Inter-observer variability in histopathological assessment of liver biopsies taken in a pediatric open label therapeutic program for chronic HBV infection treatment

AIM： To our knowledge, the inter-observer variability of the liver biopsy findings in HBV-infected children have not been studied as yet. Hence, we aimed to compare different pathologist＇s assessment of grading and staging in liver biopsies obtained from children prior to interferon treatment. MeTHODS： We collected 920 biopsies from 11 medical centers. The biopsies were independently reviewed by 6 pathologists from academic centers who assessed Batts-Ludwig score for grading and staging. satisfactory agreement among observers was defined as at least 60% of observers having the same opinion. satisfactory dispersion between maximal and minimal score for the same biopsy specimen was defined as a maximum 1 point. ReSuLTS： satisfactory inter-observer agreement for grading was obtained in 51.6% and for staging in 75.7% of biopsies. satisfactory dispersion for grading scores was observed in 44.5% and for staging in 72.7% of cases. CONCLuSION： Our study demonstrates that：（1） pathologists differ in their assessment of grading and staging of liver biopsies; （2） inter-observer variability for staging is lower than that for grading; and （3） regardless of the inter-observer variability of assessments, the majority of children with chronic HBV infection have mild to moderate inflammation and mild to moderate fibrosis.

Dear editor,fascin actin-bundling protein 1(fascin,gene name:FSCN1)is the primary actin-bundling protein that clusters filaments into stable and bundled filopodia[1].In previous studies,elevated levels of fascin promoted filopodia formation and enhanced the invasive ability of tumor cells,suggesting that fascin is a target for therapeutic intervention[2,3].Therefore,it is necessary to elucidate the detailed molecular mechanism by which fascin bundles F-actin filaments.

AIM:The rote of the sphincter of Oddi(SO)in ethanol (eTOH)-induced pancreatitis is *** aim was to characterise the effect of e-I-OH on basal and stimulated SO motility. MeTHODS:SOs removed from white rabbits were placed in an organ bath(Krebs solution,pH7.4,37℃).The effects of 2 mL/L,4 mL/L,6 mL/L and 8 mL/L of eTOH on the contractile responses of the sphincter were determined. SOs were stimulated with either 0.1 μmol/L carbachol,1 μmol/L erythromycin or 0.1 μmol/L cholecystokinin(CCK). ReSuLTS:eTOH at a dose of 4 mL/L significantly decreased the baseline contractile amplitude from 11.98±0.05 mN to 11.19±0.07 ***,no significant changes in the contractile frequency were ***(0.6%) significantly decreased both the baseline amplitude and the frequency compared to the control group(10.50±0.01 mN, 12.13±0.10 mN and 3.53±0.13 c/min,5.5±0.13 cycles(c)/min, respectively).Moreover,0.8% of eTOH resulted in complete relaxation of the ***(0.1 μmol/L)or erythromycin (1 μmol/L)stimulated the baseline amplitudes(by 82% and 75%,respectively)and the contractile frequencies (by 150% and 106%,respectively).In the carbachol or erythromydn-stimulated groups 2-6 mL/L of e-IOH significantly inhibited both the amplitude and the ***, a 4-5 min administration of 6 mL/L eTOH suddenly and completely relaxed the ***(0.1 μmol/L)stimulated the baseline amplitude from 12.37±0.05 mN to 27.40±1.82 mN within 1.60±0.24 *** this peak,the amplitude decreased to 17.17±0.22 mN and remained constant during the *** frequency peaked at 12.8±0.2 c/min, after which the constant frequency was 9.43±0.24 c/min throughout the rest of the *** at a dose of 4 mL/L significantly decreased the amplitude from 16.13±0.23 mN to 14.93±0.19 ***,no significant changes in the contractile frequency were *** at a dose of 6 mL/L inhibited both the amplitudes and the frequencies in the CCK-stimulated group,while 8 mL/L of e

Non-Stoichiometry effects on the extreme magnetoresistance in Weyl Semimetal WTe2

Non-stoiehiometry effect on the extreme magnetoresistanee is systematically investigated for the Weyl semimetal WTe2. magnetoresistance and hall resistivity are measured for the as-grown samples with a slight difference in Te vacancies and the annealed samples with increased Te vacancies. The fits to a two-band model show that the magnetoresistanee is strongly dependent on the residual resistivity ratio （i.e., the degree of non-stoichiometry）, which is eventually understood in terms of electron doping that not only breaks the balance between electron-type and hole-type carrier densities, but also reduces the average carrier mobility. Thus the compensation effect and ultrahigh mobility are probably the main driving force of the extreme magnetoresistance in WTe2.

Assessing impact of land use and climate change on regulating ecosystem services in the Czech Republic

ecosystem services,defined as benefits provided by ecosystem functioning to society,are essential to human *** to global environmental change and related anthropogenic drivers,ecosystems are often degraded,which hinders the delivery of ecosystem *** study aims to quantify the impacts of land use and climate change on two regulating ecosystem services-carbon sequestration and water purification in terms of nitrogen retention in the Czech *** employing approaches of scenarios and modeling,we illustrate current and potential future status of these ecosystem *** results show that among the ecosystem change drivers,one of the dominant domestic land-use change of ALARM BAMBu(Business-As Might Be usual)scenario is change of arable land and grassland to forest area that increases by 4.5%in 2080 compared to *** results of ecosystem service modeling based on BAMBu scenario for the years 2050 and 2080 indicate that the highest yearly carbon sequestration rate occurred in 2000-2050,reaching 640 GgC·yr^(−1),2000-2080 shows decline in this regulating service by 16%.Average nitro-gen leaching to water streams reached 0.75 kgN·ha^(−1)·yr^(−1)for BAMBu in 2050 scenario and 0.80 kgN·ha^(−1)·yr^(−1)for BAMBu in 2080 scenario as a result of decreasing nitrogen load,which suggested a decrease in nitrogen pollution compared to *** ecosystem services have not been extensively mainstreamed into research and policies in eastern european countries,we aim to contribute to improvement of knowledge on current status and potential future pathways of the provision of regulating ecosystem services in the Czech Republic.

