AIM:To assess the efficacy and safety of non-penetrating deep sclerectomy(NPDS)with uveoscleral implant plus subconjunctival and intrascleral collagen matrix overcoming the superficial scleral flap lips(modified deep sclerectomy technique,DS)and minimal use of mitomycin c in glaucoma ***:A retrospective review of 47 consecutive glaucoma patients who underwent NPDS with DS between January 2017 and May ***-corrected visual acuity,intraocular pressure(IOP),post-operative need for glaucoma medications,visual field mean deviation(MD),re-interventions,needling revisions and laser goniopuncture were *** success was defined as IOP≤18 mm Hg without topical *** success was defined as the same criteria but with the addition of any antihypertensive *** over 18 mm Hg on two consecutive followup visits was considered as a ***:Fifty-two eyes of 47 patients were *** preoperative IOP was 25.37±6.47 mm Hg,and decreased to 15.04±4.73 at 12mo and 12.21±4.1 at 24mo(all Pcal medications dropped from a mean of 3.06±0.25 per patient to 0.51±0.99 and 1.11±1.23 respectively after 12 and 24mo(all Pcations were required in 45.5%of patients after *** and absolute success rate at 24mo were 85.5%±5%and 48.5%±7.4%,***:DS is a safe and effective nonpenetrating glaucoma surgery *** aims to retain the patency of all pathways created for aqueous humor drainage:the intrascleral bleb,the supraciliary space and the open communication between intrascleral and subconjunctival compartments.

Factors Which Influence Intraluminal Temperature during Ho:YAG-Laser Exposure at an In-Vitro URS

Introduction: The Ho:YAG-Laser is categorized as a potentially dangerous lithotripsy device (DIN: class 4) for perforation which is mainly caused by the photonic energy the laser emits. Long time complications like ureteral strictures seem to be directed by thermal and mechanical injury. In this study different energy settings a) are being investigated, a DJ (double J stent) is placed beside the laser to simulate a therapy of a forgotten stent with reduction of the lumen b) due to the volume exploitation of the DJ, and direct contact between the laser fiber and the DJ in the ureter c) is simulated during laser exposure. Materials and Methods: We used the Ho:YAG-laser (Vera PulseTM, coherent, Santa clara USA) with a 365 μm diameter laser fiber. The settings of the laser were 0.6 J and 1 J pulse energy with a frequency of 5 Hz. The experimental setup was closely aligned with the clinical situation. The tip of the thermometer was attached inside the catheter through a puncture. The laser fiber was guided by means of a rigid URS video device (11.5 ch). We had four different settings for a), b) and c) during the measurement: 1) Distance of 0.5 cm between the laser and the thermometer;without irrigation, 2) Distance of 0.5 cm between the laser and the thermometer;with irrigation, 3) Distance of 1 cm between the laser and the thermometer;without irrigation, 4) Distance of 1 cm between the laser and the thermometer;with irrigation. Results: The temperature in an empty ureter rises approximately by 5°c, when the laser energy is increased from 0.6 J to 1 J. When a DJ is inserted in the artificial ureter there is surprisingly almost no difference in the maximum temperature between the lower energy level (0.6 J) and the high energy level (1 J). However the time needed to reach the maximum temperature is noticibly less when using high energy levels. PI control based on fuzzy set-point weighting tracking for hydraulic crane boom system

A PI control strategy based on fuzzy set-point weighting following was proposed for the active damping control of a hydraulic crane boom system （HcBS）. Two valve-controlled PI controllers, which include a proportional feedforward controller based on fuzzy set-point weighting following and a limited semi-integrator（LSI）, are designed respectively. LSI is used to limit output signal and to prevent wind up at the low frequency of the spectrum. By using a range camera and an electronic feedback control, the tip damping on the HcBS can be adjusted artificially. A collaborative control simulation technique of HOPSAN and MATLAB/SIMULINk is applied to the controller design. Simulation results show that the proposed PI control system has less overshoot as well as faster response. The tip damping on the HcBS during operation is improved.

常绿和落叶树种共存于中国东南部的亚热带森林中。为探究常绿和落叶树种应对亚热带磷限制所采用的碳获取策略,我们选取阳际峰常绿阔叶林中75种共存的木本植物,包括44种常绿树种和31种落叶树种,测定了其叶片的光合和呼吸速率,以及与碳(c)、氮(N)和磷(P)利用效率和再吸收效率的相关性状。落叶树种的光合氮利用效率(PNUE)、单位氮和磷的呼吸速率(R_(d,N)和R_(d,P))均显著高于常绿树种,但光合磷利用效率(PPUE)在二者之间无显著差异。对于任意给定的叶片P,常绿树种的碳利用效率(cUE)高于落叶树种。此外,落叶和常绿植物的氮再吸收效率(NRE)、磷再吸收效率(PRE)和N:P均无显著差异。这些结果表明,常绿树种维持高cUE使其在P限制的亚热带森林中成为优势树种。这些结果对其他生物群落中落叶和常绿物种的养分策略比较具有重要意义,也为未来的气候变化下的亚热带森林的群落动态建模提供了新见解。

