High Frequency Transcricoid Jet Ventilation Prior to Neck Surgery—A 10-Year Retrospective Case Series Analysis

Difficult airway is still a challenging field in anesthesia, emergency departments and intensive care unit and is the cause of severe patient injury such as irreversible hypoxic brain damage or even death. Therefore, safer techniques are needed to maintain the airway in patients, who are difficult to intubate. The aim was to analyze the safety of the technique of transcricoid high frequency jetventilation (TCHFJV) by the investigation of complications. We performed a 10-year retrospective analysis (1/2009-1/2019) of patients subjected to TCHFJV at the hospital of the Ludwigs-Maximilians-University, Munich, Germany. TCHFJV was applied before anesthesia induction in awake, spontaneously breathing patients, with suspected or known difficult airway, scheduled for head and neck surgery. During the 10-year study period, we identified 39,477 patients, who underwent neck surgery. Of these, 1489 (3.8%) patients were managed with TCHFJV (1090 males, 399 females;62 ± 13 years, BMI 25 ± 0.2 kg?m-2, ASA class 3 ± 0.5). TCHFJV was successful in 1479 (99%) patients. Subsequent endotracheal intubation was performed in most cases (93%). Occasionally, emergency cricothyroidotomy (n = 2) or tracheotomy (n = 6) was necessary following TCHFJV. TCHFJV-related complications occurred in 1.5% of the cases. A majority (83%) of the complications were judged to be non-severe (grade 1, 2A) and 17% were severe (grade 3B), while there were none that were very severeC or lethalD. This retrospective study indicates that TCHFJV in experienced hands is a relatively safe alternative airway management strategy for patients undergoing neck surgery.

Background: Polyamines stimulate DNA transcription and m RNA translation for protein synthesis in trophectoderm cells, as well as proliferation and migration of cells; therefore, they are essential for development and survival of conceptuses(embryo/fetus and placenta). The ovine conceptus produces polyamines via classical and non-classical pathways. In the classical pathway, arginine(Arg) is transformed into ornithine, which is then decarboxylated by ornithine decarboxylase(ODC1) to produce putrescine which is the substrate for the production of spermidine and spermine. In the non-classical pathway, Arg is converted to agmatine(Agm) by arginine decarboxylase(ADC), and Agm is converted to putrescine by agmatinase(AGMAT).Methods: Morpholino antisense oligonucleotides(MAOs) were designed and synthesized to inhibit translational initiation of the m RNAs for ODC1 and ADC, in ovine ***: The morphologies of MAO control, MAO-ODC1, and MAO-ADC conceptuses were normal. Double knockdown of ODC1 and ADC(MAO-ODC1:ADC) resulted in two phenotypes of conceptuses; 33% of conceptuses appeared to be morphological y and functional y normal(phenotype a) and 67% of the conceptuses presented an abnormal morphology and functionality(phenotype b). Furthermore, MAO-ODC1:ADC(a) conceptuses had greater tissue concentrations of Agm,putrescine, and spermidine than MAO control conceptuses, while MAO-ODC1:ADC(b) conceptuses only had greater tissue concentrations of Agm. The synthetic predator odor 2,5-dihydro-2,4,5-trimethylthiazoline (TMT) was used to induce innate fear in male GAD67 mice and behavioral changes were correlated with c-fos mRNA levels as marker for neuronal activation to reveal underlying activated fear circuits. Results show the same amount of increased freezing and decreased rearing and grooming behavior of TMT-exposed GAD67 mice and wild type littermates, and therefore suggest that heterozygous knock-in of GFP in the GAD67 gene that is associated with a fifty percent decreased GAD67 protein level in the brain, has no impact on TMT-induced behavioral effects. Exposure to TMT significantly increased the number of c-fos mRNA positive cells in the main olfactory bulb (MOB), the lateral septum (LS), the bed nucleus of the stria terminalis (BNST), the central amygdala (CeA), the anterior-ventral (MeAav), the anterior-dorsal (MeAad) and the posterior-ventral (MeApv) part of the medial amygdala in GAD67 mice. Thus, to further investigate the role of GABAergic neurons in TMT-induced uncontrollable stress responses GAD67 mice that provide the advantage of prelabeled GABAergic neurons through the GABA neuron specific expression of GFP could be a suitable model organism.

A key question in the coronavirus disease 2019(COVID-19)pandemic is the duration of specific T cell responses against the severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)post primary infection,which is difficult to address due to the large-scale COVID-19 vaccination and re-exposure to the ***,we conducted an analysis of the long-term SARS-CoV-2-specific T cell responses in a unique cohort of convalescent individuals(CIs)that were among the first to be infected worldwide and without any possible antigen re-exposure since *** magnitude and breadth of SARS-CoV-2-specific T cell responses correlated inversely with the time that had elapsed from disease onset and the age of those *** mean magnitude of SARS-CoV-2-specific CD4 and CD8 T cell responses decreased about 82%and 76%,respectively,over the time period of ten months after ***,the longitudinal analysis also demonstrated that SARS-CoV-2-specific T cell responses waned significantly in 75%of CIs during the ***,we provide a comprehensive characterization of the long-term memory T cell response in CIs,suggesting that robust SARS-CoV-2-specific T cell immunity post primary infection may be less durable than previously expected.
