Osteomyel.tis is a d.vastating d.sease caused.by microbial.infection of bone. Whil. the frequency of infection fol.owing el.ctive orthoped.c surgery is l.w, rates of reinfection are d.sturbingl. high. Staphyl.coccus aureus is responsibl. for the majority of chronic osteomyel.tis cases and.is often consid.red.to be incurabl. d.e to bacterial.persistence d.ep within bone. Unfortunatel., there is no consensus on cl.nical.cl.ssifications of osteomyel.tis and.the ensuing treatment al.orithm. Given the high patient morbid.ty,mortal.ty, and.economic burd.n caused.by osteomyel.tis, it is important to el.cid.te mechanisms of bone infection to inform novel.strategies for prevention and.curative treatment. Recent d.scoveries in this fiel. have id.ntified.three d.stinct reservoirs of bacterial.biofil. incl.d.ng: Staphyl.coccal.abscess communities in the l.cal.soft tissue and.bone marrow, gl.cocal.x formation on impl.nt hard.are and.necrotic tissue, and.col.nization of the osteocyte-l.cuno canal.cul.r network(Ol.N) of cortical.bone. In contrast, *** intracel.ul.r persistence in bone cel.s has not been substantiated.in vivo, which chal.enges this mod. of chronic osteomyel.tis. There have al.o been major ad.ances in our und.rstand.ng of the immune proteome against S. aureus, from cl.nical.stud.es of serum antibod.es and.med.a enriched.for newl. synthesized.antibod.es(MENSA), which may provid. new opportunities for osteomyel.tis d.agnosis, prognosis, and.vaccine d.vel.pment. Final.y, novel.therapies such as antimicrobial.impl.nt coatings and.antibiotic impregnated.3d.printed.scaffol.s represent promising strategies for preventing and.managing this d.vastating d.sease. Here, we review these recent ad.ances and.highl.ght transl.tional.opportunities toward. a cure.

To sol.e a probl.m, three things are necessary： awareness, means, and.wil.. The 2015 COP21 Paris accord.was a masterful. perhaps even worl.-saving, d.pl.matic ad.ance toward.making the worl. aware of cl.mate change. Some of that success may have been because publ.cations from the IPCC and.the National.Acad.my of Science were mad. avail.bl., on l.ne, as prepubl.cation offerings, in ord.r to be wid.l. viewed.before the Paris Cl.mate Conference. This provid.d.d.pl.mats and.nego- tiators with the l.test information about cl.mate change, its nearness in time, its consequences, and.how wel. current mitigation technol.gies can succeed. Whatever the rea- sons, the ParAs Cl.mate Conference, was a success. l.ad.rs of 195 nations agreed.that cl.mate change is a real.and.present d.nger to l.fe as is known to al.. This important und.rstand.ng was accompl.shed.d.spite the presentation of wel. establ.shed.scientific facts which, without very d.pl.matic hand.0}ng, coul. easil. have evoked.over- whel.ing pol.tical.opposition to an agreement and.thus another COP fail.re. In this paper, the fact that how some scientific truths, written specifical.y to be overl.oked. were presented.in ord.r to prepare COP21 participants for the conference is expl.ined. Und.rstand.ng antibiotic biod.grad.tion is important to the appreciation of their fate and.removal.from the environment. In this research an Isotope Ratio Mass Spectrometry（IRMS）method.was d.vel.ped.to eval.ate the extent of biod.grad.tion of the antibiotic,sul.hanil.mid., in contaminated.ground.ater. Resul.s ind.cted.an enrichment in δ^（13）C of8.44‰ from-26.56（at the contaminant source） to-18.12‰（300 m d.wnfiel. of the source）.These resul.s confirm red.ctions in sul.hanil.mid. concentrations（from 650 to 10 mg/l.across the contaminant pl.me to be attributabl. to biod.grad.tion（56%） vs. other natural.attenuation processes, such as d.l.tion or d.spersion（42%）. To und.rstand.the control. on sul.hanil.mid. d.grad.tion ex-situ microcosms assessed.the infl.ence of sul.hanil.mid. concentration, red.x cond.tions and.an al.ernative carbon source. Resul.s ind.cated. high l.vel. of anaerobic capacity（~50% mineral.sation） to d.grad. sul.hanil.mid. und.r high（263 mg/l., mod.rate（10 mg/l. and.l.w（0.02 mg/l. substrate concentrations. The ad.ition of el.ctron acceptors; nitrate and.sul.hate, d.d.not significantl. enhance the capacity of the ground.ater to anaerobical.y biod.grad. sul.hanil.mid.. Interestingl., where al.ernative carbon sources were present, the ad.ition of nitrate and.sul.hate inhibited.sul.hanil.mid. biod.grad.tion. These resul.s suggest, und.r in-situ cond.tions, when a preferential.carbon source was avail.bl. for biod.grad.tion, sul.hanil.mid. coul. be acting as a nitrogen and.or sul.hur source. 全膝置换术中髌骨是否置换存在争议。本研究比较了行髌骨置换和未行髌骨置换的全膝置换术的长期临床结果，是以前报告的更新。86例（118膝）患者接受初次全膝置换术，并被随机分成两组：一组行髌骨置换，而另一组未行髌骨置换。结果评价标准包括：根据KneeSociety临床等级系统所得评分、髌股特定41题患者问卷调查所得评分、患者满意度、全膝（gl.bal.痛和膝前痛评分、影像学发现、并发症和翻修率等。57例（78膝）患者最少随访10年。两组患者在关节活动度、KneeSociety评分、满意度、全膝痛或膝前痛方面无显著差异。髌骨未置换组患者膝关节翻修率在118膝中占12％，而髌骨置换组占9％。髌骨未置换组中7例患者（占12％）和髌骨置换组2例患者（占3％）由于髌股方面的问题接受了翻修手术。基于这些发现，我们得出结论：对患者所接受的全膝置换术类型，不论是否行髌骨置换，其结果相似。
