Atherosclerotic process has a predilection for .bifurcation due to the complex hemodynamics and resultant altered shear stress that contributes to the localization and progression of atheromatous plaques. Coronary intervention, especially the placement of stents further alters the hemodynamics and shear forces. These factors may account for the increased incidences of in-stent restenosis, stent thrombosis, side-branch （SB） restenosis, etc, that continue to plague bifurcation intervention.

Background: The novel Coronavirus disease 2019 (COVID-19) has rapidly evolved into a global emergency with far-reaching consequences. Multiple models predict mounting morbidity and mortality in the absence of appropriate interventions. Screening of sub-clinical cases through technological tools has the potential to eliminate virus containment in the population. Objective: Our aim was to develop an effective application that detected early COVID-19 infections and resulted in four-dimensional (4-D) data visualization systems to develop safe networks whilst respecting user privacy. Methods: We proposed an algorithm for a novel form of contact tracing and screening that can detect infection before manifestation of clinical symptoms and signs. We developed a simulation model that demonstrated impact on the pandemic through percent change in the “trigger point”. Results: A “trigger point” based on personal risk assessment generated a visual report to the community network. The following outcomes were rendered in the app: live surveillance of metadata, hotspot mapping, targeted live health messaging to a large population, an infection control passport technology, and personal hotspot avoidance warnings. Our model suggested that higher adoption of such strategies can potentially eliminate viral carriage in the population. Conclusions: Our proposed technology-enabled screening, detection, and elimination strategy presents a novel approach to eliminate the viral containment. This app could be applied to the COVID-19 pandemic as well as other outbreaks and epidemics in the future to control the unprecedented disease spread.

In recent years, the Internet has received increasing recognition as an effective means of facilitating public health interventions. In particular, delivering prevention for substance use to school students via the Internet appears to be an area of great potential. The Climate Schools: Ecstasy and Emerging Drugs Module, a school-based prevention program, facilitated by the Internet, was developed to address the use of ecstasy and new and emerging drugs (Emerging Psychoactive Substances or Novel Psychoactive Substances). This four-lesson course was designed to be delivered to Australian adolescents (aged 15 to 16 years) during their standard health education classes at school, and is based on a harm-minimisation and social influence approach. The program was developed in response to the important public health challenge of new and emerging drugs as well as to address the prevention of ecstasy use among young people. To our knowledge, this will be the first school- and Internet-based prevention program specifically targeting these substances. This paper describes the process involved in developing this new Internet-based substance use prevention program.

Alcohol abuse is commonly associated with the development of both acute and chronic pancreatitis. Despite this close association, the fact that only a small percentage of human beings who abuse alcohol develop pancreatitis indicates that alcohol abuse alone is not sufficient to initiate clinical pancreatitis. This contention is further supported by the fact that administration of ethanol to experimental animals does not cause pancreatitis. Because of these findings, it is widely believed that ethanol sensitizes the pancreas to injury and additional factors trigger the development of overt pancreatitis. How ethanol sensitizes the pancreas to pancreatitis is not entirely known. Numerous studies have demonstrated that ethanol and its metabolites have a number of deleterious effects on acinar cells. Important acinar cells properties that are affected by ethanol include: calcium signaling, secretion of zymogens, autophagy, cellular regeneration, the unfolded protein response, and mitochondrial membrane integrity. In addition to the actions of ethanol on acinar cells, it is apparent that ethanol also affects pancreatic stellatecells. Pancreatic stellate cells have a critical role in normal tissue repair and the pathologic fibrotic response. Given that ethanol and its metabolites affect so many pancreatic functions, and that all of these effects occur simultaneously, it is likely that none of these effects is "THE" effect. Instead, it is most likely that the cumulative effect of ethanol on the pancreas predisposes the organ to pancreatitis. The focus of this article is to highlight some of the important mechanisms by which ethanol alters pancreatic functions and may predispose the pancreas to disease.

Drone Bombings in the Federally Administered Tribal Areas: Public Remote Sensing Applications for Security Monitoring

Drone bombing, as a US defense strategy in Pakistan, began under the George W. Bush administration as part of the “US War on Terrorism” and has accelerated under the Obama administration. The United States government has not confirmed the use of drones in the Federally Administered Tribal Areas of Pakistan. We identify the region within the Federally Administered Tribal Areas with the highest incidence of drone bombing activity and use QuickBird imagery to identify evidence of drone bombing damage. The city of Miram Shah in North Waziristan had the highest incidence of drone bombings before January 1, 2010. A systematic research in 1 km2 grids over the city of Miram Shah revealed potential damage of a drone bombing at one site, recent damage at one site, and an image of a drone over the landscape. Results suggest that drone bombings are very accurate and drone missions are common in the region. It is possible for the public to monitor drone bombings and other quality of life indicators in the Federally Administered Tribal Areas. The use of drones to monitor and attack enemy locations will most likely expand in the future.

Prognostic implications of type 2 myocardial infarctions

Purpose: A type 2 myocardial infarction (MI) is due to ischemia from increased oxygen demand or decreased supply. It is not an infrequent complication following non-cardiac surgery or in the setting of acute medical illness. The purpose of this study was to determine whether type 2 MIs had a worse prognosis than a type 1 MI. Methods: The hospital database of a large terti-ary-level academic medical center was queried for all patients with a troponin I ≥ 1.6 ng/mL during a one-year period. The outcomes of patients with a type 2 MI were compared to those with a type 1 MI. Results: This retrospective study found that a type 2 MI is more lethal than the usual type 1 MI, particularly among women and the elderly. There was no statistical difference in outcomes between those that occurred in the post-operative setting compared to those that occurred in the setting of acute medical illness. Conclusions: Type 2 MIs are common and are associated with increased mortality, as compared with type 1 MIs. To date, no guidelines exist for the management of this type of MI. Further research into underlying mechanisms resulting in type 2 MI as well as potential treatment strategies is needed.
