Refractory ulcerative colitis accompanied with cytomegalovirus colitis and multiple liver abscesses: A case report

Various hepato-biliary complications are an increased incidence in patients with inflammatory bowel disease,and portal bacteremia is well documented in patients with ulcerative colitis (UC). However, few reports mention UC in association with liver abscesses. Recently, there are several reports describing cytomegalovirus (CMV) infection in association with disease exacerbation and steroid refractoriness in patients with UC. Here we present a case of refractory UC accompanied with multiple liver abscesses and CMV colitis. The patient, a 72-year-old male, with a five-year history of repeated admissions to our hospital for UC, presented with an exacerbation of his *** performed on admission suggested that his UC was exacerbated, then he was given prednisolone and mesalazine orally, and betamethasone ***, he had exacerbated symptoms. Repeat sigmoidoscopy revealed multiple longitudinal ulcers and pseudopolyps in the rectosigmoid colon. Although immunohistochemical staining of biopsy specimens and the serum testing for antigenemia were negative on admission and after the repeat sigmoidoscopy, they became histologically positive for CMV. Nonetheless, the patient developed spiking fevers, soon after ganciclovir was administered. Laboratory studies revealed an increased white cell count with left shift, and Enterococcus fecalis grew in blood cultures. An abdominal computed tomography (CT) scan was obtained and the diagnosis of liver abscesses associated with UC was made, based on CT results. The hepatic abscesses were successfully treated with intravenous meropenem for 6 wk, without further percutaneous drainage. To our knowledge, this is the first reported case of multiple liver abscesses that develop during UC exacerbation complicated by CMV colitis.

Life cycle assessments(LCAs)of early-stage technologies can provide valuable insights about key drivers of emissions and aid in prioritizing research into further emissions-reduction *** this potential value,further development of LCA methods is required to handle the increased uncertainty,data gaps,and confidentially of early-stage *** study presents a discussion of the life cycle carbon footprinting of technologies competing in the final round of the NRG COSIA Carbon XPRIZE competition-a US$20 million competition for teams to demonstrate the conversion of CO_(2) into valuable products at the scale of a small industrial pilot using consistent deployment conditions,boundaries,and methodological *** competition allowed the exploration of how LCA can be used and further improved when assessing disparate and early-stage *** intensity estimates are presented for two conversion pathways:(i)CO_(2) mineralization and(ii)catalytic conversion(including thermochemical,electrochemical,photocatalytic and hybrid process)of CO_(2),aggregated across teams to highlight the range of emissions intensities demonstrated at the pilot for individual life cycle stages.A future scenario is also presented,demonstrating the incremental technology and deployment conditions that would enable a team to become carbon-avoiding relative to an incumbent process(*** emissions relative to a reference pathway producing a comparable product).By considering the assessment process across a diverse set of teams,conversion pathways and products,the study presents generalized insights about opportunities and challenges facing carbon capture and-utilization technologies in their next phases of deployment from a life cycle perspective.

Dear Editor,Nivolumab,an inhibitor of programmed cell death 1(PD-1),is an immune checkpoint inhibitor(ICI)that enhances T cell functions by preventing negative regulation of cancer immunity,and it has shown clinically significant efficacy and tolerability in various types of *** on the results of randomized phase III trials comparing nivolumab with docetaxel[1,2],nivolumab is now used in clinical practice for patients with previously treated advanced non-small cell lung cancer(NSCLC).Fur-thermore,the most recent Japan Lung Cancer Society guidelines include nivolumab monotherapy in the sys-temic treatment strategy for previously treated,locally advanced or metastatic NSCLC[3].However,ICIs induce characteristic immune-related adverse events(irAEs),which are not seen with conventional cytotoxic agents or molecular targeted *** can occur in any organ system,most typically the skin,lung,and gastrointesti-nal,hepatic,and endocrine systems.

Influence of Wheelchair Cushions Fabricated by Therapists on Stability, Comfort, and Functionality

“Ease of comfort” and “functionality” are important factors that have been cited to improve wheelchair seating. The ability to maintain balance and posture while seated in a wheelchair improves stability, comfort, and functionality and is a prerequisite for daily activities. In this study, the authors used a sitting position measuring device to scientifically verify differences in sitting posture, comfort, and functionality depending on the presence or absence of cushions and the type of cushion in elderly individuals who used wheelchairs. Results suggested that a cushion fabricated by a therapist was more useful than a commercially available air cushion. While a commercially available air cushion was the most suitable, the therapist’s self-made cushion could be expected to demonstrate the same or better applicability than an air cushion.

