目的探討2種腎小球濾過率計算方法評估腎小球濾過率(estimated Glomerular Filtration Rate,eGFR)的臨床運用價值。方法測定912例2015年8月~2015年11月在本院住院及門診的患者(年齡21歲~93歲,男475例,女437例)血漿肌酐(Creatine,Cr)及胱抑素C(Cystatin C,Cys C)水平,血漿肌酐用肌酐酶法測定,胱抑素C用增強透射免疫比濁法測定,基於血漿胱抑素C公式計算的腎小球濾過率eGFR1,與MDRD(Modification of Diet in Renal Disease,MDRD)公式計算的腎小球濾過率eGFR2進行比較,評價2種方法用於評估腎小球濾過率的價值。結果數據分為四組,每組數據eGFR1與eGFR2的相關係數分別為0.8882;0.7635;0.8786;0.6037,每組P1.0mg/L以及肌酐≤0.905mg/dL有明顯優勢。

BACKGROUND: To popularize the wide-spread use of automated external defi brillator(AED) to save life in sudden cardiac arrest, we compared the strength and weakness of different types of AEDs to enable a sound selection based on regional ***: This was a retrospective descriptive study. Different types of AEDs were compared according to the information of AEDs from manuals and brochures provided by the manufacturers. Fifteen types of AEDs were divided into 3 groups, basic, intermediate and ***: Lifeline? AUTO AED had the best performance in price, portability and user-friendly among AEDs of basic level. It required less time for shock charging. Samaritan PAD defi brillator was superior in price, portability, durability and characteristic among AEDs of intermediate level. It had the longest warranty and highest protection against water and dust. Lifeline? PRO AED had the best performance in most of the criteria among AEDs of advanced level and offered CPR video and manual mode for laypersons and clinicians ***: Lifeline? AUTO AED, Samaritan PAD def ibrillator, Lifeline? PRO AED are superior in AEDs of basic, intermediate and advanced levels, respectively. A feasible AED may be chosen by users according to the regional requirement and the current information about the best available products.

Wireless capsule endoscopy(WCE) is a promising technique which has overcome some limitations of traditional diagnosing tools, such as the comfortlessness of the cables and the inability of examining small intestine section. however, this technique is still far from mature and asks for the feasible improvements. For example, the relatively low transmission data rate and the absence of the real-time localization information of the capsule are all important issues. The studies of them rely on the understanding of the electromagnetic wave propagation in human body. Investigation of performance of WCE communication system was carried out by studying electromagnetic(EM) wave propagation of the wireless capsule endoscopy transmission channel. Starting with a pair of antennas working in a human body mimic environment, the signal transmissions and attenuations were examined. The relationship between the signal attenuation and the capsule(transmitter) position, and direction was also evaluated. These results provide important information for real-time localization of the capsule. Moreover, the pair of antennas and the human body were treated as a transmission channel, on which the binary amplitude shift keying(BASK) modulation scheme was used. The relationship between the modulation scheme, data rate and bit error rate was also determined in the case of BASK. With the obtained studies, it make possible to provide valuable information for further studies on the selection of the modulation scheme and the real-time localization of the capsules.

Reperfusion therapy is the preferred treatment for ischemic stroke,but is hindered by its short treatment window,especially in patients with diabetes whose reperfusion after prolonged ischemia is often accompanied by exacerbated *** mechanisms underlying exacerbated hemorrhage are not fully *** study aimed to identify this mechanism by inducing prolonged 2-hour transient intraluminal middle cerebral artery occlusion in diabetic Ins2Akita/+mice to mimic patients with diabetes undergoing delayed mechanical *** results showed that at as early as 2 hours after reperfusion,Ins2Akita/+mice exhibited rapid development of neurological deficits,increased infarct and hemorrhagic transformation,together with exacerbated down-regulation of tight-junction protein ZO-1 and upregulation of blood-brain barrier-disrupting matrix metallopeptidase 2 and matrix metallopeptidase 9 when compared with normoglycemic Ins2+/+*** indicated that diabetes led to the rapid compromise of vessel integrity immediately after reperfusion,and consequently earlier death and further aggravation of hemorrhagic transformation 22 hours after *** observation was associated with earlier and stronger up-regulation of pro-angiogenic vascular endothelial growth factor(VEGF)and its downstream phospho-Erk1/2 at 2 hours after reperfusion,which was suggestive of premature angiogenesis induced by early VEGF up-regulation,resulting in rapid vessel disintegration in diabetic *** reticulum stress-related pro-apoptotic C/EBP homologous protein was overexpressed in challenged Ins2Akita/+mice,which suggests that the exacerbated VEGF up-regulation may be caused by overwhelming endoplasmic reticulum stress under diabetic *** conclusion,the results mimicked complications in patients with diabetes undergoing delayed mechanical thrombectomy,and diabetes-induced accelerated VEGF up-regulation is likely to underlie exacerbated hemorrhagic transformat

