This article looks at how cosmopolitanism--the notion of universality within a diversity of multi-cultures---has been shaping the discipline of world literature. The article encompasses chiefly three parts. The first part offers an overview of the debates on the discipline widely discussed by literary scholars such as Franco Moretti, David Damrosch and Emily Apter. I take issue with the harmonic co-existence of both local and global elements---and what I define as ＂glocality＂---in literatures to exhibit the inevitable trend of the trans-cultural, supranational and cross-historical interactions among multiple centres and/or various cities especially in the twenty-first century. I thereby argue in the second part using Leung Ping kwan （1949-2013）＇s ＂Images of Hong Kong＂（1992） and Louise Ho＇s two poetry pieces written in 1994 to prove how Kantian Cosmopolitan elements have deeply embedded in the poem written in a city where the West frequently interacts with the East. I conclude by stepping in further to argue that only through tolerating and mediating between the region and the globe can world literature as a discipline find its way out without fear for marginalising any of the literary pieces.

An unprecedented outbreak investigation for nosocomial and community-acquired legionellosis in Hong Kong

Background The environmental sources associated with community-acquired or nosocomial legionellosis were not always detectable in the mainland of China and Hong Kong, China. The objective of this study was to illustrate the control measures implemented for nosocomial and community outbreaks of legionellosis, and to understand the environmental distribution of legionella in the water system in Hong Kong, China. Methods We investigated the environmental sources of two cases of legionellosis acquired in the hospital and the community by extensive outbreak investigation and sampling of the potable water system using culture and genetic testing at the respective premises. Results The diagnosis of nosocomial legionellosis was suspected in a patient presenting with nosocomial pneumonia not responsive to multiple beta-lactam antibiotics with subsequent confirmation by Legionella pneumophila serogroup 1 antigenuria. High counts of Legionella pneumophila were detected in the potable water supply of the 70-year-old hospital building. Another patient on continuous ambulatory peritoneal dialysis presenting with acute community-acquired pneumonia and severe diarrhoea was positive for Legionella pneumophila serogroup 1 by polymerase chain reaction （PCR） testing on both sputum and nasopharyngeal aspirate despite negative antigenuria. Paradoxically the source of the second case was traced to the water system of a newly commissioned office building complex. No further cases were detected after shock hyperchlorination with or without superheating of the water systems. Subsequent legionella counts were drastically reduced. Point-of-care infection control by off-boiled or sterile water for mouth care and installation of water filter for showers in the hospital wards for immunocompromised patients was instituted. Objective:Delivery of chemotherapeutic drugs to the brain has remained a major obstacle in the treatment of glioma,owing to the presence of the blood-brain barrier and the activity of P-gp,which pumps its substrate back into the systemic *** aim of the present study was to develop an intravenous formulation of HM30181 A(HM)to inhibit P-gp in the brain to effectively deliver paclitaxel(PTX)for the treatment of malignant ***:Two formulations of solubilized HM were designed on the basis of different solid dispersion strategies:i)spray-drying[polyvinlypyrrolidone(PVP)-HM]and ii)solvent evaporation[HP-β-cyclodextrin(cyclodextrin)-HM].The P-gp inhibition of these 2 formulations was assessed on the basis of rhodamine 123 uptake in cancer *** and brain pharmacokinetic parameters were also determined,and the antitumor effect of cyclodextrin-HM with PTX was evaluated in an orthotopic glioma xenograft mouse ***:Although both PVP-HM and cyclodextrin-HM formulations showed promising P-gp inhibition activity in vitro,cyclodextrin-HM had a higher maximum tolerated dose in mice than did *** study of cyclodextrin-HM revealed a plasma concentration plateau at 20 mg/kg,and the mice began to lose weight at doses above this ***-HM(10 mg/kg)administered with PTX at 10 mg/kg showed optimal antitumor activity in a mouse model,according to both tumor volume measurement and survival time(P<0.05).Conclusions:In a mouse orthotopic brain tumor model,the intravenous co-administration of cyclodextrin-HM with PTX showed potent antitumor effects and therefore may have potential for glioma therapy in humans.