Systematic Relationship between Sr Content and the Lattice Constants in Sr Substituted Hydroxyapatite Thin Films

To increase the biocompatibility of hydroxyapatite (HA), Ca10(PO4)6(OH)2, the Sr substitution of Ca into the HA structure was effected to yield Ca10-xSrx(PO4)6(OH)2(Sr-HA). For medical and dental applications, it is important that Sr-HA is prepared as a thin film so that the Sr fully substitutes the Ca sites in the HA structure and does not form segregated impurities consisting of Sr compounds. If the segregated Sr forms different amounts of different impurities, the dissolution of the Sr into the living body will not be reproducible across different samples. To confirm the Sr substitution into the Ca site in the HA structure, the systematic variation in the lattice constants of the Sr-HA with Sr content was evaluated as the first step. The a- and c-axis lengths were found to exhibit a linear relationship with the Sr content for six samples with different Sr contents, indicating that the prepared Sr-HA thin films likely possessed partial Sr substitution into the Ca sites of the HA structure. This result is an important first step in the accurate evaluation of the biological effects of Sr-HA thin films.

A new index for non-invasive assessment of liver fibrosis

AIM:To construct and evaluate a new non-invasive fibrosis index for assessment of the stage of liver f ibrosis. METHODS:A new f ibrosis index (Fibro-Stiffness index) was developed in 165 of 285 patients with chronic hepatitis C, and was validated in the other 120 patients where liver biopsy was performed. Its usefulness was compared with liver stiffness (LS) measured by FibroScan, the aminotransferase-to-platelet ratio index, the Forns index and the FibroIndex. RESULTS: The Fibro-Stiffness index consists of LS,platelet count and prothrombin time. The values of the Fibro-Stiffness index differed signif icantly between neighboring f ibrosis stages except F0-F1. The area under the receiver operating characteristics curves of the Fibro-Stiffness index for prediction of F≥2 (0.90), F≥ 3 (0.90) and F= 4(0.92) in the estimation group and those for F≥ 3 (0.93) and F =4 (0.97) in the validation group were the highest among the 5 methods examined. The accuracy of the Fibro-Stiffness index had highest values for F≥2, F≥3 and F=4 in both the estimation and validation groups. The diagnostic performance for F= 4 was improved by a combination of the Fibro-Stiffness index with serum hyaluronic acid level. CONCLUSION: The Fibro-Stiffness index was constructed and validated. It showed superior diagnostic performance to other indices for F ≥ 2,3 and 4.

Double immunostaining of p63 and low - molecular-weight cytokeratin in pleomorphic adenomas

Purpose: Myoepithelial cells may potentially contribute to the formation of different histological patterns in salivary gland tumors。We examined the expression patterns of p63 and low-molecular-weight cytokeratin (LMCK) using doble immunostaining method to clarify the histological differentiation in pleomorphic adenoma (PA) of the salivary gland。Methods: Formalin-fixed, paraffin-embedded sections of 10 specimens of PA obtained from Osaka Dental University Hospital were deparaffinized and autoclaved at 121℃ for 15 min in 1 mM EDTA solution (pH9.0) for antigen retrieval, and endogenous peroxidase activity was then blocked with 3% H202 solution。The sections were incubated with anti-p63 monoclonal antibody for 1 hr at room temperature (RT) and then for 30 min with EnVision+ at RT。they were visualized with 3',3-diaminobenzidine (DAB)。Following visualization, the sections were incubated with anti-LMCK monoclonal antibody for 30 min at RT and incubated with EnVision+ for 30 min at RT。They were subsequently visualized with nickel-DAB。Results: In the normal salivary gland, expression of p63 was observed in the nuclei of myoepithelial cells of intercalated ducts and acini and in the basal cells of striated and excretory ducts。In PA, expression of p63 was observed in the nuclei of the basal cells of luminal side, epithelioid cells of sheet-pattern, spindle cells of myxoid-pattem and clear cells。However, p63 was not seen in plasmacytoid cells。LMCKs were observed in the cytoplasm of intercalated ductal cells, basal cells of striated ducts with abundant eosinophilic cytoplasm of cells facing the luminal side, and metaplastic squamous cells。However, LMCK was not seen in the plasmacytoid cells and myoepithelial cells。

Experimental and Modeling Investigation of Shallow Water Table Fluctuations in Relation to Reverse Wieringermeer Effect

Soil column experiments and modeling investigations were performed to study the behavior of shallow water table in response to various recharge events. Hence, shallow water table fluctuations inside sandy (Toyoura sand) and clayey (Chiba light clay) soil columns in response to surface and sub-surface recharge events were investigated under laboratory conditions. Experimental results showed that small application of water could raise the shallow water table level more than 100 times in depth in the case of Toyoura sand and more than 50 times in the case of Chiba LiC, reflecting a reverse Wieringermeer effect (RWE) response type of groundwater. This rise was associated with a prompt change of pressure head values which exhibited instantaneous fluctuations of centimeters due to the addition of millimeters of water. The recharge volumes leading to such disproportionate water table rise were successfully estimated using a simple analytical model based on the moisture retention curve of the soil and considering the hysteresis effect on soil water dynamics within the capillary fringe zone.
