AIM:To investigate whether celiac disease(cD)patients with tissue-transglutaminase antibody(tTGA)≥100 U/mL are different from patients with lower tTGA ***:Biopsy-proven(MarshⅢ)pediatric cD patients(n=116)were prospectively included between March 2009 and October *** biopsies were evaluated by a single pathologist who was blinded to all of the patients’clinical *** patients were distributed into 2 groups according to their tTGA level,which was measured using enzyme-linked immunoassay:tTGA≥100 U/mL and Ttgacharacteristics,symptoms,human leukocyte antigen(HLA)genotype and degree of histological involvement were compared between the 2 ***:A total of 34(29.3%)children had tTGA valuescores(SDS)than did patients with tTGAcommon(97.1%vs 75.6%,P=0.006).More specifically,abdominal pain(76.5%vs 51.2%;P=0.012)and nausea(17.6%vs 3.7%,P=0.018)were more frequent among patients with low *** contrast,patients with solely extraintestinal manifestations were only present in the high tTGA group(18.3%,P=0.005).These patients more commonly presented with aphthous stomatitis(15.9%vs 0.0%,P=0.010)and anemia(32.9%vs 11.8%,P=0.019).In addition,when evaluating the number of cD-associated HLA-DQ heterodimers(HLA-DQ2.5,HLA-DQ2.2 and HLA-DQ8),patients with low tTGA levels more commonly had only1 disease-associated heterodimer(61.8%vs 31.7%,P=0.005),while patients with high tTGA more commonly had multiple ***,patients with tTGA≥100 U/mL more often had a MarshⅢc lesion(73.2%vs 20.6%,P≤0.001)while in patients with low tTGA patchy lesions were more common(42.4%vs6.8%,P≤0.001).cONcLUSION:Patients with tTGA≥100 U/mL show several signs of more advanced *** also carry a larger

Background:High epilepsy prevalence and incidence have been reported in areas with high onchocerciasis *** findings suggest that proper community-directed treatment with ivermectin(cDTI)is potentially able to prevent onchocerciasis-associated epilepsy(OAE).We assessed the epilepsy prevalence and onchocerciasis transmission in two Nigerian villages following more than 20 years of ***:A cross-sectional door-to-door survey was performed in two villages in the Imo River Basin reported to be mesoendomic for onchocerciasis(Umuoparaodu and Umuezeala).Individuals were screened for epilepsy using a validated 5-item *** suspected to have epilepsy were examined by a neurologist or a physician with training in epilepsy for *** was investigated via skin snip microscopy and rapid diagnostic tests for Ov16 *** were compared with previous findings from the Imo river ***:A total of 843 individuals from 257 households in the two villages were *** detected four persons with epilepsy(PWE)giving a crude epilepsy prevalence of 0.5%.This finding differs from observations reported 14 years ago which showed an epilepsy prevalence of 2.8%in the neighbouring village of Umulolo(P=0.0001),and 1.2%from 13 villages in the Imo river basin(P=0.07).The seroprevalence of Ov16 antibodies was found to be 0%.Only 4.6%of skin snips were positive compared to 26.8%in previous surveys(Pctin mass distribution coverage in the study sites in 2017 was 79.7%.conclusions:A low epilepsy and onchocerciasis prevalence was observed following more than 20 years of cDTI in the Imo River *** of Ov16 antibodies indicates minimal transmission of *** results contrast with observations from areas of high onchocerciasis transmission,where epilepsy prevalence and incidence remain *** from this study suggest that sustained efforts could eventually achieve elimination of onch

A feasibility study to determine if minimally trained medical students can identify markers of chronic parasitic infection using bedside ultrasound in rural Tanzania

BAcKGROUND: Parasitic infections pose a significant health risk in developing nations and are a major cause of morbidity and mortality worldwide. In the Republic of Tanzania, the cDc estimates that 51.5% of the population is infected with one or more intestinal parasites. If diagnosed early, the consequences of chronic parasitic infection can potentially be ***: Six first-year medical students were recruited to enroll patients in the study. They underwent ten hours of formal, hands-on, ultrasound which included basic cardiac, hepatobiliary, renal, pulmonary and FAST scan ultrasound. A World Health Organization protocol with published grading scales was adapted and used to assess for pathology in each patient's liver, bladder, kidneys, and ***: A total of 59 patients were enrolled in the study. Students reported a sensitivity of 96% and specificity of 100% for the presence of a dome shaped bladder, a sensitivity and specificity of 100% for bladder thickening, a sensitivity and specificity of 100% for portal hypertension and ascites. The sensitivity was 81% with a specificity of 100% for presence of portal vein distention. The sensitivity was 100% with a specificity of 90% for dilated ***: Ultrasound has shown a promise at helping to identify pathology in rural communities with limited resources such as Tanzania. Our data suggest that minimally trained first year medical students are able to perform basic ultrasound scans that can identify ultrasonographic markers of parasitic infections.

cell signalling pathways underlying induced pluripotent stem cell reprogramming

Induced pluripotent stem(i PS) cells, somatic cells reprogrammed to the pluripotent state by forced expression of defined factors, represent a uniquely valuable resource for research and regenerative medicine. However, this methodology remains inefficient due to incomplete mechanistic understanding of the reprogramming process. In recent years, various groups have endeavoured to interrogate the cell signalling that governs the reprogramming process, including LIF/STAT3, BMP, PI3 K, FGF2, Wnt, TGFβ and MAPK pathways, with the aim of increasing our understanding and identifying new mechanisms of improving safety, reproducibility and efficiency. This has led to a unified model of reprogramming that consists of 3 stages: initiation, maturation and stabilisation. Initiation of reprogramming occurs in almost all cells that receive the reprogramming transgenes; most commonly Oct4, Sox2, Klf4 and c Myc, and involves a phenotypic mesenchymal-to-epithelial transition. The initiation stage is also characterised by increased proliferation and a metabolic switch from oxidative phosphorylation to glycolysis. The maturation stage is considered the major bottleneck within the process, resulting in very few "stabilisation competent" cells progressing to the final stabilisation phase. To reach this stage in both mouse and human cells, pre-i PS cells must activate endogenous expression of the core circuitry of pluripotency, comprising Oct4, Sox2, and Nanog, and thus reach a state of transgene independence. By the stabilisation stage, i PS cells generally use the same signalling networks that govern pluripotency in embryonic stem cells. These pathways differ between mouse and human cells although recent work has demonstrated that this is context dependent. As i PS cell generation technologies move forward, tools are being developed to interrogate the process in more detail, thus allowing a greater understanding of this intriguing biological phenomenon.

Background:Microorganisms transferred by water,air,and human such as bacteria and viruses can reach inanimate surfaces with high rates of *** general,the objectives of the research were to evaluate the decontamination of large areas such as floors,using a mobile device with ultraviolet c light at different delivery times and also to evaluate these microbial loads in stationary or latent *** and methods:The microbial inactivation effects of this device were measured through the recovery of viable bacteria in different points of the applied ***:A significant microbial reduction(p≤0.05)of 60-87%was obtained in general for all groups and for groups with stationary phase reductions of 100%.conclusions:microbial inactivation with UV-c ultraviolet light administration rates presents the possibility of potential use on surfaces of large areas for the decontamination of microorganisms in latent and stationary phases.
