AIm.o evaluate the effect of sitagliptin vs placebo on histologic and non-histologic param.ters of non-alcoholic steatohepatitis (NASH).m.THODSTwelve patients with biopsy-proven NASH were random.zed to sitagliptin (100 m. daily) (n = 6) or placebo (n = 6) for 24 wk. The prim.ry outcom. was im.rovem.nt in liver fibrosis after 24 wk. Secondary outcom.s included evaluation of changes in NAFLD activity score (NAS), individual com.onents of NAS (hepatocyte ballooning, lobular inflam.ation, and steatosis), glycem.c control and insulin resistance [including m.asurem.nts of glycated hem.globin (HbA1C) and adipocytokines], lipid profile including free fatty acids, adipose distribution m.asured using m.gnetic resonance im.ging (m.I), and throm.osis m.rkers (platelet aggregation and plasm.nogen activator inhibitor 1 levels). We also sought to determ.ne the correlation between changes in hepatic fat fraction (%) [as m.asured using the Iterative Decom.osition of water and fat with Echo Asym.etry and Least-squares estim.tion (IDEAL) m.I technique] and changes in hepatic steatosis on liver *** was not significantly better than placebo at reducing liver fibrosis score as m.asured on liver biopsy (m.an difference between sitagliptin and placebo arm., 0.40, P = 0.82). There were no significant im.rovem.nts evident with the use of sitagliptin vs placebo for the secondary histologic outcom.s of NAS total score as well as for the individual com.onents of NAS. Com.ared to baseline, those patients who received sitagliptin dem.nstrated im.roved HbA1C (6.7% ± 0.4% vs 7.9% ± 1.0%, P = 0.02), and trended towards im.roved adiponectin levels (4.7 ± 3.5 μg/m. vs 3.9 ± 2.7 μg/m., P = 0.06) and triglyceride levels (1.26 ± 0.43 m.ol/L vs 2.80 ± 1.64 m.ol/L, P = 0.08). However, when com.ared with placebo, sitagliptin did not cause a statistically significant im.rovem.nt in HbA1C (m.an difference, -0.7%, P = 0.19) n

In this review,advances in the understanding of the controlling factors and physical m.chanism. of tropical cyclogenesis(TCG)are sum.arized from.recent(2018–2022)research on TCG,as presented in the Tenth International Workshop on Tropical Cyclones(IWTC-10).Observational,theoretical,and num.rical m.deling studies published in recent years have advanced our knowledge on the influence of large-scale environm.ntal factors on ***,studies have shown clearly that appropriate convective coupling with tropical equatorial waves enhances the developm.nt chances of *** recently,illum.nating research has been carried out on analyzing the m.chanism. by which oscillations and teleconnections(El Ni˜no Southern Oscillation(ENSO)in particular)m.dulate TCG globally,in association with changes in the sea surface tem.erature(SST).In addition to this,recent research has diligently addressed different aspects of *** studies have reported the applicability of unified theories and physical m.chanism. of TCG in different ocean ***,research has been carried out on TCG under different flow pattern regim.s,dry air intrusion,im.ortance of m.rsupial pouch,genesis of m.dicanes,wind shear,convection and vertical ***,studies have discussed the possibility of near equatorial TCG provided that there is enough supply of background vertical vorticity and relatively low vertical wind *** has been m.de to understand the role of clim.te change on global and regional ***,there are still significant gaps which need to be addressed in order to better understand TCG prediction.

AIm.To investigate the im.act of m.dication beliefs, illness perceptions and quality of life on m.dication adherence in people with decom.ensated *** One hundred adults with decom.ensated cirrhosis com.leted a structured questionnaire when they attended for routine outpatient hepatology review. m.asures of self-reported m.dication adherence(m.risky m.dication Adherence Scale), beliefs surrounding m.dications(Beliefs about m.dicines Questionnaire), perceptions of illness and m.dicines(Brief Illness Perception Questionnaire), and quality of life(Chronic Liver Disease Questionnaire) were exam.ned. Clinical data were obtained via patient history and review of m.dical records. Least absolute shrinkage and selection operator and stepwise backwards regression techniques were used to construct the m.ltivariable logistic regression m.del. Statistical significance was set at alpha = *** m.dication adherence was " High " in 42 % o f participants, "m.dium. in 37%, and "Low" in 21%. Com.ared to patients with "High" adherence, those with "m.dium. or "Low" adherence were m.re likely to report difficulty affording their m.dications(P m.nt helpfulness(P = 0.003) and stronger m.dication concerns relative to m.dication necessity beliefs(P = 0.003). People with "Low" adherence also experienced greater sym.tom.burden and poorer quality of life, including m.re frequent abdom.nal pain(P = 0.023), shortness of breath(P = 0.030), and em.tional disturbances(P = 0.050). m.ltivariable analysis identified having stronger m.dication concerns relative to necessity beliefs(Necessity-Concerns Differential ≤ 5, OR = 3.66, 95%CI: 1.18-11.40) and m.re frequent shortness of breath(shortness of breath score ≤ 3, OR = 3.87,95%CI: 1.22-12.25) as independent predictors of "Low"*** The association between "Low" adherence and patients having strong concerns or doubting the necessity or helpfulness of their m.dications should be explor

Active surveillance is an acceptable treatm.nt option in m.n with a low-risk prostate cancer. In the present study, we have retrospectively reviewed the outcom.s of 509 m.n who fit the criteria for active surveillance but selected radical prostatectom.. Then, the im.act of varying prostate-specific antigen （PSA） levels on the risk of upstaging and upgrading in these patients was assessed. Pathological characteristics of patients who fulfilled the inclusion criteria under three active surveillance criteria--those of the University of California-San Francisco, the National Cancer Institute and the European Association of Urology--were exam.ned. The proportion of m.n who were deem.d candidates for active surveillance but were subsequently upstaged or upgraded was determ.ned. Of 509 patients, 186 （36.5%）, 132 （25.9%） and 88 （17.3%） m.n fulfilled the active surveillance criteria, respectively. Upgrading （Gleason scores 7-10） ranged from.32.8% to 38.6%, while upstaging （≥ pT3） ranged from.10.2% to 12.5%, depending on the three active surveillance criteria. After a m.dian follow-up of 24 m.nths, three patients developed a biochem.cal recurrence. When the im.act of varying PSA levels was exam.ned using a test for trend analysis in the context of PSA for each protocol, rates of upstaging were lower in m.n with PSA 〈4 ng m.-1. However, there was no im.act of varying PSA levels on upgrading. In conclusion, com.only used active surveillance protocols carry the risks of upgrading and upstaging. m.re reliable and accurate m.rkers are needed to better stratify the risks of m.n who are appropriate candidates for active surveillance.
