Survival Analysis of Victims of Sulfur Oxide Air Pollution Suffering from COPD or Asthma in Yokkaichi, Japan, in Relation to Predisposing Exposure

To investigate the relationships between clinical findings and symptoms and the survival of patients with Yokkaichi Asthma, in relation to predisposing sulfur dioxide (SO2) exposure, we examined records of 1836 patients registered in the city of Yokkaichi during 1973-1988 by “Pollution-Related Health Damage Compensation Law.” Complete records were obtained from 735 patients (352 males and 383 females) until December 31, 2007, and were used for the analysis. Ambient SO2 concentrations in the Yokkaichi area were obtained from the Environmental Numeric Database of the National Institute for Environmental Science, Japan. It was found that severity of clinical symptoms and decreased pulmonary function were significantly correlated with predisposing SO2 exposure. A Cox proportional hazards analysis revealed that among all patients (COPD and asthma), age, forced expiratory volume 1.0 (sec) % and smoking affected mortality for both males and females. Significant associations between mortality, vital capacity (percent predicted) and cough and sputum were observed in males. Thus, the survival of patients with Yokkaichi Asthma was affected by severity of clinical symptoms and decreased pulmonary function, which were related to predisposing SO2 exposure. It appeared that the effects of clinical changes were greater in males than in females.

＜正＞ Human resources in science and technology （HRST） are very important in the national inno vation system. In Japan, since 1990’ s, many policy measures have been introduced in order to develop HRST, and some of them have achieved the goals partially. However, these partial achi evements have inconsistency with other policy measures and culture of employment tradition, wh ich caused new problems, such as unemployment of Ph.D. holders and low mobility of research ers. In addition to these problems, Japan also faces new challenges, such as aging and decreasin g population, globalization and knowledge-based economy. This paper describes the recent Japan ese policies related to HRST, their results and problems, and the challenges for Japan toward th e next step.

For many years, nerve transfer has been commonly used as a treatment option following peripheral nerve injury, although the precise mechanism underlying successful nerve transfer is not yet clear. We developed an animal model to investigate the mechanism underlying nerve transfer between branches of the spinal accessory nerve (Ac) and suprascapular nerve (Ss) in rats, so that we could observe changes in the number of motor neurons, investigate the 3-dimensional localization of neurons in the anterior horn of the spinal cord, and perform an electromyogram (EMG) of the supraspinatus muscle before and after nerve transfer treatment. The present experiment showed a clear reduction in the number of γ motor neurons. The distributional portion of motor neurons following nerve transfer was mainly within the neuron column innervating the trapezius. Some neurons innervating the supraspinatus muscle also survived post-transfer. Compared with the non-operated group, the EMG restoration rate of the supraspinatus muscle following nerve transfer was 60% in the experimental group and 80% in a surgical control group. Following nerve transfer, there was a distinct reduction in the number of γ motor neurons. Therefore, γ motor neurons may have important effects on the recovery of muscular strength following nerve transfer. Moreover, because the neurons located in regions innervating either the trapezius or supraspinatus muscle were labeled after Ac transfer to Ss, we also suggest that indistinct axon regeneration mechanisms exist in the spinal cord following peripheral nerve transfer.

AIM To clarify the diagnostic performance of endocytoscopy for differentiation between neoplastic and nonneoplastic colorectal diminutive *** Patients who underwent endocytoscopy between October and December 2016 at Sano Hospital were prospectively recruited. When diminutive polyps(≤5 mm) were detected, the lesions were evaluated by endocytoscopy after being stained with 0.05% crystal violet and 1% methylene blue. The diminutivepolyps were classified into five categories(EC 1 a, 1 b, 2, 3 a, and 3 b). Endoscopists were asked to take a biopsy from any lesion diagnosed as EC1 b(indicator of hyperplastic polyp) or EC2(indicator of adenoma). We have assessed the diagnostic performance of endocytoscopy for EC2 and EC1 b lesions by comparison with the histopathology of the biopsy specimen. RESULTS A total of 39 patients with 63 diminutive polyps were analyzed. All polyps were evaluated by endocytoscopy. The mean polyp size was 3.3 ± 0.9 mm. Among the 63 diminutive polyps, 60 were flat and 3 were pedunculated. The mean time required for EC observation, including the time for staining with crystal violet and methylene blue, was 3.0 ± 1.9 min. Histopathologic evaluation showed that 13 polyps were hyperplastic and 50 were adenomas. The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of EC2 for adenoma compared with EC1 b for hyperplastic polyp were 98.0%, 92.3%, 96.8%, 98.0% and 92.3%, respectively. There were only two cases of disagreement between the endoscopic diagnosis made by endocytoscopy and the corresponding histopathological *** Endocytoscopy showed a high diagnostic performance for differentiating between neoplastic and non-neoplastic colorectal diminutive polyps, and therefore has the potential to be used for "real-time histopathology".

