Monocyte chemoattractant protein-1, transforming growth factor-β1, nerve growth factor, resistin and hyaluronic acid as serum markers：comparison between recurrent acute and chronic pancreatitis

BAcKGROUND： Diagnostic parameters that can predict the presence of chronic pancreatitis（cP） in patients with recurrent pain due to pancreatitis would help to direct appropriate therapy. This study aimed to compare the serum levels of monocyte chemoattractant protein-1（McP-1）, transforming growth factor-β1（TGF-β1）, nerve growth factor（NGF）, resistin and hyaluronic acid（HA） in patients with recurrent acute pancreatitis（RAP） and cP to assess their ability to differentiate the two ***： Levels of serum markers assessed by enzymelinked immunosorbent assay（ELISA） were prospectively compared in consecutive patients with RAP, cP and in controls and stepwise discriminant analysis was performed to identify the markers differentiating RAP from ***： One hundred and thirteen consecutive patients（RAP=32, cP=81） and 78 healthy controls were prospectively enrolled. The mean（SD） age of the patients was 32.0（14.0）years; 89（78.8%） were male. All markers were significantly higher in cP patients than in the controls（P〈0.001）; McP-1NGF and HA were significantly higher in RAP patients than in the controls（P〈0.001）. Stepwise discriminant analysis showed significant difference（P=0.002） between RAP and cP for resistin with an accuracy of 61.9%, discriminant scores of ≤-0.479 and ≥0.189 indicating RAP and cP, respectively. The other markers had no differential value between RAP and ***： Serum resistin is a promising marker to differentiate between RAP and cP and needs validation in future studies, especially in those with early cP.

AIM To compare two tests for exocrine pancreatic function(EPF) for use in M-ANNHEIM staging for pancreatitis. METHODS One hundred and ninety four consecutive patients with acute pancreatitis(AP; n = 13), recurrent acute pancreatitis(RAP; n = 65) and chronic pancreatitis(cP; n = 116) were enrolled. EPF was assessed by faecal elastase-1(FE-1) estimation and stool fat excretion by the acid steatocrit method. Patients were classified as per M-ANNHEIM stages separately based on the results of the two tests for comparison. Independent Student's t-test, χ~2 test, Kruskal-Wallis test, Mann-Whitney U test and Mc Nemar's test were used as appropriate. RESULTS Sixty-one(52.5%) patients with cP had steatorrhoea when assessed by the acid steatocrit method; 79 (68.1%) with cP had exocrine insufficiency by the FE-1 test(χ~2 test, P cid steatocrit and FE-1 showed a significant negative correlation(Spearman's rho =-0.376, P cally significant difference was seen between the M-ANNHEIM stages as classified separately by acid steatocrit and the FE-1. Thirteen(6.7%), 87(44.8%), 89(45.8%) and 5(2.5%) patients were placed in M-ANNHEIM stages 0,?Ⅰ, Ⅱ, and Ⅲ respectively, with the use of acid steatocrit as against 13(6.7%), 85(43.8%), 75(38.6%), and 21(10.8%) respectively by FE-1 in stages 0,?Ⅰ, Ⅱ, and Ⅲ thereby altering the stage in 28(14.4%) patients(P c Nemar's test). cONcLUSION FE-1 estimation performed better than the acid steatocrit test for use in the staging of pancreatitis by the M-ANNHEIM classification since it diagnosed a higher proportion of patients with exocrine insufficiency.

Transjugular intrahepatic portosystemic stent shunt for medically refractory hepatic hydrothorax:A systematic review and cumulative meta-analysis

Diagnostic performance of c-reactive protein level and its role as a potential biomarker of severe dengue in adults

