Impact of surgical volume and resident involvement on patency rates after vasectomy reversaldA 14-year experience in an open access system

Objective:Evaluate the influence of fellowship training,resident participation,reconstruction type,and patient factors on outcomes after vasectomy reversals in a high volume,open access ***:Retrospective review of all vasectomy reversals performed at a single institution from January 1,2002 to December 31,2016 was *** and spouse demographics,patient tobacco use and comorbidities,surgeon training and case volume,resident participation,reconstruction type,and postoperative patency were collected and ***:Five hundred and twenty-six vasectomy reversals were performed during the study ***-up was available in 80.6%of the cohort and overall patency,regardless of reconstruction type was 88.7%.The mean time to reversal was 7.87 years(range of 0-34 years).The majority of cases included resident *** volume was high with faculty and residents logging a mean of 37.0 and 38.7(median 18 and 37)cases ***eral vasovasostomy was the most common reconstruction type(83%)and demonstrated a significantly better patency rate(89%)than all other reconstructions(p=0.0008).Overall patency and patency by reconstruction type were not statistically different among faculty surgeons and were not impacted by fertility fellowship training,resident participation or postgraduate *** analysis demonstrated that increased time to reversal and repeat reconstructions had a negative impact on patency(p=0.0023 and p=0.043,respectively).conclusions:Surgeons with a high volume of vasectomy reversals have outcomes consistent with contemporary series regardless of fellowship training in *** was better for bilateral *** was not negatively impacted by tobacco use,comorbidities,resident participation,or post-graduate year.

Are there sex-based disparities in cataract surgery?

AIM:To investigate sex-based differences in the occurrence of intra-operative and post-operative complications and associated visual outcomes following cataract ***:This was a retrospective study of patients who had phacoemulsification cataract surgery at the University of colorado School of *** collected included the patient’s health history,ocular comorbidities,operative and post-operative complications,and the post-operative best corrected visual acuity(BcVA).The data were analyzed using univariate and multivariable logistic regression with generalized estimating equations to account for the correlation of some patients having two eyes included in the ***:A total of 11977 eyes from 7253 patients were included in the *** comorbidities differed by sex,with males having significantly higher percentages of traumatic cataracts(males 0.7%vs females 0.1%),prior ocular surgery(6.7%vs 5.5%),and mature cataracts(2.8%vs 1.9%).conversely,females had significantly higher rates of pseudoexfoliation(2.0%vs 3.2%).In unadjusted analysis,males had higher rates of posterior capsular rupture(0.8%vs 0.4%)and vitreous loss(1.0%vs 0.6%),but this difference was not significant after adjustment for *** had a significantly increased risk of post-operative retinal detachment,but in multivariable analysis this was no longer *** were significantly less likely to undergo post-operative neodymium-doped yttrium aluminum garnet(Nd:YAG)laser capsulotomy for posterior capsule opacification(OR=0.8,95%cI=0.7-0.9,P=0.0005).The BcVA was slightly worse for males pre-operatively;but post-operatively,both sexes exhibited similar visual acuity of Snellen equivalent 20/***:The study finds that in a cohort of patients presenting for cataract surgery,sex differences exist in pre-operative comorbidities and surgical characteristics that contribute to higher rates of some complications for ***,observed surgical complicatio

BAcKGROUND Video-capsule endoscopy(VcE)is an efficient tool that has proven to be highly useful in approaching several gastrointestinal *** was implemented in colombia in 2003,however current characterization of patients undergoing VcE in colombia is limited,and mainly comes from two investigations conducted before the SARS-coV-2 pandemic *** To describe the characteristics of patients undergoing VcEs and establish the main indications,findings,technical limitations,and other outstanding *** A descriptive study was carried out using data from reports of VcE(Pillcam SB3 system)use in a Gastroenterology Unit in Bogotá,colombia between September 2019 and January *** and clinical variables such as indication for the VcE,gastric and small bowel transit times(GTT,SBTT),endoscopic preparation quality,and limitations were described[n(%),median(IQR)].RESULTS A total of 133 VcE reports were *** were in men with a median age of 70 *** majority had good preparation(96.2%),and there were technical limitations in 15.8%of *** main indications were unexplained anemia(91%)or occult bleeding(23.3%).The median GTT and SBTT were 14 and 30 minutes,*** frequencies of bleeding stigma(3.79%)and active bleeding(9.09%)were low,and the most frequent abnormal findings were red spots(28.3%),erosions(17.6%),and vascular ectasias(12.5%).cONcLUSION VcE showed high-level *** main indication was unexplained *** bleeding was the most frequent *** with artificial intelligence,VcE can improve diagnostic precision and targeted therapeutic interventions.

Dear editor,The incidence of emergency department(ED)visits from gastroparesis and cannabinoid hyperemesis syndrome has increased by two folds over the past decade.[1-3]Roldan et al[4]reported that patients with these conditions often have prolonged ED stays and are likely to be hospitalized because of the danger of dehydration and challenges inherent in relieving *** analgesia is a particularly troublesome treatment modality;these conditions are often chronic with frequent recurrences,and repeated opiate therapy can cause opioid dependency and narcotic bowel syndrome.[5]Haloperidol(HP)and droperidol,members of the butyrophenone class,have potent antidopaminergic activity in the chemoreceptor trigger zone.[6,7]Intramuscular administration of HP is being used increasingly in the ED to treat nausea,vomiting secondary to gastroparesis,[6]intractable vomiting,[8]and cyclical vomiting.

Application of Image Analysis Based on SEM and chemical Mapping on Pc Mortars under Sulfate Attack

The degradation mechanisms of cementitious materials exposed to sulfate solutions have been controversial, despite considerable research. In this paper, two methodologies of image analysis based on scanning electron microscope and chemical mapping are used to analyse Portland cement mortars exposed to sodium sulfate solution. The effects of sulfate concentration in solution and water to cement ratio of mortar, which are considered as the most sensitive factors to sulfate attack, are investigated respectively by comparing the macro expansion with microstructure analysis. It is found that the sulfate concentration in pore solution, expressed as sulfate content in c-S-H, plays a critical role on the supersaturation with respect to ettringite and so on the expansion force generated.

Square wave manoeuvre for apogeotropic variant of horizontal canal benign paroxysmal positional vertigo in neck restricted patients

Objective:We aimed to describe the clinical features of the apogeotropic variant of horizontal canal benign paroxysmal positional vertigo(Hc BPPV-AG)in a cluster of patients with restrictive neck movement disorders and a new therapeutic manoeuvre for its ***:In a retrospective review of cases from an ambulatory tertiary referral center,patients with Hc BPPV-AG in combination with neck movement restriction that prevented any classical manual repositioning procedure or who were refractory to canalith repositioning manoeuvres,were treated with a new manoeuvre comprised of sequential square-wave pattern of head and body supine rotations while nystagmus was being monitored,until either an apogeotropic to geotropic conversion or resolution of the nystagmus was ***:Fifteen patients were *** but one[14/15 cases]showed a positive therapeutic response to the repositioning procedure in a single *** two cases,a direct relief of vertigo and elimination of nystagmus was observed without an intermediate geotropic *** in three patients the affected ear was not initially identified,it was ultimately identified and successfully treated by the square wave manoeuvre in all of ***:The square-wave manoeuvre is an alternative for Hc BPPV-AG treatment in either cases with neck restriction,where the affected side is not well identified at the bedside or when other manoeuvres fail to resolve the Hc BPPV-AG.
