Clinical utility of glycated albumin and 1,5-anhydroglucitol in the screening and prediction of diabetes:A multi-center study

Non-conventional modeling of extreme significant wave height through random sets

The analysis and design of offshore structures necessitates the consideration of wave loads. Realistic modeling of wave loads is particularly important to ensure reliable performance of these structures. Among the available methods for the modeling of the extreme significant wave height on a statistical basis, the peak over threshold method has attracted most attention. This method employs Poisson process to character- ize time-varying properties in the parameters of an extreme value distribution. In this paper, the peak over threshold method is reviewed and extended to account for subjectivity in the modeling. The freedom in selecting the threshold and the time span to separate extremes from the original time series data is incorpo- rated as imprecision in the model. This leads to an extension from random variables to random sets in the probabilistic model for the extreme significant wave height. The extended model is also applied to different periods of the sampled data to evaluate the significance of the climatic conditions on the uncertainties of the parameters.

Schistosomal appendicitis:A serious and often misdiagnosed condition

Schistosomal appendicitis(SA)is a rare but serious complication of schistosomiasis,a parasitic disease affecting over 250 million people worldwide.A recent retrospective study by Wang et al provides important insights into the clinicopathological characteristics of *** study compared 136 cases of SA to 5418 cases of non-SA over a ten-year *** findings include a higher average age of SA patients(61.73 years vs 35.8 years for non-SA),a higher proportion of acute on chronic appendicitis(33.1%vs 16%),and a significantly higher incidence of colorectal cancer(11.7%vs 2.2%).Despite these differences,SA remains a diagnostic challenge due to its nonspecific clinical presentation and lack of specific laboratory *** study also highlights the persistent prevalence of SA,accounting for 1.6%-3.4%of all appendicitis cases each year from 2013 to *** findings underscore the need for enhanced awareness,early detection,and prompt treatment of SA in endemic *** the association with colorectal cancer,patients with SA require thorough screening and *** research into the pathogenesis and diagnostic markers of SA is *** the global battle against schistosomiasis continues,targeted efforts to diagnose and manage SA can significantly improve patient outcomes.

目的探索气态污染物(SO_(2)、NO_(2)、CO、O_(3))对广东省7个城市心血管疾病入院人数、住院费用和住院时间的影响。方法纳入2017年1月至2019年12月广东省7个城市的2 010 905例心血管疾病患者为研究对象。从广东省电子医疗信息系统中获取心血管疾病住院患者人口学特征和住院情况,并在中国高分辨率高质量近地表空气污染物数据集提取SO_(2)、NO_(2)、CO和O_(3)每日暴露浓度。采用多步骤时间序列分析方法估计了气态污染物对广东省7个城市心血管系统疾病的影响。结果 lag0时(入院当天)SO_(2)、NO_(2)、CO浓度与心血管疾病入院人数、住院费用、住院时长均呈正相关,且NO_(2)影响最大,另外NO_(2)、CO的负面作用存在滞后效应,而O_(3)浓度在lag0-7的时间窗内均与心血管疾病入院人数、住院费用、住院时长呈负相关。结论 SO_(2)、NO_(2)、CO短期暴露可能与增加心血管疾病患者的疾病负担有一定相关性,且由于NO_(2)较大的负面作用,仍然建议加强NO_(2)的监控。但考虑本研究为回顾性研究,需要进一步开展深入研究。

Clinical factors affecting rejection rates in liver transplantation

With improvements in survival, liver trans- plant recipients now suffer more morbidity from long-term immunosuppression. Considerations were given to develop individualized immunosuppression based on their risk of re- jection. METHOD： We retrospectively analyzed the data of 788 liver transplants performed during the period from October 1991 to December 2011 to study the relationship between acute cel- lular rejection （ACR） and various clinical factors. RESULTS： Multivariate analysis showed that older age （P=0.04, OR=0.982）, chronic hepatitis B virus infection （P=0.005, OR= 0.574）, living donor liver transplantation （P=0.02, OR=0.648） and use of interleukin-2 receptor antagonist on induction （P〈0.001, OR=0.401） were associated with fewer ACRs. Patients with fulminant liver failure （P=.004, OR=4.05） were more likely to develop moderate to severe grade ACR. CONCLUSIONS： Liver transplant recipients with older age, chronic hepatitis B virus infection, living donor liver trans- plantation and use of interleukin-2 receptor antagonist on in- duction have fewer ACR. Patients transplanted for fulminant liver failure are at higher risk of moderate to severe grade ACR. These results provide theoretical framework for developing individualized immunosuppression.