目的考察茜草对人神经胶质瘤u87细胞生长的影响,并从抗肿瘤作用分子机制及丙酮酸代谢角度探讨作用机理。方法体外培养u87细胞,以MTT法考察茜草不同提取物对u87细胞的生长抑制作用;人全基因组表达谱芯片初步筛选茜草醇提物抑制u87细胞的可能机制;逆转录PCR验证部分差异表达基因;碘化丙啶(PI)单染法检测细胞周期分布;分光光度法检测细胞内H_2O_2含量、丙酮酸含量、乳酸含量、Caspase-3活力、丙酮酸激酶(PK)活力、乳酸脱氢酶(LDH)活力、苹果酸脱氢酶(MDH)活力、琥珀酸脱氢酶(SDH)活力;Western Blot检测超氧化物歧化酶1(SOD1)、过氧化氢酶(Catalase)、p-Histone H2A.X(Ser139)、p-乳腺癌易感蛋白1(BRCA1)(Ser1524)、人mutS同源蛋白6(MSH6)、增殖细胞核抗原(PCNA)、单羧酸转运蛋白1(MCT1)的蛋白表达。结果以不同剂量的茜草醇提物、水提物(终浓度分别为200,100,50,25,12.5 mg·L^(-1))作用于u87细胞24,48,72 h后,u87细胞生长受到明显抑制,抑制作用具有剂量依赖和时间依赖性,醇提物优于水提物。茜草醇提物50 mg·L^(-1)作用于u87 24 h后,表达谱芯片结果揭示其作用机制与细胞周期、凋亡、氧化-还原平衡、DNA损伤及修复、单羧酸转运等有关。茜草醇提物100,50 mg·L^(-1)处理u87 24 h进行后续实验,逆转录PCR验证MSH6、Catalase、BAX、硫氧还蛋白相互作用蛋白(TXNIP)、PCNA mRNA表达与芯片结果一致,而DNA复制解旋酶/核酸酶2(DNA2)mRNA表达仅100 mg·L^(-1)组与芯片结果相符。茜草醇提物可使u87细胞阻滞在S期、提高Caspase-3活力;还增加了细胞内H_2O_2含量,而Catalase和SOD1蛋白表达减少;细胞内DNA双链断裂的标志p-Histone H2A.X(Ser139)蛋白表达增加,DNA修复相关的p-BRCA1(Ser1524)、MSH6、PCNA蛋白表达减少。茜草醇提物处理后,u87细胞内丙酮酸含量减少、乳酸含量增加;PK、LDH、MDH、SDH活力增加、MCT1蛋白表达减少。结论茜草能有效抑制人神经胶质瘤u87细胞在体外的生长活力,作用呈现一定剂量依赖和时间依赖,且醇提优于水提。其作用机制涉及多方面,与细胞周期阻滞、Caspase-3活化、影响氧化-还原平衡、DNA损伤及修复、细胞内丙酮酸代谢有关。茜草是一味具有开发潜力的抗肿瘤中药。

注意缺陷多动障碍(Attention-deficit/hyperactivity disorder,ADHD)是癫痫患儿一种常见且富有挑战的共患病。国际抗癫痫联盟(ILAe)儿科委员会共病工作组发现,关键问题在于识别和管理癫痫患儿中的ADHD。对支持这些问题的方法的证据进行了系统评价,并根据美国神经病学学会实践参数的标准进行了整理和分级。遵循系统评价和Meta分析优先报告的条目(Preferred Reporting Items for Systematic Reviews and MetaAnalyses,PRISma)要求,使用PROSPeRO注册(CRD42018094617)。与女性癫痫患儿相比,男性患儿无更高的共患ADHD风险(Level A)。孕期使用丙戊酸钠与子代注意力不集中和多动相关(1项I级研究),且有智力和发育障碍的患儿合并ADHD的风险更高(A级)。起病年龄对是否合并ADHD的影响尚不明确(u级),但是发作控制欠佳的患儿合并ADHD的风险更高(B级)。ADHD的筛查应该从6岁或者明确诊断时开始,并且每年筛查一次(u级),在抗癫痫药物调整后也应该重新评估(u级)。ADHD的诊断应该由受过ADHD专业培训的医务工作者参与(u级)。支持使用优势和困难问卷筛查工具(B级)。强烈建议有学习障碍的癫痫患儿进行正式的认知测试(u级)。多药治疗的患儿出现行为问题的比例较单药治疗者更高(C级)。丙戊酸钠可以加剧失神发作患儿的注意力集中障碍(A级)。哌甲酯在癫痫患儿中耐受性及疗效均良好(B级)。支持阿托莫西汀耐受性好的证据尚有限(C级)。多学科参与过渡期和成人ADHD的诊治至关重要(u级)。总之,虽然可以对一些研究问题提出建议,但这项系统评价强调了开展更全面、更有针对性的大样本前瞻性研究的必要性。
