is an important study that provides the evidence for another game. We arrived at our conclusion by evolving various checkers players at different ply depths and by playing them against one another, again at different ply depths. This was combined with the two-move ballot （enabling more games against the evolved players to take place） which provides strong evidence that depth of the look-ahead is important for evolved checkers players.

目前,中国正在经受环境问题的鞭打和各种社会压力,因为过去十年突飞猛进的经济增长已经严重影响了该国家作为一个整体的可持续性。除了有害的雾霾和劳资纠纷,中国在产品质量和生产线的声誉上也面临着来自外国人和国内业务的极大怀疑。认识到这些问题,同时面临着来自外部组织的压力,中国中央政府和企业已经增加了他们关于企业社会责任(CSR)的意识和法规。然而,即使有这些强大的激励因素,来自社会和消费者的作用是不容忽视的。事实上,组织合法性理论(OL)表明,承担并展示了众多与社会共同价值观的公司,将被其利益相关者接受并信任。通过将社会的价值观融入商业计划并不断转播的这种方式,一个公司将可以在其身份认同和获得利润两方面都获得可持续发展。使用OL的概念框架,并把它应用到研究中国的企业社会责任,本研究的目的旨在更好地了解如何使企业社会责任成为中国的企业与消费者关系间的重要因素。通过面谈的方法,并基于卡罗尔(1991)关于企业社会责任的著作,这个项目定性评估了中国年轻人如何识别一个“负责任的”公司,以及这种公司所必需承担的责任。第二个主要的研究问题在于对企业社会责任活动的报告,由此理解社交媒体是否为一个合法的沟通工具。本报告的研究结果表明,中国年轻的专业人士认为,一家负责任的企业,不仅在道德上,而且在社会中是积极的助推者。支持这种观点的核心价值是“和谐”,这是儒家哲学的关键内容之一,同时也描述了各方将牺牲和协调来实现更大的幸福的内在联系。针对报告企业社会责任在社会化媒体上的体现的第二个研究问题而言,本文并没有明确的结果,但需求进一步研究的问题。事实上,虽然通过微信的潜在力量来分享企业社会责任信息的情况目前还不清楚,但受访者对企业社会责任未满足的需求和他们对共享企业社会责任的企业的信任程度表明,企业社会责任信息对企业在中国的名誉和底线上寻求可持续发展具有重大影响。本研究在结论中恳求进一步的研究去分析有效地达到中国受众的方式,这些受众不仅希望获得更多的企业社会责任报告,也将积极回应此类消息和活动。
