Pemphigoid Gestational (PG), an uncommon autoimmune bullous dermatosis of pregnancy, stands out between the specific dermatological diseases of the gestational period. Despite being self-limited and benign, it has a significant impact on women’s health, both physiologically and psychologically. In addition, it promotes pathological changes that can be complicated during pregnancy. This paper reports the case of a pregnant patient with blistering lesions throughout the whole body. The diagnostic hypothesis initially involved PG and the specific treatment with corticosteroids was initiated, but after the anatomopathological result of the biopsy there has been diagnostic confusion with drug hypersensitivity, since clinical and histological manifestations were exuberant for both pathologies. As the lesions recurred in the puerperium, due to the corticoid abstention that was administered for the treatment of the differential diagnosis, a diagnostic review with immunofluorescence was made and confirmed PG. It is important to establish a correct diagnosis therefore, in order to improve the quality of care offered to the mother-baby binomial.

The study seeks to report the clinical evolution of a patient who suffered an inadvertent subarachnoid puncture and evolved with a herpetic viral meningitis, confirmed by cerebrospinal fluid (CSF) analysis. Female, 48 years old, submitted to epidural anesthesia, for surgery, with median puncture, between L2-L3 and inadvertently accessed the subarachnoid space, with clear cerebrospinal fluid. The same puncture was used and the solution with 15 mg hyperbaric bupivacaine and 80 mcg morphine was administered. There was no complication during the entire surgical procedure. On the first postoperative day, she started complaining of headache and nausea, being treated with dipyrone, non-steroidal anti-inflammatory drugs (NSAIDs) and ondansetron. On the third day, she presented two episodes of seizure that ceased with the use of diazepam 10 mg. On the fifth day, the patient presented fever and seizures, which did not stop with medication, requiring sedation, orotracheal intubation and transfer to the intensive care unit. Lumbar puncture was performed for CSF analysis, which was positive for Herpes simplex type I. The patient was extubated, on the second day of hospitalization, maintained with a maintenance dose of phenytoin, sumatriptan, dipyrone, metoclopramide and acyclovir 500 mg, remaining intravenous, without presenting new seizures for 45 days, when she was discharged. After 30 days, he returned for a review consultation without making a complaint.

AIMTo describe factors associated with treatment failure and frequency of resistance-associated substitutions （RAS）.METHODSHuman immunodefciency virus （HIV）/hepatitis C virus （HCV） coinfected patients starting a first direct-acting antiviral （DAA） regimen before February 2016 and included in the French ANRS CO13 HEPAVIH cohort were eligible. Failure was defned as：（1） non-response [HCV-RNA remained detectable during treatment, at end of treatment （EOT）]; and （2） relapse （HCV-RNA suppressed at EOT but detectable thereafter）. Sequencing analysis was performed to describe prevalence of drug class-specifc RAS. Factors associated with failure were determined using logistic regression *** 559 patients, 77% had suppressed plasmaHIV-RNA 〈 50 copies/mL at DAA treatment initiation41% were cirrhotic, and 68% were HCV treatmentexperienced. Virological treatment failures occurred in22 patients and were mainly relapses （17, 77%） thenundefined failures （3, 14%） and non-responses （29%）. Mean treatment duration was 16 wk overall. Posttreatment NS3, NS5A or NS5B RAS were detected in10/14 patients with samples available for sequencinganalysis. After adjustment for age, sex, ribavirin useHCV genotype and treatment duration, low platelecount was the only factor signifcantly associated with ahigher risk of failure （OR： 6.5; 95%CI： 1.8-22.6）. CONCLUSIONOnly 3.9% HIV-HCV coinfected patients failed DAAregimens and RAS were found in 70% of those failingLow platelet count was independently associated withvirological failure.