Objective: To determine the relationship between c-reactive protein(cRP) level and the severity of dengue and the potential use of cRP in predicting acute dengue ***: A prospective observational study was performed on dengue patients admitted to a tertiary care hospital in southern India. All patients of age above 18 years, diagnosed with dengue were included in the study. The detailed laboratory parameters pertaining to dengue were recorded. cRP levels were estimated and compared between groups i.e. severe and non-severe dengue. cRP cut-off value was detected using the receiver-operator ***: Totally 98 patients with a mean age of 40 years were included. Among them, 11.2% of the patients suffered from severe dengue, 54.1% of the cases had non-severe dengue without warning signs, and 34.7% had non-severe dengue with warning signs. The median cRP was significantly higher in patients with severe dengue compared to patients with non-severe dengue(96.2 mg/d L vs. 5.3 mg/d L). Univariate logistic regression analysis showed that the odd’s ratio(OR) of cRP was 1.053(P≤0.001, 95% cI=1.029-1.078). cRP at a cutoff value of 21.6 mg/L(0.929 AUc) had excellent sensitivity(100%) and specificity(81.6%) in predicting severe dengue infection. Multivariate logistic regression analysis showed that cRP(OR=1.089, P=0.013) and ALT(OR=1.010, P=0.034) were statistically significant independent predictors of dengue ***: cRP level could be used as a potential biomarker to predict severity of dengue in adults.

Merits and limitations of TiO2-based photocatalytic pretreatment of soils impacted by crude oil for expediting bioremediation

Heavy hydrocarbons （HHcs） in soils impacted by crude oil spills are generally recalcitrant to biodegrada- tion due to their low bioavailability and complex chemical structure. In this study, soils were pretreated with varying concentrations of ultraviolet radiation A （UVA） or ultraviolet radiation c （UVc） activated titanium dioxide （TiO2）（1%-5%） under varying moisture conditions （0%- 300% water holding capacity （WHc）） to enhance biodegradation of Hccs and shorten remediation time- frames. We demonstrate that pretreatment of impacted soils with UVc-activated TiO2 in soil slurries could enhance bioremediation of HHcs. Total petroleum hydrocarbon （TPH） removal after 24 h exposure to UVc （254 nm and 4.8 mW/cm2） was （19.15：1.6）% in slurries with 300% WHc and 5 wt-% TiO2. TPH removal was non-selective in the c15-c36 range and increased with moisture content and TiO2 concentration. In a 10-d bioremediation test, TPH removal in treated soil increased to （26.05：0.9）%, compared to （15.45：0.8）% for controls without photo- catalytic pre-treatment. Enhanced biodegradation was also confirmed by respirometry. This suggests that addition of UVc-activated TiO2 to soil slurries can transform recalcitrant hydrocarbons into more bioavailable and biodegradable byproducts and increase the rate of subsequent biodegradation. However, similar results were not observed for soils pretreated with UVA activated TiO2. This suggests that activation of TiO2 by sunlight and direct addition of TiO2 to unsaturated soils within landfarming setting may not be a feasible approach. Nevertheless, less than 1% of UVA （7.5 mW/cm2） or UVc （1.4 mW/cm2） penetrated beyond 0.3 cm soil depth, indicating that limited light penetration through soil would hinder the ability of TiO2 to enhance soil bioremediation under land farming conditions.

Effect of Sm_2O_3 on structural and thermal properties of zinc fluoroborate glasses

Glasses based on Sm3+ doped zinc fluoroborate were synthesized and characterized. Formations of glasses were investigated in the 30 ZnF2-20 TeO2-(50-x)B2O3-x Sm2O3 matrix. Fast quenching and adequate heat treatment are required to prevent melt crystallization and to diminish thermal stress, which result in an efficient amorphous material. The differential scanning calorimetry(DSc), scanning electron microscopy(SEM), energy dispersive X-ray analysis(EDAX), X-ray photoelectron spectroscopy(XPS) were employed to record, calculate, measure and analyze the stability, density and refractive index of the glass samples with different concentrations of Sm3+ ranging between 0 to 2.5%(mass fraction). XPS result shows the values of core-level binding energy(Zn 3s, Sm 4d, Te 3d, B 1s, O 1s and F 1s) of(ZnF2-TeO2-B2O3-Sm2O3) glass matrix and indicates the good fusibility of the present glass samples. Density of the glass samples increases as dopant concentration increases and glass transition temperature tg ranges between 395 °c and 420 °c.
