Medication beliefs predict medication adherence in ambulatory patients with decompensated cirrhosis

AIM To investigate the impact of medication beliefs, illness perceptions and quality of life on medication adherence in people with decompensated *** One hundred adults with decompensated cirrhosis completed a structured questionnaire when they attended for routine outpatient hepatology review. Measures of self-reported medication adherence(Morisky Medication Adherence Scale), beliefs surrounding medications(Beliefs about Medicines Questionnaire), perceptions of illness and medicines(Brief Illness Perception Questionnaire), and quality of life(Chronic Liver Disease Questionnaire) were examined. Clinical data were obtained via patient history and review of medical records. Least absolute shrinkage and selection operator and stepwise backwards regression techniques were used to construct the multivariable logistic regression model. Statistical significance was set at alpha = *** Medication adherence was " High " in 42 % o f participants, "Medium" in 37%, and "Low" in 21%. Compared to patients with "High" adherence, those with "Medium" or "Low" adherence were more likely to report difficulty affording their medications(P n of treatment helpfulness(P = 0.003) and stronger medication concerns relative to medication necessity beliefs(P = 0.003). People with "Low" adherence also experienced greater symptom burden and poorer quality of life, including more frequent abdominal pain(P = 0.023), shortness of breath(P = 0.030), and emotional disturbances(P = 0.050). Multivariable analysis identified having stronger medication concerns relative to necessity beliefs(necessity-Concerns Differential ≤ 5, OR = 3.66, 95%CI: 1.18-11.40) and more frequent shortness of breath(shortness of breath score ≤ 3, OR = 3.87,95%CI: 1.22-12.25) as independent predictors of "Low"*** The association between "Low" adherence and patients having strong concerns or doubting the necessity or helpfulness of their medications should be explor

The Role of High-sensitivity C-reactive Protein, Interleukin-6 and Cystatin C in Ischemic Stroke Complicating Atrial Fibrillation

This study examined the role of high-sensitivity C-reactive protein (hsCRP), interleukin-6 (IL-6) and cystatin C in ischemic stroke complicating atrial fibrillation (AF) and the relationship of systemic inflammation with this disease in order to identify AF patients who are at high risk of stroke and need optimal anticoagulant therapy.A total of 103 AF patients, simple (n=75) or complicated by ischemic stroke (n=28), and 112 control subjects were ***-6 level was detected by using enzyme linked immunosorbent *** C and hsCRP levels were measured by means of a particle-enhanced immunonephelometric *** results showed that the AF patients had higher levels of hsCRP (P=0.004), IL-6 (P=0.000), and cystatin C (P=0.000) than control *** hsCRP level was increased in the AF patients with ischemic stroke as compared to the patients with simple AF (P=0.036).The AF patients who had the level of hsCRP exceeding 3.83 mg/L were at a higher risk than those with hsCRP level lower than 3.83 mg/L (P=0.030).After adjusting for other factors, cystatin C remained positively associated with IL-6 (r=0.613) and hsCRP (r=0.488).It was concluded that hsCRP is positively correlated with ischemic stroke complicating AF and may be a risk factor independent of other risk factors for *** cystatin C level is also indicative of the increased risk of AF.

The effect of the CHAMP intervention on fundamental motor skills and outdoor physical activity in preschoolers

Purpose: Physical activity(PA) and fundamental motor skills are important components of current and future trajectories of health in young children. This study examined the effects of a 5-week motor skill intervention on preschoolers' motor skill competence and their PA behaviors while participating in the motor skill intervention or outdoor free play(recess).Methods: A total of 102 preschoolers served as participants and were part of a motor skill intervention group(n = 64) or a control/outdoor free play group(n = 38). Children's motor skills were assessed before and after the intervention using the Test of Gross Motor Development-3 rd edition. PA during the motor skill intervention and outdoor free play was assessed using accelerometers both immediately before the start(baseline, Week 0 or Week 1) and end(late, Week 5 or Week 6) of the ***: All children significantly improved their motor skills from baseline to late assessment(p n in the motor skill intervention demonstrated greater rates of change(p nd scored higher on all motor skills at the late assessment compared with the control group(p no effect of group(control vs. intervention), but there was a significant effect of sex on children's PA during outdoor free play at baseline(p no effect of group on PA during days with the movement program(intervention vs. outdoor free play) at either time point, but boys were more active than girls at the late assessment(p n in the intervention engaged in more PA while participating in the intervention toward the end of the intervention than at the ***: The 5-week motor skill intervention was effective at improving preschoolers' motor skills and rates of change in motor skills were higher for children who completed the intervention compared with children in the control group. Phosphorus utilization response of pigs and broiler chickens to diets supplemented with antimicrobials and phytase