Postinflammatory hyperpigmentation (PIH) and postinflammatory erythema (PIE) in acne vulgaris are important and refractory complications for patients with acne vulgaris. To clarify the effects of 2% isostearyl-L-ascorbic acid (ISAA) against PIH and PIE in acne vulgaris, a clinical pilot study with topical 2% ISAA gel was performed in 25 acne patients with PIH and PIE. Topical ISAA gel was applied on the whole face with PIH and PIE in acne vulgaris twice a day for 3 months. Regarding PIH and PIE, investigator’s global improvement rating (IGIR) was evaluated in 7-point scales according to the reduced area of PIH and PIE before and after the study. Remarkable improvement in PIH was observed in 7 patients (28.0%) and in PIE in 12 (48%) of the 25 patients. No adverse reactions were observed during the treatment. Topical ISAA application can be an alternative, non-invasive available treatment for PIH and PIE in acne vulgaris.

Background:Established treatments for non-alcoholic steatohepatitis (NASH) are few, thus it is imperative to develop novel dietary strategies that can prevent NASH. A fermented mixed tea (FMT) made with Camellia japonica (Japanese camellia) and third- crop green tea leaves by tea-rolling processing was reported to reduce body weight and adipose tissue weight in Sprague-Dawley (SD) rats. Because visceral fat is one of the most important factors for the development of hepatic steatosis, this FMT supplementation can be a candidate dietary strategy for the prevention of NASH. Methods:Nine-week-old male SD rats were fed a high-fat and high-cholesterol (HFC) diets with or without FMT (camellia and third-crop green tea leaves at ratios of 1:5, 1:2 and 1:1) for 9 weeks (n=6–7/group). Histopathology, serology and expressions of ifbrogenetic, proinlfammatory, oxidative stress and lipid metabolism-related genes in the liver were evaluated. Results:Histologically, HFC diet with FMT at a ratio of 1:5 dramatically reduced NASH progression (14%) compared to the HFC diet without FMT (100%). FMT at a ratio of 1:5 reduced hepatic steatosis due to the activation of microsomal triglyceride transfer protein, and FMT at a ratio of 1:2 reduced mRNA levels of some proinlfammatory, lipid metabolism-related, ifbrogenic and oxidative stress marker genes. Conclusions:Our data suggest that FMT at a ratio of 1:5 or 1:2 likely possesses a preventive effect on NASH progression.

As the power density of electronic devices increases,there has been an urgent demand to develop highly conductive polymer composites to address the accompanying thermal management *** to the ultra-high intrinsic thermal conductivity,graphene is considered a very promising filler to improve the thermal conductivity of ***,graphene-based polymer composites prepared by the conventional mixing method generally have limited thermal conductivity,even under high graphene loading,due to the failure to construct efficient heat transfer pathways in the polymer ***,a spiral graphene framework(SGF)containing continuous and highly ordered graphene microtubes was developed based on a modified CVD *** embedding into the epoxy(EP)matrix,the graphene microtubes can act as efficient heat pathways,endowing the SGF/EP composites with a high through-plane thermal conductivity of 1.35 W·m^(-1)·K^(-1) at an ultralow graphene loading of 0.86 wt%.This result gives a thermal conductivity enhancement per 1 wt%filler loading of 710%,significantly outperforming various graphene structures as *** addition,we demonstrated the practical application of the SGF/EP composite as a thermal interface material for efficient thermal man-agement of the light-emitting diode(LED).

Incidence rates for esophagogastric junction cancer are rising rapidly worldwide possibly due to the economic development and demographic ***,increased attention has been paid to the prevention,diagnosis,and the treatment of esophagogastric junction *** there are discrepancies in the treatment strategy between Asian and Western countries,surgery remains the mainstay of treatment for esophagogastric junction *** developments of perioperative multidisciplinary treatment may lead to better therapeutic effect,higher complete resection rate,and better control of the residual diseases,thus result in prolonged *** this review,we will focus on the treatment of locally advanced resectable esophagogastric junction cancer,and discuss the current status and future perspectives of the perioperative treatment including chemotherapy,radiation therapy,and immunotherapy,as well as the surgical *** understanding of the latest treatment strategy and future overlook may enable to standardize and individualize the treatment for esophagogastric junction cancer,thus leading to better prognosis for those patients.