Beishan Terrane, located in the northeast of the Tarim Block, in northwest China, has developed a 500-km long and 100-km wide belt of Permian mafic–ultramafic intrusions One of these mafic–ultramafic intrusions, the Xuanwoling Intrusion, is composed of dunite, troctolite, olivine gabbros and gabbros, with cumulate texture and rhythmic layering The crystallization sequence is olivine ? spinel ? plagio clase ? pyroxene, indicating that the crystallization pressure is lower than 0.5–0.8 GPa and that the intrusion has undergone variable degrees of crustal contamination, increasing from dunite to gabbros. The olivines found in the Xuanwoling Intrusion have high Fo values(up to 90), suggesting a primary magma with a high composition of mg. It is likely that this high-mg magma was produced at extremely high temperatures(1,330–1,350 °C), and as a result, Nd–Sr isotopic compositions similar to oceanic island basalts are found in the Xuanwoling Intrusion, which we propose arose from the mantle plume.

A characterization of photophysical properties of 1,3-dihydro-1,3,3-trimethylspiro[2H-indole-2,3’-[3H]naphtha[2,1-b] [1,4]oxazine] (spirooxazine) and1’,3’-dihydro-1’,3’,3’-trimethyl-6-nitrospiro[2H-1-benzopyran-2,2’-(2H)-indole] (spiropyran) compounds hosted in poly(methyl methacrylate) (PMMA) was carried out. The parameters which influenced the photochromic behavior of both compounds were investigated. After ultraviolet (UV) irradiation, it was demonstrated that the temperature was the dominant factor which influenced the deactivation of both materials. For spiropyran, it was demonstrated that the process could be photo-induced with a certain wavelength which we have specified in this manuscript. It was also found that X-Ray did not affect the photo physical properties of spirooxazine and spiropyran. Finally, both materials could be used as low-cost germicidal UV indicators.

AIM:To investigate the risk association and compare the onset age of hepatocellular carcinoma(HCC) patients in Taiwan with different genotypes of MDM2- SNP309. METHODS:We analyzed MDM2-SNP309 genotypes from 58 patients with HCC and 138 cancer-free healthy controls *** of MDM2-SNP309 was conducted by restriction fragment length polymor- phism assay. RESULTS:The proportion of homozygous MDM2- SNP309 genotype(G/G)in cases and cancer-free healthy controls was similar(17.2%vs 16.7%).Multi-variate analysis showed that the risk of G/G genotypeof MDM2-SNP309 vs wild-type T/T genotype in patients with HCC was not significant(OR=1.265,95% CI=0.074-21.77)after adjustment for sex,hepatitis B or C virus infection,age,and cardiovascular disease/ ***,there was a trend that GG genotype of MDM2-SNP309 might increase the risk in HCC patients infected with hepatitis virus(OR=2.568, 95%CI=0.054-121.69).Besides,the homozygous MDM2-SNP309 genotype did not exhibit a significantly earlier age of onset for HCC. CONCLUSION:Current data suggest that the asso- ciation between MDM2-SNP309 GG genotype and HCC is not significant,while the risk may be enhanced in patients infected by hepatitis virus in Taiwan.

Hypertensive-Nimodipine Therapy for Middle Cerebral Artery Vasospasm after Resection of Glioblastoma Multiforme: A Case Report and Literature Review

Delayed cerebral ischemia (DCI) due to post-brain tumor resection vasospasm is an often unrecognized yet debilitating complication. We present a patient with DCI after the resection of glioblastoma multiforme (GBM). To our knowledge, this is the first report on DCI after GBM resection. A 52-year-old female patient with headache for one month underwent subtotal resection of a left temporal GBM encasing the proximal middle cerebral artery (MCA). She was well during the immediate postoperative period but developed right upper limb dense monoparesis on postoperative day four with computed tomographic angiography confirming left MCA vasospasm. Symptoms were significantly alleviated with weeklong hypertensive therapy and nimodipine administration;however they recurred soon after cessation of treatment. A high index of clinical suspicion is needed for the diagnosis of post-tumor resection DCI. Any new postoperative neurological deficit that cannot be explained by hemorrhage, seizures or infection should be expeditiously investigated by angiography or transcranial Doppler sonography. Prompt initiation of hypertensive and nimodipine therapy can possibly reverse neurological deficit. Treatment should be guided by Doppler, angiographic or perfusion imaging studies and not by clinical improvement alone.