Three experiments were conducted to evaluate the phosphorus(P) utilization responses of pigs and broiler chickens to dietary supplementation with antimicrobials and phytase and to determine if P digestibility response to phytase is affected by supplementation with antimicrobials. Experiment 1 used 4 diets(a basal negative control formulated to contain 0.41% total P and 0.71% calcium [Ca] without added antimicrobials, basal negative control with added carbadox, basal negative control with added tylosin, or basal negative control with added virginiamycin) and six 18-kg barrows in individual metabolism crates per diet. There was no effect of antimicrobials on P and Ca digestibility or retention. Carbadox supplementation increased(P nd retention of gross energy(GE) and supplementation with tylosin increased(P n retention relative to the basal negative control diet. Experiment 2 used eight 19-kg barrows in individual metabolism crates per treatment and 9 dietary treatments arranged in a 3×3 factorial of antimicrobials(none, tylosin, or virginiamycin) and phytase(0, 500, or 1,500 FTU/kg).Phytase addition to the diets linearly increased(Pnt total tract digestibility or retention of P, Ca, nitrogen(n) and GE. Supplementation with antimicrobials did not affect apparent total tract digestibility or retention of P, Ca, n or GE. There were linear effects(P n Ca utilization in diets that were not supplemented with antimicrobials but only tendencies(P n diets supplemented with tylosin or virginiamycin. Phytase linearly improved(P n utilization in diets supplemented with tylosin or virginiamycin but not in diets without added antimicrobials. Abnormal Adipokines Associated with Various Types of Obesity in Chinese Children and Adolescents

Objective To explore the role of adipokines including insulin, resistin, leptin, adiponectin, acylation stimulating protein （ASP） and complement C3 （C3） in various types of obesity （peripheral obesity, abdominal obesity and mixed obesity） in Chinese children and adolescents, and their relationships with body size and pubertal development. Methods Children and adolescents （n=3 508） aged 6 to 18 years, with 1 788 boys and 1 720 girls were assessed for body mass index, waist circumference, pubertal development, blood insulin, resistin, leptin, adiponectin, ASP and C3 levels. Three types of obesity [peripheral obesity （n=43）, abdominal obesity （n=473）, mixed obesity （n=1 187）] and non‐obese control （n=1 805） were defined with combined use of Chinese body mass index and waist circumference criteria. Results Serum resistin, leptin and adiponectin levels were higher in girls than those in boys （all P0.01）. Insulin and leptin increased and adiponectin decreased across five Tanner stages in both girls and boys （all P0.001）, while ASP changed only in girls （P0.001） and C3 only in boys （P0.001）. Insulin, leptin and ASP were higher, but adiponectin was lower in all three types of obesity vs. the non‐obese control （all P0.05）. The greatest abnormalities of all six adipokines were found in the mixed obesity group. With inclusion of body mass index and waist circumference in simultaneous regression analyses, both body size indices were independently and significantly correlated with insulin, leptin and adiponectin after age and gender adjustment. Compared with waist circumference, the body mass index was stronger in interpreting insulin, leptin, adiponectin and ASP levels, whereas it was weaker in explaining variance of plasma C3. Conclusions Obese children have a worse metabolic profile with high insulin, resistin, leptin, ASP and C3, and low adiponectin levels. Lifestyle modifications and erectile dysfunction： nhat can be expected？

Erectile dysfunction （ED） is a common medical disorder whose prevalence is increasing worldwide. Modifiable risk factors for ED include smoking, lack of physical activity, wrong diets, overweight or obesity, metabolic syndrome, and excessive alcohol consumption. Quite interestingly, all these metabolic conditions are strongly associated with a pro-inflammatory state that results in endothelial dysfunction by decreasing the availability of nitric oxide （nO）, which is the driving force of the blood genital flow. Lifestyle and nutrition have been recognized as central factors influencing both vascular nO production, testosterone levels, and erectile function. Moreover, it has also been suggested that lifestyle habits that decrease low-grade clinical inflammation may have a role in the improvement of erectile function. In clinical trials, lifestyle modifications were effective in ameliorating ED or restoring absent ED in people with obesity or metabolic syndrome. Therefore, promotion of healthful lifestyles would yield great benefits in reducing the burden of sexual dysfunction. Efforts, in order to implement educative strategies for healthy lifestyle, should be addressed.