Objective: A variety of ion channels have been implicated in breast cancer proliferation and *** K+(Kv) channels not only cause repolarization in excitable cells,but are also involved in multiple cellular functions in non-excitable *** this study we investigated the role of Kv channels in migration of BT474 breast cancer ***: Transwell technique was used to separate migratory cells from non-migratory ones and these two groups of cells were subject to electrophysiological examinations and microfluorimetric measurements for cytosolic *** migration was examined in the absence or presence of Kv channel ***: When compared with non-migratory cells,migratory cells had much higher Kv current densities,but rather unexpectedly,more depolarized membrane potential and reduced *** transcriptasepolymerase chain reaction(RT-PCR) analysis revealed the presence of Kv1.1,Kv1.3,Kv1.5,Kv2.1,Kv3.3,Kv3.4 and Kv4.3 *** migration was markedly inhibited by tetraethylammonium(TEA),a delayed rectifier Kv channel blocker,but not by 4-aminopyridine,an A-type Kv channel ***: Taken together,our results show that increased Kv channel expression played a role in BT474 cell migration,and Kv channels could be considered as biomarkers or potential therapeutic targets for breast cancer *** mechanism(s) by which Kv channels enhanced migration appeared unrelated to membrane hyperpolarization and Cainflux.

Mesenchymal stem cells(MSCs) possess immunomodulatory properties, which confer enormous potential for clinical application. Considerable evidence revealed their efficacy on various animal models of autoimmune diseases, such as multiple sclerosis, systemic lupus erythematosus and uveitis. MSCs elicit their immunomodulatory effects by inhibiting lymphocyte activation and proliferation, forbidding the secretion of proinflammatory cytokines, limiting the function of antigen presenting cells, and inducing regulatory T(Treg) and B(Breg) cells. The induction of Treg and Breg cells is of particular interest since Treg and Breg cells have significant roles in maintaining immune tolerance. Several mechanisms have been proposed regarding to the MSCs-mediated induction of Treg and Breg cells. Accordingly, MSCs induce regulatory lymphocytes through secretion of multiple pleiotropic cytokines, cell-to-cell contact with target cells and modulation of antigen-presenting cells. Here, we summarized how MSCs induce Treg and Breg cells to provoke immunosuppression.

BACKGROUND: Intravenous fluid(IVF) is commonly used in acute clinical management. This study aimed to review the choice and primary considerations in IVF prescriptions and to evaluate the adequacy of guidelines and trainings on it in the New Territories West Cluster(NTWC) of hong ***: This is a descriptive study based on data collected from an online survey. Data were processed by SPSS for statistical analysis. This study focused on a general description and doctor-nurse between group comparison. Participants were asked the choice of IVF for nine acute clinical scenarios and provide reason. A 1–10 scale was used to assess the sufficiency of guideline, training and information, and time for revision on IVF ***: 0.9% sodium chloride was the most familiar IVF(36%), followed by 5% Dextrose solution(26%). In the nine scenarios, the most chosen IVF was 0.9% sodium chloride(37%–61%). There was significant difference in the choice of IVF between doctors and nurses in 7 cases. The second most chosen IVF for doctors was Plasma-Lyte A while that for nurses was Gelofusine. Departmental practice was the most chosen reason to account for the prescription. The adequacy of guideline, information and training, and time for revision was rated 5. Doctors had significantly more time at work than nurses to update knowledge in IVF prescription(5.41 versus 4.57).CONCLUSION: 0.9% sodium chloride was mostly chosen. The choice of IVF was mainly based on departmental practice. Adequacy of guideline, information and training, and time for revision on IVF prescription were average, indicating significant training deficit.