In this article, we report the results of the work carried out on the Verde River, a predominantly urban river located in the city of Ponta Grossa, Paraná, Brazil. Developed in three stages, it aimed to evaluate the water quality through physical-chemical parameters, the presence of caffeine as a chemical tracer, and the compound Triclosan (TCS), considered a pollutant of emerging concern. An attempt was made to identify possible polluting sources along the river. Samples were collected at 4 points from the spring region, with sampling upstream and downstream of a Sewage Treatment Plant (STP). The physical-chemical parameters were determined in situ, through portable analysers, as well as in the laboratory, in this case, through analyses using molecular absorption spectroscopy. For the determination of TCS and Caffeine, high-performance liquid chromatography (HPLC) was used, preceded by a pre-concentration step in the solid phase (SPE), and the techniques were implemented after validation. The physicochemical evaluations of the surface samples showed a decline in water quality after the STP. Mean total phosphorus concentrations of 1.0 mg·L-1 (±0.4), as well as dissolved oxygen (DO) levels below 5.0 mg·L-1 were verified downstream of the STP. Caffeine concentrations for Verde River were between 49.0 to 299.0 μg·L-1, verified upstream and downstream of the STP. The results demonstrated the relationship between the presence of caffeine and impaction, either by the input of raw sewage or by treated effluent. TCS was detected by chromatographic analysis, downstream of the STP, with an average concentration of 27.6 (±1.7) μg·L-1, noting that its presence was associated with the reactor maintenance procedure, with the STP in the process called bypass. Obtaining quality data from Verde River is important for the protection of the environment and affects the health of the population.

Introduction and Hypothesis: The Pelvic Floor Dysfunction (PFD) is a common gynecological condition that causes significant morbidity in the affected population, increasing clinical and financial challenge for the Public Health Services. Methods: This study aimed to review the available evidence and interventions to promote the health of women with PFD. We used the following databases to select papers for this review: Scopus, PubMed, CINAHL, LILACS and Cochrane. The sample consists of seven articles, with evidence levels 1 and 2. Results: The results of the articles point out to a greater number of activities related to the management of Urinary Incontinence (UI) behavioral support to the patient and physical therapy of the pelvic floor muscles. They were both beneficial to UI prevention and treatment. Conclusions: The physical therapy approach, when associated with cognitive and behavioral strategies, leverages UI outcomes in patients.

Shrinking lung refers to a rare complication of systemic lupus erythematosus and is characterized by unexplained dyspnea, a restrictive pattern in lung function tests, and elevation of the diaphragmatic hemicuples. It is postulated to have a predilection for female involvement and occurs mainly during late stages of the disease. Chest X-rays usually show small, diaphragmatic lungs. Occasional basal atelectasis may be present. Chest tomography usually shows reduced lung volumes with diaphragmatic elevation, occasional basal atelectasis, without severe pulmonary or pleuropulmonary disease. Shrinking lung can cause significant morbidity and occasional mortality. There is no definitive therapy, while corticosteroids may decrease symptoms and improve lung function in some patients. The objective of this study was to describe the main imaging findings in Shrinking Lung, an important pulmonary alteration in lupus patients. We highlight the characteristics observed on radiography and computed tomography, with an emphasis on computed tomography. It is important that every radiologist is prepared to recognize these findings and understand the possible clinical repercussions.

Energy use characteristics and benchmarking for higher education buildings

Higher education buildings serve complex functions by providing spaces for various activities and disciplines. This study aims to understand energy use characteristics of different types of buildings in higher education campuses and to establish an energy benchmark system. The data was collected form 80 university campus buildings in Australia. Energy consumption（ EC） and energy use intensity（ EUI） as well as related space types and occupancy conditions were analysed. Based on a comparative study of several statistical methods,the stochastic frontier analysis（ SFA） method was selected as the most appropriate benchmarking technique for this research. The benchmark values for various activities and disciplines were determined using the SFA statistical technique. Regarding activities,buildings w hich were used mostly for research had the highest benchmark EUI value at 216 kW ·h/m2/year and buildings for academic offices had the lowest benchmark value at 137 kW ·h/m2/year. When considering disciplines,buildings for Science had the highest benchmark EUI value at 164 kW ·h/m2/year and buildings for Health had the lowest benchmark value at 136 kW ·h/m2/year. The energy benchmarks developed for each building type can guide university authorities to promote energy efficiency by evaluating energy use,determining feasible energy saving techniques,and forecasting future planning development.

It is generally expected that the Rio de Janeiro 2016 Olympic and Paralympic Games will bring health and social benefits to their host city and to Brazil. This assumption comes from “common sense”, as a logical conclusion arising from the fact that host cities “inspire” and stimulate lifestyle changes. Benefits are also expected on tourism, self-image, architecture and the economy of the country as a whole. But are these expectations real and evidence-based? What parts of these “facts” are concrete and which ones are not? This paper suggests available ways of quantifying positive effects of hosting an Olympic Game, and puts the focus of this approach on the Rio de Janeiro 2016 Olympic Games and their true legacy, seeking scientific certainties.