Recurrent thrombotic occlusion of a transjugular intrahepatic portosystemic stent-shunt due to activated protein c resistance

The transjugular intrahepatic portosystemic stent-shunt (TIPS) has successfully been used in the management of refractory variceal bleeding and ascites in patients with portal hypertension. Major drawbacks are the induction of hepatic encephalopathy and shunt dysfunction. We present a 59-year-old woman with alcoholic liver cirrhosis who received a TIPS because of recurrent bleeding from esophageal varices. Stent occlusion occurred 4 mo after placement of the TIPS. Laboratory testing revealed resistance to activated protein c (APc). combination therapy with low-dose enoxaparin and clopidogrel could not prevent her recurrent stent occlusion. Finally, therapy with high-dose enoxaparin was sufficient to prevent further shunt complications up to now (follow-up period of 1 year). In conclusion, early occlusion of a TIPS warrants testing for thrombophilia. If risk factors are confirmed,anticoagulation should be intensified. There are currently no evidence-based recommendations regarding the best available anticoagulant therapy and surveillance protocol for patients with TIPS.

Over 11% of all pregnancies in the US result in preterm birth, greatly contributing to perinatal morbidity and mortality （Goldenberg and Rouse, 1998）. Preterm birth etiologies remain largely unknown, and effective prevention methods have yet to be developed. The use of biofluid （e.g., serum or urine） for the analysis of the naturally occurring peptidome （MW 〈 4000） as a source of biomarkers has been reported for different diseases （Villanueva et al., 2006; Ling et al., 2010a, 2010b, 2010c, 2011）. Mass spectrometry-based profiling of naturally occurring peptides can provide an extensive in- ventory of serum peptides derived from either high-abundant endogenous circulating proteins or cell and tissue proteins （Liotta and Petricoin, 2006）.

First Intraluminal Temperature Measurement during Ho:YAG-Laser Exposure at an <i>In-Vitro</i>URS

Introduction: The laser is a high energy instrument which can melt metals like nitinol. So it is very important to know under which conditions it is dangerous to perform an endourologic lithotripsy. We measure the temperature increase during laser exposure in an underwater in-vitro ureter model. For comparison, temperatures with and without irrigation and with different distances from the laser fiber to the thermometer are measured. Materials and Methods: We used the Ho:YAG-laser (Vera PulseTM, coherent) with a 365 μm laser fiber. The settings of the laser were 0.6 J with a frequency of 5 Hz which is the minimum setting for that type of laser. The experimental setup was closely aligned with the clinical situation. A metal container was filled with 0.9% sodium chloride (Nacl) solution (Temp. 36.8°) and a catheter with an inner diameter of 4 mm was attached to the rim of the container. The tip of the thermometer was attached inside the catheter through a waterproof hole. The laser fiber was guided by means of a rigid URS video device (11.5 F). We had four different settings during the measurement: 1) Distance of 0.5 cm between the laser and the thermometer;without irrigation, 2) Distance of 0.5 cm between the laser and the thermometer;with irrigation, 3) Distance of 1 cm between the laser and the thermometer;without irrigation, 4) Distance of 1 cm between the laser and the thermometer;with irrigation. Results: The maximum overall temperature was recorded in the 1) and 3) setting, both featuring no irrigation. The maximum temperature was ~50°c in both settings, with the 1) setting reaching the maximum temperature after 50 seconds and hence approximately twice as fast as the 3) setting. During measurements with a Nacl solution flow we couldn’t detect any noticeable increase in temperature, neither at short nor at long distance between the laser fiber and the thermometer. conclusion: There is a relevant heating in the ureter beside an endourologic lithotripsy. Rock Magnetic characterization of Fine Particles from car Engines, Brake Pads and Tobacco: An Environmental Pilot Study on Oahu, Hawaii, USA

Today, it is well known that small airborne particles are very harmful to human health. For the first time in Hawaii we have conducted an environmental pilot study of fine magnetic particles on the island of Oahu, Hawaii, of particulate matter (PM) PM = 60, PM = 10, and PM = 2.5. In order to do a rock magnetic characterization we have performed low field susceptibility vs. temperature [k-T] experiments to determine the curie points of small particles collected from exhaust pipes, as well as from brake pads of four different types of car engines using gasoline octane ratings of 87, 89, and 92. The curie point determinations are very well defined and range from 292°c through 393ce:normal;">°c and up to 660°c. In addition, we have conducted magnetic granulometry experiments on raw tobacco, burnt tobacco ashes, as well as on automotive engine exhaust, and brake pads in question. The results of the experiments show ferro and ferrimagnetic hysteresis loops with magnetic grain sizes ranging from superparamagnetic-multidomain [SP-MD], multidomain [MD] and pseudo-single domain [PSD] shown on the modified Day et al., diagram of Dunlop (2002). Thus far, the results we have obtained from this pilot study are in agreement with other studies conducted from cigarette ashes from Bulgaria. Our results could be correlated to the traffic-related PM in Rome, Italy where the SP fraction mainly occurs as coating of MD particles originated by localized stress in the oxidized outer shell surrounding the un-oxidized core of magnetite-like grains.
